The Clinical Effects of Non Sugical Spinal Decompression Treatment on HIVD (요추추간판탈출증환자 15예의 척추감압치료 병용에 대한 임상적 고찰)

  • Objectives : These studies are designed to make a survey of the effectiveness of the non surgical decompression treatment. Methods : These studies 15 cases of L-spine herniated lumbar intervertebral disc(HIVD) which was treated with non surgical spinal decompression and other medicine treatment(acupuncture, cupping, interferential current therapy, ultra-sound therapy and hot poultice). Each patient has been treated with spine decompression and other medicine treatment. And degree of improvement has been evaluated by Visual Analogue Scale and the Roland-Morris Disability Questionnaire score for low back pain. Results and Conclusions : Through the results, the medical treatment proved to have valid effect for L-spine HIDV. And further clinical studies comparing non surgical spinal decompression treatment and other medicine treatment is necessary.

  • Hiccups, also known as singultus, are spasmodic involuntary contractions of respiratory muscles that shorten respiration. The characteristic sound is caused by rapid closure of the glottis. In oriental medicine, the mechanism of hiccups is "reversed flow of Ki". Hiccups have been associated with neoplasm, infections, seizures, diabetes. renal failure, alcohol ingestion, various drugs, and ischemic events of the myocardium or central nervous system. We present a case of chronic hiccups after cerebral infarction that was successfully treated using moxibustion therapy.

  • Ok-jin Gong's hunchback dance is considered to be a strange dance in Korea. However, there have been no reports of the effects of music and dance therapy as a pain treatment. The aim of this article is to argue that watching and listening to the hunchback dance of Ok-jin Gong may have significant effects as a form of pain treatment. The creator of this style of dance suspected that dance in general can be an outlet for our inner emotions. Her dance in this case has received critical and complimentary reviews as well. However she has been cited as the main culprit behind the destruction of traditional dance forms in the Korean dance world. Nonetheless, her bewitching stage presence attracted much attention. She satirized the feelings of the under privileged' through music and dance. We tend to feel somehow relative poverty and small in the presence of a rich and successful man. On the other hand, the artist felt that it is the public who would be relatively comforted and satisfied in the presence of the weak and handicapped. A free spirit that makes us forget pain is the very essence of her dancing. Her dance and music sound as if she sublimated her sadness and ugliness, changing it to happiness and beauty. She puts herself in a low position and spreads a sense of freedom and relief to the world. Hence, the author felt that innumerable people have been comforted in these ways by her dance.

The Compensatory Articulation in the Patients with Cleft Palate having Velopharyngeal Insufficiency (구개열로 인한 연인두 폐쇄 부전 환자의 보상조음)

  • Background and Objectives The compensatory articulation not only influences general speech intelligibility, but also prevents precise assessment of the velopharyngeal function. This study was performed to investigate frequently affected phonemes, prevalence and the characteristics of compensatory articulation in the patients with cleft palate having velopharyngeal insufficiency. Material and Method An archival review was taken on 103 cleft palate subjects. Their age ranged from 2.6 to 63 years (mean age of 9.8 years). They were grouped into two : preschool group (n=71) and older patient group (n=32). The prevalence and patterns of compensatory articulation were examined on oral high pressure consonants such as plosives, fricatives and affricates. Results : Compensatory errors were observed in $49.5\%$ of the subjects and were mostly glottal stops with the exception of 4cases who had pharyngeal fricatives in addition to glottal stops. The most frequently substituted phonemes were velar plosives and tense sound. There was no significant difference of prevalence in both groups. However, errors for bilabial and alveolar plosives were more frequently observed in preschool group. Conclusion High prevalence of compensatory articulation observed in both preschool and older age group indicates that their articulation errors tend to remain unless appropriate speech therapy is provided. To improve speech intelligibility of the patients with cleft palate having velopharyngeal insufficiency, it is advisable to address and correct the compensatory articulation errors in their earlier ages.

Wheelchair martial arts practitioners have similar bone strength, sitting balance and self-esteem to healthy individuals

  • Objective: The aim of this study was to compare the radial bone strength, sitting balance ability and global self-esteem of wheelchair martial arts practitioners and healthy control participants. Design: Cross-sectional study. Methods: Nine wheelchair martial art practitioners with physical disabilities and 28 able-bodied healthy individuals participated in the study. The bone strength of the distal radius was assessed using the Sunlight Mini-Omni Ultrasound Bone Sonometer; sitting balance was quantified using the modified functional reach test (with reference to a scale marked on the wall); and the self-administered Rosenberg self-esteem (RSE) scale was used to measure the global self-esteem of the participants. The velocity of the ultrasound wave (speed of sound, m/s) traveling through the outer surface of the radial bone was measured and was then converted into a T-score and a Z-score. These ultrasound T-score and Z-score that represent bone strength; the maximum forward reaching distance in sitting (cm) that represents sitting balance; and the RSE total self-esteem score that indicates global self-esteem were used for analysis. Results: The results revealed that there were no statistically significant between-group differences for radial bone-strength, maximum forward reaching distance, or self-esteem outcomes. Conclusions: The wheelchair martial arts practitioners had similar radial bone strength, sitting balance performance and self-esteem to able-bodied healthy persons. Our results imply that wheelchair martial arts might improve bone strength, postural control and self-esteem in adult wheelchair users. This new sport-wheelchair martial arts-might be an exercise option for people with physical disabilities.

  • The technology of 'Virtual Reality' has placed in advanced tools for human beings' joy and anger together with sorrow and pleasure in our generation. It has recently tried in a variety ways to use as an implication for treatment in the field of Cognitive Psychology. Especially, it widely approaches to human in terms of that a sense of reality in a virtual world through the five senses should reinterpret the meaning of cognition in the real world. Based on this paradigm shift, it allows for new treatment using the technology of virtual reality. A typical example is a field of Therapy in order to overcome panic disorder. It has advantages that in particular development of flexible interaction technologies in a virtual space can lead patients to experience psychological environments rather than physical one. the interaction technology provides environments in which users' five senses can be actively stimulated, it is very useful that information from the experiences in the virtual world allows people to learn through real experiences by renewing potential energies, advantages of Virtual Reality Therapy can be customized treatment by depending on symptoms in patients with panic disorder and are capable of differentiate application for the cure at each stage. It is to treat by leading patients to get accustomed to environments and situations in real world through care process with each symptom and stage. It is helpful that based on A Human-Sensibility Ergonomics, technologies like immersive virtual reality equipment, force-relative feedback and stereophonic sound, and like stimulating the sense of smell make people to induce experiences by stimulating human's five senses. There are many advantages of immersion in virtual world in that the phenomenon such as challenge, interaction, reality, illusion, and cooperation is expanded. As an application for therapy by growing such augmented reality, virtual space and sharing of data through the Internet and also inexpensive its availability have recently expanded the base. There are other benefits of Virtual Reality Therapy offering active interaction environments for cognitive experience which can provide appropriately adjusted environments for patients who are hard to overcome the real situation because of phobia. In addition to that it is safe and economical and patients' confidentiality is assured. Moreover, due to the principles of applying real-time navigation the Virtual Reality Therapy makes modification and supplementation easier and also it can reduce cybersickness because of the supply of Lenticular allowing people to see stereoscopy without eyeglasses, which makes sense of presence clearer. On top of that due to the development of interactive technologies, it is becoming close to sense of reality similar to real world by leading users to navigate by themselves and to operate objects in a virtual space. This paper will therefore examine, although it is of limited, characteristics of application of virtual reality technology based on A Human-Sensibility Ergonomics used for treatment for a disorder. this paper will analyse a range of its application and problems and it will suggest the future possibilities.

Alveolar Fricative Sound Errors by the Type of Morpheme in the Spontaneous Speech of 3- and 4-Year-Old Children (자발화에 나타난 형태소 유형에 따른 3-4세 아동의 치경마찰음 오류)

  • Korean alveolar fricatives are late-developing speech sounds. Most previous research on phonemes used individual words or pseudo words to produce sounds, but word-level phonological analysis does not always reflect a child's practical articulation ability. Also, there has been limited research on articulation development looking at speech production by grammatical morphemes despite its importance in Korean language. Therefore, this research examines the articulation development and phonological patterns of the /s/ phoneme in terms of morphological types produced in children's spontaneous conversational speech. The subjects were twenty-two typically developing 3- and 4-year-old Koreans. All children showed normal levels in three screening tests: hearing, vocabulary, and articulation. Spontaneous conversational samples were recorded at the children's homes. The results are as follows. The error rates decreased with increasing age in all morphological contexts. Also, error percentages within an age group were significantly lower in lexical morphemes than in grammatical morphemes. The stopping of fricative sounds was the main error pattern in all morphological contexts and reduced as age increased. This research shows that articulation performance can differ significantly by morphological contexts. The present study provides data that can be used to identify the difficult context for articulatory evaluation and therapy of alveolar fricative sounds.

The Relationship between Dental Treatment and Temporomandibular Disorder (치과치료와 턱관절장애의 연관성에 관한 연구)

  • The risk of temporomandibular joint disorder (TMD) can be increased during dental treatment due to excessive mouth opening and change of occlusion. The aim of this study is to find the relationship between dental treatment and TMD in the patients who developed TMD after dental treatment. The subjects of this study were 21 patients, who developed TMD after dental treatment and were treated with active TMD therapy in Seoul National University Bundang Hospital from June 2003 to February 2007. The subjects were examined with preceding dental treatment, symptom, diagnosis, treatment method of TMD and prognosis of TMD. The obtained results were as follows. 1. Preceding dental treatments were : Implant treatment, 14 cases ; Tooth extraction, 3 cases and others. 2. TMD symptoms were : pain on TMJ, 12cases : Sound on TMJ, 3cases ; Mouth opening limitation, Headache and others. 3. Diagnoses of TMD were : Synovitis and/or capsulitis, 10 cases ; 8 cases of Internal derangement and others. 4. Most TMD were treated by stabilization splint. 5. Prognoses of TMD were : Improvement, 6cases ; sustained 11 cases. In conclusion, the risk of TMD is increased during implant treatment. Prognoses of TMD after dental treatment were bad. It might be that these patients were non-cooperative and have distrust of dental treatment. Because the overloading on TMJ is possible in dental treatment of patients with underlying TMD, prior explanation and knowledge TMJ treatment are very important in these cases.

  • This report is a analysis of 72 cases of mediastinal tumors which are originated in mediastinum or probably metastasized from other organs, and classified on the basis of histopathological types. And all are experienced in the Depart. Thoracic and cardiovascular Surgery, National Medical Center from 1959 to April 1976. Among these cases, 58 cases were confirmed by histopathologically, and remained 14 cases were considered as mediastinal tumor by clinically and roentgenologically. In this series, dermoid cyst and teratoma was most frequent tumor among histopathologically confirmed cases(27.6%), and thymoma 20.7%, neurogenic tumors 17.2%, carcinoma 19%, lymphoid tumor 8.6% and others was 5.9%. The cases of not verified histopathologically were 14 cases. The main clinical symptoms were dyspnea on exertion or orthopnea (62.55%), productive or irritative cough (59.77%), decreased breathing sound (43. 09%). S.V.C syndromes was seen in 25.02%, and there were no definitive symptom in 4.2% of all cases. The main treatment method was surgical removal and irradiation therapy.

Double-layered reconstruction of the nasal floor in complete cleft deformity of the primary palate using superfluous lip tissue

  • After cleft lip repair, many patients suffer from nasolabial fistulas, asymmetrical nasal floor, or an indistinct nostril sill, as well as intraoral wound dehiscence and subsequent scar contracture of surgical wounds leading to vestibular stenosis. For successful primary nasolabial repair of complete cleft deformity of the primary palate, cleft surgeons need special care in reconstructing the sound nasal floor. Especially when the cleft gap is wide or when any type of nasoalveolar molding therapy was not performed, three-dimensional reconstruction of the nasal floor is critical for a balanced nasal shape. In this study, the author describes an effective method for reconstructing a double-layered nasal floor using two mucosal flaps from both sides of the fissured upper lip. This is a report of six patients with unilateral or bilateral complete cleft of the primary palate with a detailed description of the surgical technique and a literature review.
