Osseous metaplasia of the palate: a case report

  • Osseous metaplasia is defined as the formation of lamellar bone inside soft tissue structures where bone normally does not exist. It results from the transformation of non-osseous connective tissue into mature bone. This condition is rare in the oral and maxillofacial region. We report a case of osseous metaplasia of the maxilla, a rare benign tumor in an uncommon region. A 60-year-old male patient visited our clinic complaining of foreign body sensation and asymptomatic swelling on the right palatal side. However, he did not experience pain and reported no local trauma that he could remember. Intra-oral examination revealed an exophytic lesion on the right palatal portion. On computed tomography, there was a round hard-tissue mass approximately 2 cm in diameter on the right palate area. The mass was biopsied and diagnosed as an osseous metaplasia. We review the clinical, radiographic, and histologic features and common causes of osseous metaplasia and report a rare case of osseous metaplasia of the palate.

  • A submucous cleft palate(SMCP) is characterized by a midline deficiency or lack with/without incorrect positioning of muscular tissues in the soft palate, and by a bony defect in the midline or the center of the hard palate. Velopharyngeal incompetence(VPI) related to this SMCP has been managed by various surgical and prosthetic techniques. Because the individual diagnosis and treatment of SMCP patients was not easy to the speech pathologist and to the maxillofacial reconstructive surgeons, and for the better understanding and for the ideal approaches to the SMCP patients, we reviewed several recent articles about grading system in the SMCP caused by VPI, and summarized in this review article.

  • Epithelial cysts are benign epithelium-lined lesions that contain fluid or semisolid material. Most epithelial cysts in the oral cavity occur in the anterior part of the mouth floor. Cysts arising on the uvula in a cleft palate patient are rare. Intraoral examination in a 14-month-old boy with a complete cleft lip and palate revealed a cystic lesion on the right uvula. The lesion was excised and push-back palatorrhaphy with Z-plasty on the uvula was performed. Histopathological examination diagnosed an epithelial cyst. We report a case of an epithelial cyst of uvula in a patient with a unilateral complete cleft lip and palate.

Use of a buccinator myomucosal flap and bilateral pedicled buccal fat pad transfer in wide palatal fistula repair: a case report

  • Primary palatoplasty for cleft palate places patients at high risk for scarring, altered vascularity, and persistent tension. Palatal fistulas are a challenging complication of primary palatoplasty that typically form around the hard palate-soft palate junction. Repairing palatal fistulas, particularly wide fistulas, is extremely difficult because there are not many choices for closure. However, a few techniques are commonly used to close the remaining fistula after primary palatoplasty. Herein, we report the revision of a palatal fistula using a pedicled buccal fat pad and palatal lengthening with a buccinator myomucosal flap and sphincter pharyngoplasty to treat a patient with a wide palatal fistula. Tension-free closure of the palatal fistula was achieved, as well as velopharyngeal insufficiency (VPI) correction. This surgical method enhanced healing, minimized palatal contracture and shortening, and reduced the risk of infection. The palate healed with mucosalization at 2 weeks, and no complications were noted after 4 years of follow-up. Therefore, these flaps should be considered as an option for closure of large oronasal fistulas and VPI correction in young patients with wide palatal defects and VPI.

PERINEURAL SPREAD IN ADENOID CYSTIC CARCINOMA (신경주위 전파를 수반한 유선낭종암)

  • This is a report of adenoid cystic carcinoma occurred in the palate in 30-year-old patient with a complaint of exophytic mass. The authors diagnosed it as adenoid cystic carcinoma by the clinical examination, radiographic findings and histopathological findings. The obtained results are as follows: 1. In clinical examination, asymptomatic exophytic mass of palate was observed. 2. In radiographic findings, soft tissue mass infiltrated the left maxillary sinus, nasal cavity, infraorbital fossa, hard palate, pterygopalatine fossa and pterygoid plate, and enhanced soft tissue mass was also observed in CT. 3. In histopathological findings, tubular and solid patterns of glandular structures were observed and the infiltration of tumor cells into the nerve fibers was also observed. 4. Two years after radical surgery, radiation therapy and chemotherapy, the perineural spread to orbital area was observed. 5. Much longer follow-up than 5 years is needed for early diagnosis of recurrence and distant metastasis.

CEPHALOMETRIC PREDICTORS OF OBSTRUCTIVE SLEEP APNEA (폐쇄성 수면 무호흡증에 있어 두부 규격 방사선 계측학적 기여 인자)

  • Purpose : This study was intended to perform cephalometric comparison between the patients with and without obstructive sleep apnea (OSA). The factors influencing the OSA in the lateral cephalogram was also investigated. Patient and Method : Twenty four patients who visited Sleep Disorder Clinic in Dongsan Medical Center, Keimyung University and evaluated with polysomnograph(PSG) and cephalogram were included in the study. The patients had apnea-hypopnea episode(AHI) over 10 times per hour was diagnosed as OSA after overnight PSG. To evaluate hard and soft tissue profile, cephalometric radiogram were taken at maximal intercuspation(P1) and mandibular protruding position(P2). The diffefence between the OSA and normal group were evaluated statistically and the stepwise regression analysis was applied to analyse the cephalometric influencing factors to OSA. Result : The OSA Group(n=14) had significantly higher Body Mass Index(BMI) than control group(n=10). Lower facial height(ANSGn) was longer in OSA group. However statistically significant difference was not detected in other anteroposterior craniofacial measurements. The soft palate lenth (PNS-P), hyoid position (MP-Hyoid) had positive correlation between AHI (r=0.496, r=0.413, respectively, p<0.05). However, the measurements of oropharyngeal airway was not different between the two groups. The hypothesis, the antero-posteriorly narrow oropharyngeal airway might aggravate the airway resistance and can give rise to higher AHI, was not accepted in the study. This can be attributed by inclusion of the patients performed uvulopalatopharyngoplasty because of the tonsilar or soft palate hypertrophy in the present study. The results of regression analysis revealed that PNS-P, upper airway width(Nph1), upper facial heght(N-ANS), and lower facial height(ANS-Gn) could influence the degree of AHI (F value < 0.0001, $R^2$ = 0.829). Conclusion : We suggest lateral cephalogram may utilized as a useful method to evaluate OSA. The patient with long soft palate, narrow upper airway width, long upper & lower facial height can be expected to have high risk of OSA. However, it should be emphasized the comphrehensive intraoral inspection including soft palate and tonsilar hypertrophy because lateral cepahlogram cannot visualize oropharyngeal status completely.

Early outcomes of cleft and palatal width following anterior palate repair (vomerine flap) in infants with wide cleft lip and palate

  • Background Anterior palatal repair performed during cleft lip repair using a vomerine flap may assist in recruiting additional soft tissue for subsequent completion of palatoplasty, especially in patients with a wide cleft. We present our early results in the hope of triggering a re-evaluation of this technique regarding its advantages for maxillary growth through further studies of patients with a wide cleft. Methods A retrospective analysis of patients with complete unilateral and bilateral cleft lip and palate was performed, including cleft and palatal measurements taken during initial surgery (lip repair together with anterior palate repair) and upon completion of palatoplasty. Results In total, 14 patients were included in this study, of whom nine (63.3%) had unilateral cleft lip and palate and five (37.5%) had bilateral cleft. All patients had a wide cleft palate. Lip and anterior palate repair was done at a median age of 3 months, while completion of palatoplasty was done at a median age of 10.5 months. Measurements taken upon completion of palatoplasty showed significant cleft width reduction in the mid-palate and intertubercle regions; however, the palatal arch distances at nearby landmarks showed non-significant marginal changes. Conclusions Anterior palate repair using a vomerine flap significantly reduced the remaining cleft width, while the palatal width remained. Further research is warranted to explore the long-term effects of this technique in wide cleft patients in terms of facial growth.

Maxillary Nerve Block for Patient with Soft Palate Cancer Pain - A case report - (연구개암 환자에서의 상악신경차단 - 1예 보고 -)

  • In early stages, cancer is not usually painful. However, many patients with recurrent or metastatic cancer eventually experience pain, which becomes progressively worse. Chemotherapy, sympathetic surgery, physical therapy and nerve block can be used to control cancer pain. A 60-year old patient had severe pain of the soft palate due to squamous cell carcinoma. We successfully treated this patient with maxillary nerve block using pure alcohol by a lateral approach. Four months after maxillay nerve block, the patient is still pain free.

  • Velopharyngeal function refers to the combined activity of the soft palate and pharynx in closing and opening the velopharyngeal port to the required degree. In normal speech, during the production of oral consonant sounds elevation of the soft palate, along with the superior constrictor muscle, occludes the oropharynx from the nasopharynx. Inadequate velopharyngeal function caused by congenital or acquired insufficiency or incompetency may result in abnormal speech characterized by hypernasality, nasal emission and decreased intelligibility of speech due to weak consonant production. The speech aid is often helpful in improving the speech of individuals with velopharyngeal incompetency. In this article, the pathogenesis and treatment of velopharyngeal incompetence are discussed and a speech aid appliance that was constructed for the patient is described.

  • 구개인두성형술이 코골음과 수면무호흡증에 유용한 수술적 방법으로 널리 이용되고 있으나 코골음과 수면무호흡증의 유발기전이 어리기지 복합적인 원인에 의하므로 구개인두성형술의 적응증을 결정하는데 많은 어려움이 있어, 수술전에 이 방법의 적응이 되는 환자를 보다 정확히 선택함이 중요하므로 많은 사람들에 의하여 주된 병변부위를 검시하는 방법들이 시행되어 왔다. 예를 들면 Mueller maneuver 중 내시경검사, Cephalometrics, polysomnogram, Multiple sleep latency test, CT scan 등의 방법이 환자선택에 이용되었다. 저자들은 비교적 쉽고 경비가 들지 않는 술전 검사법으로 양 후구치봉선(retromolar raphe)을 연결하는 선상에서 구인두궁 (soft palate arch)의 길이를 비교하는 방법을 총 89례에 적용 시행하여 그중 추적관찰이 가능한 46례에서 코골음은 86%, 수면무호흡증은 82%에서 자각적 증상의 개선이 있었기에 구개인두성형술을 시행함에 있어 양 후구치봉선거리와 구인두궁 거리를 비교하는 방법이 간단하고 경제적인 술전 진단방법으로 사료되어 문헌고찰과 함께 보고하는 바이다.
