Background: The appropriate management of diabetes mellitus(DM) can help reduce its relapse and economic burden, but the level of management of DM in Korea is reported to be insufficient. This study aims to identify the management level of DM by figuring out the HbA1c examination performance rate of the diabetics and analyzing the variation according to the characteristic of a diabetic. Methods: This study used the Korean National Health Insurance Database which includes E10-14(ICD-10 code) as a primary or secondary disease as of 2006. Study population is 1,892,062 diabetics excluding 393,784 patients with the first attack of DM in 2006, 33,440 diabetics who died in 2006, and 21,299 patients with DM having no record of ambulatory care among the 2,340,585 DM patients in total. Results: The HbA1c examination performance rate of all DM patients in our country is estimated to be 41.5% as of 2006 and shows variation according to the characteristic of individual DM patients. The highest performance odds was shown by the patients who were below 19 of age, insured for health insurance, attended more than 3 ambulatory care providers, made ambulatory care visits more than 10 times annually, attended a specialized general hospital as their main attending medical institution, had a record of hospitalization or had co-morbidity. Conclusion: This study propose that is necessary to make politic preparations for the appropriate management of diabetes at a national level, and particularly, the patients with advanced age, the ones dependent on Medical Aid, and the ones using hospitals or clinics, whose appropriate management seems vulnerable, demand a careful management.
Objectives : This study aimed to develop an evaluation protocol of disability weights using person trade-off, and to test the reliability of the developed protocol in a Korean context. Methods : To develop the valuation protocol, the Global Burden of Disease(GBD) and the Dutch studies were replicated and modified. Sixteen indicator conditions were selected from the Korean version of disease classification, which was based on that of the GBD Study, and the person trade-off method referred to the Dutch method. Results : The disability weights were valued in a two step panel sfudy. The first step was a carefully designed group process by three panels, using person trade-off to establish the disability weights for sixteen selected indicator conditions. The second step consisted of interpolation of the remaining diseases, on a disability scale, by the individual members of three panels. The members of three panels were all medical doctors, with sufficient knowledge of the consequences of a broad variety of diseases. The internal consistency of the Korean disability weights was satisfactory. Considerable agreement existed within each panel and among the panels. Conclusions : It was feasible to use a modified evaluation protocol from those used in GBD and Dutch studies, This would provide a rational basis for an international comparative study of disability weights.

The repository for the disposal of LILW which is generated from nuclear power plants and industries is expected to be completed in 2014. For the disposal of LILW, it is important to secure a disposal facility itself, but it is also very important to establish a reasonable charging system which all shareholders are satisfied with. Korea's disposal fee for LILW is higher than other countries' fee, which is a burden to waste generators as well as the waste management organization. The partial reason for the high disposal fee is put on the high social and construction cost when compared with other countries. However the major reason is put on the excessive borrowing cost that is used for the construction of the LILW disposal facility. In this study, we proposed the way to reduce the excessive borrowing cost for sustainable project managements of LILW disposal by analyzing a cost structure.
This paper investigated the operating environment for the representative of each agency and the facility workers on the basis of analytical result of recognition changes of the operating environment changes under the operating the long-term care insurance. It was described plans to take positive effect on the operating as follows. The first, on the result of regression analysis, the service administrative range takes the biggest effect on the general recognition of executing the long-term care insurance off and on. The affirmative recognition of the service administrative range had the general recognition on the system be positive effect. But the operator of facility asserts that the care manager's professionalism related quality of service be strengthened. The second, on the result of regression analysis, in the financial accounting administrative it is revealed the more positive recognition it is, the more positive effects it has. From the difference verification of an operation size from operation subject, the small operation size and personal facility recognize the long term care insurance positively. On the other side the facilities where the operation size is big recognize the system negatively. The long-term care facility should rearrange a support program newly and the government needs to promote the donation activity, because it is needed to reduce the financial burden of facilities.

The purpose of this study is to examine university student's awareness and utilization plan of Smart Learning of the university education. For this purpose, a needs analysis is performed through a survey of 345 university students. Through the survey, first, students of media devices, including the average daily usage time of four hours at the most common smart devices, the intended use was used SNS, games and entertainment, datebook, and more. Second, the benefits of the smart learning, which students learn to recognize the challenges and reducing the burden most common obstacle was the difficulty in immersion classes. Third, the demand for university education smart learning, the most common opinion about the classroom building. On the basis of the above research findings, the utilization plans are proposed.
The purpose of this study was to identify health care providers' perceptions about the goal achievement and benefit/loss caused by the separation policy of drug prescribing and dispensing after the policy implemented on July 1, 2001. Uslng stratified sampling method based on the administration area, Ku, 315 physicians and pharmacists were sampled from the rosters of physician and pharmacist association in the city of Busan on 2001. There were 122 and 115 responses from physician and pharmacist sample, respectively. 78.3% of physicians and 50.4% of pharmacists evaluated that the goal of the policy was not achieved. Moreover, 75.3% of physicians and 40.7% of pharmacists did not support the policy. Most physicians and pharmacists considered preventing the citizens with drug abuse and misuses as the most important benefit derived from the policy. However, physicians and pharmacists concerned over raising health care cost that could be patients' burden. The most important physicians' benefit derived from the policy was free choice of all possible medicine that might result in effectiveness of medication. In physicians' the most important loss, most physicians worried about that breaking traditional patient and physician relationship might cause physicians' authority in treating diseases to be damaged. Pharmacists considered the most important policy benefit as hiked social status resulted from enforcement of profession due to the policy whereas they considered the most significant loss as expected financial problems of small pharmacies compared to that of large pharmacies or pharmacies adjacent to hospitals. In the current problems of the policy, physician and pharmacists blamed the government for inadequate preparations of the policy implementation. Physicians and pharmacists also considered citizens' mature attitudes toward the policy as a crucial success factor.
Objectives: The purpose of this study was to analyze the medical cost of facial paralysis in payer perspective and to estimate the practice pattern of patient using 2011 Health Insurance Review & Assessment Service-National Patients Sample(HIRA-NPS). Methods: Basic statistical system was used for descriptive analysis of NPS dataset. A table for general information (table20) was extracted by disease code, and social demographic characteristics, distribution of the use among inpatients and outpatients, utilization of each kind of medical care institutions, medical cost were analyzed. Subgroup analysis was conducted for assuming the practice pattern of korean medicine and western medicine. Results: A total of 8,219 people and 64,345 claims data were identified as having facial paralysis. Proportion of outpatient was 95.23%, inpatient 0.84% and patient using both services 3.93%. Mean patient charges was 44,229 won per outpatient, 178,886 won per inpatient and 523,542 won per patient using both services. Utilization of korean medical care institutions was 68.81%(claims), 40.46%(patients), utilization of western medical care institutions was 31.19%(claims), 59.54%(patients). The amount charged by korean medical care institutions was 52.61% and western medical care institutions was 47.39%. Cost per claim was higher than those of the korean treatment and cost per patient of western treatment was lower than those of the korean treatment. Conclusions: The research assessed the medical cost and practice pattern associated with facial paralysis. These findings could be used in health care policy and subsequent studies.
This paper examines the types and functions of politeness in German communication and how politeness strategy can be realized. 'Politeness' is not a grammatical terminology in German, but it can be found in many places in grammar. The criteria for politeness are not only organized according to the rules of the language system, but the boundaries are ambiguous because non-language factors affect communication. Politeness is an important strategic element as well as social value. The polite expression first appears in the grammatical level of invariant with the form of address according to the familiarity between the conversational parties, verb modus, and modal particle. Modal particle with familiarity is considered to be a positive politeness strategy that limits the listener's speech by weakening or avoiding face threatening act. Modal verbs is classified as polite expressions that do not impose a psychological burden by not forcing the listener to make a direct request. The results of this study are therefore expected to suggest a rationale for empirical research on politeness in German communication.
Purpose: The purpose of this study is to discover the nature from the life experience of a person with unilateral acquired blindness and his/her family after losing the eyesight and adapting in the environment and to find the meaning of life and how to solve the problem in psychosocial aspect. Methods: This study uses one of the qualitative research methods which explains how families with the unilateral acquired blind perceive blindness after experiencing it and observes how they signify it. starts with interest in lifestyles of individuals and their families and tries to understand the subjective existences of participants in accessible ways and draw the experiences after becoming one-side blind. It cyclically uses deductive verification process through inductive method and establishing hypothesis using materials. Results: According to the results of this study, unilateral acquired blindness studies, due to shattered life, they did not know what to do. Also, discomfort from struggling in a big tunnel and even will to live were found. trying to go out to the world, seeing the new world, and trying to encourage myself, strong attachment to life was shown to by saying, appeared. Each includes sub-topics such as feeling abandoned after confirmed the blindness, feeling disappointed to doctors, family, and friends, trying to live with hope, struggling in a tunnel with thinking how to live, closing the mind from the world, seeing outside the world in the midst of struggling, trying to forget the past with the will of life, having hope to live with care of family, and trying to keep the rest vision. Conclusion: Firstly, in nursing aspect for their adaptation, programs for disable people and nursing intervention focused on their families should be developed. Secondly, since it can be economic and psychological burden for their families and acquaintances, it is necessary to support the blind so that they can find fitted rehabilitation programs and come back to society. Thirdly, active participation of health care providers may influence social interest the improvement of national welfare policy for the unilateral acquired blind.
The purpose of this study was to examine unmet dental care needs and related factors among adults in Korea. The study included a nationally representative sample of Koreans (Korea National Health and Nutrition Examination Survey 2104). The dependent variable was unmet dental care need and the independent variables were socioeconomic status and oral health status. The chi-square test and logistic regression analysis were performed to identify the associations between explanatory variables and unmet dental care needs. The major causes of unmet dental care needs included economic burden, work life, and academic reasons; in addition, dental treatment was considered to have lower priority. The factors that had statistically significant relationships with unmet dental care needs were sex, age, self-rated oral health status, and difficulty in mastication. The findings of the study, suggest a need for lower dental insurance copayments in keeping with the policies and principles aimed at strengthening the national health insurance system. In addition, groups with limited access to dental services should be identified, and effective health care policies and services should be established for these individuals.
