보건소 금연클리닉 신규등록자와 재등록자의 금연 성공요인 분석 (The Factors Influencing on success of Quitting Smoking in new enrollees and re-enrollees in Smoking Cessation Clinics)

  • This study has comparatively analyzed the primary success factors in smoking cessation among new enrollees and re-enrollees of a smoking cessation clinic in order to find out how to efficiently operate smoking cessation clinics at public health centers. The study was conducted with 262,837 smokers aged 19 or over who were provided with smoking cessation services for more than 6 months after being registered with the smoking cessation clinic at public health centers(250 clinics nationwide) from July 16, 2006 to July 15, 2007. After dividing smokers into re-enrollees and new enrollees of the smoking cessation clinic, the success rate of and success factors for smoking cessation over 6 months have been investigated. The success factors in smoking cessation have been compared between new enrollees and re-enrollees of smoking cessation clinics. The results can be summarized as follows: First, the success rate of smoking cessation for 6 months at smoking cessation clinics of public health centers was higher in new enrollees (46.3%) than in re-enrollees (41.1%). Second, the common factors that had an influence on the success of smoking cessation of both new enrollees and re-enrollees of the smoking cessation clinic included age, social security, service, frequency of counseling, number of cigarettes per day, and alcoholic problems. Third, compared to new enrollees, re-enrollees had a higher success rate of smoking cessation as they got older. In terms of the success rate of health insurance, on the contrary, new enrollees were better than re-enrollees. Fourth, the study showed a higher success rate in smoking cessation in both new and re-enrollees if they had no alcoholic problems. In particular, a higher success rate was observed in re-enrollees when there were no alcoholic problems. To efficiently operate smoking cessation clinics at public health centers, this study confirmed that counseling should be tailored depending on the types of enrollees in the program.

  • Objectives: This study identified the factors associated with successful smoking cessation for 6 months at a smoking-cessation clinic of a public health center in an urban area. Methods: The subjects of this study were 670 visitors to the smoking cessation clinic of Dongjak-Gu public health center in Seoul, from September 6, 2005 to March 24, 2006. The 274 visitors of them responded to the questionnaire with registration, which contained the information related to the study except that of registered card for the clinic. A dependent variable was success or failure in smoking cessation during the 6 month-smoking cessation program, measured the status of smoking cessation in each week by self-report. Independent variable included demographic information, the characteristics in using the clinic, health status and smoking related behaviour, exposure to other smokers in daily life and motivation. Multiple logistic regression model was used to find the factors associated with success of smoking cessation. Results: The success rate in smoking cessation for 6 months was 33.6%. Five pretreatment characteristics were identified as univariate predictors of continuous abstinence. Finally, age, job, and practice oriented motivation were associated significantly with the success of smoking cessation for 6 months from a multiple logistic regression analysis. The lower socio-economic smokers such as people having lower literacy level, lower income people, unskilled workers, and recipients by Medical Assistant Program were more likely to fail in continuous abstinence for 6 months. Conclusions: In order to increase the success rate in smoking cessation clinics of public centers, counselors should activate self confidence and practice oriented motivation of participants for smoking cessation.

다층모형을 통한 금연성공에 영향을 미치는 요인 분석 (A Multilevel Model Analysis on the Determinants of Smoking Cessation Success Rates)

  • Objectives: In this study, a multilevel analysis model has been designed to investigate the effect of personal characteristics associated with smoking cessation on anti-smoking determinants with a goal of finding out the factors which have influence on smoking cessation among the entrants of smoking cessation clinic in a public health center. Methods: A total of 253,136 male smokers who received smoking cessation services for more than six(6) months in a smoking cessation clinic of public health center from July 16, 2007 to July 15, 2008 were examined. For technical analysis, SPSS Version 2.0 has been used. For multilevel analysis on smoking cessation determinants, in addition, HLM 7.0 has been adopted. Results: According to the unconditional model of multilevel analysis, the success rates of smoking cessation among the entrants of a smoking cessation clinic were 47.3%. In an unconditional slope model test to which regional variables were added, a negative effect was observed in average smoking amount, total smoking period, nicotine dependence and services while a positive effect was found in age, stress and type of social security in terms of the log of the odds of smoking cessation. In a conditional model test, a positive effect was observed in Non-Smoking Campaigns (NSC) and Frequency of Counseling (FC) in terms of the log of the odds of smoking cessation in regional variables. Conclusions: It is important to approach smokers individually and, at the same time, build healthy environment for a local community to increase smoking cessation rates among the entrants of smoking cessation clinic in a public health center.

범이론적 모형을 기초로 한 보건소 금연클리닉 등록자의 6개월 금연성공요인 (Factors affecting the Success of Smoking Cessation for Six Months in the Smoking Cessation Clinic of a Public Health Center Based on the Trans?theoretical Model)

  • Purpose: The purpose of this study was to identify factors affecting on the successful cessation of smoking for six months in the smoking cessation clinic of a public health center. Methods: The subjects of this study were 568 adult smokers who had registered in the smoking cessation clinic of a public health center in B City. Data were analyzed through $x^2$-test, t-test, and logistic regression using the SPSS/WIN 12.0 program. Results: Identified factors affecting the successful cessation of smoking for six months were service type, number of counseling, alcohol problem, previous quit-smoking trial, dependency on nicotine, CO level in expiration at the first visit time, self-reevaluation, helping relationship, negative/affective situational temptation, and Cons. Conclusion: Trans-theoretical Model variables were confirmed as factors affecting the success of smoking cessation. Based on the results of this study, a smoking cessation program was suggested, which uses Trans-theoretical Model variables affecting smoking cessation for six months in a public health center.

금연프로그램에 대한 인지도, 이용의도 및 영향요인 (Factors Related to the Recognition and Behavioral Intention for Smoking Cessation Programs)

  • This study was conducted to evaluate factors related to the recogniton and behavioral intention for smoking cessation programs. Five effective smoking cessation programs were considered: acupuncture, nicotine patch, clinic program, mass education, and alliance programs. To explain the health behavior for smoking and smoking cessation programs, a five-stage behavioral intention model was built, and 500 questionnaires were completed through a telephone survey. Stages of the model included recogniton of the programs, past experiences, present smoking status, intention for smoking, and behavioral intention for smoking cessation programs. The results showed that the recogniton rate of the programs were low in general, therefore strategies of education, public relations, and advertisement need to be pursued. Nicotine dependency resulted in the fact that success rates were low although trial rates of smoking cessation were high among smokers. The necessity for smoking cessation programs was suggested. And the significant factors related to the intention for smoking cessation were individual attitude and reluctancy to pay time and money. Others' attitude was insignificant to subjects' smoking cessation. Purchase rates for nicotine patches were 11.3% for male and 27.3% for female, those for acupunture were 7.6% for male and 10.0% for female. There were very low purchase rates for clinic, mass education, and alliance programs. In conclusion, evidence-based and effective smoking cessation programs need to be promoted by medical doctors. Strategies in education, public relations, and advertisement also need development. In addition, continuing legal and systematic support for smoking cessation would lower the smoking rate and ultimately contribute to the nation's health.

금연 상담프로그램을 통한 금연이 금연 성공자의 호기 CO농도, BMI, 혈압, 간기능 및 지질대사에 미치는 영향 (The Effects of Smoking Cessation through a Smoking Cessation Counselling Program on Expiratory CO Concentration, BMI, Blood Pressure, Liver Function, and Lipid Metabolism in Smoking Cessation Successes)

  • Purpose: This study was performed in order to examine the effects of an smoking cessation counselling program for smoking cessation success. Method: Among a total of 468 persons who had ceased from smoking for 6-months and had visited the smoking cessation clinic of a public health center from January 2nd to December 31th in 2006, 61 in all who had a negative reaction in the urine nicotine check were selected for this study. Collected data were expiratory CO concentration, BMI, blood pressure, liver function, and lipid metabolism. These data were analyzed by descriptive statistics, repeated measured ANOVA and paired t-test with the SPSS/PC(Version 12.0) program. Result: There were significant changes in expiratory CO concentration, SBP, DBP, AST, ALT, and TG, but not in BMI, $\gamma$-GTP, TC, HDL-C, and LDL-C. Conclusion: This study showed that smoking cessation through a smoking cessation counselling program has partially positive effects for smoking cessation success. The results of this study show that the smoking cessation counselling program at the smoking cessation clinic of a public health center should be continued for smoking cessation success.

광역시 소재 일개 보건소 금연클리닉 이용자의 6개월 금연 성공 관련요인 (The Factors associated with Success of Smoking After a 6 Month Cessation at Smoking Cessation Clinic in Public Health Center of One Metropolitan City)

  • 이 연구의 목적은 2007년 대전시 보건소 금연 클리닉 이용자 중 금연 성공자와 금연 실패자의 일반적 특성과 흡연특성, 금연특성, 그리고 중재요인 등을 분석하여 금연 성공에 영향을 미치는 요인을 알아보고자 했다. 2007년 대전시 보건소 금연 클리닉에 등록된 2,125명을 분석하였다. 참가자의 특성파악을 위한 빈도분석과 특성에 따른 금연성공률의 유의성 검증을 위한 $X^2$-검정, 그리고 각각의 특성이 금연성공률에 미치는 영향력을 파악하기 위해 로지스틱 회귀분석을 시행하였다. 6개월간의 전체 금연 성공률은 39.8% 이었다. 금연 성공 집단에서 남자가 여자보다 더 높은 성공률을 보였다. 또 65세 이상인 집단이 40세 미만인 집단보다 더 금연 성공률이 높았다. 중구보건소가 다른 곳에 비해 높은 금연성공률을 보였다. 금연 성공률에 영향을 미치는 요인에 대한 로지스틱 회귀분석은 나이, 보건소, 2단계 니코틴 패치, 총 상담횟수와 유의성을 보였다. 금연성공률에 대한 총 상담횟수는 다른 유의성 있는 요인보다 높은 비차비를 보였다. 따라서 우리는 총 상담횟수가 금연 성공률과 높은 연관을 가지며 상담이 금연클리닉에서 중요한 역할을 할 것이다.

금연클리닉 프로그램을 통한 금연침 효과의 후향 연구 (Effects of the Acupuncture on the Smoking-cessation Clinic Program of Public Health Center in Urban Area: Retrospective Case Control Study)

  • Objectives : This study identified the effects of the acupuncture associated with successful cessation at a smoking-cessation clinic of public health center in an urban area. Methods : The subjects of this study were 1121 patients to the smoking cessation clinic of the Anjung Public Healthcare Center in Pyeongtaek, from June 1, 2007 to December 31, 2008. The 123 patients of them were treated by the auricular acupuncture. The experimental group were treated by the acupuncture, and the control only participated in the smoking-cessation clinic program. The data were analysed by SPSS(version 17.0). Chi-square analysis were conducted to see the different of effects between experimental and control group. Also, to find out the factors that affect the success of smoking cessation, logistic regression and Fisher's exact test were done. Results & Conculusions : 1. As the result of the analysis, it showed that 73.2% of the experimental were able to quit smoking for 4 weeks and 6months. 2. Logistic regression and Fisher's exact test showed that number of Cigarette per day, Nicotine Dependence, CO figure(p=.004<.01), and Number of Acupuncture(p=.000<.001) were the factors that affect the successful smoking cessation. 3. The smoking cessation were more likely to success as patients have lower CO figure(B=-.074), more number of the acupuncture(B=1.718). The more number of acupuncture increased 1 time, the more smoking cessation success did 5.576 times(e$^{(1.718{\times}x)$=e$^{(1.718{\times}1)$ = 5.576). 4. As the result of the analysis between experimental and control group, the success rate have the significance difference(p=.000<.001). 73.2% of the experimental were able to quit smoking, but 45.1% of the control did.

보건소 금연클리닉 활성화를 위한 방문자 이용현황과 금연성공 및 만족도 조사 (Implementing a Smoking Cessation Clinic at a Public Health Center in Korea: Evaluating the Outcomes and the Smokers' Perceptions)

  • Purpose: To examine the outcomes of a smoking cessation clinic at a public health center, in Hwasung city in 2006 and to examine the perceptions of the users regarding the clinic services. Method: 620 smokers who visited the smoking cessation clinic from January, 2006 to November, 2006 were enrolled. An instrument developed by the researcher was used to collect their perceptions. The instrument comprised 24 questions, on a 5-point Likert scale investigating the clinic services in terms of counseling time, environment, readiness of the counselors, counseling information, campaign method, medication use, and follow-up services. The general characteristics of the participants were collected from the database developed by the Ministry of Health & Welfare. The data were analyzed with frequencies, percentage, mean and standard deviation methods. Results: The outcomes of the smoking cessation clinic in Hwasung city in 2006 showed some limitations in terms of low-income group, female smokers, and young adult smokers. Conclusion: Strategies for solving these limitations were necessary to increase the effectiveness of the smoking cessation clinic. Clinic services focused on cognitive-behavioral therapy needed to be developed in accordance with chemotherapy. More effective publicizing campaigns of the clinic services also needed to be developed for more people to know about the clinic. Strategies to increase the accessibility for the people living far from the clinic were also necessary. Assessment of the smokers who did not visit clinic was also recommended in order to determine their intentions, or barriers to visit the clinic.

일 지역사회 간호사들의 금연지도활동에 대한 조사연구 (Smoking Cessation Counselling Activity among Nurses in a Community)

  • Purposes: The purposes of this descriptive correlational study were to assess knowledge and attitude toward smoking and smoking cessation counseling activity among nurses in a community, and to identify predictors of their smoking cessation counseling activity. Method: Nurses employed by institutions such as university hospital, hospital, clinic, and health department in a community were surveyed using a structured questionnaire. Of the nurses invited to participate in this study, 760 (88.5%) responded with completed questionnaires. Results: Nurses had relatively positive attitude toward their roles and responsibilities about smoking cessation counseling activity. However, smoking cessation counseling activity was not a routine part of their nursing practice. Moreover, the level of engagement with smoking cessation counseling activity was significantly different by working place. Nurses who were working at the university hospital, hospital, and health department were more actively engaged with smoking cessation counseling activity than nurses who were working at the clinic. Smoking cessation counseling activity was significantly correlated with knowledge of smoking. attitudes toward smoking-related issues, and self-efficacy for smoking cessation counseling activity. In the final stepwise multiple regression, smoking cessation activity was predicted by the nurses' working place, attitudes toward smoking-related issues, and self-efficacy for counseling knowledge and skills. Conclusion: In conclusion, nurses need to participate routinely and actively in smoking cessation counseling activity. To help nurses counsel and intervene patients regarding smoking cessation more effectively, it is essential to integrate educational information on smoking cessation intervention into curriculums of nursing schools as well as to offer smoking cessation intervention as a continuing education program available for currently practicing nurses.
