Hyaluronidase: An overview of its properties, applications, and side effects

  • Hyaluronidase, an enzyme that breaks down hyaluronic acid, has long been used to increase the absorption of drugs into tissue and to reduce tissue damage in cases of extravasation of a drug. With the increasing popularity of hyaluronic acid filler, hyaluronidase has become an essential drug for the correction of complications and unsatisfactory results after filler injection. For this reason, when performing procedures using hyaluronic acid filler, a sufficient knowledge of hyaluronidase is required. In order for hyaluronidase to dissolve a hyaluronic acid filler, it must interact with its binding sites within the hyaluronic acid. The reaction of a filler to hyaluronidase depends on the hyaluronic acid concentration, the number of crosslinks, and the form of the filler. Hyaluronidase is rapidly degraded and deactivated in the body. Therefore, in order to dissolve a hyaluronic acid filler, a sufficient amount of hyaluronidase must be injected close to the filler. If the filler is placed subcutaneously, injection of hyaluronidase into the filler itself may help, but if the filler is placed within a blood vessel, it is sufficient to inject hyaluronidase in the vicinity of the vessel, instead of into the filler itself. Allergic reactions are a common side effect of hyaluronidase. Most allergic reactions to hyaluronidase are local, but systemic reactions may occur in infrequent cases. Since most allergic responses to hyaluronidase are immediate hypersensitivity reactions, skin tests are recommended before use. However, some patients experience delayed allergic reactions, which skin tests may not predict.

Preserved Respiratory Function after Reconstruction of a Large Chest Wall Defect

  • A case report of a patient who developed radiation-induced sarcoma in the left chest wall is presented. The patient had partial mastectomy and adjuvant radiation therapy (total dose, 5,220 cGy) and chemotherapy. Five years later, she visited with rapidly growing mass with central ulceration in the irradiated chest wall. The mass was diagnosed as malignant fibrous histiocytoma. The chest wall mass resected en bloc ($23{\times}18cm$) including five consecutive ribs. After the defected thoracic cage was reinforced using a polytetrafluoroethylene patch, omental flap and split thickness skin graft was done for soft tissue coverage. We applied negative pressure wound closer system for effective suction of omeantal exudate. The wound healed without complications. The patient suffered no perioperative pulmonary complications. Pulmonary function tests showed no significant changes. Each of Gore-Tex, omental flap, negative pressure wound therapy and skin graft is widely used method. However, If these methods are used in combination, we can reconstruct the large defect of chest wall including multiple ribs without any repiratory function problems.

  • Necessity of removable soil nailing has been arisen in the soil nailing system because of problem such as civil petition for geotechnical environment and invasion of the ground boundary line and payments for use besides geotechnical engineering. Removable soil nailing system is improved soil nailing system that fixed socket arranged in boring hole for increasement of skin friction. In this study, field pull-out tests are carried out more 4 times considering installed distance of fixed socket and analyze skin friction behavior characteristics in fixed socket through analysis of measurements of strain gauge attached to fixed socket. Also, to evaluate application for wedge type removable soil nailing system analyzing displacement aspects through field pull-out tests by trial construction.

모래지반에 타입된 모형 개단강관 말뚝의 지지력 분석 (Bearing Capacity of Model Open -Ended Steel Pipe Pile Driven into Sand Deposit)

  • 말뚝이 설치되는 동안과 하중이 재하되는 동안에 유발되는 관내토의 거동과 관내토가 개단 말뚝의 지지력에 미치는 영향, 그리고 관내토의 하중전이 메카니즘 등을 알아보기 위하여, 개단말뚝의 단면부에 작용하는 지지력과 관내토에 의한 지지력 및 외주면마찰력을 분리하여 측정할 수 있도록 제작된 모형 개단강관말뚝과 압력조절이 가능한 토조를 이용하여 모래지반을 대상으로한 모형말뚝실험을 실시하였다. 실험결과로부터, 개단말뚝의 폐색정도는 폐색길이비(PLR)보다는 특정회수율(${\gamma}$)에 의하여 더 정화히 성의될 수 리음을 알 수 있으며, 내주면마찰력의 대부분은 말뚝 선단부로부터 말뚝내경의 3배까지의 영역에 존재하는 관내토에 의하여 발생되는 것으로 나타났다. 또한 전체 지지력에서 내주면마찰력이 차지하는 비율은 외주면마찰력이 차지하는 비율보다 상당히 크기 때문에, 개단말쪽의 지지력 산정시 내주면마찰력은 필수적으로 고려되어야 한다.

DEVELOPMENT OF POLYETHOXYLATED RETINAMIDE AS AN ANTI-AGING AGENT

  • A novel retinol derivative, polyethoxylated retinamide(Medimin A) was synthesized, as an anti-aging agent. Collagen synthesis, skin permeation, stability, and toxicity of Medimin A were evaluated and compared with those of retinol and retinyl palmitate. In vitro collagen synthesis was evaluated by quantitative assay of $[^3H]-proline$ incorporation into collagenase sensitive protein in fibroblast cultures. For in vitro skin permeation experiments, Franz diffusion cells(effective diffusion area: 1,766 $cm^2$) and the excised skin of female hairless mouse aged 8 weeks were used, The stabilities of retinoids were evaluated at two different temperature($25^{\circ}C\;and\;40^{\circ}C$) and under UV in solubilized state and in O/W emulsion. To estimate the safety, acute oral toxicity, acute dermal toxicity, primary skin irritation, acute eye irritation and human patch test were performed. The effect of Medimin A on collagen synthesis was similar to that of retinol. The skin permeability of Medimin A was higher than those of retinol and retinyl palmitate. The Medimin A was more stable than retinol and retinyl palmitate. Medimin A was nontoxic in various toxicological tests. These results suggest that Medimin A would be a good anti-aging agent for enhancing bioavailability and stability.

니코틴 패취제로부터 니코틴의 피부투과를 반영하는 방출시험법의 설정 (Establishment of a Release Test Reflecting in vitro Skin Permeation of Nicotine from Commercial Patches)

  • Various release test methods have been applied for the evaluation of nicotine release in vitro from commercial patches. However, whether and how the release data reflect the permeation of nicotine across the skin, is not fully elucidated. To predict in vivo bioavailability from in vitro release tests, correlation between in vitro release and in vitro skin permeation was assessed in the present study. Release of nicotine from three commercial patches was measured for 24 hours under nine experimental conditions which were classified depending on the apparatus (i.e., paddle over disk, cylinder and reciprocating holder) and dissolution media (i.e., phosphate buffer pH 7.4, water and the 1 % phosphoric acid pH 1.5). In vitro permeation of nicotine from the patches across the human cadaver skin was also measured using a diffusion cell. The release of nicotine was better explained by the Higuchi's equation rather than by the first order rate equation. Correlation between the release rate and the in vitro skin permeation differed among the patches. However, in general, the cylinder method, in which water is used as a dissolution medium, showed the highest correlation among the nine release test conditions.

충주지역 주민 피부 알레르기 반응율에 관한 연구 (A Study on Allergen Skin Test Prevalence Among the Residents of Chungju City as a Non-industrial Area)

  • Objectives: The main purpose of this study is to produce background data which can be compared with several large-scale industrial complex regions. Methods: The framework of this study was designed to evaluate and monitor health effects associated with low-level and long-term exposure to environmental pollutants. In this study, 1,009 local residents were recruited in the city of Chungju using personal questionnaires, medical check-ups, and allergen skin-prick tests for 12 common allergens. The prevalence rate of skin allergens was reviewed together with that from large-scale industrial areas. Results: The diagnosis prevalence and medical treatment prevalence of asthma was 5.6% and 1.7%, respectively. Allergic rhinitis was 25.4% and 16.7%, allergic dermatitis was 16.6% and 9.9% and allergic conjunctivitis was 18.7% and 7.9%, respectively. These results were similar with those of industrial complexes. The prevalence of allergen skin-prick test was 16.4% and the most common allergen was dust mites (27.9-31.6%). This result was lower than those of the industrial complexes (22.5-39.9%) and Gangneung (18.2-24.7%). Conclusion: Further study is required to find the cause of the regional differences in skin prick prevalence.

SIP말뚝의 주면마찰특성 및 주면 경계요소의 수치모델에 관한 연구 (A Study on the Skin Friction Characteristics of SIP and Numerical Model of the Interface Between SIP and Soils)

  • 최근에 건설공사에서 발생하는 환경문제에 대한 관심이 고조되는 가운데 저소음, 저진동 공법인 매입말뚝공법이 건설현장에 많이 적용되고 있다. 이러한 건설여건의 변화에 부흥하여 국내에서는 지반을 굴착하고 시멘트 페이스트를 주입한 후 기성말뚝을 삽입하는 SIP공법(Soil-Cement Injected Precast Pile)의 적용이 늘고 있으나, 현재 국내 지반조건에 맞는 고유의 관계식이 없는 실정이다. 이와 같은 문제해결에 기여하기 위해, 본 연구에서는 SIP말뚝 주면과 지반사이의 저항력을 직접전단시험으로 측정하여 그 경향을 분석하였다. 직접전단시험 결과 SIP말뚝의 주면 저항력 성분이 수직하중에 가장 크게 영향을 받는다는 것을 밝혀내고 국내 주요 토질인 SM, SC 토질에서 이를 인자로 한 최대주면저항력 산정식을 제안하였다. 또한 말뚝 표면과 주면 지반과의 전단저항 거동 특성을 수치적으로 정확히 묘사하기 위해 Duncan(1971)의 쌍곡선 모델식과 새로운 비선형 수치모델식에 따른 계수산정식을 제안하였다.

Dosimetric comparison of axilla and groin radiotherapy techniques for high-risk and locally advanced skin cancer

  • Purpose: Radiation therapy targeting axilla and groin lymph nodes improves regional disease control in locally advanced and high-risk skin cancers. However, trials generally used conventional two-dimensional radiotherapy (2D-RT), contributing towards relatively high rates of side effects from treatment. The goal of this study is to determine if three-dimensional conformal radiation therapy (3D-CRT), intensity-modulated radiation therapy (IMRT), or volumetric-modulated arc therapy (VMAT) may improve radiation delivery to the target while avoiding organs at risk in the clinical context of skin cancer regional nodal irradiation. Materials and Methods: Twenty patients with locally advanced/high-risk skin cancers underwent computed tomography simulation. The relevant axilla or groin planning target volumes and organs at risk were delineated using standard definitions. Paired t-tests were used to compare the mean values of several dose-volumetric parameters for each of the 4 techniques. Results: In the axilla, the largest improvement for 3D-CRT compared to 2D-RT was for homogeneity index (13.9 vs. 54.3), at the expense of higher lung $V_{20}$ (28.0% vs. 12.6%). In the groin, the largest improvements for 3D-CRT compared to 2D-RT were for anorectum $D_{max}$ (13.6 vs. 38.9 Gy), bowel $D_{200cc}$ (7.3 vs. 23.1 Gy), femur $D_{50}$ (34.6 vs. 57.2 Gy), and genitalia $D_{max}$ (37.6 vs. 51.1 Gy). IMRT had further improvements compared to 3D-CRT for humerus $D_{mean}$ (16.9 vs. 22.4 Gy), brachial plexus $D_5$ (57.4 vs. 61.3 Gy), bladder $D_5$ (26.8 vs. 36.5 Gy), and femur $D_{50}$ (18.7 vs. 34.6 Gy). Fewer differences were observed between IMRT and VMAT. Conclusion: Compared to 2D-RT and 3D-CRT, IMRT and VMAT had dosimetric advantages in the treatment of nodal regions of skin cancer patients.

Time effect of pile-soil-geogrid-cushion interaction of rigid pile composite foundations under high-speed railway embankments

  • Centrifuge model tests were used to simulate pile-raft composite foundation and pile-geogrid composite foundation with different pile spacing for researching the time effect of negative skin friction of rigid piles in high-speed railways. The research results show that the negative skin friction has a significant impact on the bearing capacity of composite foundation. Pile-raft composite foundation has higher bearing capacity compared to pile-geogrid composite foundation to reduce the effect of negative skin friction on piles. Both the foundation settlement and negative skin friction have significant time effect. The distribution of skin friction can be simplified as a triangle along the pile. The neutral point position moves deeper in the postconstruction stage at larger pile spacing. For pile-geogrid composite foundation, the setting of pile-cap affects the position of neutral point in the post-construction stage. Reinforced cushion with geotextile may promote the better performance of cushion for transmitting the loads to piles and surrounding soils. Arching effect in the cushion of the composite foundation is a progressive process. The compression of the rigid piles contributes less than 20% to 25% of the total settlement while the penetration of the piles and the compression of the bearing stratum below the pile tips contribute more than 70% of the total settlement. Some effective measures to reduce the settlement of soils need to be taken into consideration to improve the bearing capacity of pile foundation.
