노인의 성별 만성동통 호소의 차이에 대한 조사연구 (Difference of Pain Description According to Gender in the Elderly)

  • Despite acknowledgment that pain is likely to be a major problem for many older adults, it is difficult to estimate the frequency of pain problems for this population. The main purpose of this study was to identify the various characteristics of chronic pain in the elderly by gender. It examines by gender, pain frequency, pain intensity, number of chronic pain sites, localization of pain, impact on activities, methods of pain management, and effects of chronic pain management. The subjects were 189 elderly people(65 years and older) living in an urban area. They were surveyed at their homes. They were surveyed by interview using a closed-ended questionnaire. The survey was done from Nov. 6th. to Dec. 6th. 1997. Descriptive statistics were used to determine all of the reported pain variables. Chi-square tests were used to determine crude differences between pain intensity and gender. T-test was used to determine differences in number of pain sites between men and women. The findings are as follows ; Of the 189 subjects, 83.6% reported experiencing pain, and men reported a lower prevalence (69.5%) than women(89.2%). Women had significantly more severe pain than did men(p=.001). Lower back pain(20.2%) and leg pain(20.2%) were the most prevalent, followed by knee pain(17.4%), arm pain(13.3%), neck and shoulder pain(11.6%), and headache(9.9%). Women experienced more multiple localizations of pain(p=.0001), the greatest impact was on movement (83.5%) , followed by usual activities(60.8%), sleep(49.4%), visiting(29.1%), and hobbies and recreations(50.0%). No differences were observed between men and women in the proportion of subjects reporting a negative impact on each of these activities. The methods of pain management were hospital visit(70.9%), drug store medication(46.7%), oriental medicine clinic(32.9%), endurance(13.3%), self-management(6.3%). Drug therapy was the most effective pain management strategy(94%), followed by physical therapy(63%) and accupuncture.byssocausis(55%). The conclusion : Pain is a symptom of great clinical importance that is often associated with disability, loss of independence, and reduced quality of life. In this study chronic pain symptoms were common but unevenly distributed in men and women. The results further advance understanding of the experience and impact of pain by gender. Future studies should incorporate questions that gather systemic and more detailed information on the characteristics of pain, especially by gender and by age.

임시주거용 조립주택 개선사항 도출을 통한 리모델링(안) 개발 연구 (A Study on the Development of Remodeling (plan) by Deriving Temporary House Improvements)

  • 연구목적 방법: 본 연구에서는 포항지진 피해자들에 대한 설문 면담조사를 통해 이재민들에게 지원하는 임시주거용 조립주택의 개선방안 도출을 통한 리모델링(안) 제시를 목적으로 한다. 연구결과: 시설적 측면에서의 각 공간별 주요 개선사항은 다음과 같다. 첫째, 화장실 조리공간의 확장을 위해 기존에 있던 중문과 가벽을 없애고 화장실의 폭을 확장, 화장실의 확장 폭 만큼 조리대를 추가 배치하여 불편을 최소화한다. 둘째, 한정된 면적에서 수납공간 확보를 위해 별도의 수면구획을 설정, 하부에 수납장을 설치하여 수면공간을 수납공간으로 활용한다. 이때, 수면공간의 크기는 더블베드 사이즈로 설정한다. 셋째, 프라이버시 확보를 위해 양쪽 창문에 커튼 블라인드를 설치하여 사생활 보호는 물론 심리적 안정을 도모한다. 결론: 본 연구에서 제안한 임시주거용 조립주택의 리모델링(안)을 활용 적용한다면 보다 나은 거주환경을 제공할 수 있을 것으로 판단된다. 또한, 이재민들의 니즈를 반영하여 불편사항을 최소화함으로써 공간의 효율성을 높이고 기존의 공간적 한계를 극복할 수 있을 것으로 사료된다.

  • 국내 건설산업의 기술자격제도는 노동부주관 $\ulcorner$국가기술자격법$\lrcorner$ 에 의하여 운영되며, 대표적인 국가자격은 건축사, 기술사 및 기사 등을 들 수 있다. 1990년 중반에 도입된 건설사업관리(CM)는 건설산업을 활성화하기 위한 필수불가결의 조건이 되고 있으나 국가자격으로 공인되지 못하고 있으며, 현안문제로서 가장 시급하다고 판단되는 전문인력 양성은 민간 CM교육기관들의 "공인민간자격"으로서 각각의 교육프로그램과 별도의 인증자격을 가지고 이루어지고 있는 실정이다. 문제는 국제공인자격으로서 신뢰성을 확보하고 있는 유일한 미국CM협회(CMAA)의 "CCM"인증자격과 미국VE협회(SAVE)의 "CVS"인증자격의 사례로 볼 때, 외국에 비하여 CM인증자격이 과다하며, 신뢰성과 효율성을 확보하지 못한 상태로 인증자격자득에게 혼란을 주면서 양산되고 있다는 점이다. 본 고에서는 국내CM인증자격의 문제점을 살펴보고 인증자격의 통합 및 체계화를 통하여 국가공인자격으로 발전하기 위한 개선방안을 제시하고자 한다.

경남지역 백세인의 장수요인과 일상생활수행능력(ADL) 및 도구적 생활수행능력(IADL)에 대한 연구 (A Study on the factors of longevity, ADL and IADL of the eldery people over 100 years in Gyeongnam province)

  • 본 연구는 경남지역의 백세인(100세 이상 자) 88 명을 대상으로 2011년 4월부터 6월까지 약 90여 일간장수 요인과 일상생활 수행능력 도구적 생활수행능력 정도를 파악하기 위한 목적으로 수행하였다. 장수요인으로는 7시간 이상의 충분한 수면(93.6%), 규칙적인 식사(95.5%,) 짠 음식 적게 섭취(69.8%), 그리고 자살을 생각하지 않는 낙관적 사고(86.9%)와 금연(92.0%) 과 절주(86.4%,)로 나타났다. 일상생활수행능력(ADL) 중 목욕하는 것이 가장 힘들었으며 도구적 생활수행능력(IADL)의 경우는 버스나 전철 혼자타기가 가장 힘든 것으로 나타났다. 모든 영역에서 여성이 남성에 비해 일상생활수행능력, 도구적 생활수행능력이 높은 것으로 나타났으나 통계적으로 유의한 차이는 없었다(p<0.05). 따라서 향후 연구에서는 본 연구의 결과를 토대로 점차 늘어나고 있는 백세인의 신체수행능력 저하를 보완 지지할 수 있는 정책의 개발이 필요한 것으로 사료된다.

Prediction Model of User Physical Activity using Data Characteristics-based Long Short-term Memory Recurrent Neural Networks

  • Recently, mobile healthcare services have attracted significant attention because of the emerging development and supply of diverse wearable devices. Smartwatches and health bands are the most common type of mobile-based wearable devices and their market size is increasing considerably. However, simple value comparisons based on accumulated data have revealed certain problems, such as the standardized nature of health management and the lack of personalized health management service models. The convergence of information technology (IT) and biotechnology (BT) has shifted the medical paradigm from continuous health management and disease prevention to the development of a system that can be used to provide ground-based medical services regardless of the user's location. Moreover, the IT-BT convergence has necessitated the development of lifestyle improvement models and services that utilize big data analysis and machine learning to provide mobile healthcare-based personal health management and disease prevention information. Users' health data, which are specific as they change over time, are collected by different means according to the users' lifestyle and surrounding circumstances. In this paper, we propose a prediction model of user physical activity that uses data characteristics-based long short-term memory (DC-LSTM) recurrent neural networks (RNNs). To provide personalized services, the characteristics and surrounding circumstances of data collectable from mobile host devices were considered in the selection of variables for the model. The data characteristics considered were ease of collection, which represents whether or not variables are collectable, and frequency of occurrence, which represents whether or not changes made to input values constitute significant variables in terms of activity. The variables selected for providing personalized services were activity, weather, temperature, mean daily temperature, humidity, UV, fine dust, asthma and lung disease probability index, skin disease probability index, cadence, travel distance, mean heart rate, and sleep hours. The selected variables were classified according to the data characteristics. To predict activity, an LSTM RNN was built that uses the classified variables as input data and learns the dynamic characteristics of time series data. LSTM RNNs resolve the vanishing gradient problem that occurs in existing RNNs. They are classified into three different types according to data characteristics and constructed through connections among the LSTMs. The constructed neural network learns training data and predicts user activity. To evaluate the proposed model, the root mean square error (RMSE) was used in the performance evaluation of the user physical activity prediction method for which an autoregressive integrated moving average (ARIMA) model, a convolutional neural network (CNN), and an RNN were used. The results show that the proposed DC-LSTM RNN method yields an excellent mean RMSE value of 0.616. The proposed method is used for predicting significant activity considering the surrounding circumstances and user status utilizing the existing standardized activity prediction services. It can also be used to predict user physical activity and provide personalized healthcare based on the data collectable from mobile host devices.

  • 6차 산업은 농업인 소득감소 및 침체되어가는 농촌지역 경제를 활성화할 대안으로서 주목받고 있다. 6차 산업은 1차 산업에 기반 한 2차 산업의 제조 및 가공, 3차 산업의 유통 및 서비스를 통합 또는 연계하는 것을 의미한다. 박근혜 정부는 농업농촌에서 창조경제를 실현하는 구체적 대안으로 6차 산업을 평가하고 다양한 정책적 대안을 제시하고 있다. 또한 이를 뒷받침 하기 위해 모델개발이나 선진사례 분석등의 연구가 진행되고 있는 실정이다. 그러나 6차산업화를 수행하는 경영체에 대한 경영수준 기술수준 리더역량 등을 종합적으로 평가하는 평가지표 개발에 관한 연구는 미흡한 실정이다. 이러한 점에서 본 연구는 6차산업화를 시행하는 경영체에 대한 경영수준 기술수준 리더역량 평가지표 개발을 위해 AHP기법을 활용하여 종합적으로 평가지표를 개발 하였다. 분석결과 한국이 여러 국가와 FTA가 이루어지면서 값싼 농산물 수입이 증대되면서 6차 산업과 관련된 경영체의 수익이 악화될 가능성이 매우 높은 시점에 무한경쟁에서 살아남기 위한 전략 중 가장 중요한 것은 각 개별 경영체가 독자적으로 사업을 운영하기 보다는 내부 역량 강화와 더불어 타 산업과의 연계성을 강화함으로써 상품 및 서비스 판매를 촉진시키고 이를 통해 경영체의 수익을 향상시켜야 한다는 것이다. 따라서 6차 산업 경영체 모두 각자의 위치에서 소비자 신뢰를 얻기 위한 노력들이 필요할 것이며 더욱이 각 경영체간에 협력체계를 구축할 수 있도록 다양한 방안을 모색해야 할 것이다. 또한 스마트 시대에 고객의 니즈가 빠른 속도로 변함에 따라 상품 및 서비스의 수명주기는 점점 빨라지고 있으며 소비자는 새로운 신상품에 찾는 경향이 점점 높아지고 있다. 따라서 6차 산업 경영체는 고객 및 시장 트랜드 변화를 신속하고 정확하게 예측하여 새로운 상품 및 서비스를 시장에 출시하는 노력이 더욱 필요할 것으로 사료된다.

퇴원환자의 가정간호요구와 가정간호사업의 효과 분석 - 일 종합병원을 중심으로 (A Study of Home Care Needs of Patients at Discharge and Effects of Home Care -Centered on Patients Discharged from a Rural General Hospilal-)

  • The study was carried out at W. hospital, an affiliated hospital of Y university, involved a total of 163 patients who were discharged from the hospital between May 1990 und March 199J. Data collection was twice, just prior to discharge and a minimum of three months post discharge. Thirty patients who lived within a hour travel time of the hospital received home care during the three months post discharge. Nursing diagnoses and nursing interventions For these patients were analyzed in this study. The results of the study are summarized as follows : 1. Discharge needs for the subjects of the study were analyzed using Gordon's eleven Functional categories and it was found that 48.3% of the total sample had identified nursing needs. Of these, the needs most frequently identified were in the categories of sexuality, 79.3 %, health perception, 68.2 % self concept, 62.5 %, and sleep and rest 62.5 %. Looking ut j he nursing diagnosis that were made for the 30 patients receiving home care, the following diagnoses were the most frequently given; alteration in sexual pattern 79.3%, alterations in health maintenance, 72.6%, alteration in comfort, 68.0%, depression, 64.0%, noncompliance with diet therapy, 6.3.7%, alteration in self concept, 55.6%, and alteration in sleep pattern, 53%. 2. In looking at the effects of home nursing care as demonstrated by changes in the functional categories over the three month period, it was Found that of the 11 functional categories, the need level for health perception, nutrition, activity and self concept decreased slightly over the three month period. On the average sleep patterns improved, but restfulness was slightly less and bowel elimination patterns improved but satisfaction with urinary elimination was slightly less. On the other hand, role enactment, sexuality, stress management and spirituality decreased slightly. The only results that were statistically significant at the 0.05 level were improvement. in digestion and decrease in pain. No statistically significant changes were found in ability related to ADL, the total ADL Score at discharge was $19.78{\pm}8.234, and after 3 months $19.01{\pm}8.12$. Considering that a majority of the patients were over 60 years of age and that many had brain or spinal cord injuries, the fact that their ADL ability did nor deteriorate after discharge can be interpreted as related to a positive impact by the home health care nurses. Similarly there was a slight be not statistically significant decrease in the quality of life scores between the two lest times(l47.83 at discharge and 113.02 at the three month period). Again, when the chronic nature of thee problems facing these patients is considered this maintenance of quality of life can be interpreted as a positive impact by the home health care nurses. 3. One of the home care nursing activities was diagnosis. For this activity it was found that for nine functional health categories(sexuality and spirituality excepted) there were 20 nursing diagnoses. The most frequent were noncompliance, alteration in skin integrity both actual and potential, and impaired physical mobility in that order. 4. Delivery of home health care by the home health nurses included the following nursing activities; assessment, patient education, demonstration of care activities, counselling, direct care to the patient and referrals. Direct care included changing dressings, bladder irrigations, changing Foley catheters, measurement of residual urine, perineal care, position change, back care, oral hygiene, exercise and massage of motion exercises, cleansing enemas, tracheostomy suctioning and tracheostomy care, care of dentures, applications of heat and other similar nursing activities. In conclusion almost 50% of (he sample indicated a need for continued nursing care at the time of discharge and for the patients in the sample who received home care there was a slight decrease in nursing needs but while the patients had chronic and debilitation problems there was ill decrease in ADL abilities or in quality of life. Further study needs Lo be done La increase the reliability and validity of the tool that was used to measure home health care needs. It is also recommended that study by done using a randomized sampling with a control group to compare patients who receive home care with those who do not.

대학간호교육목표의 시안개발 (Development of the Tentative Educational Objectives for the Nursing College)

  • This thesis is a study to develop the tentative objectives and present the professional and courses for the nursing college. The conclusions conducted by the purpose of this study are as follows. First, on the basis of the concepts of the nursing and the view-points of nursing education, the tentative goals for the nursing education are set up as follows. 1. To understand human being's life, dignity and their physical, mental, and social aspects. 2. To understand the basic concepts and the principles of human sciences, social sciences, natural sciences, and medical sciences. To apply their knowledges to nursing practices. 3. To diagnose and assess the problems of individual, family and community in terms of nursing practice. To develop the ability of planning, negotiation, management, and evaluation for the nursing education. 4. To develop appropriate knowlege, attitudes, and skills to promote the clients' health and treat their illness. 5. To accomplish all tasks effectively as a trained and qualified professional nurse through the endless studies. Second, the nursing areas and courses for the nursing college in terms of validity, Importance, continuity, relationship, utility and appropriateness are listed as follows. 1. To decide the number of the objectives in each course on the basis of the numbers of the syllabus and the level of its comprehensiveness, 5. To correct, supplement or eliminate the stated objectives by a professional or professional groups in that area.

제2형 당뇨병 환자의 혈당 비조절 관련 요인분석: 국민건강영양조사(2010-2012) 자료이용 (Factors Associated with Poor Glycemic Control among Patients with Type 2 Diabetes Mellitus: The Fifth Korea National Health and Nutrition Examination Survey (2010-2012))

  • Background: Glycemic control is an effective way to reduce the cardiovascular complications of diabetes, but more than half of the adults with diabetes in Korea are improperly controlling their glycemic levels. The purpose of this study is to identify the factors associated with poor glycemic control in type 2 diabetes patients. Methods: This study analyzed 1,261 subjects ${\geq}30years$ old diagnosed with type 2 diabetes who participated in the fifth Korean National Health and Nutrition Examination Survey (2010-2012). Poor glycemic control rates were defined as hemoglobin A1c (HbA1c) level ${\geq}7%$. To shed light on the causes of poor glycemic control, socio-demographics, diabetes severity, health status, and health behavior factors were adjusted and logistic regression was done. Results: Of the total 1,261 patients, 53.0% of patients with type 2 diabetes had HbA1c ${\geq}7%$. After running a logistic regression model, the odds ratio of poor glycemic control was higher in high school graduates than elementary school graduates; in people living in Chungcheong and Jeolla/Jeju than those living in Seoul; in the group with diabetes for over 5 years had diabetes less than 5 years; in a group with insulin and oral hypoglycemic agent treatment than non-treatment; in a group with hypertriglyceridemia than without hypertriglyceridemia; and in the group with slept less than 6 hours slept 7-8 hours. Conclusion: We need a comprehensive public health policy to reduce the poor glycemic control rates in type 2 diabetes patients. We should recognize the education levels, duration of diabetes, diabetes treatment, hypertriglyceridemia, and sleep duration were associated with poor glycemic control.

요통환자의 통증행위에 대한 조사연구 (The Pain Behavior of Patients with Back Pain)

  • The purposes of this study were; 1) to gather data relevant to demographic features. major main management practices, and the level of impairment of the activities of daily living (ADL) of patients with back pain, 2) to test the sensitivity of the Korean Pain Rating Scale and the Graphic Rating Scales, and 3) to identify indirect indicators of back pain by analysing pain related-behaviors. The level of pain was measured by Korean Pain Rating Scale(KPRS) and Graphic Rating Scales(GRS) developed by the reserchers. The GRS consists of two dimensions; the pain intensity (sensory) and unpleasantness (affective) measures. Of the 1,650 diagnosed back pain patients, from January 4 through June 30, 1987 by visiting outpatients' clinics of orthopedic and neurosurgical departments at 11 university hospitals in different districts of Korea, 330 men and women patients were self-selected by responding to the mailed questionnaires. The results were summarised as follows: Male exceeded female patients in number and onset of back pain were more prevalent in the age groups of 20s and the 30s. The average duration of suffering from the pain were 11 months, sixty three (19.1%) of the subjects retired from their jobs, one third(36.7%) have teen hospitalized for the treatment of back pain. In two thirds(64.8%) of the cases pain was characterized as lower back pain. The average sleep hour was 6.8 hours per 24 hours and the average rest hour during the day was 3.3 hours. The mean percentage of pain measured by GRS was higher than that of KPRS. The level of sensory intensity as well as the affective level of pain measured by KPRS and GRS were not highly correlated (sensory intensity r=0.4986, affective r=0.5029) which indicated low discriminative power. On the other hand, intercorrelation between sensory and affective dimension measured by KPRS and GRS showed moderate interrelation(r=0.7247; r=0.7899). One-third(32.5%) of the subjects complied with the hospital prescribed treatment while the other one-third(31.5%) depended on self-remedy and traditional practices, and the last one-third did not imply any pain management practices. The following 6 pain-related behaviors such as length of hospitalization, rest hour during day hours, varieties of pain management practice implied, number of pain sites, need for ADL and discomfort accompanied by ADL revealed to be important indicators of back pain. An investigation of sociodemographic features of patients with back pain in a larger context, i.e. with bigger number of respondents is recommended. Tests for construct validity of KPRS, i.e. factor analysis is further recommended.
