A study of the Office for Saving Lives (活人署), a government office in the Joseon, through its history and use of a standing prescription (조선 시대 활인서 연구 - 연혁 및 상비처방을 중심으로)

  • The Office for Saving Lives (活人署) (OSL) was the office in charge of the treatment and relief of the poor in the Joseon Dynasty. This study disputes prior scholarship on the OSL by analyzing the use of a ready-made prescription and by focusing on the personality of the OSL's medical institutions. The work of the three government offices, the Office of Great Mercy (大悲院) (OGM), which was the formal office of OSL, the Office of Benefiting People (惠民署) and the Office of Aiding Life (濟生院), overlapped in the area of relief of the common people. But OGM was different from the other two in that it was not a purely medical office, had no educational function, and did not manage medicine. By analyzing a standing prescription, this article argues: 1) Heojun's influence on the composition of a standing prescription is absolute. 2) Epidemic warm disease (溫疫) was a major social problem in terms of emergency medical care at the time. 3) In the late Joseon Dynasty, the treatment of epidemic warm diseases became more sophisticated than the previous era.

Factors Influencing Antibiotics Prescribing of Primary Health Physicians in Acute Upper Respiratory Infections (급성상기도질환에서 일차의료의사의 항생제 처방에 영향을 주는 요인)

  • Objectives : To explore the factors influencing antibiotics prescription by primary health physicians for acute upper respiratory infections(URI). Methods : We performed a survey of 370 primary health physicians randomly sampled in April, 2003. The questionnaire consisted of a prescription on the scenario of acute bronchitis case, along with opinions and reasons for prescribing antibiotics on URI. Results : We found that 54.7% of the physicians prescribed antibiotics on the example case of acute bronchitis which is known as not needing antibiotics. Female physicians and ENT physicians had a greater tendency to prescribe antibiotics. The factors influencing antibiotics prescription on URI were the belief about the effectiveness of antibiotics, preference for their own experiences rather than clinical guidelines, perception of patients' expectations, and perception of competitive environment. The prescription of antibiotics in the example case was affected by how much they usually prescribe antibiotics (OR=2.400, 95% CI=1.470-3.917) and the physicians who thought that antibiotics were helpful for their income prescribed antibiotics more than others (OR=6.773, 95% CI=1.816-25.254). Conclusion : These findings demonstrated that the false belief on the effectiveness of antibiotics, patient's expectation of medication and fast relief of symptoms, and perception of competitive environment all affected the physicians prescription of antibiotics on URI. It may help to find barriers to accommodate scientific evidence and clinical guidelines among physicians and to specify subgroups for education about appropriate prescription behaviors.

Smart Exercise Prescription of Elderly Users using Visual Path Map (비쥬얼패스맵을 이용한 고령자 대상의 스마트 운동처방)

  • Exercise programs for elderly users are operated by each department and facility, but it is not enough to visualize exercise prescription and effect followed by elderly users physical conditions. The purpose of this study is to suggest exercise prescription for elderly users with a visual path map. A visual path map is to visually present types of users classified according to physical strength conditions, the process of exercise prescription, and effects of exercise. Exercise prescription is divided into four stages: analysis of physical conditions, exercise prescription by the visual path map, smart exercise prescription, and exercise for elderly users. The first stage, analysis of physical conditions is to classify physical conditions by each type by mechanically learning elderly users' physical test values. The second stage, exercise prescription by the visual path map, is to present exercise prescription suitable for elderly users' physical conditions. The third stage, smart exercise prescription, is to offer exercise prescription of the day when exercise is carried out using elderly users' smart phones in consideration of their situations. The fourth stage, exercise for elderly users, is to provide information by their smart phones when they exercise. In conclusion, this study will be able to induce elderly users to do continuous exercise by motivating them.

Trend on the Curtailments of Medical and Drug Expenditure Before and After the Separation between Prescription and Dispensing in General Hospitals (의약분업 전후 일부 종합병원 진료비 및 약제진료비 삭감추이)

  • Fiscal crisis in the medical insurance system has put the pressure upon hospitals by increasing the rate of curtailment, since the implementation of the separation of prescription and dispensing of medicine. The purpose of this study is to analyse the curtailment of mdical and drug expenditure before and after the system of separation between prescribing and dispensing and to suggest the problems about current inspection system. Data were obtained from 13 general hospitals and used for analysis of trends on medical & drug expenditure, and curtailment in 1999-2000 at three months intervals. The results were as follows; The scale of curtailment for drug expenditure has been increased on outpatient and inpatient since 2000. For the curtailed drug cost with outpatient, the ratio of curtailed drug expenditure has been increased in the case of prescription within the hospital. These results suggest that review system in social insurance were over-focused to control the cost and it might to impede the validity of review function in insurance system. Therefore, it' s needed to develope the scientific and reasonable criteria for Inspection and evaluation of durg expenditure.

  • This paper deals with the social system from the point of system thinking consisting the fundamental construct of system dynamics. The Bertalanffy's general system theory, having been criticized because of its ambiguity, and the complex science theory, emerging system theory, are integrated by using the system thinking which is characterized with three concepts, 'feedback thinking', 'dynamic thinking', 'operational thinking'. In the integration, system thinking suggests the dynamic pattern of the social system have not only an equilibrium status but also complex status. The science of complexity gives an implication to system dynamics the important of the uncertainty and complexity if we interpret the social system as an open system. To show more concrete description, I simulate the cooperation model based on the iterated prisoner dilemma. The simulation results show the diverse patterns of cooperation and betrayal. Especially the sensitivity of initial payoff will cause the chaotic strategic landscapes as the game gose on. These results mean that we should not give the hasty prescription to control social system artificially. Because social system retains the self-organizing force in itself.

  • Nowadays, a patient's private medical data which is exposed to the outside world has a severe effect on not only the patient's private life but also his/her social activities and environment. So, it is important to securely protect the patient's private medical data from the illegal manipulation. This paper studies the method to store the electronic prescription information in an IC card. For that, an access control for users, such as a doctor, a nurse, a medical institute member, a pharmacy, a pharmacist, or a patient, is proposed to access the data stored in an IC card. The certificate is issued using the Crypto API of a certificate management model supported by Windows 2000. The public/private key is created by the Cryptographic Service Provider program, and the electronic prescription is signed using the digital signature. The proposed system, therefore, can improve the quality of medical services by securing the safety and integrity of the electronic prescription, stored in an IC card.

  • Recently, mental patients are increasing by secondary gain, that is, purpose for avoiding a military service as the social prejudice about mental disease decreases. In particular, it is unique diagnostic tool to a patient's subjective symptoms complaint and a doctor's special judgment, in occasion of psychiatry problem that an objective diagnostic tool does not exist. In this paper, we provide an objective basis to help in a quick decision-making of discharging from military service using the datamining, that analyzes mental patient's prescription to find a special rule. Therefore, we propose the decision support system of discharging from service using the analysis of mental patient's prescription.

A Prediction of Number of Patients and Risk of Disease in Each Region Based on Pharmaceutical Prescription Data (의약품 처방 데이터 기반의 지역별 예상 환자수 및 위험도 예측)

  • Recently, big data has been growing rapidly due to the development of IT technology. Especially in the medical field, big data is utilized to provide services such as patient-customized medical care, disease management and disease prediction. In Korea, 'National Health Alarm Service' is provided by National Health Insurance Corporation. However, the prediction model has a problem of short-term prediction within 3 days and unreliability of social data used in prediction model. In order to solve these problems, this paper proposes a disease prediction model using medicine prescription data generated from actual patients. This model predicts the total number of patients and the risk of disease in each region and uses the ARIMA model for long-term predictions.

A Comparative Study on the Herb Network of Prescriptions in the Dongui-Bogam Wind Chapter (동의보감 풍문 내 중풍증과 비병증, 역절풍증, 파상풍증 처방의 본초 조합 네트워크 비교)

  • Objectives: This study was carried out to investigate whether a prescription's composition varies according to the disease being caused by wind, which is one of the migratory pathogenic factors. Methods: An initial database and binary matrix of Pungmun in Dongui-Bogam, including its herbs and prescription, was constructed. With this data, a network map about wind stroke, arthralgia, acute arthritis, and tetanus in Dongui-Bogam was constructed. Results: Analysis of the network map about Pungmun in Dongui-Bogam revealed that the complete prescription network has more isolated nodes than does each disease's network map. Conclusions: The composition of prescriptions in Dongui-Bogam Pungmun differ according to the disease being caused by wind.
