A Study on the Optical Influence by Photosensitizer in Vitro (In Vitro에서 광증감제에 의한 광학적 영향에 관한 연구)

  • The propagation of light radiation within tissues is an important problem that confronts the dosimetry of therapeutic laser delivery and the development of diagnostic spectroscopy. In the clinical application of photodynamic therapy(PDT) and in photobiology, the photon deposition within a tissue determines the spatial distribution of photochemical reactions. Scattered light is measured as a function of the distance (r) between the axis of the incident beam and the detection spot. Consequently, knowledge of the photosensitizer(Chlorophyll-a) function that characterizes a phantom is important. To obtain the results of scattering coefficients(${\mu}s$) of a turbid material from diffusion described by experimental approach. It was measured the energy fluency of photon radiation at the position of penetration depth. From fluorescence experimental method obtained the analytical expression for the scattered light as the values of $(I\;/I_o)_{wavelength}$ vs the distance between the center of the incident beam and optical fiber in terms of the condition of "in situ spectroscopy(optically thick)" and real time by fluorometric measurements.

Applicability of bilateral transfer paradigm to the rehabilitation programs for children with hemiplegic cerebral palsy (편마비 아동의 재활프로그램에 대한 양측성 전이 패러다임의 적용가능성)

  • The purpose of this study was to provide experimental evidence and theoretical background for the applicability of bilateral transfer paradigm to the rehabilitation programs for children with hemiplegic cerebral palsy. Children with hemiplegia, which means unilateral motor disabilities, display abnormal motor and postural patterns of the affected side due to hemiparesis, spasticity, and sensory disorders, resulting in a decreased motor abilities of the affected side compared to unaffected side. Accordingly, they tend to rely on the unaffected limb for everyday activities, which further deteriorates the functions of the affected side by causing associated reaction, abnormal postural patterns, and hypertonus. Rehabilitation programs developed for children with hemiplegic cerebral palsy include neurodevelopmental treatment, application of cast or splint to unaffected limb, neuromuscular electrical stimulation, and task oriented model. These programs, however, have several drawbacks, such as discontinuity in treatment effect and psychological hatred to the force use of the affected side. In order to solve these problems and enhance the efficiency of the rehabilitation programs, it is required to maximize the use of the affected side without hatred. Characteristics of the control system, such as temporal coupling and spatial assimilation between limbs and neural crosstalk at different levels of central motor pathway, suggest that the bilateral transfer paradigm may enhance the efficiency of the rehabilitation programs for children with hemiplegic cerebral palsy.

Effects of Audiovisual Biofeedback(RAPAEL Smart Pegboard) on Cognitive Function of Stroke Patients (시청각 바이오피드백 훈련(RAPAEL Smart Pegboard)이 뇌졸중 환자의 인지기능에 미치는 영향)

  • Objective : The purpose of this study is to investigate the effects of audiovisual biofeedback training using RAPAEL Smart Pegboard on the cognitive function of stroke patients. Methods : A total of 20 participants were divided into an intervention group and a control group, with 10 patients per group K-MoCA and NCSE were used for pre and post intervention evaluation. The intervention method of the intervention group provided traditional occupational therapy and audiovisual biofeedback training, and the control group provided traditional occupational therapy and participated in self-activity programs. Results : K-MoCA revealed a significant difference in visuo-spatial and executive functions, attention, memory, and overall score in the internvention group compared to the control group (p<.05). In addition, the results of NCSE showed that there was a significant difference in attention, memory, and overall score in the intervention group compared to the control group (p<.05). Conclusion : We confirmed that the audiovisual biofeedback training had a positive effect on the cognitive function of stroke patients. Therefore, it could be useful tool for the rehabilitation of stroke patients in future occupational therapy clinical environments.

The Variations in Gait Parameters and the Muscle Activities on the Non-Affected Side of the Shoulder Girdle According to Arm Sling Type in Patients with Hemiplegia (팔걸이 형태에 따른 편마비 환자의 보행 변수 및 비마비측 견갑대의 근활성도 변화)

  • The purpose of this study was to investigate the variations in gait parameters in terms of the type of arm sling used in hemiplegic patients. Ten patients with hemiplegia and ten healthy adults participated in this study and walked at self-selected speeds on a GAITRite-instrumented carpet. The activities of the opposite shoulder girdle muscle including the latissimus dorsi, anterior deltoid, and posterior deltoid were simultaneously recorded using surface EMG during gait. They were randomly assigned a condition: without an arm sling, a single strap arm sling, a Harris hemi arm sling, a Rolyan humeral cuff arm sling, and a Bobath roll arm sling. The following gait variables were analyzed: the temporo-spatial parameters of velocity, step length, stride length, swing phase, stance phase, single support, step time and toe in/toe out. The statistical analysis was one-way ANOVA with repeated measures to compare the variation of each variable. In comparison of parameters in each trial in the hemiplegia group, the non-affected side stride length, single support, and toe in/toe out resulted in statistically significantly changes (p<.05). But without an arm sling group did not show any gait parameter differences with arm slings. This study found that several arm slings varied gait patterns in patients with hemiplegia and in healthy adults. In the EMG analysis, the Rolyan humeral cuff arm sling and the Bobath roll arm sling were higher muscle activity for the latissimus dorsi muscle than did the single strap ann sling. Further study should examine the problems that appeared in patients who worn arm slings by focusing on a larger number of subjects and by studying the variety of responses in more detail using an assessment tool that measures variation.

Case Report of Two Stroke Patients with Hemi-spatial Neglect Treated with Traditional Korean Medicine (중풍 환자에게 발생한 편측 무시 한방 치험 2례 보고)

  • Objectives: This study reports on two cases of the clinical application of traditional Korean medicine on stroke patients with hemi-spatial neglect.Methods: We applied several traditional Korean medicine treatments and then evaluated the patients’ symptoms with Albert’s test, the clock-drawing test (CDT), the Catherine Bergego scale (CBS), and the numeric rating scale (NRS).Results: The scores of all of the scales showed improvement. In case 1, the Albert’s test score decreased from 2.5% to 0%, the CDT score increased from 6 to 10, and the CBS score decreased from 16 to 11. In case 2, the Albert’s test score decreased from 5% to 2.5%, the CDT score increased from 3 to 6, and the CBS score decreased from 23 to 16.Conclusion: We found that acupuncture, moxibustion, herbal medicine, and physical therapy appeared to be effective treatments for hemi-spatial neglect in these two stroke patients.

The Effect of 131I Therapy by Taking in Laxatives (방사성요오드 치료 시 완하제 투여 효과)

  • Purpose: Our goals were to evaluate the effect of high dose radioiodine treatment for thyroid cancer by taking in laxatives. Materials and Methods: Twenty patients(M:F=13:7, age $46.3{\pm}8.1\;yrs$) who underwent high dose radioiodine treatment were seperated into Group 1 taking $^{131}I$ 5,500 MBq and Group 2 with the use of laxatives after taking $^{131}I$ 5,500 MBq. The whole body was scanned 16 hours and 40 hours after taking radioactive iodines by using gamma camera, the ROIs were drawn on the gastro-intestinal tract and thigh for calculation of reduction ratio. At particular time during hospitalization, the radioactivity remaining in the body was measured in 1 meter from patient by using survey meter (RadEye-G10, Thermo Fisher Scientific, USA). Schematic presentation of an Origin 8.5.1 software was used for spatial dose rate. Statistical comparison between groups were done using independent samples t-test. P value less than 0.05 was regarded as statistically significant. Results: The reduction ratio in gastro-intestinal 16 hours and 40 hours after taking laxatives is $42.1{\pm}6.3%$ in Group 1 and $72.1{\pm}6.4%$ in Group 2. The spatial dose rate measured when discharging from hospital was $23.8{\pm}6.7{\mu}Sv/h$ in Group 1 and $8.2{\pm}2.4{\mu}Sv/h$ in Group 2. The radioactivity remaining in the body is much decreased at the patient with laxatives(P<0.05). Conclusion: The use in combination with laxatives is helpful for decreasing radioactivity remaining in the body. The radioactive contamination could be decreased at marginal individuals from patients.

Effects of Ultrasound on Cell Proliferation from Chicken Articular Cartilage Explants Culture (초음파가 닭의 관절연골에 미치는 영향)

  • This study was performed to examine the effects of therapeutic doses of ultrasound on cell migration distance and proliferation of biopsies from articular cartilage. Articular cartilage biopsies were isolated from proximal part of the tibial of chicken, and cultured. Cartilage explants were exposed for a single 5 min to ultrasound with $0.1{\sim}1.6\;W/cm^2$ (spatial average-temporal average) at a frequency of 1 MHz. A control group was treated with the ultrasound generator switched off. The cell migration distance and cell proliferation analysis were performed on day 6 after stimulation of ultrasound. The results revealed that ultrasound influenced cell migration distance and cell proliferation in intensity-dependent manner. It was found that ultrasounds at $0.2\;W/cm^2$, $0.4\;W/cm^2$, and $0.8\;W/cm^2$ were significantly increase respectively both cell migration distance and cell proliferation (p<0.05). However, cell migration distance and cell proliferation were not affected by exposure ultrasound at $0.1\;W/cm^2$ and $1.6\;W/cm^2$ compared with control group. These results suggest that low-intensity ultrasounds at $0.2\;W/cm^2$, $0.4\;W/cm^2$, and $0.8\;W/cm^2$ may stimulate cell proliferation of the chondroblasts, and reflect a potential role in cartilage repair.

Effects of a Newly Designed Pelvic Belt Orthosis on Functional Mobility of Adults with Post-Stroke Hemiparesis

  • Purpose : Lower extremity orthoses have been used as conservative methods to recover gait of the stroke patients. The purpose of this study is to examine how newly designed pelvic belt orthosis can improve gait ability and dynamic balance of adults with Hemiparesis after stroke. Methods : 22 patients who had hemiparesis after stroke participated in this study. Two groups were randomly created by assigning 10 subjects to the experimental group and the rest of the 12 subjects to the control group. The control group was treated by conventional physical therapy and occupational therapy. Identical therapy protocols were used to treat the experimental group who were assigned to wear the pelvic belt orthosis during post measurement. This study has a group of independent variables including group, gender, age, height, MAS, lesion side, cause and a group of dependent variables including gait speed, cadence, step length, stride length, and dynamic balance. The GAITRite system was used to measure spatial-temporal gain parameters and the balance system SD to measure dynamic balance. The data was analyzed using R version 3.3.1. Random forest, boosting algorithm, and MANOVA test were conducted to determine the effects of independent variables on dependent variables. Results : This study has a group of independent variables including group, gender, age, height, MAS, lesion side, cause and a group of dependent variables including gait speed, cadence, step length, stride length, and dynamic balance. The independent variable "group" has the most important value, which is approximately 25.42 (%IncMSE) representing a value three times greater than the second important predictor "height." Conclusion : As a result of this research, the hypothesis is validated with conclusion that Pelvic Belt orthosis could be effectively used for improving gait ability and balance of the patients with post-stroke hemiparesis.

Deformable image registration in radiation therapy

  • The number of imaging data sets has significantly increased during radiation treatment after introducing a diverse range of advanced techniques into the field of radiation oncology. As a consequence, there have been many studies proposing meaningful applications of imaging data set use. These applications commonly require a method to align the data sets at a reference. Deformable image registration (DIR) is a process which satisfies this requirement by locally registering image data sets into a reference image set. DIR identifies the spatial correspondence in order to minimize the differences between two or among multiple sets of images. This article describes clinical applications, validation, and algorithms of DIR techniques. Applications of DIR in radiation treatment include dose accumulation, mathematical modeling, automatic segmentation, and functional imaging. Validation methods discussed are based on anatomical landmarks, physical phantoms, digital phantoms, and per application purpose. DIR algorithms are also briefly reviewed with respect to two algorithmic components: similarity index and deformation models.

Design Features and Operating Characteristics of the MC-50 Cyclotron (MC-50 싸이클로트론의 설계 특징과 동작 특성)

  • The MC-50 cyclotron at Korea Canter Center Hospital is now operational for neutron therapy and medical radioisotope production. Design features, mechanical structures and operating characteristics of the MC-50 are described in this paper. Optimum operating condition for this cyclotron has been determined by the repetitive running, and the performances of the internal beam have been investigated through the measurements of intensity and spatial distribution of the internal beam as a function of the radius of the cyclotron. Routinely, the 40 $\mu$A of 50 MeV protons have been obtained at first Faraday cup with a extraction efficiency of 61%.
