Solitary fibrous tumor of Pleura - 1 case report - (흉막에 발생한 고립성 섬유종 - 외과적 치험 1례 -)

  • Solitary fibous tumor of pleura is a rare and slow vowing neoplasm. The histogenesis of this tumor is thought as mesenchymal origin by immunohistochemical and ultrastructural bases. This case report is of a 69-year-old female who presented severe dyspnea and orthopnea with long-term history. Preoperative chest X-ray an chest CT revealed huge and hemithorax-occupying heterogenous mass and cardiomegaly. She underwent surgical resection and a 22 X 18 X 10 cm sized visceral pleural based mass was evacuated. Histologic findings of tumor showed patternless proliferation in a fibrocollagenous and well vasculized stroma. Immnohistochemical staining revealed strong positive in CD34, that are compatible with solitary fibrous tumor of pleura. The patient was discharged without any complications and no evidence of recurrence was found after 2 years.

  • The Socialist Republic of Vietnam has become one of the top ten nations with the highest aging rate. The proportion of their aging population increased from 7.2% to 10.95% from 1989 to 2017 and entered into the aging society six years earlier than what had been anticipated in 2011. The main issues in such a society are the problems associated with the elderly living by themselves and their solitary deaths. This study attempts to find a solution which would mitigate the burdens of aging or aged population who are living in a lonely and solitary living condition focusing on the system used for the purpose of managing or monitoring of their daily lives to prevent any undesirable outcomes including solitary deaths. The study also discusses the possibility of introducing the system into Vietnam.

On the Interaction of a Solitary Wave and a Wave-Packet (고립파와 파도패킷의 상호작용)

  • In this paper, numerical experiments are performed to examine the collision between a solitary wave and a wave-packet (dispersive wave) in shallow water. We attempt to introduce the improved Boussinesq equation governing the experiments, which is solved by using a semi-analytical approach, called Pseudo-parameter Iteration method(PIM). Using various numerical experiments, we have observed that the wave-packet (propagating dispersive wave) experiences a phase shift after collision with a solitary wave. This phenomenon may be considered as a nonlinear wave-wave interaction in shallow water.

  • Solitary plasmacytomas are rare and account for 5-10% of all plasma cell disorders. These tumors are categorized as solitary plasmacytomas of bone(osseous) or extramedullary plasmacytomas(non-osseous). About a half of solitary plasmacytomas of bone occur in the spine but rarely in the skull. We report a case of solitary plasmacytoma of the skull presented with a painless palpable left parietal calvarial mass in an otherwise asymptomatic 38- year-old man. Skull radiographs showed a large radiolucent lesion with well defined non-sclerotic margins. Computed tomograph scan demonstrated a markedly enhancing mass extending from the epidural to the subcutaneous space. The patient underwent surgery and tumor was completely excised. Pathological examination showed tumor to be a plasmacytoma synthesizing IgG. Postoperatively, the patient received radiotherapy. There was no evidence of systemic involvement on postoperative laboratory wokups. Our recommended treatment is a complete surgical excision combined with postoperative radiation therapy. The patient should be follwed carefully for more than 10 years because of either local recurrence or possible progression to multiple myeloma.

Differentiation between Glioblastoma and Solitary Metastasis: Morphologic Assessment by Conventional Brain MR Imaging and Diffusion-Weighted Imaging

  • Purpose: Differentiating between glioblastoma and solitary metastasis is very important for the planning of further workup and treatment. We assessed the ability of various morphological parameters using conventional MRI and diffusion-based techniques to distinguish between glioblastomas and solitary metastases in tumoral and peritumoral regions. Materials and Methods: We included 38 patients with solitary brain tumors (21 glioblastomas, 17 solitary metastases). To find out if there were differences in the morphologic parameters of enhancing tumors, we analyzed their shape, margins, and enhancement patterns on postcontrast T1-weighted images. During analyses of peritumoral regions, we assessed the extent of peritumoral non-enhancing lesion on T2- and postcontrast T1-weighted images. We also aimed to detect peritumoral neoplastic cell infiltration by visual assessment of T2-weighted and diffusion-based images, including DWI, ADC maps, and exponential DWI, and evaluated which sequence depicted peritumoral neoplastic cell infiltration most clearly. Results: The shapes, margins, and enhancement patterns of tumors all significantly differentiated glioblastomas from metastases. Glioblastomas had an irregular shape, ill-defined margins, and a heterogeneous enhancement pattern; on the other hand, metastases had an ovoid or round shape, well-defined margins, and homogeneous enhancement. Metastases had significantly more extensive peritumoral T2 high signal intensity than glioblastomas had. In visual assessment of peritumoral neoplastic cell infiltration using T2-weighted and diffusion-based images, all sequences differed significantly between the two groups. Exponential DWI had the highest sensitivity for the diagnosis of both glioblastoma (100%) and metastasis (70.6%). A combination of exponential DWI and ADC maps was optimal for the depiction of peritumoral neoplastic cell infiltration in glioblastoma. Conclusion: In the differentiation of glioblastoma from solitary metastatic lesions, visual morphologic assessment of tumoral and peritumoral regions using conventional MRI and diffusion-based techniques can also offer diagnostic information.

Numerical Study on the Reflection of a Solitary Wave by a Vertical Wall Using the Improved Boussinesq Equation with Stokes Damping (고립파의 수직 벽면 반사와 Stokes 감쇠에 관한 개선된 부시네스크 방정식을 이용한 수치해석 연구)

  • In this paper, we simulate the collision of a solitary wave on a vertical wall in a uniform water channel and investigate the effect of damping on the amplitude attenuation. In order to take into account the damping effect, we introduce the Stokes damping whose dissipation is dependent on the velocity of wave motion on the surface of a thin layer of oil. That is, we use the improved Boussinesq equation with Stokes damping to describe the damped wave motion. Our work mainly focuses on the amplitude attenuation of a propagating solitary wave, which may depend on the Stokes damping together with the initial position and initial amplitude of the wave. We utilize the method of images and a powerful numerical tool (functional iteration method) for solving the improved Boussinesq equation, yielding an effective numerical simulation. This enables us to find the amplitudes of the incident wave and reflected one, whose ratio is a measure of the (wave) amplitude attenuation. Accordingly, we have shown that the reflection of a solitary wave by a vertical wall is dependent on not only the initial amplitude and position of a solitary but the Stokes damping.

A Study on Stable Generation of Tsunami in Hydraulic/Numerical Wave Tank (수리/수치파동수조에서 안정적인 쓰나미 조파를 위한 고찰)

  • This study considered the existing approximation theories of solitary wave for stable generation of it with different waveforms in a hydraulic/numerical wave tank for coping with the tsunami. Based on the approximation theory equations, two methods were proposed to estimate various waveforms of solitary wave. They estimate different waveforms and flow rates by applying waveform distribution factor and virtual depth factor with the original approximate expressions of solitary wave. Newly proposed estimation methods of solitary wave were applied in the wave generation of hydraulic/numerical wave tank. In the result, it was able to estimate the positional information signal of wave generator in the hydraulic wave tank and to find that the signal was very similar to an input signal of existing hydraulic model experiment. The waveform and velocity of solitary wave was applied to the numerical wave tank in order to generate wave, which enabled generate waveform of tsunami that was not reproduced with existing solitary wave approximation theory and found that the result had high conformity with existing experiment result. Therefore, it was able to validate and verify the two proposed estimation methods to generate stable tsunami in the hydraulic/numerical wave tank.

The Prognostic Factors of Solitary Pulmonary Nodule (고립성 폐결절의 예후에 관여하는 인자)

  • The solitary pulmonary nodule is considered as a round or ovoid lesion with sharp, circumscribed borders, surrounded by normal appearing lung parenchyme on all sides, and found on a simple chest X-ray without any particular symptoms or signs. There is a wide spectrum of pathologic conditions in the solitary pulmonary nodules prove to be malignant tumors, either primary or metastatic. Most Benign granulomas and other benign conditions can also be seen as solitary nodules. The resection of solitary malignant nodules results in a surprisingly high 5-year survival rate. On the contrary, most benign nodules do not need to be resected and a period of prolonged observation and nonsurgical management is usually indicated. Therefore, the best approach to the controversial management of solitary pulmonary nodules depends on finding factors affecting the probability of malignancy. In this article, clinical records and chest roentgenographies of 60 patients operated on over the past 8 years at the Department of Thoracic and Cardiovascular Surgery, Seoul National University Hospital were reviewed. There were 15 malignant nodules and 45 benign nodules and the prevalence of malignancy was 25%. The most common pathologic entity was tuberculoma [21 cases]. The mean age was 55.5*9.6 years in the malignant group, 45.8>12.5 years in the benign group and there was a significant statistical difference between the two groups [P < 0.05]. The malignant ratio in each age group increased with advancing age. The average smoking amount was 35.6*12.9 cigarettes per day in malignant smokers, 20.9* 12.0 cigarettes per day in benign smokers, and there was a significant statistical difference between the two groups [p< 0.05]. The malignant ratio also increased with the increasing smoking amount. Comparing the appearance of the nodule on chest films, 6 calcifications and 7 cavitations were found only in benign nodules, not in malignant nodules. Therefore, calcification and cavitation can be considered as preferential findings for benignity. Previous cancer history was also a significant factor deciding the prognosis of the nodule [p< 0.05]. The average diameter on chest X-ray was 3.07*0.82 cm in malignant nodules, 3.25*1.04 cm in benign nodules and there was no significant statistical difference between the two groups [p< 0.05]. The author used Bayes theorem to develop a simple method for combining individual clinical or radiological factors of patients with solitary nodules into an overall estimate of the probability that the nodule is malignant. In conclusion, patient age, smoking amount, appearance of nodule on chest film such as calcification and cavitation, and previous cancer history were found to be strongly associated with malignancy, but size of nodule was not associated with malignancy. Since these prognostic factors have been found retrospectively, prospective controlled studies are needed to determine whether these factors have really prognostic significance.

Wave Control by Submerged Breakwater under the Solitary Wave(Tsunami) Action (고립파(지진해일) 작용하의 수중방파제에 의한 파랑제어)

  • Present study examined the functionality of the solitary wave (tsunami) control of the two-rowed porous submerged breakwater by numerical experiments, using a numerical wave tank which is based on the Navier-Stokes equation to explain fluid fields and uses a Volume of Fluid (VOF) method to capture the free water surface. Solitary wave was generated by the internal wave source installed within the computational zone in the numerical wave tank and its wave transformations by structure were compared with those in the previous study. Comparisons with the precious numerical results showed a good agreement. Based on these results, several tow-dimensional numerical modeling investigations of the water fields, including wave transformations, reflection, transmission and energy flux, by the one- and two-rowed permeable submerged breakwater under solitary waves were performed. Even if, it is a research of the limited scope, in case of two-rowed permeable submerged breakwater with $h_0/h=0.925$ ($h_0$ is height of submerged breakwater and h is water depth), the wave height damping in range of $l/L_{eff}>0.4$($L_{eff}$ is effective distance of solitary wave) can reach nearly 60% of the incident wave height. In addition, it is found that reflection coefficient increases nearly 47% and transmission coefficient decreases nearly 18% than one-rowed one. The numerical results revealed that the tow-rowed submerged breakwater can control the incident solitary wave economically and more efficiently than the one-rowed one.

Effectiveness of a Wave Resonator under Short-period Waves and Solitary Waves (공진장치를 이용한 단주기파랑과 고립파의 제어)

  • The performance evaluation of a conventional Wave Resonator at the entrance of harbors against solitary wave has been performed using 3D numerical wave flume. A wave resonator has been designed for the attenuation of the transmitted wave energy by trapping the short periodic incident waves only. In this study, however, the controlled performance of the wave resonator by its various widths has been numerically investigated for solitary waves. Source distribution method based on the Green function and the 3D one-field Model for immiscible TWO-Phase flows (TWOPM-3D) using 3D numerical wave flume were used for the short-periodic waves and the solitary waves, respectively, and these models were verified through the comparisons with the previous experimental and numerical results by other researchers. It was confirmed that the wave resonator is effective enough to control the solitary waves as well as the periodic waves when it compares with the case of no resonance system. Further, it was found that there is the optimal width of a wave resonator to attenuate the target solitary waves.
