Purpose: Soft tissue chondroma is a rare benign tumor, found mainly on the palm and sole and grows slowly. Typically, mature hyaline cartilage is the dominant pathological feature. There are reports that assert soft tissue chondromas to be a cause of median nerve entrapment syndrome. However, this is the first case report showing soft tissue chondroma to be a cause of simultaneous median and ulnar neuropathy. Methods: A 62 year-old woman presented with chief complaints of numbness and hypoesthesia of her right palm for 4 to 5 years, and a palpable mass on her right palm that had been increasing in size slowly for 3 years. Physical examination revealed a firm, mobile, non-tender and about $3{\times}3\;cm^2$ sized mass in the center of the right palm. Electromyography showed entrapment neuropathy of the median and ulnar nerve. Ultrasonography showed an approximately $5.7\;cm^2$ mass below the flexor tendon of ring finger. Upon surgical excision, a $3{\times}3\;cm^2$ mass attached to the flexor digitorum profundus of ring finger and redness and hypertrophy of both the median and ulnar nerve were discovered. Mass excision was performed gently and the specimen was referred for histopathologic study. Mass excision resulted in median and ulnar nerve release. Results: The pathology report confirmed the mass to be a soft tissue chondroma with mature hyaline cartilage. The patient exhibited post-operative improvement of her symptoms and did not show any complications. Conclusion: This is the first case report showing soft tissue chondroma to be a cause of simultaneous median and ulnar neuropathy.

골격외 연골종은 골이나 골막과의 연결이 없이 초자연골을 주성분으로 하는 연부조직의 연골종양을 이른다. 골격외 연골종은 매우 드물게 발병하며, 대부분 수부나 족부의 관절근처의 연부조직 및 건 활액막 근처에서 흔하게 관찰된다. 본 증례에서는 저자들이 경험한 젊은 여성의 수지에서 발생한 골격외 연골종 1예를 보고하고자 한다.

Soft-tissue chondroma was very rare in incidence and thought to be benign. Recently, we operated upon a 13 year-old female with a chondroma of the middle mediastinum, which was incidentally detected in chest X-ray as mediastinal mass, measured about 10x8x7 cm in size and completely resected via thoracotomy.

관절강내 및 관절주위 연골종은 큰 관절 중 주로 슬관절에 생기는 드문 양성 종양이다. 저자들은 동통을 동반한 만져지는 슬개골하 종괴를 주소로 내원한 15세 여자 환자에서 Hoffa씨 지방층에 발생한 관절강내 및 관절주위 연골종을 경험하였다. 이학적 검사에서 슬개골하 부위에 단단하고 이동성이 있는 종괴가 만져졌으며 자기공명영상에서 이 종괴는 국소 석회화를 동반한 경계가 좋은 연조직 종괴였다.

골격외 연골종은 수부와 족부에 드물게 발생하는 양성 종양으로 방사선학적 및 조직학적으로 연골 육종과 유사하여 감별 진단이 필요하며 국소 절제술로 치료가 가능하다. 저자들은 본원에서 64세 남자의 족저부에 발생한 골격외 연골종을 시험하였기에 문헌고찰과 함께 보고하는 바이다. 본 증례는 제술 후 12개월의 추시결과 재발의 소견은 없었고, 통증없이 정상적인 보행이 가능한 양호한 상태이다.

연골 지방종은 연골모양 화생을 가진 지방종으로 드문 양성 연부조직 종양이며, 아직까지 이 종양에 대한 충분한 역학 조사나 영상의학적 보고가 없다. 1960년부터 2008년까지 골성/연골성 분화를 가진 지방종은 몇몇의 연구에서만 발표되었으며, 그중에 이 종양에 대한 영상학적 소견을 기술한 것은 매우 소수였다. 이에 77세 여성의 쇄골 상부에서 발생한 연골 지방종의 증례를 보고하고자 한다.

주변골과 연결성을 가지지 않는 것을 특징으로 하는 골격계 외의 연골종에는 활액막성 연골종증, 관절 내 및 관절 주위 연골종, 연부 조직성 연골종 등이 있으며, 후자의 두 경우는 매우 드물다. 본 증례는 20세 군인이 심한 훈련을 한 후 발생한 슬관절의 압통, 굴곡시의 통증과 충만감, 운동 제한을 주소로 내원하여 시행한 x-ray와 MRI 검사에서 발견한 종괴에 대한 것으로, 이 종괴에 대해 절제술이 시행되었으며, $5.5{\times}4{\times}3$ cm 크기의 단단한 고립성 종괴였다. 현미경 소견 상 종괴는 소엽상을 이루는 초자 연골이 바깥쪽의 섬유성 피막에 의해 싸여져 있었으며, 이에 관절 주위 연골종으로 진단하였다. 저자들은 슬관절 슬개하 지방체에서 발생한 관절 주위 연골종 1 예를 경험하여 임상적, 방사선학적, 조직학적 소견에 대해 문헌 고찰과 함께 보고하고자 한다.

Benign cartilaginous tumors, known as chondrogenic tumors, show cartilage components in the microscopic diagnosis. We present two clinical cases with cartilaginous tumors of the toes showing distinctive clinical manifestations. Two juvenile patients visited our outpatient clinic due to tumors with toenail deformities. A 10-year-old girl presented with a palpable mass with a nail deformity on the left third toe. The initial pathology report was soft tissue chondroma until complete resection. Another 15-year-old male patient visited the dermatology department with a toenail deformity and underwent a punch biopsy. The pathology report was fibrosis with myxoid degeneration. Excisional biopsies were performed for both patients. In the operative field, we observed exophytic tumors connected to the distal phalangeal bones. The final pathology reports were subungual osteochondroma on both patients. The specimen exhibited mature bone trabeculae with a focal cartilaginous cap. Benign cartilaginous tumors have a slow, progressive course and do not show significant symptoms. However, tumors in subungual areas are accompanied by toenail deformities and they can cause pain. Their clinical characteristics lead to a delayed diagnosis. Surgeons can be confused between soft tissue and chondrogenic tumors. When they conduct physical examinations, these categories should be considered in the differential diagnosis.

골막하에 발생하는 결절종은 매우 드물며, 골막 연골종, 지방종, 건초 거대 세포종, 혈종, 기타 염증 그리고 방골성 골육종 등과 감별이 필요하다. 현재까지 보고된 골막하 결절종의 발생 부위는 경골이 가장 흔하며, 요골, 척골, 대퇴골에서도 발생이 보고되었지만, 비골에 발생한 경우는 1예만 보고되어 있다. 이에 저자들은 30세 여자 환자의 우측 원위 비골 부위에 발생한 골막하 결절종을 경험하였기에 이를 문헌 고찰과 함께 보고하는 바이다.
