  • The Ministry of Health and Welfare of Korea has implemented various social security programs to ensure a basic standard of living and raise overall quality of life for all citizens. The Korean social security system provides social insurance, public assistance, and social welfare services. To achieve adequate drug benefits, the Drug Management Department of Health Insurance Review and Assessment Service (HIRA) implement drug management duties including drug listing, upper price limit setting, scope of benefits, and post-factum management. When a manufacturer or an importer wants to apply for National Health Insurance (NHI) coverage of the drug that has obtained safety and efficacy approval, the pharmaceutical benefit assessment committee of HIRA evaluates the drug's clinical efficacy and cost-effectiveness to determine whether or not to include the drug into the benefit package. The benefit standards for a listed drug (ingredient) are set either for the whole permitted range or a part of range with conditions. To increase the coverage rate for new drugs, the listed drugs are regularly reviewed for their value. The status of listed drugs can be adjusted or eliminated from the benefit package if the clinical efficacy turns out to be insignificant. Therefore, through these pharmaceutical management procedures, high-quality drugs are provided at reasonable prices, which save healthcare expenditure by price determination and selective coverage in consideration of economic evaluation.

  • South Korea is not a wasteland of publicly funded health care-instead, it has a good medical social security system known as the national health insurance (NHI). The NHI of Korea has three unique features; (1) low premiums, low insurance fees, and low coverage; (2) obligatory designation of medical institutions; (3) and allowance of non-benefit services. These features have made hospitals and doctors interested in profit-seeking. However, the commercialization of medical institutions has taken place in both private- and public-established sectors. A basic problem of commercialization is the co-existence of the obligatory designation of medical institutions and non-benefit services. The problem became worse in the Kim Dae-Jung government because it officially permitted non-benefit services. Since 2000, the Korean government has consistently pursued benefit extension policies, but the coverage rates of the NHI have stagnated. In addition, premiums and current medical expenses have markedly increased because policy-makers have emphasized accessibility to the NHI, while ignoring important principles of medical social security such as a needs-based approach and patient-referral system. In order to resolve the commercialization problem, the obligatory designation of medical institutions to the NHI should be changed to a contract system, and non-benefit services should be prohibited at NHI institutions. We must re-establish the patient-referral system via a needs-based approach. We also need to build a primary healthcare system and public health policies. We should make a long-term plan for healthcare reform.

The data were analyzed by descriptive statistics, t-test, Kruskall-Wallis test, Pearson's correlation coefficient. and path analysis. Results: 1. The quality of life and the level of health promoting lifestyle were moderate or more, but were not significantly different by sex, age, marital status, educational level. occupation, and monthly household income. 2. Self-efficacy was significantly correlated to be a perceived benefit, social support and health-promoting lifestyle. Perceived benefit had a significant correlation with social support and health promoting lifestyle. • Child care cash benefit policy in Korea started in 2009 limited to the those under 2 years old in the poverty group entitled to National Basic Living Security Act and the near poor group (100,000 won a month). However, in 2013 the coverage has been expanded to everyone under 5 (200,000 won for those between babies in their first year, 150,000 won for the second year and 100,000 won for those between 2 to 5 years old) regardless of the household income level. The policy change between year 2012 and year 2013 requires a rapid increase in child care budget - 760% increase. This paper examines this exceptional expansion in child care cash benefit using policy network analysis. We found that local election as well as general election immediately effect the interaction between policy actors and the types of networks. This suggest that policy actors recognize child care cash benefit to be more directly related to the election result compared with child care services. Also conflictive interaction between the parties and government bodies with budget restraint also facilitated the diversification on the child care cash benefit discussion. The policy making process of child care cash benefit was led to policy adoption immediately after the presidential election suggesting that policy formation process and the policy adoption had an close relationship in the Korean child care cash benefit policy process.
