The Effect of Mandibular Protrusion on Dynamic Changes in Oropharyngeal Caliber (하악의 전방이동이 구인두 내경의 동적 변화에 미치는 영향)

  • The purpose of this study was to determine the sites of narrowing/obstruction and to measure the regional severity of narrowing through the evaluation of dynamic changes in upper-airway of healthy subjects. The selected 9 subjects were proved not to have any sleep-related disorder such as snoring or obstructive sleep apnea through clinical examination, radiological examination, sleep study with a portable recording system. Afterward, the Electron Beam Tomography was performed during the waking and sleeping state of subjects, with their mandible in resting and protruded position. Intravenous injection of Dormicum$^{(R)}$ was used for the induction of sleep. The maximum and minimum cross-sectional areas at each airway level during tidal ventilation were measured and the Collapsibility Index for each level of cross-section was also computed. In a comparison with results under variable conditions, the result was showed that the significant difference between each airway level divided with upper, middle, lower region of upper airway is not observed in the average minimum cross-sectional areas and Collapsibility Index. The significant difference only between in wake and sleep state was observed in the average minimum cross-sectional area at the lower region. Also, in wake state, the significant difference between resting and protrusive position of mandible for the average minimum was also observed in cross-sectional area at middle region. In sleep state, no significant difference between resting and protrusive position of mandible was observed in cross-sectional area and the Collapsibility Index.

Postoperative Analgesia of Intrathecal Morphine and Intramuscular Caroverine and Tiaprofenate in Transurethral Resection of the Prostate (지주막하 Morphine과 근주 Caroverine과 Tiaprofenate의 경요도 전립선 절제술후 진통효과)

  • Background: Intrathecal injection of morphine is widely used in the management of postoperative pain because it provides long-lasting analgesia. Intramuscular caroverine and tiaprofenate are used to produce postoperative pain relief. This study was designed to evaluate the analgesic efficacy and quality of sleep achieved with intrathecal morphine and those of intramuscular caroverine and tiaprofenate in transurethral resection of the prostate (TURP). Methods: Forty patients undergoing elective TURP were randomly allocated into 2 groups as follows: Group M (n=20); 0.25 mg of morphine hydrochloride mixed in 7.5 mg of 0.5% hyperbaric bupivacaine was administered at the time of induction of spinal anesthesia. Group S (n=20); 7.5 mg of 0.5% hyperbaric bupivacaine was administered intrathecally and caroverine and tiaprofenate intramuscularly at every 8 hr and 12hr postoperatively for management of postoperative pain. We evaluated the analgesic efficacy with visual analog scale (VAS), quality of sleep, and side effects. Results: VAS at 6, 12 and 24 hours after operation were significantly less (p<0.01) in the group M than in the group S. Group M was superior to group S with respect to quality of sleep (p<0.01). In the group M, the incidence of nausea was 30% (6/20) and that of pruritus was 35% (7/20) and clinical respiratory depression did not occur. Conclusions: Intrathecal 0.25 mg morphine provides good postoperative analgesic effect. but intramuscular caroverine and tiaprofenate does not.

THE EFFECTIVENESS OF TWO SEDATIVE REGIMEN USED FOR CHILD PATIENT WHO FAILED TO ORAL CHLORAL HYDRATE (Chloral hydrate 경구 진정에 실패한 소아환자를 대상으로 사용한 두 가지 진정요법의 효과)

  • Chloral hydrate is one of the most widely used sedative agents to control the difficult-to-treat young age group in the dental clinic. We are often frustrated to see the patient still awake and cry with agitation even after far more than the normal onset time of Chloral hydrate. In such a case, the patient has to be rescheduled for another sedation visit with different agents and/or routes which greatly disappoints the guardians. This study was designed to test the efficacy of one sedative regimen that can possibly help the clinician complete scheduled treatment without postponement. We have tried sleep induction with mixed gas of Enflurane(2vol%) and $N_2O(50%)-O_2(50%)$ for $60\sim120$ seconds to 35 patients of those who failed to respond properly to the dose(70mg/kg)of oral Chloral hydrate. The Result of this regimen was compare to those of two oral regimen of Chloral hydrate/Hydroxyzine and Chloral hydrate only Analyses of result on vital signs and behavior pattern were performed. The outcome of the study suggest that sleep induction by a short inhalation of low dose of $Enflurane/N_2O-O_2$ provide dentist with suitable condition for the completion of scheduled treatment in the patient who failed to oral Chloral hydrate. Evidence of adverse effect was not detected or reported during and/or after the procedures.

  • Astrocytes are the most abundant cells in the central nervous system that play roles in maintaining the blood-brain-barrier and in neural injury, including cerebral malaria, a severe complication of Plasmodium falciparum infection. Prostaglandin (PG) $D_2$ is abundantly produced in the brain and regulates the sleep response. Moreover, $PGD_2$ is a potential factor derived from P. falciparum within erythrocytes. Heme oxygenase-1 (HO-1) is catalyzing enzyme in heme breakdown process to release iron, carbon monoxide, and biliverdin/bilirubin, and may influence iron supply to the P. falciparum parasites. Here, we showed that treatment of a human astrocyte cell line, CCF-STTG1, with $PGD_2$ significantly increased the expression levels of HO-1 mRNA by RT-PCR. Western blot analysis showed that $PGD_2$ treatment increased the level of HO-1 protein, in a dose- and time-dependent manner. Thus, $PGD_2$ may be involved in the pathogenesis of cerebral malaria by inducing HO-1 expression in malaria patients.

Biofeedback Treatment for Insomnia (불면증의 바이오피드백 치료)

  • Insomnia is associated with numerous psychiatric and physical conditions, and hyperarousal is known to play an important role in the development of insomnia. There are a lot of limitations to use hypnotic medications for the treatment of insomnia. As psychological factors can greatly affect the development and progress of insomnia, several non-pharmacological treatment methods have been used for insomnia. Biofeedback is effective in the treatment of insomnia and its treatment effect lasts considerably long. Biofeedback together with relaxation techniques can reduce levels of arousal in insomnia patients so that they are effective for induction and maintenance of sleep. In conclusion, biofeedback treatment seems to be very helpful for insomnia patients who show high levels of arousal and have limitations in the use of hypnotic medications.

Two Cases of Acute Renal Failure due to Rhabdomyolysis Complicating Doxylamine Succinate Intoxication (독시라민 과다복용에 의한 횡문근 용해 및 급성 신부전 2례)

  • Doxylamine succinate is an antihistamine used primarily as a sleep-induction. It can be gotten without a doctor's prescription in Korea, so it' s overdoses were frequently encountered. There were several reports that the overdoses of doxylamine might cause rhabdomyolysis, but few cases have been reported that it is related to acute renal failure (ARF). In cases that ARF occur, most of them are not severe enough to require hemodialysis. We experienced two cases of severe rhabdomyolysis complicating ARF after doxylamine overdose and treated with hemodialysis. Clinicians should be aware of the potentially lethal complications of rhabdomyolysis in patients who ingest doxylamine succinate and the needs for prompt intervention and careful assessment of renal function.

  • With the development of IT convergence technology, autonomous driving has gradually developed; however, the vehicle is still operated by the driver, who should always be in good health - but sometimes, this is not the case. It is especially dangerous to drive when drowsy, and unable to fully concentrate on driving, such as when taking certain medicines, or through fatigue. Drowsy driving is at least eight times more dangerous than normal driving, and as dangerous as drunk driving. Previous research has looked at technology to detect drowsiness, in order to wake up drivers when necessary, or to safely stop the vehicle. Furthermore, many studies have been conducted to find out when drowsiness occurs. However, it is more desirable for the driver to take sufficient rest during a break, in order to be able to continue to focus and drive. In other words, it is important to maintain a normal state before drowsiness. In this study, we introduce a sound source to increase driver concentration and prevent drowsiness, another that can improve the quality of sleep, and a system that produces these sound sources. The proposed system has a noise reduction effect of about 15 dB. We have confirmed that the proposed sound induces an EEG of the desired form.

  • Customized services to a sleep induction for better sleepcare are more effective because of different satisfaction levels to users. The EOG data measured at the frontal lobe when a person blinks his eyes can be used as biometric data because it has different values for each person. The accuracy of measurement is degraded by a noise source, such as toss and turn. Therefore, it is necessary to analyze the noisy data and remove them from normal EOG by filtering. There are low-pass filtering and high-pass filtering as filtering using a frequency band. However, since filtering within a frequency band range is also required for more effective performance, we propose a machine learning model for the filtering of EOG data in this paper as the second filtering method. In addition, optimal values of parameters such as the depth of the hidden layer, the number of nodes of the hidden layer, the activation function, and the dropout were found through experiments, to improve the performance of the machine learning filtering model, and the filtering performance of 95.7% was obtained. Eventually, it is expected that it can be used for effective user identification services by using filtering model for EOG data.

  • Though, these days there are increasing many patient of insomnia, there was not considered literatual study on insomnia. So, the result of consideration about cause and process of insomnia from thirty kinds of literatures, are as follows. 1. Early literature like Huangjaenankyung(黃帝內經), Sanghanron(傷寒論) reffered to insomnia as accompanying symptom. on the other hand, Myung(明), Chung(淸)'s literatures reffered to that as chief symptom or distinguished chief symptom from accompanying symptom. 2. There were tendency of deductive expanding of various masters and tendency of induction of simple theory that was based on Huangjaenankyung(黃帝內經). 3. Huangjaenankyung(黃帝內經) showed basic process of the sleep disorder that 'exhausting of Yin and excess of Yang (陰盡陽盛)', protecting energy does not invade Yin portion(陽氣不入於陰). And Huangjaenankyung(黃帝內經) showed cause of insomnia that deficiency of vital energy and blood, imbalanced of spleen and stomach, a fever as a invasion in the outside, lung system's disease. This became a basic cause and process of the sleep disorder in ancient period. 4. Sanghanron(傷寒論) occurred to insomnia as accompanying symptom in progress, remedy of a fever invaded outside, Kumkyeyoriak showed as origination in weakness, fatique, various diseases. Out of that, there is a significance in description of insomnia from some disease like histery or neurosis. 5. Jaebyungwonhuron(諸病源候論), Chunkumbang(千金方), Kukbangseo(局方書.) occured to a fire of heart(心慤) and a deficiency of heart and gall bladder(心膽虛) in defails. Insomnia is caused by agony of seven emotion, delivering of a child, are similar to insomnia is caused by psychologic disorders. Injaesanghanyusu(仁劑傷寒類書) occured to exhausting of Yin and excess of Yang (陰盡陽盛), imbalanced of stomach(胃不和) invasion of coldness(傷寒) are brought a conclusion of assumption of sap(津液耗損) brought about unreturn of yin energy. 6. Manbyunghuechum(萬病回春) in Myung period (明代) made much of portion of phlegm's influence about spiritual function. Kyungakjunsu(景岳全書) valued much of treatment divided according to excess and deficiency. Junginmaekchi(證因脈治) occurred to concept of pyorihesil(表裏虛實), Dongyibokam(東醫寶鑑) synthesised various theories. 7. Hyuljungron(血證論), Byunjungkimun(辨證奇聞), Suksilbirok(石室秘錄) made much of surprisemeni(驚恐) Consideration(思慮), liver's dryness(肝燥) is caused by liver's weakness(肝虛), imbalance of haert -kidney(心腎不交), seven emotion(七情). Especially, ftyujungchijae(類證治載) said that heart, liver, gall-bladder, kidney, surprisment, consideration baought to a conclusion of inbalance of Yang and Yin (陽不交陰). There is a tendency in literature mostly that literature showed separation of insomnia as a chief symptom. 8. These days there are increasing many patient of insomnia. So, it is needed to study about insomnia as a psychologic disease. Saying in conclusion, it is needed that we have to recognize in modern style based on ancient style of cause and process of insomnia. It is regarded to study about insomnia definitely and experimently.

The Role of Transglutaminase-2 in Fibroproliferation after Lipopolysaccharide-induced Acute Lung Injury (리포다당질로 유도된 급성 폐손상 후 섬유화증식에서 Transglutaminase-2의 역할)

  • Background: Transglutaminase-2 (TG-2) has been reported to play an important role in the process of fibrosis. However, TG-2 studies on fibroproliferation of acute lung injury (ALI) are absent. The purpose of this study was to investigate the role of TG-2 in the fibroproliferation of lipopolysaccharide (LPS)-induced ALI. Methods: The male C57BL/6 mice of 5 weeks age were divided into 3 groups; control group (n=30) in which $50{\mu}L$ of saline was given intratracheally (IT), LPS group (n=30) in which LPS 0.5 mg/kg/$50{\mu}L$ of saline was given IT, and LPS+Cyst group treated with intraperitoneal 200 mg/kg of cystamine, competitive inhibitor of TG-2, after induction of ALI by LPS. TG-2 activity and nuclear factor $(NF)-{\kappa}B$ were measured in lung tissue homogenate. Tumor necrosis factor (TNF)-${\alpha}$, interleukin (IL)-$1{\beta}$, IL-6, myeloperoxidase (MPO), and transforming growth factor (TGF)-${\beta}1$ were measured using bronchoalveolar lavage fluids. Histopathologic ALI score and Mallory's phosphotunistic acid hematoxylin (PTAH) for collagen and fibronectin deposition were performed. Results: The TG-2 activities in the LPS group were significantly higher than the control and LPS+Cyst groups (p<0.05). The TNF-${\alpha}$ and IL-$1{\beta}$ concentrations and $NF-{\kappa}B$ activity were lower in the LPS+Cyst group than the LPS group (p<0.05). The LPS+Cyst group showed lower MPO, ALI score, TGF-${\beta}1$ concentration, and Mallory's PTAH stain than the LPS group, but the differences were not significant (p>0.05). Conclusion: Inhibition of TG-2 activity in the LPS-induced ALI prevented early inflammatory parameters, but had limited effects on late ALI and fibroproliferative parameters.
