소양인체질병증 임상진료지침: 망음병 (Clinical Practice Guideline for Soyangin Disease of Sasang Constitutional Medicine: Yin-Depletion (Mangeum) Symptomatology)

  • Objectives This research was proposed to present Clinical Practice Guideline(CPG) for Soyangin Disease of Sasang Constitutional Medicine(SCM): Yin-Depletion (Mangeum) Symptomatology. The CPG was developed by the national-wide experts committee consisting of SCM professors. Methods The CPG was developed by the national-wide experts committee considering of the society of Sasang Constitutional Medicine. It was performed by search and collection of literature related SCM, opinion of SCM experts and journal search and it was followed by CPG's guideline. Results & Conclusions The CPG of Yin-Depletion (Mangeum) Symptomatology in Soyangin Disease include classification, definition and standard symptoms of each pattern. Yin-Depletion (Mangeum) Symptomatology is classified into severe and critical pattern by severity. The severe pattern of Yin-Depletion (Mangeum) Symptomatology is classified into the pattern of Heat-related diarrhea accompanied by headache (Sinyeol-dutong Mangeum) and the advanced pattern of Heat-related diarrhea accompanied by headache (Sinyeol-dutong Mangeum). The critical pattern of Yin-Depletion (Mangeum) Symptomatology is classified into the pattern of Cold-related diarrhea accompanied by the abdominal pain (Sinhan-bocktong Mangeum) and the advanced pattern of Cold-related diarrhea accompanied by abdominal pain (Sinhan-bocktong Mangeum).

  • Objective The purpose of this study was to examine the correlation between the Cold-damage Six-meridian disease of Qibo (岐伯六經病證) and Sasang Constitutional Symptomatology (四象體質病證), presented in Discourse on the Origin of Eastern Medicine (醫源論) of Longevity and Life Preservation in Eastern Medicine (Donguisusebowon, 東醫壽世保元). Method The process of development from Cold-damage Six-meridian disease of Qibo (岐伯六經病證) to Six Meridians Physical Symptoms and Medicines (六經形證用藥) in the chapter Cold(寒門) of the Treasure Mirror of Eastern Medicine (Donguibogam, 東醫寶鑑) was investigated. And the correlation between Six Meridians Physical Symptoms and Medicines (六經形證用藥) and Sasang Constitutional Symptomatology (四象體質病證) was considered. Results and Conclusions 1. The Cold-damage Six-meridian disease of Qibo (岐伯六經病證) in the chapter Heat Treatise (熱論篇) of Basic Questions (素問) had evolved into Six Meridians Physical Symptoms and Medicines (六經形證用藥) in the chapter Cold(寒門) of the Treasure Mirror of Eastern Medicine (Donguibogam, 東醫寶鑑) through Book for Life Saving (Huorenshu, 活人書), a work of Zhu Gong (朱肱), Six Books on Cold Damage disease (Shanghanliushu, 傷寒六書), a work of Tao Hua (陶華) and Introduction to Medicine (YixueRumen, 醫學入門), a work of Li Chan (李梴). 2. The correlation between the Cold-damage Six-meridian disease of Qibo (岐伯六經病證) and Sasang Constitutional Symptomatology (四象體質病證) can be analyzed and understood through Six Meridians Physical Symptoms and Medicines (六經形證用藥) in the chapter Cold(寒門) of the Treasure Mirror of Eastern Medicine (Donguibogam, 東醫寶鑑). 3. Greater Yang meridian disease of Qibo (岐伯) is related to Soyangin early stage of Lesser-Yang Wind-Injury symptomatology and Soyangin early stage of Chest-Heat symptomatology, Yang Brightness meridian disease and Greater Yin meridian disease to Taeeumin Liver-Heat symptomatology, Lesser Yin meridian disease to Soyangin Chest-Heat symptomatology, Lesser Yang meridian disease to Soyangin early stage of Lesser-Yang Wind-Injury symptomatology and Reverting Yin meridian disease to Soeumin Reverting Yin symptomatology of Greater Yang disease.

Methods First, it was performed that search and collection of literature related SCM such as "Dongeuisusebowon", Textbook of SCM, Clinical Guidebook of SCM and Fundamental research to standardize diagnosis of Sasang Constitutional Medicine. And journal search related to clinical trial or Human complementary medicine of SCM was performed domestic and overseas. Finally, 1 articles were selected and included in CPG for Lessor-Yang Wind-Injury (Soyang-sangpung) Symptomatology of Spleen Cold-based Exterior Cold (Bisuhan-pyohan) disease in Soyangin Disease. Results & Conclusions The CPG of Lessor-Yang Wind-Injury (Soyang-sangpung) Symptomatology in Soyangin Disease include classification, definition and standard symptoms of each pattern. Lessor-Yang Wind-Injury (Soyang-sangpung) Symptomatology is classified into mild and moderate pattern by severity. Lessor-Yang Wind-Injury (Soyang-sangpung) Symptomatology mild pattern is classified into initial pattern and advanced pattern. Lesser-Yang Wind-Injury (Soyang-sangpung) symptomatology moderate pattern is classified into Chest-binding (Gyeolhyoong) pattern and Chest-binding (Gyeolhyoong) advanced pattern.

  • Objectives This research was proposed to present Clinical Practice Guideline(CPG) for Soyangin Disease of Sasang Constitutional Medicine(SCM): Diagnosis and Algorithm. This CPG was developed by the national-wide experts committee consisting of SCM professors. Methods We searched the literature and articles related to Soyangin Symptomatology diagnosis and algorithm. For developing diagnosis and algorithm, we searched the classification, ordinary symptom, present symptom of the Soyangin Symptomatology. Results & Conclusions We classify the Soyangin Symptomatology by 4 steps: Exterior-Interior disease, favorable-unfavorable pattern, mild-moderate-severe-critical pattern (initial-advanced pattern). And at the unfavorable pattern, ordinary symptom is very important. So doctors need to focus on the symptom of unfavorable's ordinary symptom such as temperament inclined symptom, diarrhea, and diurnal body fever.

Methods This CPG was developed by the national-wide experts committee consisting of SCM professors. First, collection and organization of literature related to SCM such as Donguisusebowon, Text book of SCM, Clinical Guidebook of SCM and Fundamental research to standardize diagnosis of Sasang Constitutional Medicine was performed. Secondly, journals related to clinical trial or Human complementary medicine of SCM were searched. Finally, 4 articles were selected and included in CPG for Chest-Heat congested(Hyunggyeok-yeol) Symptomatology of Stomach Heat-based Interior Heat disease in Soyangin disease. Results & Conclusions CPG of Chest-Heat congested(Hyunggyeok-yeol) symptomatology in Soyangin disease includes classification, definition and standard symptoms of each pattern. Chest-Heat congested(Hyunggyeok-yeol) symptomatology is classified into mild and moderate pattern by severity. Chest-Heat(Hyunggyeok-yeol) symptomatology Mild pattern is classified into Chest-Heat congested(Hyunggyeok-yeol) initial pattern and Chest-Heat congested(Hyunggyeok-yeol) advanced pattern. And Chest-Heat congested (Hyunggyeok-yeol) moderate pattern is classified into Clear Yang Failure of Stomach(Weguck-cheongyang Bulsagnseung) pattern (Upper wasting-thirst(Sangso) pattern), Clear Yang Failure of Large Intestine (Daejang-cheongyang Bulsangseung) pattern (Middle wasting-thirst (Jungso) pattern).

  • Objectives This research was carried out to establish the clinical practice guideline(CPG) for Yin-Deficit Diurnal-Heat (Eumheo-oyeol) symptomatology of Soyangin disease. Methods Dongeuisusebowon(sinchuk edition) and several kinds of literatures including journal articles concerning this symptomatology of Soyangin disease were collected and classified. Sasang constitutional specialists' conference was held to make an agreement on the conflicting issues as well. Consensus was drawn as a result of the conference. Results & Conclusions 3 papers were selected as an inclusion and exclusion criteria for the relevant articles to Yin-Deficit Diurnal-Heat (Eumheo-oyeol) symptomatology of Soyangin disease. Yin-Deficit Diurnal-Heat (Eumheo-oyeol) symptomatology consists of two aspects : Yin-Deficit Diurnal-Heat (Eumheo-oyeol) severe pattern and Yin-Deficit Diurnal-Heat (Eumheo-oyeol) critical pattern. In Yin-Deficit Diurnal-Heat (Eumheo-oyeol) severe pattern contains 1 disease, namely, Clear Yang Depletion of Large Intestine (Daejang-cheongyang Moson) pattern (Lower wasting-thirst (Haso) pattern). In Yin-Deficit Diurnal-Heat (Eumheo-oyeol) critical pattern contains 2 diseases, Yin-Deficit Diurnal-Heat (Eumheo-oyeol) pattern and Yin-Deficit Diurnal-Heat (Eumheo-oyeol) advanced pattern. Yin-Deficit Diurnal-Heat (Eumheo-oyeol) symptomatology has several kinds of symptoms like dry mouth, disliking to drink much water, diurnal body fever, coldness on the back and nausea as well as body fever, chest discomfort, constipation or dry stool as a common symptoms of Interior Heat disease. Clear Yang Depletion of Large Intestine (Daejang-cheongyang Moson) pattern (Lower wasting-thirst (Haso) pattern) has above mentioned symptoms and much urine/turbid urine, thin thigh and knee joints and twinge of joint pain over the body. Yin-Deficit Diurnal-Heat (Eumheo-oyeol) pattern has symptoms like indigestion and epigastric discomfort, abdominal pain and vomiting in addition. Yin-Deficit Diurnal-Heat (Eumheo-oyeol) advanced pattern has symptoms like hematemesis as well.

  • 1. Objectives The purpose of this case is to report the effects of Sibimijihwang-tang(十二味地黃湯) which is based on Sasang constitutional Medicine for Yin-Deficit Diurnal-Heat Symptomatology(陰虛午熱證) in Soyangin. 2. Methods We treated a Soyangin patients that had Fever Unknow Origin. We prescribed Sibimijihwang-tang(十二味地黃湯) for her physical symptoms. The improvement of her diseases was evaluated by her fever and other physical symptoms. 3. Results After the Sasang constitutional medication of Sibimijihwang-tang(十二味地黃湯) was given, the patient's fever and physical symptoms were improved. 4. Conclusions This case study show an efficient result of using Sibimijihwang-tang(十二味地黃湯) in the Yin-Deficit Diurnal-Heat Symptomatology(陰虛午熱證) of Soyangin.

  • Objectives This study was aimed to develop the clinical practice guideline for Soyangin symptomatology. It discussed the principle and method of application of clinical practice guideline for Soyangin symptomatology which focuses on symptomatology, not disease. Methods Based on the previous guidelines, we assessed the guidelines by Appraisal of Guidelines for Research and Evaluation (AGREE II). After AGREE II assessment, we chose and revised the clinical practice guideline. Member of writing committee reviewed and examined "Donguisusebowon" and many articles for developing clinical practice guidelines. Draft of clinical practice guideline was reviewed by advisory committee and approved by Society of Sasang Constitutional Medicine. Results & Conclusions By researching and discussing the Soyangin symptomatology, we establish the evaluation criteria for diagnosis including classification, definition and develop diagnostic algorithm and treatment assessing tool.

소양인 동출일속 병증에 관한 연구 (A Study on the Soyangin Pathologies that originate from a Common Root Pathologic Category)

Objectives: This study examines the pathologies that originate from a Common Root Pathologic Category(CRPC; 同出一屬) in the Soyangin that are suggested in the Sasang Constitutional Medicine(SCM). 2. Methods: The changes in concepts and perceptions on the Soyangin pathologies were compared across the different editions of Donguisusebowon, "Donguisusebowon, Gabo edition(東醫壽世保元甲午本) (DGO)" and "Donguisusebowon, Sinchuk edition (東醫壽世保元辛丑本) (DSC)". 3. Results and Conclusions: The Soyangin pathologies originating from a CRPC that are described in Donguisusebowon are actually detailed classifications of the Yin-deficiency Diurnal-heat symptomatology (陰虛午熱證) and the Chest-congestion symptomatology (結胸證). Lee Jema had introduced the concept of "Common Root Pathologic Category(CRPC)" to approach pathologies of similar exterior/interior classification or severity stage (mild/moderate/severe/critical) by combining them in a comprehensive, integrative pathology system. This comprehensive approach, which promotes better understanding of the Soyangin pathology and maximizes the therapeutic efficiency of SCM, constitutes the "Comprehensive Therapeutic Methodology" that is the hallmark of Sasang therapy.

  • 1. Objectives: The purpose of this study is to develop the algorithm, which can help clinicians diagnose Soyangin's symptomatology, based on the indexes for energy and fluid and those for nutrient material. 2. Methods: The items of "Donguisusebowon(東醫壽世保元)" were analysed to figure out the inevitable and sequential indexes of Soyangin's symptomatology diagnosis, in order of exterior-interior pattern differentiation, favorable-unfavorable pattern differentiation, and mild-severe-dangerous-urgent pattern differentiation. 3. Results and Conclusions: 1) 1st step: Soyangin's exterior pattern and interior pattern are differentiated in terms of heat and cold, respectively. Aversion to cold and feces are used to confirm the difference. 2) 2nd step: The existence of diarrhea is used to find out that an exterior pattern is with or without favor, while the indexes of back cold, skinniness of thigh-knee and turbid urine are used to identify an interior pattern with or without favor. 3) 3rd step: The favorably exterior-heat pattern can be either mild or severe by the indexes of stuffiness/rigidity/pain below the heart and digestion, while the unfavorably exterior-heat pattern can be either dangerous or urgent by the ones of cold-heat and specific pain. And, the favorably interior-cold pattern can be either mild or severe mainly by feces and subsidiarily by delirious speech and digestion, while the unfavorably interior-cold pattern can be either mild or severe by afternoon tidal fever and vomiting.
