A Clinical Analysis of Pediatric Head Injuries (소아 두부외상의 임상적 분석)

  • Objectives : With the advancement of a social life, the pediatric head injuries(PHI) occur greater than ever. Since the PHI differs from adult head injury with regards to mechanism of trauma, prognosis, and mortality, it is important to identify the characteristics of the PHI for its proper treatments and prognosis. Methods : For this study, a series of 365 PHI patients under 15 years of age who were admitted to our hospital, were evaluated from January 1991 to December 1996. The clinical variable studied were age, sex, Glasgow coma score(GCS), causes of trauma, diagnosis, symptoms, associated injuries and Glasgow outcome score (GOS). The characteristics of PHI were evaluated according to presentations of skull fractures, intracranial hemorrhages, associated injuries, GCS at admission and GOS. Results : Mean age of the studied patients was 6.51 years of age. The majority of PHI patients were under the 7 years of age(66.7%). The ratio of male to female was 2.2:1. Seasonally, PHI occurred more frequently during March to August(61.6%). The main causes of the injuries were accidental falls and traffic accidents(47.1% and 46.3%). One hundred ninety seven(54%) patients suffered from skull fractures and 110(30.1%) patients were developed intracranial hemorrhages and acute epidural hematomas(17.8%) which were the most common intracranial hemorrhages. There was statistical significance between skull fractures and intracranial hemorrhage (p=0.032) and between GCS and GOS(p=0.001). However, there was no statistical significance between skull fractures and intracranial hemorrhage(epidural hematomas, subdural hematomas, and intracerebral, intraventricular and subarachnoid hemorrhage)(p=0.061, 0.251 and 0.880). Also there were no significance of prognosis between under the seven and over the 8 years of age(p=0.349). Conclusions : The core management for PHI is prevention from its occurrences. However, when unexpected accident occurs, early diagnosis and treatment for PHI by through examination for associated injuries and other damages even if there is no skull fracture are essential in managing patient's outcome.

A Study of Anxiety of Families of Psychiatric Patients at Discharge (정신과 환자 퇴원시 가족들이 느끼는 불안에 관한 연구)

  • This study investigated the anxiety of families of psychiatric patients at discharge. The purpose was to contribute to the improvement of psychiatric nursing care, rehabilitation and social adjustment of psychiatric patients and community mental health. The objectives of this study were to identify the acceptance of the psychiatric nurse by the families, their anxiety at the time of discharge, whether any help was wanted to reduce anxiety, the attitude toward the patient after discharge and feelings about the patients. The population studied consisted of 180 family members of patients from 10 mental hospitals (including local clinic) in Seoul and Kyung- Ki province, from March I to April 30, 1977. The date were collected by an interview schedule, and compared and analysed by Computer usings х$^2$- test. Results were as follows : 1. Many of the families(83.6%) expressed a acceptance of psychiatric nurse. 2. A little more than half of the families(51.1%) expressed happiness but a largo portion (38.9%) had "anxious" feelings at discharge. 3. Almost all families(92.6%) wanted a physician′s help to reduce discharge anxiety. Younger families tended to want the physician′s help more. 4. Many of the families(83.1%) wanted a nurse′s help. Families of parents patients admitted for the 1 st time wanted the nurse′s help more. 5. Comparing the feelings at a previous discharge with the present discharge, 49.1% of the family expressed greater happiness at tile latter than the former. 6. More than half the families responded positively toward the patient. Unmarried family members responded more positively than married Families of 1 st admission patients responded more positively than families of readmission patients. 7. Many families(78.8%)had positively feelings toward the patients. More negative responses came from women than from men, from lower education levels, lower incomes and readmission patients.

Korean Medicine approach to traffic accident victims (교통사고 후유증관리에 관한 한의학적 임상 고찰)

  • Objective : Korean medicine treatment method is noted as alternative in treating traffic accidents(TA) victims recently. The main purpose of this research is to make a survey of the effective way of the Korean medicine treatment about TA victims. Methods : In following research, 389 cases of traffic victims who hospitalized in the Youngsaeng Korean Medicine Hospital & Youngsaeng Clinic from March 1, 2001 to November 30, 2001 were surveyed. The research is focused on finding out the distribution, such as sex and age, damaged part of the body of 389 TA victims, nature of damage of 389 TA victims, chief complaint of 389 TA victims, nature of damage by Korean medicine code classification, treatment given to 389 TA victims, Extract(Ex) medicine given to 389 TA victims, days in hospital of 389 TA victims. Results : The patients reached korean medicine hospital in fewer numbers from most of TA victims. As neck pain and L-spin sprain, most of symptoms was light and the treatment of medicine as well was limited for TA. The treatment method which was used for treating TA was acupuncture, cupping therapy, physical therapy, Ex medicine. Few ways could use the Ex medicine as well. Conclusions : The achieving rate of treatment was marked highly by using Haenggi(行氣) Hwalhyeol(活血) Guyea(祛瘀) and we confirmed a possibility for the treatment of TA in korean medicine treatment method. It needs to increase all kinds of treatment-ways which can treat successfully and needs to come into application of insurance. Korean medicine method will have possibility to take part in TA injury admission, by presenting basis to prove effectiveness of treatment-ways.

  • STEAM and S / W education in public education are effective in fostering talented people and the talents of the 4th industrial revolution era. It is necessary to expand the teachers for this purpose, to find out and apply various learning materials, and to improve education environment for fusion talent education. An open learning platform is effective in reducing private education costs and supplementing public education. Especially, it is useful for flip learning combined with classroom (off-line). In this case, teacher's role can be transformed into active teaching activities and research activities, which can speed up normalization of public education and reduce private education.In particular, the core functions of the MOOC platform for elementary education are 'creative instructional design and contents development function', 'digital teaching and learning curation', 'big data based learner customization', 'learning participation' flip learning and social Learning function.Through this study, it is expected that discussion on the introduction of MOOC for career and admission education for adolescents including elementary education will be established and the Korean youth MOOC platform will be developed and developed as a global advanced model of education democratization.

  • The conditions of university education are also changing as society changes. In particular, the rapid decline in the school-age population heralds a change in the environment of university education. In this study, we would like to draw up educational issues that arise with changes in the educational environment, and propose the university's efforts to resolve them. For example, the challenges of change include expanding the scope of admission resources, providing substantial education, producing competent talent, and administrative and financial bases for implementing educational tasks. Based on this, it is necessary to understand the change in educational paradigm for all members to have a solid education, starting with opening university doors to adult learners. In addition, efforts to change need to be made in consideration of various categories and factors in order to strengthen the educational power of universities in relation to the administrative and financial foundations for implementing them.

A Case Report on the Use of Korean Medicine Treatment for a Patient with Dysarthria Caused by Progressive Bulbar Palsy (PBP)

  • Objectives : To introduce a rare case of a patient with Progressive Bulbar Palsy (PBP) and suggest the possibility of treatment using electroacupuncture and Korean Medicine. Methods : A 61-year-old man with PBP, complaining of dysarthria, was treated with electroacupuncture and Korean Medicine, from June 16 to July 10, 2015. Improvements in symptoms were measured using the speech mechanism screening test (SMST), measurement of tongue and orbicularis oris motility, and speech handicap index (SHI). Results : The scores of SMST, motility of tongue and orbicularis oris showed a tendency for gradual improvement with 25 days of Korean Medicine Treatment after admission, but conversation was still impossible. In SHI scores, one point increased in the speech domain and one point decreased in the psycho-social domain, and three points increased in the other domain. Conclusion : Electroacupuncture and Korean Medicine Treatment improved dysarthria caused by PBP, but not completely. Korean Medicine Treatment seems effective in the management of accompanying symptoms such as black hairy tongue, dry mouth, and general condition. The symptoms of PBP are similar to those of amyotrophic lateral sclerosis and there is controversy regarding the classification of PBP. The most important aspect of treating a patient with PBP is an early diagnosis and devising appropriate rehabilitation strategies.

Factors Influcing Elderly Care Burden of A Working Family in Korean Nursing Facilities: A Convergence Approach (한국 요양시설을 이용하는 직장인 가족의 노인 돌봄 부담감에 영향을 미치는 요인)

  • The purpose of this study was to identify factors influencing elderly care burden of a working family. Methods: A descriptive correlational design was used. Participants was 153 family member of elderly at five nursing facilities. Data were collected through self-reported questionnaires from July to October 2019. Data were analyzed using an independent t-test, one-way ANOVA, Pearson's correlation, and hierarchical multiple regression analysis with the SPSS WIN 25.0 program. Analysis result, explaining 20,8% of the variance, which indicated that subjects for giving care, monthly admission fee, burden of expenses, and satisfaction for fee service were significant predictors of working family's care burden. Various individual characteristics significantly influenced working family's care burden in aspects of emotional, social, economical, and physical burdens. Therefore, interventions to decrease working family's care burden must focus on their individual factors.

Differences between Major Satisfaction, University Life Adjustment, Self-Esteem and Career Identity according to the Time of Department Selection of Students in the Department of Occupational Therapy at One University (일 대학 작업치료학과 학생의 학과선택 시기에 따른 전공만족도, 대학생활적응, 자아존중감, 진로정체감의 차이)

  • The purpose of this study is to examine the differences between major satisfaction, university life adjustment, self-esteem, career identity according to the time of department selection of students in the department of occupational therapy. Frequencies, descriptive statistics, chi-square test, ANOVA was conducted using IBM SPSS Statistics 22. The major satisfaction and career identity have statistically significant difference according to the time of department selection. As the time of department selection was earlier(in high school), major satisfaction and career identity turned out to be higher than other times(before application period or application period). Thus, in order to effective life guidance and career guidance for the students in the department of occupational therapy after admission, the carrer programs that convergent the characteristics of the department of occupational therapy are needed.

Analysis on the Relating Factors of Profitability of Korean Public Corporation Medical Centers(KPCMCs) (지방공사 의료원의 수익성 관련요인 분석)

  • The objective of this study is to analyze a current trend of and relating factors on profitability of the Korean Public Corporation Medical Centers(KPCMCs, hereinafter, hospitals) in Korea. There are 34 hospitals in Korea as of 2004. Among these hospitals some are red ink hospitals, others are black inks in terms of profitability. Data were collected by Korea Health Industry Development Institute(KHIDI) Statistics for Hospital Management 2000-2002 and Ministry of Health and Welfare(MOHW) financial data of public hospitals which was planned to coordinate public health care services roadmap in the long run. The samples are 32 hospitals. Profitability was measured in the aspect of profit rate with normal profit to total assets, and normal profit to gross revenues as dependent variables in respective. Independent variables were classified by general factors, i.e., location, intern/resident training, period of opening, number of beds, and managerial factors(current ratio, fixed ratio, liability to total assets, total assets turnover, personnel costs, materials cost, administrative cost), and finally factors related to patient treatment(average length of stay, bed occupancy rate, admission ratio of outpatients). The methods of analysis are correlation and multiple regression analysis. This study shows firstly, a lot of hospitals are optimal current ratio. Hospitals in upper 100% current ratio are 81.2%. And the personnel cost in total costs are high. Secondly, the trend of normal profit to gross revenues of hospitals are deteriorating gradually. And lastly, as a result of multiple regression analysis, the factors had on significant effect on normal profit to total assets are fixed ratio(+), liability to total assets(-), bed occupancy rate(+), admissions of outpatients(+), etc. And the factors had on significant effect on normal profit to gross revenues are current ration(+), fixed ratio(+), personnel cost(-), administrative expenses(-), admissions of outpatients(+), etc. In conclusion, to improve the profitability of hospitals, the efforts to reduce personnel cost and average length of stay might be needed. And also beds utilization rate need to be increased.

Analysis on educational experience of graduates' career success (진로 성공 대학 졸업생의 교육 참여 경험 탐색)

  • The purpose of this study is to explore the experience of graduates' career success who actively participated during their school days. The experience of participation in education has been explored from three perspectives: input, environment, and output. The first factor which is the input includes the admission process, motivation for application, and methods of appropriating tuition fees. The second factor which is the environment includes the interrelationships with both in and out of school activities, in-class activities, and in-college members. Lastly, the output factor of university education includes the results of university education and the perception of social advancement. The subjects of the study were 21 students who succeeded in entering society based on their goals. The interviews and surveys were conducted, and the factors affecting their career success were analyzed by dividing them into three upper categories and each category is to create implications for the future direction of university education.
