Parosteal Lipoma of the Proximal Radius Causing Posterior Interosseous Nerve Palsy - A Case Report - (근위부 요골에 발생하여 후골간 신경마비를 일으킨 방골성 지방종 - 1례 보고 -)

  • Lipoma is the most common benign soft tissue tumor. But the parosteal lipoma which occurs in deep tissue is very rare. The authors experienced a case of parosteal lipoma causing posterior interosseous nerve palsy around the proximal radius. A 53-year old male patient, who has motor weakness on right wrist and finger extension for 3 weeks visited. He was diagnosed as a parosteal lipoma causing postrior interosseous nerve palsy of the proximal radius. 6 months after the marginal excision, he was recovered from motor weakness.

Microsurgical Reconstruction of the Injured Limb (미세혈관 수술법을 이용한 결손사지의 재건술)

  • From Fabuary 1982 to May 1995, 396 patients had undergone reconstructive surgery of the upper and lower limb with microsurgical technique at department of orthopaedic surgery, Yonsei University of Medicine. The results were as follows; 1. Average age at the time of operation was 23.4years(2-64 years), and there were 277 male and 119 female patients. 2. Among 324 patients of soft tissue flap(87 inguinal flap, 132 scapular flap, 38 latissimus dorsi flap, 11 latissimus dorsi and scapular combind flap, 6 gracilis flap, 12 deltoid flap, 3 tensor facia lata flap, 11 dorsalis pedis flap, 6 lateral thigh flap, 12 wrap around flap, 1 lateral arm flap, 5 musculocutaneous flap), 274 cases(85.5%) were succeed. 3. Among 37 patients of vascularized bone graft(18 fibular bone graft, 11 iliac bone graft, 7 toe to finger transplantation,1 vascular pedicle rib graft), 30 cases(80.1%) were succeed. 4. In 26 cases of segmental resection and rotationplasty at lower extremity, 23 cases were succeed. 5. In 7 cases of Tikhoff-Linberg procedure and in 2 case of segmental resection and replantation, all case was succeed. Overall success rate of microscopic reconstructive surgery was 85.6%. In conclusion, microsurgical technigue is valuable for reconstruction of tissue defect or function loss of the limb.

Treatment of Subungual Glomus Tumor (손톱밑에 발생한 사구종의 치료)

  • Glomus tumor is uncommon tumor with an incidence of about 1% in soft tissue tumors and the single most common location is the subungual region of the finger. Many reports have emphasized that glomus tumors were difficult to diagnose and that the result of surgical removal was good, but nail deformity has received little attention. Most of authors removed the entire nail or partially splitted the nail to exposure the nail bed and tumor. Nobody reported whether nail deformity after operation was present or not. But, an abnormal nail is both a cosmetic and a functional problem in that catches on objects. In this study, our attention was postoperative nail deformity as well as the results of surgical removal. Seven patients(eight cases) who were diagnosed as a subungal glomus tumor in the hand were treated by surgical excision without removal of the nail and followed over twenty two months. And then we report on our experience with 8 cases of subungual glomus tumor which were successfully treated by complete excision without nail deformity.

The feature of Microcapsule Involving Ultraviolet Rays Absorbent

  • A new method was developed to prepare microcapsules involving hydrophobic components. A totally new "silicone-resin-polypeptide" was used as the wall materials. The polypeptide was made by hydrolysis of collagen and silk protein and so on, and that was combined with silicone. This microcapsule was easily prepared from silicone-resin-polypeptide in water solution. The ratio of encapsulation in the microcapsule was not only high level as 90%, which had never been reached, but also the particle size could be controlled to obtain very small size (average particle size: 2${\mu}{\textrm}{m}$). Moreover, these microcapsules were resistant to high shearing forces and were stable over a long time period. This stable microcapsule was not crushed in pressure with finger spreading, so the core materials hardly touch the skin directly. Application in cosmetics by using microcapsule involving UV absorbents (2-ethylhexy1-4-methoxycinnamate (OMC) and 4-tert-butyl-4' -methoxydibenzoyl-methane (BMDBM)) was examined. It was possible to apply organic UV absorbents in water-rich formulations without any surfactant by using this microcapsule. This formulation demonstrated a good moisturizing and soft skin feel. Therefore, the microcapsule was applied to hair care products. As a result, the sunscreen hair lotion with microcapsule was able to prevent from damaging and decoloring of hair color by UV rays. As just, it was suggested that this microcapsules were be widely applied in cosmetics.cosmetics.

Correction of Hook Nail Deformity with Composite Graft (복합조직이식을 이용한 갈고리 손톱 변형 교정의 임상례)

  • Purpose: Hook nail deformity is caused by inadequately supported nail bed due to loss of distal phalanx or lack of soft tissue, resulting in a claw-like nail form. A composite graft from the foot bencath the nail bed gives adequate restoration of tip pulp. Methods: From September of 1999 to March of 2004, six patients were treated for hook nail deformity and monitored for long term follow up. Donor sites were the lateral side of the big toe or instep area of the foot. We examined cosmetic appearance and nail hooking and sensory test. The curved nail was measured by the picture of before and after surgery. Results: In all cases, composite grafts were well taken, and hook nail deformities were corrected. The curved nail of the 4 patients after surgery were improved to average $28.7^{\circ}$ from average $55.2^{\circ}$ before surgery. The static two point discrimination average was 6.5mm and the moving two point discrimination average was 5.8mm in the sensory test. Conclusion: Composite graft taken from foot supports the nail bed with the tissue closely resembling the fingertip tissue, making it possible for anatomical and histological rebuilding of fingertip.

Congenital Hyperostosis in Pig Litters (신생자돈의 선천성 전지과골증의 발생예)

  • Two cases of congenital hyperostosis at a pig farm were observed and examined pathologically in Landrace piglets. Twenty two piglets from 2 litters: 10 alive piglets in a litter and 2 stillborn and 10 alive piglets in another, were born with thickened forelimbs and some of them with thickened hindlimbs too. These piglets stowed symptoms of staggering and incoordination. Only one of these piglets could be reared normally and all the others died within 3 days of age. The thickened parts of their limbs were felt firm and not well pitted by finger pressing. An apparent ossification was identified on the central regions of the cross sections of the thickened limbs, and some pale, gray and ossiferous signs were found in the peripheral soft areas. On histopathological examination of the thickened limbs, it was found that the bone trabeculae were newly proliferated in a concentric form in central regions and in a radiated form in peripheral regions. The peripheral regions of the periosteum and perichondrium of new bone were proliferated with connective tissue and the edematous muscle bundles were atrophic and degenerative. Both the sows which produced piglets of congenital hyperostosis were mated with a different boar at next breeding and farrowed normal piglets. It was concluded that the swine congenital hyperostosis in the present cases was expressed by autosomal recessive gene of first mating boar.

  • 최근 스마트 기기의 활용 요구 증가로 인한 실내 위치 인식 시스템 수요가 급증함에 따라, BLE(Bluetooth Low Energy) 비콘을 이용한 실내 측위 시스템이 각광받고 있다. 본 논문은 BLE 비콘 기반에 중심을 두고 RSSI 신호를 이용하여 거시적인 삼변 측량 기법을 이용하여 산출한다. 그 결과값을 근사치 위치에만 Fingerprinting을 적용하여 위치 측위 기본 연산량을 줄임과 동시에 에너지 효율을 증대시킨다. 또한 선정된 Fingerprinting Cell 주위의 노드만을 이용하여 사용자의 단말 위치의 정밀성을 보장하는 정밀 삼변 측량 연산을 수행하여 에너지 효율을 고려한 BLE 핑거프린팅 기반의 정밀 실내 측위 알고리즘을 제안한다.

A Study on Bone Proportional Measurement of the Forearm (전완부의 골도분촌법에 관한 연구)

  • Objectives : The purpose of this study was to establish an accurate and effective method of locating acupoints of the forearm by studying bone proportional measurement. Methods : A total of 60 volunteers, 30 men and 30 women, participated in this study. 7 parts of the body were measured by a soft ruler or digital vernier calipers. The cun of the upper limb was calculated and analyzed. And the results were compared with the standard cun. Results : The bone proportional cun of the forearm based on height and femur was close to 11. It was different from the standard 12.5, 12 or 10. The cun of the forearm by thumb or finger width measurement was different from the standard too. Conclusions : We suggest that locating acupoints of the forearm needs to be done based on 11 cun. The confusion on locating acupoints of the forearm will be reduced by reflecting the result of the actual measurement.

Development and Structural Design of Textile Touch Sensor Easily Implemented (구현방식이 용이한 텍스타일 터치센서 개발 및 구조적 설계)

  • This study presents and develops a textile type touch sensor structural design that is easy to implement. First, the design of the touch sensor circuit finds the size of the switch with the easiest finger contact and selects a structure with a long circuit with the lowest resistance value. An experiment is performed on a change in an electrostatic capacitance value that accompanies the distance on the electrode and the magnitude of the electrode area of the structure; however, the structure having the distance on the electrode and the large electrode area shows the best resistance change. The laundry assessment was conducted three times at a time and ten times at a time with an average standard deviation less than one ohm, with little change in resistance. Consequently, there were no problems with durability and performance for laundry. Finally, in the bending evaluation, the difference in resistance can be seen between 1-2 ohms and was developed as a smart wearable in the future; in addition, there was no problem as a difference in resistance can be seen between 1 and 2 ohms.

Serious Complications of the Percutaneous A1 Pulley Release: Case Reports and Literature Review

  • Percutaneous first annular pulley (A1 pulley) release, which has been increasingly used to treat trigger fingers, has been widely established as a safe and simple procedure. Multiple studies have reported positive results of percutaneous A1 pulley release. In this study, however, we report cases of patients who developed complications after undergoing percutaneous A1 pulley release at local clinics. A total of six patients visited our hospital for infectious complications after percutaneous A1 pulley release. Various sequelae such as damage to normal structures, insufficient procedure, and tissue necrosis were observed during the exploration. A retrospective study was conducted to identify the cause and trend of the observed complications by instruments (HAKI knife or needle). In the HAKI knife group, there was a tendency for damage to normal structures, while in the needle group, an insufficient release or serious soft tissue necrosis was observed. Based on these cases, our findings confirm the existence and characteristics of infectious complications following the percutaneous A1 pulley release. We further identify that the type of instrument used predicts the nature of complications. Thus, reliable and skilled performance of the procedure by experts is essential for safe treatment.
