The Availability of Diagnostic and Treatment Planning Computer in 700 Cancer Patients and Magnification Devices for CT (암환자 700예의 진단 및 치료 CT 이용율과 CT 확대장치)

  • It has been evident since 1972 that computed tomography(CT) can play an important role in treatment and managment of the cancer patients as four steps; diagnosis, satging Treatment and follow-up. In this paper, we intended to investigate the availability of CT scan and treatment planning computer in 700 cancer patients who have undergone radiation therapy at the division of radiation therapy, Kangnam St. Mary's Hospital, Catholic Medical College between Mar. 1983 and Dec. 1985. The result were as follow; 1. Of 700 irradiated cancer patients, 342 patients ($48.9\%$) were performed CT scan prior to radiation therapy. 2. The distribution of lesions in 342 patients having CT scans was like this; CNS (83 of 104 patients, $79.8\%$), abdomen (44 of 76 patients, $57.9\%$), pelvis (100 of 188 patients, $53.2\%$) etc. in order. 3. The treatment planning computer were used in 280 cancer patients ($40\%$). 4. Of the 280 cancer patients using treatment planning computer, 167 patients ($59.6\%$) applied diagnostic CT scan and remaining 113 patients ($40.4\%$) were made body contour to be used for radiation therapy planning by the treatment planning computer. Authors also made some magnification devices used for small multiformat CT images to magnify into life size, consisting of overhead projector (3M) I.V. stand and mirror. These enabled us to make less errors in tracing the small-sized CT images during input of the anatomical data into the treatment planning computer.

Primary Malignant Lymphoma of Lung -A Case Report- (원발성 폐 림프종 치험 1례)

  • Primary malignant lymphomas of the lung are rare and known often to be localized, solitary pulmonary lesions, in chest radiograph. Because they are highly treatable contrast to the other primary lung cancer, the distinction is important. A 35-year old man who was admitted for a solitary pulmonary nodule in the right middle lobe. Percutaneous needle aspiration disclosed diffuse, small cell lymphoma. Bone marrow biopsy showed no evidence of neoplastic lymphoid cell infiltration. There were a walnut sized mass involving right middle lobe with a small satellite nodule at 2cm distal to the right upper lobe bronchial orifice. The histopathology of the bilobectomized specimen showed diffuse, small cell, malignant, non-Hodgkin`s lymphoma. Immunologic subtype was defined as B-cell type.

Factors Associated with Cancer Screening Rates of Manufacturing Workers (제조업 근로자의 암 검진 관련 요인)

  • Purpose: Regular cancer screening for workers can help prevent cancer or allow early treatment, and is thus beneficial in improving health, reducing costs, and increasing companies' productivity. However, there has been little research on cancer screening rates of workers and related factors. The purpose of this study was to identify cancer screening rates and relevant factors among manufacturing workers. Methods: Using a questionnaire, data were collected from 335 workers aged over 40 years recruited from seven manufacturing companies. The collected information included cancer screenings (stomach, colorectal, breast, and cervical), attitude towards cancer screening, and demographic and job characteristics. Results: 72.5% of workers received stomach cancer screening (SCS), and 43% received colorectal cancer screening (CRCS). Among 86 women, 68.6% received breast cancer screening and cervical cancer screening. The attitude towards cancer screening was $29.5{\pm}3.78$. Workers aged over 51, married, and those working in mobile manufacturing were more likely to have undergone SCS. Workers aged over 51, married, those working in mobile manufacturing, those in workplaces with more than 1,001 employees, and those with more positive attitudes toward cancer screening were more likely to have undergone CRCS. Finally, attitude affected cervical cancer screening. Conclusion: Workers in small-sized workplaces had lower cancer screening rates. Thus, follow-up research should assess the health environment of workplaces and develop educational programs on cancer screening that reflect attitudes towards screening.

Incremental Expression of 14-3-3 Protein Beta/Alpha in Urine Correlates with Advanced Stage and Poor Survival in Patients with Clear Cell Renal Cell Carcinoma

  • We investigated the urinary levels of 14-3-3 protein beta/alpha to evaluate their diagnostic significance with regard to clear cell renal cell carcinoma (ccRCC) and angiomyolipoma (AML). Urine samples from 91 patients with ccRCC, 16 patients with AML and 24 healthy volunteers were assessed. We used an enzyme-linked immunosorbent assay (ELISA) to quantify 14-3-3 protein beta/alpha levels in urine. Values were higher in patients with ccRCC than in those with AML and in healthy volunteers. High levels were associated with pathologic stage, lymph node status, distant metastasis and poor survival. Urinary levels of 14-3-3 protein beta/alpha were significantly increased in patients with small-sized carcinoma, irrespective of being less than 4.0 cm and 2.0 cm, compared with levels in patients with AML. This study is the first to report that increased expression of 14-3-3 protein beta/alpha in urine is associated with advanced stage and poor survival in patients with ccRCC. In addition, urinary 14-3-3 protein beta/alpha may differentiate AML from RCC, even when small sized. These results suggest that examination of urinary 14-3-3 protein beta/alpha could serve as a diagnostic and prognostic marker in patients with ccRCC.

Tuberculous Esophageal Perforation -Report of A case- (결핵성 식도천공 수술치험 1예)

  • This is a report of a case of tuberculous esophageal perforation, which was surgically treated.. The patient was 32-year-old Korean female patient, who complained swallowing difficulty for` 4 weeks duration. Esophagogram was shown irregular filling defects in the upper one third of esophagus, about 4 cm in length. It was noticed that a small amount of contrast media was leaked out from the involved area of esophagus into the right mediastinum. It was highly suggested that abscess formation was due to perforation of esophageal cancer. Esophagoscopy revealed no definitive evidence of perforation of esophagus, but punch biopsy specimen of esophageal mucosa was shown acute and chronic inflammatory changes. Operation was performed under impression of esophageal cancer. In the right-sided upper mediastinum, a walnut-sized abscess cavity which was connected with esophagus through a fistulous tract was noted. A portion of cavity submitted for frozen section was shown tuberculous inflammation. The abscess cavity, fistula tract, and involved esophageal wall were removed. The perforated esophagus was closed layer by layer. The tuberculous changes were confirmed by histopathologic examination postoperatively. The postoperative course was uneventful.

Mixed Exocrine and Endocrine Carcinoma in the Stomach: A Case Report

  • We report a rare case of the coexistence of a gastric small cell neuroendocrine carcinoma with a gastric adenocarcinoma. A 62-year-old man presented with epigastric soreness for 1 month. Esophagogastroduodenoscopy revealed a Borrmann type I tumor at the lesser curvature of the lower body of the stomach. The patient underwent a distal gastrectomy with D2 lymph node dissection and the resected specimen exhibited a $3.5{\times}3.5$ cm sized, fungating lesion. Two separated, not intermingling, lesions with non-adenocarcinoma components encircled by well differentiated adenocarcinoma components were identified microscopically. The non-adenocarcinoma component showed neuroendocrine features, such as a solid and trabecular pattern, and the tumor cells showed a high nuclear grade with minimal cytoplasm, indistinct nucleoli, and positive response for synaptophysin, CD56. The final pathological diagnosis was a gastric mixed exocrine-endocrine carcinoma (MEEC) composed of an adenocarcinoma and small cell neuroendocrine carcinoma of the collision type.

Loss of the Retinoblastoma Gene in Non-Small Cell Lung Cancer (비소세포폐암에서의 망막모세포종유전자의 소실)

  • Background: Inactivation of retinoblastoma gene (Rb) has been observed in a variety of human cancers. Loss of heterozygosity (LOH) of Rb which is a common mode of allelic inactivation of Rb, has been known as a frequent genetic event in small cell lung cancer but it has been detected less frequently in non-small cell lung cancer. To define the role of Rb deletion in lung cancer, we investigated the genomic DNAs of 43 non-small cell lung cancers and 1 small cell lung cancer paired with normal lung tissues obtained by thoracotomy. Methods: The genomic DNAs were obtained by the digestion with proteinase K followed by phenol-chloroform extraction method. The genomic DNAs were digested by restriction endonuclease (EcoRI), separated by agarose gel electrophoresis, transferred to nylon membrane by Southern blot transfer and then hybridized with labelled Rb 1 probe which contains. 1.4 kb sized DNA sequence containing N-terminal portion of Rb. Results: In 26 squamous cell lung cancers, 16 cases were informative after EcoRI digestion and LOH of Rb was found in 10 cases (62.5%). In 17 adenocarcinomas of lung, 11 cases were informative and LOH of Rb was found in five cases (45.4%). The analysis of clinical parameters revealed no significant differences between the two groups with or without LOH of Rb in the aspects of age, sex, degree of differentiation, stage and smoking amount. Conclusions: These results suggest that Rb inactivation is also significantly involved in the molecular pathogenesis of non-small cell lung cancer.

Computer Simulation of the Transanal Endoscopic Microsurgery for the Improvement of Optimal Operation Range (경항문 내시경 수술 도구에서의 최적 활동 반경 개선을 위한 컴퓨터 시뮬레이션 연구)

  • Conventional devices for transanal endoscopic microsurgery that are currently used clinically for intestine or rectal cancer patients have 40 mm external diameter of rectoscope tube - induces anal damage and long-term postoperative pain for small-sized patients. In this paper, we designed rectum and rectoscope models and calculated the changing trends of operation area of the surgical tools in accordance with the step-by-step variation of design parameters of the rectoscope tube - external diameter, axial length, and distal angle. Using the results of computer simulation, we suggested an optimal set of design parameters that minimizes external diameter of the rectoscope tube and at the same time, maintains similar operation area of the surgical tools compared with commercialized devices (${\geq}\;4274.7mm^2$). The results of the simulation showed that the optimal design parameters were 35 mm external diameter, 100 mm axial length, and $45^{\circ}$ distal angle of the rectoscope tube. This result can be applied to the development of endoscopic microsurgery device that can minimize side effects to the intestine or rectal cancer patients.

A Case of the Soft Palate Reconstruction Using the Bilateral Palatal Mucomuscular Flap and Pharyngeal Flap after Wide Resection (연구개 및 구개수 암종의 광범위 절제 및 국소 점막근 피판 재건술 1예)

  • The soft palate of carcinoma limited to the uvular region is infrequent among oropharyngeal cancers. The oropharynx regulates swallowing and speech through dynamic motions. Failure to reconstruct after surgical resection of the oropharynx structure can lead to permanent velopharyngeal insufficiency. Therefore, suitable reconstruction is important in establishing proper functional outcomes while maintaining oncological safety. We present a case of a 66-year-old male who was diagnosed with oropharynx cancer limited in the uvula accompanied by lymph node metastasis. After surgical resection, reconstruction was performed with the united arrangement of bilateral palatal mucomuscular flap and superiorly based posterior pharyngeal flap. There was no aspiration or reflux after feeding and epithelialization completely occurred after 1 month postoperatively. We report a successful case that the reconstruction with the local flap described above could preserve proper oropharyngeal function after primary surgery in small-sized oropharyngeal cancer.

XRCC1 Polymorphisms are Associated with Cervical Cancer Risk and Response to Chemotherapy: a Systematic Review and Meta-analysis

  • Background: Functional single nucleotide polymorphisms of x-ray repair cross-complementing protein 1 (XRCC1) have been suspected to contribute to uterine cervical cancer risk for a long time; however, most previous case-control studies were small sized and biased. Additionally, recent studies suggested that XRCC1 polymorphisms could be a biomarker of response to platinum-based chemotherapy. Methods: A comprehensive search was conducted to retrieve eligible studies and odds ratios (ORs) and 95% confidence intervals (95% CIs) were calculated to measure association strength. Results: A total of 13 studies were identified and analyzed. We found that the Arg194Trp polymorphism (Trp vs. Arg, OR=1.342, 95% CI: 1.176) was associated with increased risk of cervical cancer, while no significant association was found with Arg280His (His vs. Arg, OR=1.059, 95% CI: 0.863, 1.299) or Arg399Gln (Gln vs. Arg, OR=1.144, 95% CI: 0.938, 1.394). As for response to platinum-based chemotherapy, the variant XRCC1 399Gln allele (Gln vs. Arg, OR=0.345, 95% CI: 0.163, 0.729) was linked with a poor response; however, the Arg194Trp polymorphism (TrpArg vs. ArgArg, OR=6.421, 95% CI: 1.573, 26.205) predicted a good response. Conclusion: The Arg194Trp polymorphism of XRCC1 increases risk of cervical cancer; the variant 399Gln allele predicts poor response to platinum-based chemotherapy, while the Arg194Trp polymorphism indicates a good response.
