The Efficacy and Safety of Tetrachlorodecaoxide in Comparison with Super-oxidised Solution in Wound Healing

  • Background Some of the relatively newer, more efficacious, and potent topical wound dressing solutions include tetrachlorodecaoxide and super-oxidised solution. This study compares the efficacy and safety of these two drugs. Methods This is a block-randomised, double blind, parallel-arm, post-marketing study. One hundred fifty patients with ulcers (75 blocks uniform for sex, ulcer aetiology, diabetes mellitus, and wound area score) were randomised into the two treatment arms. Patients were observed for eight weeks with weekly assessments. One hundred and twenty patients completed the study. Wound healing was objectively assessed by measurement of wound area, scoring of wound exudation and tissue type, and using the pressure ulcer scale of healing Tool (validated for multiple wound aetiologies). Subjective improvement in pain was noted using a visual analogue scale. Both groups were compared using Mann-Whitney U test on all indicators. Results Difference in change in wound tissue type in the two groups was significant (${\alpha}$=0.05) by intention-to-treat (ITT) and per-protocol (PP) analysis at the end of week two (ITT and PP, P<0.001) and week four (ITT, P=0.010; PP, P=0.009). P-values for other comparisons were not significant (P>0.05). No study-related adverse events were observed. Conclusions Both drugs are efficacious. Tetrachlorodecaoxide yields healthy granulation tissue earlier. Both drugs appear to be safe for application.

A Study on Relation between the Quality of Life and Factors for recurrent Stress of Alcohol Dependents (일부 알코올 의존자의 삶의 질 및 재발 스트레스 요인과의 관련성 연구)

  • This study intended to provide fundamental data to take countermeasures against recurrent stress by analyzing the health-related quality of life and factors for recurrent stress of alcohol dependents. Structured questionnaire was prepared for patients who quitted drinking after they had been hospitalized for alcohol dependence to take medical treatment but suffered recurrence, and analysis on health-related quality of life and environmental factors with drinking risk was conducted. The quality of life was at a relatively satisfactory level, where pain was found at the highest level and general health condition was found at the lowest level. Environmental factors with drinking risk were found to be a moderate level, drinking for the reason of family and friends in terms of complications with other persons was found to be highest, and then job, self control, bad emotions, and social pressure on drinking were found in order. For health-related quality of life, the quality of life was found to be high in case of the only son, professional job, well-educated persons, and no history of hospitalization. Regarding factors for recurrent stress, stress was found to be high in case of the eldest son and being without religion, and the lower was the quality of life, the higher were recurrent stress and environmental factors with drinking risk. It was found that demographic factors including age, marriage, and job influenced the recurrence of alcohol dependence, and factors for recurrent stress, etc. also influenced recurrent alcohol dependence with drinking risks. Specially, the lower was the quality of life, the higher were factors for recurrent stress, and drinking risks, therefore measures to improve the quality of life are required to be taken to prevent alcohol dependence from recurrence.

A Study on the Correlation between Self-efficacy and Self-care in Hemodialysis Patients (혈액투석환자의 자기효능감과 자가간호에 관한 연구)

  • The purpose of this study was to examine the correlation between self-efficacy and self-care of hemodialysis patients. The subjects consisted of 140 hemodialysis patients who underwent hemodialysis at 2 university hospitals. The data were collected by questionnaires and chart review. The questinnaire used the self-efficacy tool developed by Kim Ju Hyune(1995) and the self-care tool developed by literature review and indepth open openended questions to 10 patients. Also, the questionnaire included physiologic data which collected through review of the patients' charts. The statistical analysis was used the SPSS program for frequency, mean, t-test, ANOVA and Pearson correlation. The results were as follows : 1. The mean score for general self-efficacy of hemodialysis patients was 3.103(1-4point) and there were significant differences according to percieved health. The mean score for specific self-efficacy of hemodialysis patients was 3.113 (1-4point) and there were significant differencies according to percieved health, side effects and complications which related hemodialysis. 2. The mean score for self-care of hemodialysis patients was 3.822(1-5point) and there were significant differencies according to marital status and economic level. 3. The relationship between general self-efficacy and self-care was a positive correlation(P=.000). The relationship between specific self-efficacy and self-care was a positive correlation(P=.000). In conclusion, this study revealed the level of self-efficacy and self-care, and the positive correlation between self-efficacy and self-care in hemodialysis patients. Therefore, intervention is needed to promote self-efficacy for self-care of hemodialysis patients. Considering the vulnerable self-care area same as checking blood pressure and weight, fluid restriction, social adjustment, exercise and rest, luther studies should develop self-efficacy promoting programs for self-care of hemodialysis patients.

An Analysis of Management Methods for Traditional Village in Japan -The Case of Tumago Village and Shiragawa Village- (일본 전통마을의 유지.관리방법 분석 -쯔마고마을과 시라카와마을을 사례로-)

  • The traditional village, which is subject for this paper, can be defined as a community having unique combinations of natural, cultural, and social characteristics of that nation, which reflects settlement environments during hundreds of years. Now, in spite of tis potential power of traditional village, national strategies do not find satifsactory directions in Korea. In terms of this concenrs, this paper tries to analyze and diagnose about successful precedents(Tsumage village and Shiragawa village, Japan) with focus on the village management. And this paper aims to explore concrete management systems of Japanese cases, to find clues for practical application, and to suggest several instructive concepts in the light of management system of traditional village management. The analysis is progressed in three viewpoints(village space, village attraction, and village community). And it is extracted that diverse management systems are necessary to secure sustainable traditional village and their way of life, particularly in the face of the pressure of tourism. As a result of exploring of Japanese two cases, representative characteristics, which are found, are as follow; Fist, objective of management is not tourism development but maintenance of sustainable life system. Second, management systems are having not compulsory but spontaneous open decision making process, and in particular, village management is mainly operated by the local government and semipublic institute(inhabitants-oriented0 which have the strongest influential power in village composition units. Finally, village management programs are mostly experience-type and are composed of public law and regulations, diverse operation program, and individual efforts of inhabitants for village management. Because this paper mainly focused on two precedents, there should be more diverse cases. however, extracted conclusions have practical meanings for village management and can be used in re-establishing management concepts of Korean traditional villages.

Development of Seawater Intrusion Vulnerability Index Using AHP (계층화 분석기법을 이용한 해수침투 취약성지수 개발)

  • Sea level rise due to global warming causes seawater intrusion into aquifers in coastal areas. Seawater intrusion vulnerability index was developed using PSR (Pressure, State, Response) model and analysis hierarchy process (AHP). Coastal regions in Korea, Gangwon-do Sokcho-si, Incheon-si Ganghwa-gun, Chungcheongnam-do Taean-gun, Jeollanam-do Yeosu-si, Jindo-gun were chosen and 14 indicators were selected by considering the humanities, economic, social, environmental aspects. Re-scaling method was used for the standardization of indices and questionnaire survey was performed to calculate weight values for each index. The results showed that Yeosu-si was selected as the most vulnerable region to seawater intrusion. The seawater intrusion index developed in this research can be used to analyze the vulnerable regions to seawater intrusion and to establish a policy to minimize the seawater intrusion problems in coastal regions.

The Spatial and Social Characteristics of the Farmland Reduction Area in Urban Vicinity - Focusing on Gimhae City in Gyeongsangnamdo - (도시주변 농지감소지역의 공간 및 사회적 특성 - 경상남도 김해시를 중심으로 -)

  • This study was conducted for the purpose of deriving implications by observing the changing patterns and characteristics of the farmland reduction area in urban vicinity with Gimhae city, Gyeongsangnam-do as the subject. In order to achieve this goal, we first examined the problems and possibilities of farmland reduction area in urban vicinity through a theoretical review. Additionally, the characteristics of land use and community were examined for Gimhae city, Gyeongsangnam-do. The results of the study are summarized as follows. First, for 35 years from 1981 to 2015, Gimhae decreased $50.52km^2$ of farmland, which is about 17.4 times that of Yoido, and about 69.4% of the decreased farmland area. Second, the decrease in agricultural land has been expanding to the whole of Gimhae City from 1990 to 2010, and has been continuing since 2010 around dong-area. Third, in the farmland reduction area in urban vicinity, the number of settlements increases rapidly, but the aging population also increases. Fourth, the composition of the community is getting complicated with the change of the members. Taken together, it is necessary to manage the area efficiently because rapid change is present in the farmland reduction area in urban vicinity. Based on this, the implications are summarized as follows. First, there is a change in land use due to the reduction of farmland not designated as agricultural development region. Therefore, it is necessary to supplement the farmland-conversion standard. Second, despite the fact that land use management has been carried out, there have been problems such as uncontrolled development due to the development pressure beyond institutional management, and therefore it is necessary to improve the structural defects of the pertinent legal system. Fourth, while the traditional farming activities are decreasing with the decrease of agricultural land area, the increase in farms with secondary jobs and the urban-rural interchanges organization's efforts can lead to increased visits from outsiders that seek rural tourism and experiential learning.

Correlation among triceps surae muscle structure, balance, and gait in persons with stroke

  • Objective: This study aimed to compare muscle structure, balance, and gait parameters between healthy adults and persons with stroke and to analyze the correlation among these variables. Design: Cross-sectional study. Methods: Twenty persons with stroke (11 male, 9 female) and twenty healthy participants (9 male, 11 female) were included. Ultrasound images of the triceps surae and the tibialis anterior were acquired in sitting resting, sitting co-contraction, and standing resting positions and also during the functional reach test (FRT) and single leg anterior reaching test (SLART). Muscle thickness and fascicle length were measured. Spatiotemporal parameters of gait were measured using a pressure walkway. Gait speed, cadence, step length, stride length, stance time, and swing time were measured. Results: Changes in percent fascicle length were significantly greater in the gastrocnemius and soleus (SOL) muscles of healthy adults in the sitting co-contraction position (p<0.05). The percent fascicle length of the SOL in FRT and SLART were significantly greater in healthy adults (p<0.05). The mid-stance phase of stroke patients was shorter than healthy adults (p<0.05). A negative correlation was observed between percent fascicle length of the SOL in the sitting co-contraction position and the proportion of the mid-stance phase (p<0.05). Conclusions: The function of the triceps surae is affected in persons with stroke when compared with healthy adults. This can lead to difficulty in performing tasks that involve forward transfer of weight. If the triceps surae is not sufficiently secured, the possibility of compensation in the stance phase increases during gait.

Dental hygienists work-related musculoskeletal symptoms and factor analysis (치과위생사의 직무관련 근골격계 자각증상과 요인분석)

  • Objectives : This research was conducted to find the subjective symptoms of musculoskeletal system for main jobs and job posture. Methods : A survey was conducted from September 15, 2011 to October 7, 2011 and 357 dental hygienists were responded. The survey was consisted of the working environment and medical equipments, psycho-social characteristics, health habits and education, job posture, subjective symptoms of musculoskeletal system pain, and so on. Results : In the survey on the frequency of job posture of dental hygienists by jobs, said they cast down or tilt their head by 15 degrees or more or twist or bend their back. The result of the analysis on the subjective symptoms of musculoskeletal system by main job showed that they experienced a pain in neck, shoulder, and back, although the order differed. The order of subjective symptoms of musculoskeletal system for casting down or turning head by 15 degrees or more, twisting or bending back, having shoulders not parallel to the ground, sitting at the end of a chair, and supporting the whole body with one leg or pressing on a pedal for treatment was from the shoulders to the neck to the back. The order of subjective symptoms of musculoskeletal system for having arms too far from the body or hunching for treatment and bending wrists inward or outward for treatment was from the shoulders to the back to the neck. Conclusions : Due to the characteristics of the job which requires one to have a proper view of a patient's oral cavity, most of the postures included casting down of head or bending of back. Therefore a development of new postures is needed to replace the existing job postures which pressure the musculoskeletal system. And shift rotation can be used as a preventive measure.

  • This study examined the factors affecting submarine crews' turnover intention. This study reviewed previous research-related turnover in military organizations. The measurement was designed based on the Kano model, which was originally used to survey the customers' satisfaction with a service or a product. The authors conducted a questionnaire survey of 408 submarine crew, from 1, Nov to 30, Nov, 2013. The results showed that the factors that impact the submarine crews' turnover were the sense of insecurity due to the a poor working environment of the submarine, physical risks due to the water pressure, anxiety due to the closed space, and insecure rank-limited promotion system. The paper discusses the implications of the results from the perspective of military social welfare and provides recommendations for setting up the policy for submarine crews' welfare based on the study result.

A Study on Factors Influencing User's Satisfaction of OTT Service (OTT 서비스의 이용만족도에 영향을 미치는 요인에 관한 연구)

  • In multi-media, multi-channel era, video viewing using various digital devices via the Internet, not the existing broadcasting network, is emerging as a new broadcasting viewing behavior. OTT service is regarded as the main service of this change of viewing behavior. It is a video service that can seamlessly use any content desired by the user at any time, at any time, any device, any contents, if the Internet connection is available. The purpose of this study is to investigate the factors affecting the satisfaction of OTT service by applying the technology acceptance model (TAM). As a result of analyzing through 303 questionnaires of the early users of OTT service, social pressure, perceived popularity, perceived cost, user reputation, individual innovation, and esthetics set as external factors in this study are partially affects perceived usefulness, perceived ease of use, and perceived enjoyment. In addition, perceived usefulness, ease of use, and enjoyment are directly influencing satisfaction. Based on these results, we discuss the theoretical and practical implications and propose future research direction.
