An Application Effect of Rhythmic Movement Program for the Health Promotion in the Elderly (노인의 건강증진을 위한 율동적 운동프로그램의 적용효과)

  • Every year the number of the elderly increases in Korea thanks to the improvemen of social and economical levels and the development of medicine. However, many problems such as insufficent care and the isolation of the elderly have been commonplace. This trend exists not only because of increased lifespan but also the changing social structure of the nuclear family. Accordingly, inspite of the development of medicine, geriatric diseases including circulatory diseases are increasing in proportion of elderly population, as well as the severity. Therefore, it is important to emphasize that health care programs provide the best possible health care and functional capacities in terms of healthy elderly lifestyles. Especially, the phenomena of aging and geriatric diseases taking place with the elderly naturally are affected by lifestyle and the drastic changes in exercise patterns. This study aims to improve geriatric health by introducing a rhythmic movement program for the elderly to estabilish a health-promoting self-care system and by developing quality of life, perceived health status, their physical and physiological functions and emotional state. The theoretical framework used in this dissertation is derived from the Health-Promoting Self-Care System Model (Simmons, 1990), which integrates the Self-Care Deficit Nursing Theory (Orem, 1985), the interaction model of Client Health Behavior (Cox, 1982) and the Health Promotion Model (Pender, 1987). As a quasi-experimental design, the nonequivalent control group pretest-posttest design is utilized for this study. The subjects of this study consist of 64 people, over 65 years old who live in 2 nursing homes for the aged located in S city , Kyong-gi province and volunteered for this study from July, 12, 1999 to September, 17, 1999. They are divided into two groups:33 in the experimental group and 31 in the control group. The experimental group particpated in the Rhythmic Movement Program at the nursing home, which was comprised of 45 minutes a session, 5 sessions a week during 9 weeks. In order to measure the results of the Rhythmic Movement Program, aspects of perceived health status, balance, flexibility, grip strength, leg strength, heart rate, blood pressure, depression, anxiety and the quality of life were measured before and after participating in the Rhythimic Movement Program for the experimental group after 9 weeks, as well as the control group. The collected data were processed by SPSS PC+ and analyzed by the X2 test, t-test, ANCOVA and the Pearson Correlation Coefficient. The results of this study are as follows: 1. The perceived health status conditions in the experimental group show statistically significant improvement when compared to the control group (F=17.51, p=.000). 2. The physical and physiological functions, that is, balance (F=17.51, p=.000), flexibility (F=8.01, p=.006), grip strength (F=3.21, p=.018) and leg strength (F=25.78, p=.000) in the experimental group are higher than the control group. The vital signs, that is, the number of heart rate (F=.022, p=.884), systolic pressure (F=1.73 p=.193), and diastolic pressure (F=2.74, p=.103) in the experimental group compared to the control group decreased, but doesn't show statistically significant differences. Immune responses (F=5.13, p=.003) showed statistically significant increases in the experimental group when compared to the control group. 3. The emotional state are improved, that is, degree of depression (F=11.56, p=.001) and degree of anxiety (F=9.14, p=.004) in the experimental group showed statistically significant decreases. 4. The quality of life in the experimental group (F=3.03, p=.037) showed statistically significant differences compared to the control group. 5. The observations of the relationships among the perceived health status, emotional state , the quality of life, the relationships between the perceived health status, the degree of depression (r=-.653, p=.000) and the degree of anxiety (r=-.786, p=.000) were in contrary propotions, while the relationships between the perceived health status and the quality of life (r=.234, p=.008) were in direct propotion. In conclusion, the Rhythmic Movement Program used in this study for geriatric nursing care is simple and safe for application to the elderly and shows significant effects by implementing 5 sessions a week for 9 weeks. The Rhythmic Movement Program improves the quality of life, maintains as well as improves the physical and physiological fuctions and emotional state, therefore this program is strongly recommended for positive applications for independant geriatric nursing health care.

  • Encouraged by the investment in the rice paddy, the introduction of new varieties (Tong-Il), and the price support program, there has been great success in increasing rice production. Meanwhile, the demand for rice has decreased rapidly as per capita income continues to rise. Rice self-sufficiency has been attained, and a new over-supply problem is emerging. Moreover, the Uruguay Round Agricultural Negotiation would prohibit government price support for agricultural products. In October the Korean government decides the government purchase amount and support price, which works as the price guideline. All interested parties exert political efforts to influence the decision. The continued increase of the government purchase price of rice due to political pressure pushed the government purchase price above the market wholesale price in 1988. Also, the farmers preferred to sell to the government than to the wholesaler. This has discouraged the market mechanism, and the government is to take over the three functions of the market mechanism: stockpiling, seasonal price fluctuation adjustment, and circulation. Another big increase may cause the government purchase price to rise above the consumer price, which might lead to arbitrage opportunities for the farmers and suffocate the market mechanism. However, the current political situations limits the options for the Korean government. This paper argues that a supply control policy will reduce the social cost resulting from the high level of producer price support, and it proposes several second best policies: First, the production of new varieties should be reduced rapidly. Second, the old rice in the government warehouse should be auctioned or disposed of in order to reduce the government handling and management costs. Third, the acreage diversion program should be launched in order to control rice paddy acreage. Fourth, a social welfare program in rural areas should be introduced, since the share of population over 60 is increasing rapidly. Fifth, instead of the price support which is forbidden by the Uruguay Round, Korea should restructure the agricultural industry by developing new crops, by enhancing productivity and by improving the agricultural infrastructure.

A Reflective Inquiry of the Qualification System and Jobs for the Teacher Librarian in Korea (사서교사 자격 제도 및 직무에 대한 반성적 고찰)

  • The international standard for the school library suggests that school librarians qualified for librarian and teacher should run the school library. And library assistants should support the teacher librarian to carry out a role as a teacher. Despite the difference of requirements, librarians without teachers' license are under the human resources for school library administration. Furthermore, the job description between the school librarian and librarian is not well-defined in Korea. The different requirements in the library standards and the school library related laws have led to weaken professionalism and social status of librarianship. The condition that teacher librarians are not in the school library have brought the pressure to overcome imperfect functions of the school library and unfairness of educational services. In order to solve these problems, it is necessary to strengthen teacher librarians' qualification system to improve their identity as educational subject specialist librarians. We ought to also build proper foundations of social recognition about librarianship through classifying the types of human resources in the school library into the teacher librarian, library paraprofessional and volunteers. Especially we should hold the job analysis based on administrator, teacher, service, leader, collaboration, and partner.

  • While the term of the institution as social manifestation has been discussed intensively through various theoretical approaches over the last few years in Social Sciences, such a debate has been missing so far in Communication Sciences. This paper attempts a theoretical discussion about the media as an institution in the field of Communication Sciences by applying the theory of organizational sociological neo-institutionalism. This research started out with the question which influence exerts the media on organizations and participants. The media is understood as an institution in the sense of permanent monitoring systems which create a) normative expectations b) which become stronger in order to assert such normative expectations c) the norms are applied by the participants d) in this application process, the participants accept the organizations, look primarily for the chosen paragraphs and exert an influence to change the norms. Organizations orient themselves at the institutional rules, because this way, they want to gain legitimacy and support. The media unfold their influence on organizations through certain obligations in addition to the pressure of the participants who deal with the organization and the media. Thus, media cannot exert influence independent from the organizationbut the participants accept the situation, which is generated by the many conflicting processes within the organization, they analyse them and transfer them.

The Relationship Between Systemic Diseases and Oral Volatile Sulfur Compound (전신질환과 구취의 휘발성 황화합물 상관관계)

  • This study was conducted to investigate the relationship between systemic diseases and oral malodor. The author measured the volatile sulfur compound(VSC) of the patients who visited Pusan National University Health Promote Center for a comprehensive medical testing. The patients were examined gingival bleeding on probing, CPI index, tongue coating. Their systemic diseases were diagnosed by the specialist. 182 patients consisted of 112 males and 70 females. In this study, Oral $Chroma^{(R)}$ was used to measure oral malodor. This equipment could measure the concentration of intraoral VSC (hydrogen sulfide, methyl mercaptan, dimethyl disulfide). All data were analylized using Statistical Package for the Social Science $12.0^{(R)}$. The result of this study was the followings. 1. There was significant difference of numbers of patient who visited health care center according to the VSC concentration level and the Community Periodontal Index, bleeding on probing, tongue coating. 2. The subjects with hyperlipidemia showed the high level of $CH_3SH$ concentration (p=0.036). The concentration of $H_2S$ tends to be high in the group with abnormal findings on pulmonary fuction test(p=0.086). The concentration of $CH_3SH$ in the groups with abnormal findings on lipid profile test(p=0.130) and bone mineral density test(p=0.099) and abdominal ultrasonograpy(p=0.088) tends to be higher than the other group. 3. The concentration of $(CH_3)_2S$ in the group with abnormal findings on blood pressure test(p=0.113), hepatitis B virus serology(p=0.069), Abdominal ultrasonograpy(p=0.091) tend to be higher than the other group.

Is BTC Oil Pipeline Good or Bad for Azerbaijan Economy? (BTC 파이프라인이 아제르바이잔 경제에 미치는 영향 분석)

  • Since 2000, as importance of sourcing energy emphasized caused by instability of international oil price, interests toward Caspian countries as an alternative markets has increased. Especially, Azerbaijan, as middle Asian emerging exporting country, has performed drastic economic boom because of massive amount of foreign capital flowed in and construction of BTC pipeline. However, despite this economic surge, there are unbalanced economy which is merely focusing on energy industry and pressure from increase in real exchange rate and inflation. In order to analyze the sustainability of Azerbaijan economy, the total sample time period of this paper is from January 2001 to December 2007 and the term is divided into before and after BTC line construction. Vector Error-Correction Model has been applied to analysis confirming short-term and long-term effect. As a result, Azerbaijan now face the symptoms of the recession during the time period and this is due to high oil price and increase in export influenced by BTC oil pipeline resulting in decrease in real interest rate. This conclusion is to affect competitiveness of manufacturing industry, base industry for economic proliferation, in a negative way.

A Study on Stress in Poor Families with School Children (학령기 자녀를 둔 빈곤가족의 스트레스 -일 지역 빈곤가족의 실태와 경험을 중심으로-)

  • In the family life cycle, the most important task the families with school children should perform is ‘child education’ and ‘socialization’ However, economic stress on poor families with school children presents multiple problems through the shortage of resources necessary for child education and socialization, inappropriateness of family appraisal, and the insufficiency of the control channels for the efficient management of these confined resources and appropriate appraisal. The objectives of this article are : First, to report research on the actual conditions of the poor families in one area of Cheju and on the relevant health welfare policy, and to examine the appropriateness of the direction and the substance of this policy in terms of the intervention in economic stress on the poor families under the categories of resource management and control of appraisal. Second, to analyze qualitative data extracted from the memoirs of single-parent families living in several areas of Korea under the conceptual framework constructed by literature review in order to get a better understanding of the stress which poor families with school children have experienced. And third, to confirm the factors that can be risk factor but, at the same time, strength to these poor families from presented data and to use them as the basic data from which an intervention model can be developed, based on resource management and control of appraisal. The findings of this article are : First, while the number of absolute poor families in one area of Cheju is increasing and, as a result, the danger of the possibility that multiple problems will occur is also growing, the supporting level of the current policy is no more than a direct resource offer and there is no evidence that resource management is being professionally carried out. When it comes to control of appraisal, due to absence of the professional human resources in this matter, policy performances such as technical education training can have a negative impact. Second, a conceptual framework introduced in this article, ; Economic Pressure → Helplessness → Poor self-esteem → Marital Conflict → Parent-Child financial conflict → Inappropriate socialization → Poor child social competence : is partly verified. And third, judging from the results of the qualitative data, it is confirmed that the healing families, having overcome poverty, show several positive characteristics including : hope, strength, and social support. These findings are identified with the factors of resiliency considered above. According to these results, this article suggests the following. The anti -poverty policy in the future should focus not only on a direct resource offer but also on resource management and the control of appraisal that can magnify its effects. In particular, close attention should be paid to school children since they are in the most crucial period for socialization. Moreover, an emotional labor is such an essential resource for intervention that skilled nurses should play pivotal roles.

A Study on the Activation Measures of Human Tissue Donation in Korea through Comparing with Foreign Countries (해외 주요 국가와 비교 고찰을 통한 인체조직기증 활성화 방안)

  • In modern society, there is a growing demand for human tissue donation along with organ donation. However, the awareness of tissue donation and actual donation rates remain very low in South Korea. This study was undertaken to evaluate the current status of domestic laws and systems, and to compare them with the operation systems of major foreign countries, by reviewing literature and web sites of organ donation and registration. The authors present three measures to promote human tissue donation in Korea: integration of a dual legal system in a legal aspect, vitalization of the Opt-out system in terms of system operation, and activation of public relations in terms of social and cultural aspects. The Opt-out system, in particular, is the most effective way to activate transplants in the form of presumed consent of countries without undue pressure. However, the presumed consent method requires various stages of social public debate, and the requirement is a proper domestic understanding of the registration system for rejection. In conclusion, we believe the solution towards a positive inclination for organ donation is a public policy to increase the supply for organs and human tissue transplants, and positive perception of donations, public promotion, and support for postmortem donors and their families.

Risk Assessment of Stationary Hydrogen Refueling Station by Section in Dispenser Module (고정식 수소충전소에서의 Dispenser Module 내 구역별 위험성 평가)

  • Demand for hydrogen as a renewable energy resource is increasing. However, unlike conventional fossil fuels, hydrogen requires a dedicated refueling station for fuel supply. A risk assessment of hydrogen refueling stations must be undertaken to secure the infrastructure. Therefore, in this study, a risk assessment for hydrogen refueling stations was conducted through both qualitative and quantitative risk assessments. For the qualitative evaluation, the hydrogen dispenser module was evaluated as two nodes using the hazard and operability (HAZOP) analysis. The risk due to filter clogging and high-pressure accidents was evaluated to be high according to the criticality estimation matrix. For the quantitative risk assessment, the Hydrogen Korea Risk Assessment Module (Hy-KoRAM) was used to indicate the shape of the fire and the range of damage impact, and to evaluate the individual and social risks. The individual risk level was determined of to be as low as reasonably practicable (ALARP). Additional safety measures proposed include placing the hydrogen refueling station about 100m away from public facilities. The social risk level was derived as 1E-04/year, with a frequency of approximately 10 deaths, falling within the ALARP range. As a result of the qualitative and quantitative risk assessments, additional safety measures for the process and a safety improvement plan are proposed through the establishment of a restricted area near the hydrogen refueling station.

Validation and Development of the Driving Stress Scale (운전 스트레스 척도(Driving Stress Scale: DSS)의 개발과 타당화 연구)

  • This study was intended to validate and develop the driving stress scale. In a preliminary investigation, literature studies on the stress and open questionnaire were administered and examined in four regions in Korea. As a result, 121 items driving stress questionnaire were developed. In the study, this driving stress questionnaire was examined to 450 drivers located seven regions in Korea. The factors analysis revealed 5 meaningful factors[(Progress Obstacle: PO), (Traffic Circumstance: TC), (Accident & Regulation: AR), (Regulation Observance: RO), (Time Pressure: TP)] with 38 items. When internal consistency for each 5 factor was calculated, all sub-scale revealed a satisfactory level of Cronbach's α. Also, correlations with Driving Behaviour Inventory-General Driver Stress(DBI-GEN) and risk driving behaviors(speed driving, drunken driving, offence accident, defence accident) supported consistently validity of the Driving Stress Scale(DSS). Finally the result were discussed and implications are suggested for future studies.
