The authors studied 5 cases of idiopathic CNS hypersomnia who visited Division of Sleep Studies, Seoul National University Hospital in 1995. Detailed medical history was taken and nocturnal polysomnography(NPSG), multiple sleep latency test(MSLT) and human leukocyte antigen(HLA) typing were performed. Neither cataplexy nor hypnagogic hallucination was reported in all cases and in NPSGs, there were tendencies of increased sleep period time and decreased slow wave sleep time. In MSLT, all the subjects showed average sleep latencies less than 8 minutes without sleep-onset rapid eye movement period(SOREMP). In HLA typing, some correlation between idiopathic CNS hypersomnia and HLA DR4 was observed. In contrast to previous reports, overall treatment response with methylphenidate was remarkable. Therefore, the authors suggest that patients suspected of idiopathic CNS hypersomnia be actively evaluated and treated with rather optimistic perspective.
BACKGROUND/OBJECTIVES: The aim of this study was to evaluate the biological and sleep-promoting effects of combined ${\gamma}$-aminobutyric acid (GABA) and 5-hydroxytryptophan (5-HTP) using caffeine-induced sleepless fruit flies, ICR mice, and Sprague-Dawley rats. MATERIALS/METHODS: Video-tracking analysis was applied to investigate behavioral changes of Drosophila melanogaster. Pentobarbital-induced sleep test and electroencephalogram (EEG) patterns were used for analysis of sleep latency, duration, and quantity and quality of sleep in vertebrate models. RESULTS: Administration of combined GABA/5-HTP could significantly reverse the caffeine induced total distance of flies (P < 0.001). Also, individually administered and combined GABA/5-HTP significantly increased the total sleeping time in the caffeine-induced sleepless ICR mice (P < 0.001). In the caffeine-induced sleepless SD-rats, combined GABA/5-HTP showed significant differences in sleep quality between individual amino acid administrations (P < 0.05). CONCLUSIONS: Taken together, we identified inhibitory effects of combined GABA/5-HTP in locomotor activity, sleep quantity and quality in caffeine-induced sleepless models, indicating that combined GABA/5-HTP may be effective in patients with insomnia by providing sufficient sleep.

Objectives: We conducted a public health promotion program in Korean medicine for participants suffering from chronic insomnia in 2016~2017. A significant improvement effect was obtained through the 8-week herbal medicine treatment program. The object of this study is to report the results of this public health promotion program. Methods: Seventy patients diagnosed with chronic insomnia participated in the program, 49 of whom completed the program. Participants were treated with the herbal medicine for insomnia treatment for 8 weeks. A sleep diary was used to assess the total sleep time, sleep latency, and number of times the participants woke. The treatment satisfaction was then evaluated. Results: Through the 8-week herbal medicine treatment program, the total sleep time was significantly increased by $109.59{\pm}70.77$ minutes, the sleep latency was significantly decreased by $63.53{\pm}47.79$ minutes, and the number of times the participants woke was significantly decreased by $1.54{\pm}1.19$ times compared to the baseline (all, p=0.000). Among the 24 patients who were examined for the treatment satisfaction, the average score was 4.33 points of a total of 5 points. Six patients had adverse events. Except for one patient, the symptoms of other patients were mitigated by changing or discontinuing the herbal medicine. Conclusions: For patients with chronic insomnia, the 8-week herbal medicine treatment program was effective, with few side effects and high treatment satisfaction. This study has significance as this is the first report in Korea on the effect of a herbal medicine treatment program for chronic insomnia through Korean medicine public health promotion.

This sudy aimed to evaluate the effect of Punsimgieum-gamibang(分心氣飮加味方) on emotionality and sleep disorder. Punsimgieum(分心氣飮) has been used for insomnia, trembling, emotionality caused by strong mental stimulation and continuous stress in Oriental Medicine. And it was reported that Punsimgieum had anti-stress and anti-depression effect. Animals were devided into two groups : control group and Punsimgieum-gamibang group.Emotionality was tested in open-field with five indexes : walking, rearing, grooming, excretion and start latency. For the study of sleep disorder, after two groups had been given caffeine into abdomen, activity amount in animals was assessed for daytime and then the average percentage of sleep in two groups was calculated.The following results were observed.1. In peripheral and central partition, meditaion group walked more than control group. The difference between two groups was statistically significant.2. In rearing and grooming frequency, there was difference between two groups, but it was not statistically significant. 3. In grooming. excretion and start latency. there was no difference between two groups.4. There was no difference in the average percentage of sleep between two groups.
The sleep homeostatic response significantly affects the state of anesthesia. In addition, sleep recovery may occur during anesthesia, either via a natural sleep-like process to occur or via a direct restorative effect. Little is known about the effects of isoflurane anesthesia on sleep homeostasis. We investigated whether 1) isoflurane anesthesia could provide a sleep-like process, and 2) the depth of anesthesia could differently affect the post-anesthesia sleep response. Nine rats were treated for 2 hours with $ad$$libitum$ sleep (Control), sleep deprivation (SD), and isoflurane anesthesia with delta-wave- predominant state (ISO-1) or burst suppression pattern-predominant state (ISO-2) with at least a 1-week interval. Electroencephalogram and electromyogram were recorded and sleep-wake architecture was evaluated for 4 hours after each treatment. In the post-treatment period, the duration of transition to slow-wave-sleep decreased but slow wave sleep (SWS) increased in the SD group, but no sleep stages were significantly changed in ISO-1 and ISO-2 groups compared to Control. Different levels of anesthesia did not significantly affect the post-anesthesia sleep responses, but the deep level of anesthesia significantly delayed the latency to sleep compared to Control. The present results indicate that a natural sleep-like process likely occurs during isoflurane anesthesia and that the post-anesthesia sleep response occurs irrespective to the level of anesthesia.

Purpose: To determine the type and degree of effect that a hot footbath has on sleep quality and fatigue level in older Korean adults. Methods: A non-equivalent control group quasi-experimental design was used. Fifty participants from a long-term care facility in Kwangju, South Korea, were randomly selected and assigned to two groups: experimental group (27 participants) and control group (23 participants). The participants in the experimental group received hot footbaths in a temperature-controlled water tub of $42^{\circ}C$. They soaked their feet up to 20cm above the ankles for 30 minutes before going to bed for 3 nights. Actigraphy was used to measure their sleep patterns. The data were analyzed using the SAS program. Results: After the intervention, the total sleep satisfaction rate of the participants increased, while sleep latency and fatigue decreased significantly in the experimental group compared to the control group. Conclusion: The study results suggest that hot footbaths are beneficial for older Korean adults in enhancing sleep quality as well as reducing fatigue. Therefore, hot footbaths are recommended as a nursing intervention to improve sleep quality and to reduce fatigue in older Korean adults.
Purpose: In this study rest-activity rhythm, sleep pattern and quality of life of patients with restless legs syndrome were compared with those of a normal group. Methods: The participants in this study were 36 patients with restless legs syndrome diagnosed by a neurologist and 36 participants in the normal group. An actigraph, sleep diary, Pittsburgh Sleep Quality Index and Insomnia Severity Index scale were used as measurement tools for the study. Chi-square test, Lamda test, t-test and Kendall's correlation with SPSS 12.0 program were used to analyze the data. Results: Patients with restless legs syndrome had a higher rest-activity rhythm curve of Least 5 hr's activity(L5) and Most 10 hr's activity(M10) than those of normal group and sleep problems included decreased sleep efficiency and increased sleep latency, wake time and number of awakenings. The scores for the subscales of quality of life in patients with restless legs syndrome were lower than the normal group for general health, physical functioning, role limitations due to emotional problems, role limitation due to physical problems, social functioning, bodily pain, vitality and mental health. Conclusion: The results suggest that further studies are needed to identify rest-activity rhythm according to symptom severity and to develop nursing interventions which consider rest-activity rhythm.
Morningness-Eveningness Questionnaire (MEQ) of Horne & Ostberg(1976) was translated into a Korean version, and MEQ and life habit inventory was administrated with approximately 900 university students. The distribution of MEQ score was normal. Arising time and bedtine for weekdays and holidays came in the order of morning type, intermediate and evening-type. And the deviation for the evening-type was greatest. Also, the deviation of sleep length for the evening-type was greater than the morning- type. There were tendencies that the bed time and arising time of the male were slightly later than those of the female, and the sleep length of the male was a little longer than that of the female. It also appeared that the arising time was later in the male who completed the military services than those who did not in the chronotype. And the sleep length of the non-served male was longer than that of the served one. Such tendencies noticeably appeared especially the evening-type. The evening-type was subject to heavy variability in bedtime. arising time, and sleep length. The sleep latency of the evening- type was longer and mood on arising was worse. And the ratio of nap or light sleep was higher in these subjects than in others. These results suggested that evening-type students had more irregular and/or flexible sleep-wake habits than the morning-type.
Purpose: The purpose of this study was to investigate the relationships among quality of sleep, depression, late-life function and disability in community-dwelling older women with urinary incontinence. Methods: A stratified random sampling method was conducted to recruit participants from May 10 to August 17, 2007. Data were collected by questionnaires, which were constructed to include lower urinary tract symptoms, quality of sleep, depression, and late-life function and disability in 128 community-dwelling older women. Results: The major findings of this study were as follow: 1) 56.3% of participants belonged to urinary incontinence group. 2) There were significant relationships between depression and sleep latency, sleep duration, daytime dysfunction, quality of sleep, function component, frequency dimension, and limitation dimension. 3) Depression was significantly associated with frequency dimension, limitation dimension in capability which explained 44% of variance in depression. Conclusion: These results may contribute to a better understanding of sleep quality, depression, late-life function and disability in the community-dwelling older women with urinary incontinence. Therefore, health programs for prompting older women's health should be planned based on results of the study.
Perillae Herba has been traditionally used for the sedation in the oriental countries. Therefore, this study was conducted to determine whether Perillae Herba ethanol extract (PHEE) enhances pentobarbital-induced sleeping behaviors in animals. In addition, the possible mechanisms are demonstrated. PHEE (12.5, 25 and 50 mg/kg. p.o.) reduced the locomotor activity in mice. PHEE reduced sleep latency and augmented the total sleep time in pentobarbital (42 mg/kg, i.p.)-induced sleep in mice. Furthermore, the number of sleeping mice treated with sub-hypnotic pentobarbital (28 mg/kg, i.p.) increased. PHEE (50 mg/kg. p.o.) decreased the sleep/wake cycles and wakefulness, and increased total sleeping time and NREM sleep in electroencephalogram (EEG) of rats. In addition, PHEE (0.1, 1.0 and $10{\mu}g/ml$) increased the intracellular $Cl^-$ level through the GABA receptors in the hypothalamus of rats. Moreover, the protein of glutamate decarboxylase (GAD) was overexpressed by PFEE. It was found that PHEE enhanced pentobarbital-induced sleeping behaviors through $GABA_A-ergic$ transmissions.
