Effects of Surgery Volume on In Hospital Mortality of Cancer Patients in General Hospitals (종합병원 암 종별 수술량이 병원 내 사망에 미치는 영향)

  • Background: Although the mortality rate in cancers has been decreased recently, it is still one of the leading causes of death in most of the countries. This study analyzed the relationship between surgery volume and in hospital mortality of cancer patients. The purpose of this study is to investigate the relationship in Korean healthcare environment and to provide information for the policy development in reducing cancer mortality. Methods: The study sample was the 20,517 cancer patients who underwent surgery and discharged during a month period between 2008-2011. The data were collected in Patient Survey by Korean Institute of Social Affairs. Logistic regression was used to analyse a comprehensive analytic model that includes a binary dependent variable indicating death discharge and independent variables such as surgery volume, organizational characteristics of hospitals, socio-economical characteristics of the patients, and severity of disease indicators. Results: In chi-square test, as the surgery volume increases, the in-hospitals mortality showed a downward trends. In regression analysis, the relationship between surgery volume and mortality showed significant negative associations in all types of cancer except for pancreatic cancer. Conclusion: In the absence of other information patients undergoing cancer surgery can reduce their risk of operative death by selecting a high-volume hospital. Therefore, policies to enhance centralization of cancer surgery services should be considered.

Online Reviews Analysis for Prediction of Product Ratings based on Topic Modeling (토픽 모델링에 기반한 온라인 상품 평점 예측을 위한 온라인 사용 후기 분석)

  • Customers have been affected by others' opinions when they make a purchase. Thanks to the development of technologies, people are sharing their experiences such as reviews or ratings through online or social network services, However, although ratings are intuitive information for others, many reviews include only texts without ratings. Also, because of huge amount of reviews, customers and companies can't read all of them so they are hard to evaluate to a product without ratings. Therefore, in this study, we propose a methodology to predict ratings based on reviews for a product. In a methodology, we first estimate the topic-review matrix using the Latent Dirichlet Allocation technic which is widely used in topic modeling. Next, we predict ratings based on the topic-review matrix using the artificial neural network model which is based on the backpropagation algorithm. Through experiments with actual reviews, we find that our methodology can predict ratings based on customers' reviews. And our methodology performs better with reviews which include certain opinions. As a result, our study can be used for customers and companies that want to know exactly a product with ratings. Moreover, we hope that our study leads to the implementation of future studies that combine machine learning and topic modeling.

The Development of a Homecare Nursing Assessment Tool for Terminal Cancer Patients (말기암환자의 가정간호 사정도구 개발)

  • Purpose: The purpose of this study is to develop a homecare nursing assessment tool for terminal cancer patients, testing the validity and reliability of the tool. Methods: This was a methodological study. The tool was developed in four stages: first, preliminary items were developed based on Gordon' functional health pattern model; second, a panel of specialists reduced the number of preliminary items using validity tests for content; third, final items were selected from the results of a pre-test. Finally, from August 4th, 2011 to August 26th, 2011, reliability and validity were tested using a sample of 125 terminal cancer patients in Seoul and Gyeonggi-do. Results: The final tool consisted of 39 items, with Cronbach's ${\alpha}$ 0.70. Using factor analysis, 10 factors were extracted; the correlation coefficient of these was over 0.3. Conclusion: The tool developed in this study was identified as having a high degree of reliability and validity. Given this, the tool can be effectively utilized for implementing and improving home care for patients with terminal cancer.

  • This study aimed to develop a comprehensive group therapy program for divorced parents and their children for increased self-esteem, problem-solving technique, communication ability and emotional support. This comprehensive program was based on Solution-Focused and Satir's Experiential Family Therapy Models, as well as on therapeutic activities from various sources. Six mothers and 18 children of divorced families participated in this program which consisted of six, two-hour sessions. This program was found to be effective in enhancing the participants' self-esteem and communicative ability in addition to emotional support between mothers and their children, and among participants. Therapeutic activities were implemented in harmony with the Solution and Experiential based values and techniques, which seemed to heighten the fun and motivation for the participants to change their view points and behaviors in a positive way.

Study of Design Strategy to Reduce Energy Consumption in a Standard Office Building (사무용 건물의 에너지 절감을 위한 요소별 성능 분석 및 디자인 전략에 관한 연구)

  • Purpose: Recently energy consumption is rapidly increasing due to continuous development of social evolution in various field. In this situation, there is a lot of effort to reduce this energy consumption in many ways, especially in building energy. Preceding studies already started to analyze the housing area such as zero energy house and passive house by researching annual building energy consumption, but to apply the results of housing to office building is insufficient since it has different consumption tendency. Method: In this study, eQuest program was used for simulation and the base model is selected among standard office building in ASHRAE 90.1. Variables are divided into passive and active factors for comparison. Result: In passive factors, glazing system showed the highest energy saving rate by 21.3% with triple low-e glass and enhancing wall u-value showed the lowest energy saving rate by 3.6% with 0.15 m2/K. In active factors, VAV system showed 30.9% energy saving rate when compared to CAV system, and heat exchanger showed 10.2% energy saving rate. For regeneration energy part, photovoltaic panel generated 10.4% of base annual energy usage.

  • Social and economic changes in the world, along with changes in the culture of government and structural changes in the relationship between the state, the market and higher education institutions, have resulted in new approaches pursued by states towards higher education (Konstantinov & Philonovich, 2007; Peterka, 2011). Higher education institutions, in its turn, enjoy more freedom in choosing to focus on various tasks and goals to their academic activities. This contributes to the diversity of expectations when it comes to the various roles modern academic institutions take on with regard to their development. Among many existing concepts in the current academic environment, the concept of "entrepreneurial university" is becoming a critically important concept in Russia. In this article, we consider the concept of the Entrepreneurial University as developed in Clark and Etzkowitz's theories, and the ways the concept is being adjusted in the higher education system in Russia. We analyze comparatively both contemporary and prior historical [1990s-mid2000s] state policies pertaining to higher-education institutions in Russia in order to provide an insight into how the national market of knowledge intensive production is being established in the country. As a consequence of these analyses, we present a new model of how universities can take a more systemic and meaningful approach to creating research-based innovation entrepreneurial activities. This approach is also viewed as making universities' innovation activities more sustainable and thus less dependent on the state support.

Application of Rasch Analysis to the Activities-Specific Balance Confidence (ABC) Scale (Activities-Specific Balance Confidence(ABC)척도에 대한 라쉬분석의 적용)

  • This study was designed to examine, applying Rasch analysis based on item response theory, the questionnaires of the Activities-Specific Balance Confidence (ABC) scale for the elderly. The subjects were 99 institutional older adults and clients of social welfare facilities. The subjects (17 men, 72 women) ranged in age from 65 to 94 years (mean age 76.5 yrs). The Winsteps software was used to assess whether the ABC scale fits the Rasch model, to estimate the score and to refine the rating scale. The results are as follows. Twenty-two subjects were excluded as misfit persons. Four items were found to be misfits and the order of difficulty of the remaining 12 items was rearranged. Their balance confidence is indicated by -.64~1.12 logit, and the transformation formula is score=[(logit score+2.76)/(2.76+3.48)]${\times}$100. The most difficult item was "Walk outside in icy sidewalks" and the easiest item was "Walk around house." In conclusion, the ABC scale for the elderly has been proven reliable and valid. Therefore, it is expected to be used as an effective examination tool for treatment planning and screening for older adults.

Does Village Fund Transfer Address the Issue of Inequality and Poverty? A Lesson from Indonesia

  • This study investigates the impact of fiscal transfer, specifically the Village Fund Transfer, on rural income inequality and rural poverty. Studies on fiscal transfer offers contrasting outcomes, some argues that fiscal transfer suppresses wealth disparity, while others argue that it tends to widen disparity. This study employs descriptive analysis in estimating the elasticity of income inequality and poverty rate before and after the Village Fund Transfer. It develops multiple regressions model on panel datasets of 33 provinces in Indonesia before and after the implementation of Village Fund Transfer. This study suggests that the elasticity of income inequality is higher after the implementation of village fund transfer. Rural poverty tends to decline annually, however, the elasticity changes is lower after the implementation of village fund transfer. Furthermore, this study suggests that village fund transfer is insignificant in coping with the issue of income inequality, while education and the level of labor productivity of agricultural sector appears to be the determinant factor in tackling the issue of income inequality in the rural areas. This study further reveals the significance of village fund transfer in suppressing the rural poverty rate. This study also highlights the significance of human resources quality and agricultural sector in reducing poverty rate in rural areas.

Expanding the aging self: Investigating successful aging among Korean older adults using grounded theory

  • Purpose: This qualitative study aimed to understand older adults' perspectives on successful aging and develop a model of a successful aging process, within the Korean socio-cultural context. Methods: This study used a Grounded Theory approach. Through theoretical sampling, 14 participants were selected from older adults at a public health center and a volunteer institution in Gyeonggi Province, South Korea. Results: The basic social process identified was "expanding the aging self", which was preceded by three phases, namely, adjusting to the changes, developing the valuable self, and embracing the environment. Participants used 2-3 strategies for successful aging in each phase. For adjusting to the changes, participants used strategies such as "adopting a positive attitude", "accepting the changes", and "being at the center of life". To develop the valuable self, "re-identifying capacities", "having something to do", and "advancing to a healthy lifestyle" were used. Participants embraced their environments with "sharing with others" and "embracing younger generations". The causal condition for expanding the aging self was the changes in participants' physical, mental, or psychosocial situations. Conclusion: These findings suggest a theoretical foundation for the development of potential nursing interventions to promote self-care management and the interpersonal relationship for successful aging among Korean elderly individuals.

Development of parent Education for Family Leisure - Focus on the parent of adolescent - (가족여가부모교육 프로그램 개발 - 청소년 자녀의 부모를 대상으로 -)

  • This study aimed at developing parent education program for family leisure for parents of adolescent children to enable them to pursue happy lives as healthy families with adequate balance of cohesiveness and adaptability and smooth communication between the members of their families after parent education for family leisure. Research procedure of parent education program for family leisure proceeded with establishment of theory and requirement survey through precedent research, design of program, practice of program, evaluation of program and suggestion of revised program model after evaluation. On the basis of evaluation results, parent education program for family leisure aiming at parents of adolescent children can conclusively be revised as follows. The first session aims at understanding of family leisure; the second session aims at improvement of learning effect through education & experience-oriented activities; the third session aims at understanding and paying regard to their children through hobby-oriented activities; the fourth session aims at improvement of social relations through society-oriented activities; the fifth session aims at helping children education through check of communication in family-oriented activities and home-visiting education for etiquette; and the sixth session, the final session, aims at providing information and knowledge on family leisure and letting them gain better understanding of adolescent children.
