  • Authors investigated the pathogenesis and treatment of dennatosurgical diseases in the ImJeungJiNamUiAn(臨證指南醫案). 1. The symptoms and diseases of dermatosurgery were as follows; 1) BanSaJinRa(반사진라) : eczema, atopic dermatitis, seborrheic dermatitis, psoriasis, lichen planus, pityriasis rosea, hives, dermographism, angioedema, cholinergic urticaria, urticaria pigmentosa, acne, milium, syringoma, keratosis pilaris, discoid lupus erythematosus, hypersensitivity vasculitis, drug eruption, polymorphic light eruption, rheumatic fever, juvenile rheumatoid arthritis(Still's disease), acute febrile neutrophilic dermatosis(Sweet's syndrome), Paget's disease, folliculitis, viral exanthems, molluscum contagiosum, tinea, tinea versicolor, lymphoma, lymphadenitis, lymphangitis, granuloma annulare, cherry angioma 2) ChangYang(瘡瘍) : acute stage eczema, seborrheic dermatitis, stasis ulcer, intertrigo, xerosis, psoriasis, lichen planus, ichthyosis, pityriasis rosea, rosacea, acne, keratosis pilaris, dyshidrosis, dermatitis herpetiformis, herpes gestationis, bullae in diabetics, pemphigus, lupus erythematosus, fixed drug eruption, erythema multiforme, toxic epidermal necrolysis, toxic shock syndrome, staphylococcal scaled skin syndrome, scarlet fever, folliculitis, impetigo, pyoderma gangrenosum, tinea, candidiasis, scabies, herpes simplex, herpes zoster, chicken pox, Kawasaki syndrome, lipoma, goiter, thyroid nodule, thyroiditis, hyperthyroidism, thyroid cancer, benign breast disorder, breast carcinoma, hepatic abscess, appendicitis, hemorrhoid 3) Yeok(疫) : scarlet fever, chicken pox, measles, rubella, exanthem subitum, erythema infectiosum, Epstein-Barr virus infection, cytomegalovirus infection, hand-foot-mouth disease, Kawasaki disease 4) Han(汗) : hyperhidrosis 2. The pathogenesis and treatment of dermatosurgery were as follows; 1) When the pathogenesis of BalSa(발사), BalJin(發疹), BalLa(발라) and HangJong(項腫) are wind-warm(風溫), exogenous cold with endogenous heat(外寒內熱), wind-damp(風濕), the treatment of evaporation(解表) with Menthae Herba(薄荷), Arctii Fructus(牛蒡子), Forsythiae Fructus(連翹) Mori Cortex(桑白皮), Fritillariae Cirrhosae Bulbus(貝母), Armeniaoae Amarum Semen(杏仁), Ephedrae Herba(麻黃), Cinnamomi Ramulus(桂枝), Curcumae Longae Rhizoma(薑黃), etc can be applied. 2) When the pathogenesis of BuYang(부양), ChangI(瘡痍) and ChangJilGaeSeon(瘡疾疥癬) are wind-heat(風熱), blood fever with wind transformation(血熱風動), wind-damp(風濕), the treatment of wind-dispelling(疏風) with Arctii Fructus(牛蒡子), Schizonepetae Herba(荊芥), Ledebouriellae Radix(防風), Dictamni Radicis Cortex(白鮮皮), Bombyx Batrytioatus(白??), etc can be applied. 3) When the pathogenesis of SaHuHaeSu(사후해수), SaJin(사진), BalJin(發疹), EunJin(은진) and BuYang(부양) are wind-heat(風熱), exogenous cold with endogenous heat(外寒內熱), exogenous warm pathogen with endogenous damp-heat(溫邪外感 濕熱內蘊), warm pathogen's penetration(溫邪內陷), insidious heat's penetration of pericardium(伏熱入包絡), the treatment of Ki-cooling(淸氣) with TongSeongHwan(通聖丸), Praeparatum(豆?), Phyllostachys Folium(竹葉), Mori Cortex(桑白皮), Tetrapanacis Medulla(通草), etc can be applied. 4) When the pathogenesis of JeokBan(적반), BalLa(발라), GuChang(久瘡), GyeolHaek(結核), DamHaek(痰核), Yeong(?), YuJu(流注), Breast Diseases(乳房疾患) and DoHan(盜汗) are stagnancy's injury of Ki and blood(鬱傷氣血), gallbladder fire with stomach damp(膽火胃濕), deficiency of Yin in stomach with Kwolum's check (胃陰虛 厥陰乘), heat's penetration of blood collaterals with disharmony of liver and stomach(熱入血絡 肝胃不和), insidious pathogen in Kwolum(邪伏厥陰), the treatment of mediation(和解) with Prunellae Spica(夏枯草), Chrysanthemi Flos(菊花), Mori Folium (桑葉), Bupleuri Radix(柴胡), Coptidis Rhizoma(黃連), Scutellariae Radix(黃芩), Gardeniae Fructus(梔子), Cyperi Rhizoma(香附子), Toosendan Fructus(川?子), Curcumae Radix(鬱金), Moutan Cortex(牧丹皮), Paeoniae Radix Rubra(赤芍藥), Unoariae Ramulus Et Uncus(釣鉤藤), Cinnamorni Ramulus(桂枝), Paeoniae Radix Alba(白芍藥), Polygoni Multiflori Radix (何首烏), Cannabis Fructus (胡麻子), Ostreae Concha(牡蠣), Zizyphi Spinosae Semen(酸棗仁), Pinelliae Rhizoma(半夏), Poria(백복령). etc can be applied. 5) When the pathogenesis of BanJin(반진), BalLa(발라), ChangI(瘡痍), NamgChang(膿瘡).   • Factors which affect the capacity and the soil plug condition of an open-ended pile can be broadly divided into three categories:i.e., pile conditions, soil conditions and penetration methods. It has been found that the relative density and the horizontal stress have much effects on the soil plug behavior than other soil conditions. Also, it has been found that the pile diameter is the most important factor among pile conditions. However, a few investigations have been performed to account for both soil conditions and pile conditions. In this paper, a number of calibration chamber tests have been conducted with three different sized open-ended model piles. The model pile was driven into siliceous sand, with varying soil conditions, to clarify coupled effects of pile diameter and soil conditions on the plug behavior, the capacity, and the load trasfer mechanixm of soil plug. The model piles are composed of two stainless steel pipes so as to measure the plug capacity, the tip resistance, and the outside skin friction. separately.

나노 독성의 개념 및 나노입자에 대한 위해성 평가의 필요성 (The Concepts of Nanotoxicology and Risk Assessment of the Nanoparticles)

  • Human exposure to nano-sized particles (NSP) has increased over the last century with anthropogenic sources, and the rapid development of nanotechnology becomes an another source of such exposure. Information regarding the safety of nanotechnology and its product, nanoparticles, is urgently needed when assuming exposure through inhalation, oral intake, and penetration across skin is ever increasing as growing nanotechnology rapidly. The recent advancement of biokinetic studies with NSP and newer epidemiologic and toxicologic studies with ultrafine particles can be the basis for the nanotoxicology. Some concepts of nanotoxicology can be known from the results of these results. Specific small size of NSP, when inhaled, facilitates deposition by difusional mechanism in all regions of the respiratory tract and uptake into cells, ranscytosis across epithelial and endothelial cells into the blood and lymph circulation to reach target sites. Translocation along axons and dendrites of neuron makes an access to CNS and ganglia. These biokinetics are dependent on NSP surface chemistry. Risk assessments of NSP include appropriate and relevant doses/concentration selections, the increase effects in the organism and the benefits of possible desirable effects. An interdisciplinary team approach is desirable for nanotoxicology research and an appropriate risk assessment.

Antimicrobial Effect of Supercritical Robinia pseudo-acacia Leaf Extracts and Its Transdermal Delivery System with Cell Penetrating Peptide

  • In this paper, we present to evaluate physiological activity of Robinia pseudo-acacia leaf and its skin penetration using liposome and cell penetrating peptide. After extraction with Robinia pseudo-acacia leaf using the distilled water and supercritical, various physiological activities were examined. In antioxidants experiments, the total concentration of polyphenol compounds was determined to be 56.88 mg/g in hydrothermal extract, 45.07 mg/g in supercritical extract. The DPPH radical scavenging ability at 1,000 ㎍/mL was 33.97% in supercritical extract. The scavenging effect on SOD experiment at 500 ㎍/mL was 76.41% in supercritical extract. In the antimicrobial experiments, the hydrothermal extract had no effect, but supercritical extract represented maximum clear zone of 14.00 mm in Staphylococcus aureus strain. Liposome containing the RSE (Robinia pseudo-acacia leaf supercritical extract) reduced particle size and stabilized zeta potential. In the epidermal permeability experiment, it was confirmed that the permeation of liposome containing the RSE and cell penetrating peptides was remarkable.

Development of Clotrimazole Gels for Enhanced Transdermal Delivery

  • To develop a topical bioadhesive formulation of clotrimazole for enhanced transdermal delivery, hydroxypropyl methylcellulose gel containing permeation enhancer was formulated and permeation studies were carried out. The release characteristics of the drug from the gel formulation were examined according to the receptor medium, drug concentration, and temperature. The rate of drug release from the gel increased with increasing drug concentration and temperature. The activation energy (Ea) of drug permeation, which was calculated from the slope of log P versus 1/T plots, was 14.41kcal/mol for a 1%(w/w) loading dose. The enhancer, such as saturated, unsaturated fatty acids, pyrrolidones, propylene glycol derivatives, glycerides, and non-ionic surfactants, were incorporated onto the gels to increase the amount of drug permeation into the skin. Among the enhancers used, polyoxyethylene 2-oleyl ether showed the highest level of enhancement. These results show that clotrimazole gels containing polyoxyethylene 2-oleyl ether could be used for the enhanced transdermal delivery of clotrimazole.

$PGE_1$ 요도좌제의 제조 및 평가 (Preparation and Evaluation of $PGE_1$ Urethral Suppositories)

  • [ $PGE_1$ ] is an endogenous substance of potent vasodialator as well as inhibitor of platelet aggregation. It has been used therapeutically in peripheral arterial occlusive disease and impotence. Intracavernous injection of $PGE_1$ for erectile dysfunction has been established for several years as a treatment option for erectile dysfunction of diverse etiologies, but this mode of administration is limited by penile discomfort, pain at the injection site, inconvenience and noncompliance. As the matter of worse, the $\beta-hydroxy$ moiety of $PGE_1$ is extremely susceptible to dehydration in solution to give inactive $PGA_1$ and $PGB_1$. For the improvement of stability, rapid absorption at action site and the convenience of application, $PGE_1$ was formulated as urethral suppositories of three types of formulations, such as PEG, witepsol, and the mixture of PEG and witepsol. The stability test of $PGE_1$ and the release test in urinary suppositories were performed. Futhermore, the effect of enhancers and vehicle composition on the penetration of $PGE_1$ through excised rat skin was evaluated by permeability coefficient and enhancement ratio.

Development of Bio-ballistic Device for Laser Ablation-induced Drug Delivery

  • Transdermal and topical drug delivery with minimal tissue damage has been an area of vigorous research for a number of years. Our research team has initiated the development of an effective method for delivering drug particles across the skin (transdermal) for systemic circulation, and to localized (topical) areas. The device consists of a micro particle acceleration system based on laser ablation that can be integrated with endoscopic surgical techniques. A layer of micro particles is deposited on the surface of a thin metal foil. The rear side of the foil is irradiated with a laser beam, which generates a shockwave that travels through the foil. When the shockwave reaches the end of the foil, it is reflected as an expansion wave and causes instantaneous deformation of the foil in the opposite direction. Due to this sudden deformation, the microparticles are ejected from the foil at very high speeds, and therefore have sufficient momentum to penetrate soft body tissues. We have demonstrated this by successfully delivering cobalt particles $3\;{\mu}m$ in diameter into gelatin models that represent soft tissue with remarkable penetration depth.

프로스타글란딘 $E_1$의 안정성 및 경피흡수 (Stability and Percutaneous Transport of Prostaglandin $E_1$)

  • We have studied the stability and transdennal flux of prostaglandin $E_1\;(PGE_1)$ from various donor solutions through hairless mouse skin. Stability in HEPES buffer or in propylene glycol (PG) solution where enhancer (oleic acid (OA), propylene glycol monolaurate (PGML), transcutol (TC), ethanol (EtOH))s dissolved was investigated. $$PGE_1 was not stable in HEPES buffer. The concentration of $$PGE_1 decreased continuously for 7 days, and the degradation rate constant was $0.0028\;h^{-1}$, assuming first order reaction. The effect of current or penetration enhancer on the degradation was minimal. Percutaneous transport from HEPES buffer by passive or iontophoretic delivery without enhancer was close to nil. When OA or PGML was used together with PG, both passive and iontophoretic flux increased. PGML showed better enhancing effect than OA. Flux by cathodal delivery was about 2 times larger than that by passive delivery. Flux by anodal delivery was lower than that by passive delivery. TC and EtOH also increased the transdermal flux, but the effect was not as good as that observed when OA or PGML was used. These stability and flux data provide important information on how to formulate the patch, which will be the next step of this work, and on the polarity of current to use during iontophoresis.

Complement-mediated tail degradation of Neodiplostomum seoulense cercariae

  • The furcocercus cercariae of Neodiplostomum seoulense (Digenea: Neodiplostomidae) penetrate the skins of tadpoles and shed their tails. The speculated mechanism of this tail loss was physical efforts required to produce a vigorous zigzag motion during skin penetration; no other mechanism has been proposed. We examined the relationship between the host serum and cercarial tail loss. Cercariae of N. seoulense were collected from experimentally infected Segmentina hemisphaerula, and lots of 300 cercariae were cultured in medium 199 contained several types of sera. Cercarial tail degradation was induced in all media, but all the cercariae cultured except those cultured in media containing fetal bovine serum (FBS) died within 48 hr. After 72 hr cultivation in media containing FBS, cercarial tail degradation was induced in 67.0%; in continuous cultivation 13.3% of larvae survived for 7 days. Tail degradation did not occur in the absence of serum and when serum was heat inactivated at $56^{\circ}C$ for 30 min. The addition of 20 mM ethylenediaminetetraacetic acid (EDTA) blocked cercarial tail degradation completely. Moreover, the addition of 20 mM $MgCl_2$ restored tail degradation blocked by EDTA. These results suggest that the alternative complement pathway is related with the N. seoulense cercarial tail degradation induced by serum.

Photodynamic Therapy for Cancer without External Light Illumination by Utilizing Radioisotope-induced Cerenkov Luminescence as an Excitation Source

  • Photodynamic therapy (PDT), in which a photosensitizer (PS), light, and molecular oxygen are essential components, is a non-invasive and highly effective cancer therapeutic method. However, PDT suffers from the penetration limit of light caused by attenuation and scattering of light through tissues constraining its use to skin and endoscopically accessible cancers. Cerenkov luminescence (CL) is defined as the light illuminated when charged particles move in a dielectric medium at a velocity greater than the phase velocity of light. It is known that medical radioisotopes in preclinical and clinical settings have enough energy to generate CL, and lately, CL has been exploited as an excitation source for PDT without external light illumination. This review introduces state of the art studies of radioisotope-based PDT for cancer, in which radioisotopes are utilized as a light source.
