전문간호사의 수급 현황과 건강보험 급여화 방안 (Demand-supply of Advanced Practice Nurse (APN) and Alternative Benefit Strategies in the National Health Insurance)

  • Purpose: The purpose of this paper is to analyze the demand and supply of advanced practice nurses and suggest alternative benefit strategies in the Korean national health insurance. Methods: A revised demand & supply model was used to estimate the excess supply of APNs, and policy making process and key actors in the Korean health insurance were considered to develop a political approach to the APN issue. Results: The social demand for APNs is currently estimated to be less than 50% of its supply and the APN education program fell into difficulties in recruits. No reimbursement mechanism for APN's services in the national health insurance has given no economic incentive to hospital managers who have monopsony power in nursing labor market, which has caused the demand shortage of APNs in hospital industry. Payment for APN's services recognized as one of the most significant strategies to booster the social demand for APN's services should be carefully designed and implemented in the national health insurance. In line with this, key actors in health insurance policy decision-making include government, national assembly, labor unions, NGOs, civic groups, medical associations, and academia. Conclusion: The basic researches for APN's activities and cost-effectiveness analysis in clinical settings are required to support the strategies aforementioned. Constructing a policy network among key actors is able to make the payment strategy feasible, which will increase the socal demand for APNs.

농촌지역 사회적기업 활성화 방안 : 전남 보성군 사례를 중심으로 (Revitalization of Social Enterprise in Rural Areas : Focused on the Case of Boseong in Jeonnam)

  • 2007년 사회적기업 육성법이 제정된 이후 2012년 12월 현재 801개 기관이 사회적기업으로 인증되었다. 이 중 도시지역의 사회적기업이 훨씬 많이 활성화되어 있으며, 실제 사회적기업을 필요로 하는 낙후지역 또는 농촌지역에서 사회적기업이 활성화되기 위해서 무엇을 개선해야 하고 어떠한 방안을 적용해야 할 것인가는 실천가와 연구자에 의해 성찰되어야 할 과제이다. 본 연구에서는 보성지역에 소재하고 있는 업체들의 사회적기업 전환가능성 진단과 지역주민의 수요조사를 통해, 농촌지역 사회적기업이 활성화되기 위한 방안을 모색해보고자 하였다. 조사결과 농촌 지역의 사회적 기업이 비활성화되고 있는 이유는 인력난, 자금난, 시장 협소 및 경영 능력 부족 등 농촌 지역의 기업 환경이 도시에 비해 열악한 상황과 연관되어 있다. 농촌형 사회적기업 모델의 충족 조건들은 다음과 같은 사항들을 고려할 필요가 있다. 첫째, 생산과 사회서비스를 통합적으로 제공하며 계절적 특성을 반영한 노동시장 부문의 고려이다. 둘째, 지방자치단체를 중심으로 사회적 기업의 지역 리더 발굴 및 지원 프로그램의 개발이다. 셋째, 협동조합 및 마을기업의 사회적기업 모델 개발이다. 넷째, 대표적인 지역자원을 활용하여 지역의 광범위한 취약계층을 고용하고 지역사회로 이익을 환원하는 대표 사회적기업을 발굴하고 단계적이고 장기적 지원을 통해 지속가능성을 높이는 방안이다.

수요곡선을 이용한 무형의 음주비용 혜택의 추계 (Estimating the Alcoholic Intangible Costs and Benefits by use of Demand Curve)

  • Since the alcohol involves a wide variety of adverse health and social consequences, there is a strong need for improved estimates on economic costs and benefits of alcohol drinking to reduce the social alcohol abuse and to obtain some useful informations for targeting programs. However, there is no research for the intangible costs and benefits so far that are essential to estimate the real overall costs and benefits incurred by alcohol use. Thus this paper finds out and represents a general model for estimating the intangible costs and benefits by use of alcohol demand curve. The calculated monetary values, as a example, of them in 2002 show as big as 7,900 billion won and 4,200 billion won respectively.

직업성 긴장과 근로자 결근 (Effects of Job Strains on Absenteeism from Work)

  • Objectives : The purpose of this study was to assess the relationship between job strains and absenteeism from work. Methods : The study design was cross-sectional, and the study subjects consisted of 1,166 workers who were employed in the small-sized industries. A self administered questionnaire was used to measure the general characteristics, job characteristics(job demand, job control), and social support(coworker support, supervisor support) at work. The Job Content Questionnaire(JCQ) was used to assess job demand(2 items) and decision lattitude(10 items). Social support at work (10 items) was measured using JCQ. Sick absence was collected using self-report and were rechecked by the attendance record of their company. Odds ratios and 95% confidence intervals for the association between job strain and sick absence were estimated. The modifying effect of social support was evaluated by stratification. Logistic regression was used to estimate the relationship between job strain and sick absence. Results : In the bivariate analysis, the variables related to sick absence were age, marital status, occupation, job demand. Four distinctly different kinds of level of job strain were generated by the combination of job demand and job control: low strain group, high strain group, active group, and passive group. The crude odds ratio of high job strain was 1.78(95% CI: 1.26-2.53), and those of active group and passive group were 1.33(95% CI: 1.07-1.66) and 1.13 (95% CI: 0.88-1.47), respectively. The odds ratio of high job strain after adjusting for age and occupation were still significant The odds ratio of high job strain in low social support was 5.96(95% CI: 2.45-14.51), but that in high social support was 0.73(95% CI: 0.26-2.01). Conclusions : Job strain was associated with increased risk of absenteeism from work, and social support at work modified the association between job strain and sick absence.

  • This paper explored an effective scheduling method for the Seoul-Busan High Speed Rail. It is important to decide train frequency influencing on scheduling method. Main factors to decide train frequency are location of station, social economic index, land use of station area and travel demand. In this paper, we focused on travel demand which is critical factor to decide train frequency. And we studied on standardized scheduling method. Simulation method is used to analyze the performance of explored method.

직무요구와 직무자원이 병원행정직 간호사의 소진과 조직몰입에 미치는 영향 (The Effects of Job Demand and Job Resources on Burnout and Work Engagement of Hospital Nurse Administrators)

  • Purpose: This study aims to investigate the degree of job demand, job resources, burnout, and the organizational commitment of administrative nurses based on the job demands-resources model. Further, it seeks to confirm the influencing factors affecting nurses' burnout and organizational commitment. Methods: The participants were 188 administrative nurses working at hospitals (one tertiary hospital and six general hospitals) located in D City. The collected data were analyzed with IBM SPSS Statistics 23.0 using frequency, percentage, mean, standard deviation, t-tests, ANOVA, Pearson's correlation coefficient, and multiple regression analysis. Results: The influential factors of burnout were role conflict (β=.50), job demand (β=.18), job position (β=-.17, team leaders and above), and social support (β=-.15). The regression model had an explanatory power of 59%. The influential factors of organizational commitment were appropriate rewards (β=.59), job position (β=.15, team leader or above), working department (β=.14, referral center and health screening administration department), and social support (β=.18). The regression model had an explanatory power of 59.5%. Conclusion: The results support the job demands-resources model, and interventions should be developed to decrease job demand and provide sufficient job resources.

패션한복시대의 한복 전문교육 현황 연구 (A Study on the Current Status of Hanbok Education in the Era of Fashion Hanbok)

  • This study aims to derive suggestions to help develop Hanbok education in the future by examining its current status in domestic educational institutions in light of rising social demand for Hanbok in the era of fashion Hanbok. We assessed the Hanbok curriculum in educational institutions nationwide, including universities and specialized institutes, and conducted in-depth interviews to understand the operational status of Hanbok-specialized education. We found that the demand for education on production functions is decreasing, while the demand for in-depth education on the history of Korean costume and education integrating Hanbok with design and marketing is increasing. We also found that, in the case of universities, which play an important role in Hanbok research and education, professional research and training are shrinking in graduate schools, whereas Hanbok subjects are being maintained and new subjects are being taught in undergraduate courses. In the fashion Hanbok era, it has been suggested that Hanbok education needs to converge with design and marketing education, beyond teaching only Hanbok. In addition, the necessity of expanding liberal arts education within universities to meet social demand was proposed, and online education was expected to help spread knowledge.

뇌혈관질환자 가족의 부담감과 가정간호 요구도 (A Families's Burden on a Cerebrovascular Accident Patient and the Demand for Nursing Home)

  • Purpose: This is to find out the correlation between a families burden on a cerebrovascular accident patient and the demand for nursing home. Method: The data was established by using structured questionnaires for 102 nursing families of cerebrovascular accident patient at three university hospitals. Result: 1) The total burden was showed to be at 3.21, which was some what higher than average. Total average of demand for nursing home was at 3.30, which was higher than average. 2) This showed static correlations between the economic, social, physical dependent and psychological burden, and demand for nursing home. The result represents that the higher burden showed a significantly positive correlation to the higher demand for nursing home. Conclusion: If a systemic support of nursing home is prepared the differentiated nursing home matched perfectly on each burden, the burden on a family of the patient can be decreased effectively.

빅데이터 분석을 활용한 제5차 산림기본계획 주요 사업에 대한 평가 (Evaluation of Major Projects of the 5th Basic Forest Plan Utilizing Big Data Analysis)

  • 제5차 산림기본계획은 2008년부터 2017년까지의 10년 단위 계획으로, 2013년에 대외환경 변화 등의 이유로 변경되었다. 본 연구에서는 산림기본계획에 대한 거시적 평가를 위해 빅데이터 분석을 활용하여 연도별 정책 수요와 공급의 간극을 파악하였다. 정책수요 부분은 당해 연도의 뉴스, SNS 등에서 언급된 사업과 관련된 키워드(명사 기준)를 기준으로 하고, 정책 공급 부문은 산림청이 발행한 문서를 기준으로 하여 비정형 데이터를 수집하였다. 수집한 자료는 사회연결망 분석(social network analysis)을 활용하여 네트워크 구조를 특정하고, 수요 측면의 네트워크와 공급 측면의 네트워크를 비교하여 산림청 정책 수요와 공급의 간극을 확인한다. 분석 결과, 공급부문의 네트워크 구조는 수요 부문보다 방사형이 약한 것으로 나타나 산림 이외에 다양한 주제어가 네트워크에서 상당한 영향력을 행사하고 있는 것으로 판단되었다. 또한 27대 대표사업들을 대응한 33개 주제어에 대해 수요부문과 공급부문의 기울기를 비교한 결과, 수요는 증가하지만 공급이 감소하는 주제어의 개수는 7개로 지속가능, 산림경영, 산림생물, 산림보호, 산림병해충, 도시숲, 북한이 해당되었다. 이들 주제어는 수요와 공급의 간극이 확인된 만큼, 제6차 기본계획에서는 이들 주제어에 대한 공급 강화가 필요하며, 자유게시판 분석을 통하여 신규 임업인에 대한 정보 제공 및 교육 강화도 포함되어야 할 것으로 나타났다.

사회복지 분야 NCS기반의 교육요구 분석 (Educational Needs Analysis on NCS Based In the Field of Social Welfare)

  • 본 연구는 2014년 개발된 사회복지분야 국가직무능력표준(NCS)에 대한 교육요구분석을 통해 사회복지분야 직무별 직무능력요소의 상대적 중요도와 시급성을 분석하여 이를 기반으로 교육요구조사 및 교육과정 개발 필요성이 높은 직무능력요소를 도출하는 데 목적이 있다. 본 연구를 위해 공무원 583명, 민간종사자 644명 등 총 1,227명의 데이터를 활용하여 SPSS 18.0과 Microsoft Excel 2013을 활용하여 분석을 실시하였다. 분석결과, 공무원의 경우에는 사회복지개발 직무의 지역사회분석, 욕구조사, 지역사회자원구축, 지역사회 네트워크, 사회문제사정, 주민조직화, 복지계획수립의 6개 능력, 공공복지 직무의 복지특이민원관리, 자원발굴민간협력 2개 능력, 사회복지행정 직무의 사회복지조직재무관리, 복지계획수립 2개 능력 등 총 10개 능력이 능력개발 시급성이 높은 것으로 분석되었다. 본 연구 결과를 토대를 시사점을 제시하였다.
