염증유발 백서에서 전침자극과 미세전류자극의 항-통각과민 효과 (Anti-hyperalgesic Effects of Electroacupuncture Combination of Microcurrent Stimulation in Rat with Induced Inflammation)

  • Purpose: The purpose of this study were to examine the anti-hyperalgesic effects of combination of electroacupuncture and microcurrent on anti-hyperalgesia in local hyperalgesia zone. Methods: It used 24 rats for experiment, divided them into control group, electroacupuncture group (EA group), microcurrent group (MC group), combination of electroacupuncture with microcurrent (EA+MC group), caused hyperalgesia by injecting ${\lambda}-carrageenan$ into hindpaw. Thickness of hindpaw, mechanical pain threshold (MPT), thermal pain threshold (TPT), noxious flexion withdrawal reflex (NFR) and somatosensory evoked potential (SEP) were measured immediately after induction, at 24 hours, 48 hours and 72 hours after induction. The electrical stimulation was given once a day for three days, 20min per session. Results: Change of thickness, MPT, and TPT showed significant difference in all groups compared to control group. In particular, there were remarkable difference in EA+ME group. In particular there were remarkable differences in EA group and EA+MC group. Change of NFR(% threshold, % reaction time, % RMS) and SEP showed mainly significant differences in EA group and EA+ME group compared to control group. In particular, there were remarkable difference in EA+ME group. Conclusion: The above results suggest that appropriate combination of microcurrent with electroacupuncture for pain control will be very desirable.

뇌졸중 쥐모델에서 비침습적뇌자극치료 이후 신경생리학적 검사에서 나타난 뇌가소성과 연관된 변화 (Plasticity Associated Changes in Neurophysiological Tests Following Non Invasive Brain Stimulation in Stroke Rat Model)

  • Background: Neuromodulation therapy has been used to an adjunctive treatment promoting motor recovery in stroke patients. The objective of the study was to determine the effect of repetitive transcranial magnetic stimulation (rTMS) on neurobehavioral recovery and evoked potentials in rats with middle cerebral artery occlusion. Methods: Seventy Sprague-Daley rats were induced permanent middle cerebral artery occlusion (MCAO) stroke model and successful stroke rats (n=56) assigned to the rTMS (n=28) and sham (n=28) group. The 10 Hz, high frequency rTMS gave on ipsilesional forepaw motor cortex during 2 weeks in rTMS group. The somatosensory evoked potential (SSEP) and motor evoked potential (MEP) were used to evaluate the electrophysiological changes. Behavioral function of the stroke rat was evaluated by the Rota rod and Garcia test. Results: Forty rats ($N_{rTMS}=20;\;N_{sham}=20$) completed all experimental course. The rTMS group showed better performance than sham group in Rota rod test and Garcia test at day 11 (p<0.05) but not day 18 (p>0.05). The amplitude of MEP and SSEP in rTMS group was larger than sham group at day 18 (p<0.05). Conclusions: These data confirm that the high frequency rTMS on ipsilesional cerebral motor cortex can help the early recovery of motor performance in permanent middle cerebral artery stroke model and it may simultaneously associate with changes in neurophysiological activity in brain.

진동자극에 대한 상지 절단자의 전완부 감각 인지 크기와 불쾌감 측정 (The Measurement of the Magnitude of Sensory Perception and Displeasure to the Vibration Stimuli applied on Forearm in upper Limb Amputees)

  • Research involving discomfort or pain related to haptic vibratory stimulation the for prosthesis users of myoelectrical hand is very lacking. Our objective of this study was to evaluate the displeasure and sensitivity of areas in forearm using vibration stimulation system between upper limb amputees and non-amputees. Twenty transradial amputees and forty non-amputees (20 youth, 20 elderly) were involved. We set up custom-made vibration stimulation system including eight actuators (4 medial parts and 4 lateral parts) and GUI-based acquisition system, to investigate changes of residual somatosensory sensibility and displeasure at proximal 25% of forearm. Eight vibration actuators were attached to the circumference of proximal 25% point of forearm at regular intervals. Sensitivity tests were used to stimulate the 120Hz and discomfort experiment was used to 37 ~ 223Hz. The subjective responses were evaluated by 10 point scale. The results showed that both groups were similar in sensitive areas. Response at around of radius was most sensitive than other areas in all subjects. Elderly group do not appear discomfort of vibrotactile; however, youth group and amputee presented discomfort of vibrotactile. Prosthesis with a vibrotactile feedback system should be developed considering the sensitivity. Furthermore, Future studies should investigate the scope of application of that principle.

후천성 탈수초성 다발신경병증에서의 운동유발전위 및 체성감각유발전위 연구 (Motor Evoked Potential and Somatosensory Evoked Potential Studies in Acquired Demyelinating Polyneuropathy)

  • Background and Objectives: The proximal and distal nerve segments are preferentially involved in acquired demyelinating polyneuropathies (ADP). This study was undertaken in order to assess the usefulness of motor evoked potential (MEP) and somatosensory evoked potential (SSEP) in the detection of the proximal nerve lesion in ADP. Methods: MEP, SSEP and conventional NCS were performed in 6 consecutive patients with ADP (3 AIDP, 3 CIDP). MEP was recorded from abductor pollicis brevis and abductor hallucis using magnetic stimulation of the cortex and the cervical/lumbar spinal roots. SSEP were elicited by stimulating the median and posterior tibial nerves. Latency from cortex and cervical/lumbar roots, central motor conduction time (CMCT), EN1-CN2 interpeak latency were measured for comparison. Results: MEP was recorded in 24 limbs (12 upper and 12 lower limbs) and SSEP in 24 limbs (12 median nerve, 12 posterior tibial nerve). F-wave latency was prolonged in 25 motor nerves (25/34, 73.5%). Prolonged CML and PML were found in 41.7% (10/24) and 45.8% (11/24), respectively. Interside difference (ISD) of CMCT was abnormally increased in the upper extremity, 66.7% (4/6 pairs) in case of CML-PML. EN1-CN2 interpeak latency was abnormally prolonged in one median nerve (1/10) and LN1-P1 interpeak latency was normal in all posterior tibial nerves. Conclusions: MEP and SSEP may provide useful information for the proximal nerve and root lesion in ADP. MEP and SSEP is supplemental examination as well as complementary to conventional NCS.

건강한 성인에서 시각적 되먹임 훈련이 감각운동겉질의 뮤-리듬에 미치는 즉각적인 효과 (Immediate Effect on Mu-rhythm of Somatosensory Cortex using Visual Feedback Training in Healthy Adults)

  • PURPOSE: A visual feedback method was proposed to induce brain stimulation in a stroke patient, and among them, there was a treatment using a mirror. On the other hand, mirror therapy focuses only on the functional changes in body movements, and analysis of neurophysiological mechanisms of brain activity is lacking. In addition, studies on evaluating the activity and response generated in specific brain regions during visual feedback training using mirrors are insufficient. METHODS: Fifteen healthy adults (male: 10, female: 5, Years: 23.33 ± 1.23), who were right-handed were recruited. By attaching the C3, Cz, and C4 channels in the sensorimotor cortex using an electroencephalogram, training was performed under the conditions without mirror-based visual feedback (No-condition) and with visual feedback (Tasks-condition). At this time, the immediate activity of the mu-rhythm in response to training was separated and evaluated. RESULTS: The tasks-condition of C3, Cz, and C4 channels activated the relative mu-rhythm rather than the no-condition, and all showed significant differences (p < .05). In addition, in all channels at the start time, the tasks-condition was more active than the no-condition (p < .05). The activity of the cortical response was higher in the tasks-condition than in the no-condition (p < .05). CONCLUSION: The mu-rhythm activity can be evaluated objectively when visual feedback using a mirror is applied to healthy subjects, and a basic analysis protocol is proposed.

중완, 신궐 및 관원 경혈의 뜸 자극과 무 자극 대상군의 16채널 뇌파 변화에 관한 연구 (EEG 16 channel variations between the non-stimulation and the moxibustion stimulated subjects for CV4, CV8, and CV12 acupuncture points)

  • 본 연구에서는 건강한 남성을 대상으로 뜸 자극을 가한 대상군과 자극을 가하지 않은 대상군의 뇌파를 분석하여 어떠한 영향을 미치는지에 대해 실험하였다. 뜸 자극은 뜸 링을 사용하는 간접 뜸 자극 방식을 적용하였고, 뇌파는 국제10-20전극법을 적용하여 16채널 쌍전극 측정법으로 측정하였다. 측정된 데이터는 ${\alpha}$, ${\beta}$, ${\delta}$, ${\Theta}$파의 주파수 스펙트럼을 구하고, 16채널 전체를 평균한 뇌파변화추이와 16채널 각각의 뇌파변화추이에 대해 분석하였다. 그 결과, 중완, 관원, 신궐의 뜸 자극은 체표감각신경의 반응에 따라 두정엽(ch 2, 3, 6, 7)의 뇌파신호가 낮았고, 무 자극 대상군과 뜸 자극 대상군의 큰 차이는 후두엽(ch 4, 8)의 변화로 뜸 자극 시 온화한 온열 ($42{\sim}44^{\circ}C$)자극에서 ${\alpha}$파 증가와 ${\beta}$파 감소, 약간의 뜨거움($45{\sim}48^{\circ}C$)의 반응 시 ${\alpha}$파 감소와 ${\beta}$파 증가의 반응이 있었다. 그리고 자극 후 1시간에서 ${\beta}$, ${\delta}$, ${\theta}$파 감소와 ${\alpha}$파의 증가로 정신생리 향상효과가 있음을 알 수 있었다.

  • Dysfunction of the inferior alveolar nerve may result from trauma, diseases or iatrogenic injury. The development and refinement of an objective method to evaluate this clinical problem is highly desirable and needed, especially concerning for an increasing medico-legal issue. Evoked potential techniques have attracted considerable attention as a means of assessing the function and integrity of nerve pathways. The purpose of this study was to characterize the Sensory Evoked Potentials(SEPs) and Somatosensory Evoked Potentials(SSEPs) elicited by electrical stimulation of mental nerve. SEPs and SSEPs were measured and analyzed statistically before and after needle injury on the inferior alveolar nerve of Sprague-Dawalye rats. Measuring SEPs was more sensitive in evaluation of the recovery of sensory function from inferior alveolar nerve injury then measuring SSEPs but we measured SSEPs in the hope of providing a safe, simple and objective test to check oral and facial sensibility, which is acceptable to the patient. We stimulated mental nerve after needle injury on the inferior alveolar nerve and SEPS on the level of mandibular foramen and SSEPs on the level of cerebral cortex were recorded. Threshold, amplitude, and latency of both of SEPs and SSEPs were analyzed. The results were as follows ; 1. Threshold of SEPs and SSEPs were $184{\pm}14{\mu}A$ and $164{\pm}14{\mu}A$ respectively. 2 SEPs were composed of 2 waves, i.e., N1 N2 in which N1 was conducted by II fibers and N2 was conducted by III fibers. 3. SSEPS were composed of 5 waves, of which N1 and N2 shower statistically significant changes(p<0.01, unpaired t-test). 4. SEPs and SSEPs were observed to be abolished immediately after local anesthesia and recovered 30 minutes later. 5. SEPs were abolished immediately after injury. N1 of SSEPs was abolished immediately and amplitued of N2 was decreased($20.7{\pm}12.2%$) immediately after 23G needle injury, but N3, N4 and N5 did not change significantly. Recovery of waveform delayed 30 minutes in SEPs and 45 minutes in SSEPs. 6. The degree of decrease in amplitude of SEPs and SSEPs, after 30G needle injury was smaller than those with 23G. SEPs recorded on the level of mandibular foramen were though to be reliable and useful in the assessment of the function of the inferior alveolar nerve after injury. Amplitude of SSEPs reflected the function and integrity of nerve and measuring them provided a safe, simple and abjective test to check oral and facial sensibility. These results suggest that measuring SEPs and SSEPs are meaningful methods for objective assessment in the diagnosis of nerve injury. N1 and N2 of SSEPs can be useful parameters for the evaluation of the nerve function following a needle injury.

  • 정상 성인 남녀 36명(남:여=23:13)을 대상으로 LFCN의 SSEP검사를 시행한 결과로서 다음과 같은 결론을 얻을 수 있었다. 1. MP의 진단에 있어서는 $P_0$, $N_1$의 절대잠복기 뿐만 아니라 좌 우측 잠복기의 차이값($DP_0$, $DN_1$을 비교하여 보는 것이 더욱 중요하며 그 차이는 모두 2 msec 이하였다. 2. $P_0N_1$의 진폭만으로 비정상과 정상의 기준을 정할 수는 없으나 좌 우측 평균 진폭의 차이는 1.6배 이하였다. 3. $P_0(N_1)$, $DP_0(DN_1)$, 그리고 A(DA)에 있어 남녀군 간의 차이는 없었다.

복잡한 사고에 의해 유발되는 간질발작 2예 (Two Patients with Epilepsy Induced by Complex Thinking)

  • Reflex epilepsies are distinct but not clearly understood clinical entity. Various cerebral activities induced by simple stimulation including visual, auditory, somatosensory stimulation, as well as diverse functional tasks such as reading, calculation, complex thinking are believed to be seizure-inducing factors. We experienced two patients whose seizures were readily precipitated by complex, strenuous thinking. Both patients was teen-aged boy at the onset of seizure(13, and 15 years of age each) with normal physical and mental growth. Although first seizure was precipitated by watching TV and playing puzzles in each patient, initial diagnosis was idiopathic generalized epilepsy, possibly juvenile myoclonic epilepsy( JME). For the first few years, seizures were infrequent but mostly precipitated by the tasks needs concentration such as playing computer games, decision-making, mathematics, reading, or during the examination. EEG revealed various thinking process including reading hard books, drawing complex figure, complex calculation induced epileptic discharges even if it usually needs certain period of concentration. Phenytoin, valproic acid, clonazepam, vigabatrin, and lamotrigine sometimes abated their seizures but none of these made them seizure-free. Complex reflex epilepsy induced by thinking was proposed to be a separate type of epilepsy or a variant of JME. Age, sex, stereotypic seizure-inducing factors, clinical course, and refractory epilepsies in these patients highly suggested this type of epilepsy as a variant of JME but its refractoriness and unique provocation still needs more speculation.

발바닥의 피부 감각 변화가 보행 중에 족저압 분포에 미치는 영향 (The Effect of Changes in Foot Cutaneous Sensation on Plantar Pressure Distribution during Gait)

  • Purpose: The purpose of this study was to examine the effect of changes in foot cutaneous sensation on plantar pressure distribution during gait. Methods: Sixteen healthy young subjects participated in this experiment. All subjects performed two trials of walking under three somatosensory conditions induced by a normal facilitatory insole that provides increased plantar sensory stimulation, and application of lidocaine cream to the plantar surface of the foot to reduce the sensitivity of the soles. Semmes-Weinstein monofilaments were used for evaluation of reduced plantar sensation. The Pedar system was used for measurement of pressure distribution at the plantar surface of the foot. Results: Pressure in the lateral midfoot area showed an increase with increasing and decreasing sensory inputs. When sensory input was increased, plantar pressure showed a decrease in the forefoot area. When sensory input was decreased, plantar pressure showed an increase in the lateral forefoot area and a decrease in the hallux area. Conclusion: By altering sensory feedback, plantar pressure distribution is changed during gait. Plantar cutaneous afferents play an important role in plantar distribution.
