The Clinical Nurses' Knowledge, Attitudes, and Performance on Incontinence Associated Dermatitis (임상간호사의 실금 관련피부염에 대한 지식, 태도 및 수행 정도)

  • Purpose: Incontinence-associated dermatitis (IAD) is widespread and a common problem in healthcare settings, It is defined as irritation of the perineal skin due to contact with urine, feces, or both. It is one of the major risk factors for the development of pressure injuries are regarded as a quality indicator of nursing care. It is important to understand the knowledge and attitudes of nurses who are primarily responsible for providing appropriate nursing care that includes differentiation of IAD from other skin breakdowns, and appropriate management. Therefore, the purpose of this study was to determine the knowledge, attitudes, and performance of IAD among clinical nurses in Korea. Methods: Data about IAD knowledge, attitudes, and performance were collected from August to November 2022. 656 nurses from six hospitals participated in the study. Results: The mean scores of nurses' knowledge, attitudes, and performance were 17.98±2.91, 76.34±7.85, and 91.53±16.18, respectively. Nurses' knowledge showed a positive correlation in both attitudes (r=0.27, p<.001) and performance (r=0.09, p=.02), and attitudes and performance (r=0.34, p<.001) also showed a positive correlation. Conclusion: To improve the knowledge and performance of nurses, an educational program consisting only of incontinence-related dermatitis is needed, and case-based education and clinical skills education are needed to enhance the ability to differentiate from pressure injury.

A Study on Self-care among the Lymphedema Patients (림프부종 환자의 자가관리에 관한 실태 조사)

  • Purpose: The purpose of this descriptive study was to gain basic data to develop a self-care protocol for the lymphedema patients. Method: The subjects of this study consisted of 115 patients with lymphedema from 8 hospitals and two community health and welfare centers in Busan and Seoul. The data was collected with questionnaire by self reporting of patients between March 2001 and December 2001. Data was analyzed by mean and percentage. Result: For self-care activities in daily life, compliance of 'use skin care preparations', 'use heat and cold', 'protect from local compression on affected limbs', 'protect from insect biting', 'use aids to protect affected limbs', 'take diuretics and take protein diet' did not reach to 50%. For self-care activities related to complex physical therapy, 28.7% of subjects complied with compression garment, 14.8% with manual lymph drainage, and 13.0% with exercise. 20.0% of subjects tried to treat with acupuncture and 13.9% with heat therapy. Conclusion: From this study, it is suggested that patients need to get a self-care education with correct information about self care activities and health care professionals need to develop more convenience self-care techniques of massage and exercise.

Comparison among with Nursing Records, Nursing Intervention Priority Perceived by Nurse and Nursing Intervention Frequency of General Surgery Department (일반외과 간호기록에서의 중재, 지각한 간호중재의 중요도 및 수행 빈도)

  • Purpose: The purpose of this study was to determine core nursing intervention in nursing records and to compare perceived nursing intervention priority and nursing intervention frequency of general surgery department. Methods: Subjects were 70 nurses who work in the general surgery department. Data was collected using a nursing intervention classification and analyzed by frequency and mean. Results: The most frequent nursing interventions of nursing records were orderly risk management, coping assistance, tissue perfusion management, skin/wound management and nutrition support. Important nursing interventions were tissue perfusion management, respiratory management, electrolyte acid-base management, elimination, peri-operative care. The most frequent nursing interventions were drug management, peri-operative care, risk management, tissue perfusion management, patient education. Conclusion: This study found that nursing records were different from intervention priority and nursing frequency. So further study is needed for finding focused intervention of specific subjects and differences with priority of nursing and frequency of nursing.

A Comparative Study on Self-care Practices between the Natural Menopause Group and the Artificial Menopause Group (자연폐경 부인과 인위적폐경 부인간의 자기건강관리에 관한 비교연구)

  • This study was carried out to explore the illness-related activities of the menopaused women. To achieve such a purpose, sixty-six cases of the middle aged women were sampled the naturally and the artificially menopaused group respectively from 1st, Aug. to 31th, Aug. in 1988. For the collection of data, 1, 140 women aged between 45-54 were selected through stratified sampling techniques in urban and rural area. Among them the final subjects for analysis were restricted to only those who had experienced menopause naturally or artificially. And then, after control for age and education analysis was performed. The data was analysed by use of frequency, percentage, $X^2-test,$ t-test, Pearson correlation coefficiency and stop-wise multiple regression. The obtained results were as follows. I. As for the se1f-perception on menopausal symptoms, it was revealed than Korean women, neithe in naturally and artificially, accepted the change of menstruation itself as serious. This shows us that middle-aged women had positively receptive attitudes that the change of menstruation is follwed by amenorrhea. 2. The artificially menopaused group scored more than the naturally one: (I) on the self-control activities such as self-assessment, lay-consultation, fever check, pulse check and observation of vagina discharge, (2) on the self-decision activities such as hospital and pharmacy utilization, (3) on the self maintenance activities such as walking, aerobic, weight check, skin care, skin protect, calori control diet control, milk intake, vegetable intake, cold water drinking before meal, parasol use and BP check The above results lead us that the self care practices of the menopaused women revealed' coping wit I menopausal symptoms at the first level in community and must be developed by the adequate nursin intervention.

A study on the Body Cathexis and Appearance Management Behaviors of High School Girls (여고생의 신체만족도와 외모관리행동에 관한 연구)

  • The purpose of this study was to investigate the body cathexis and appearance management behaviors of the high school girls according to their demographic, physical, psychological. and school related variables. Data was obtained from 378 high school girls in Jeonju City by self-administered questionnaires from March to April. 2001. The results of this study were; 1. Body cathexis of high school twirls was relatively low, face and upper body cathexis were higher than other body parts. but lower-body and total body cathexis were low. 2. Body cathexis of high school twirls was in exact proportion to their parents' educational level total household income. and monthly pocket money. And it was higher in case of tall. light. external personality. higher self-esteem, art or physical high school girls. and the vocational high school twirls. with good academic records. and popular among friends. 3. High school girls showed higher appearance management behaviors such as bath, skin care. hair care. but they did risky appearance management behaviors such as drug use. diet. plastic cosmetic a tittle. The students in case of mother's higher educational level. more monthly income of household, more pocket money. external personalities. lower self-esteem, art or physical high school girls and the vocational high school girls, with good academic records, popular among friends showed higher appearance management behaviors .

  • Today all kinds of smart devices are being developed with various researches on wearable devices that support smart computing on the human body. Skin diseases continue to rise including freckles, pimples, atopy, and scalp trouble due to the environmental and genetic factors, and people pay bigger medical bills to treat their skin diseases. There is thus a need to develop a smart-phone or table-based smart healthcare imaging system of high portability and diagnostic accuracy capable of analyzing and managing various skin problems related to skin care. This study proposed an integrated system combining the Smart Mi Band, a wearable device using moisture and UV sensors based on IoT, on the hardware part with the sensor information monitoring software.

  • Newborn infants, including premature infants, are high-risk patients susceptible to various microorganisms. Catheter-related bloodstream infections are the most common type of nosocomial infections in this population. Regular education and training of medical staffs are most important as a preventive strategy for central line-associated bloodstream infections (CLABSIs). Bundle approaches and the use of checklists during the insertion and maintenance of central catheters are effective measures to reduce the incidence of CLABSIs. Chlorhexidine, commonly used as a skin disinfectant before catheter insertion and dressing replacement, is not approved for infants <2 months of age, but is usually used in many neonatal intensive care units due to the lack of alternatives. Chlorhexidine-impregnated dressing and bathing, recommended for adults, cannot be applied to newborns. Appropriate replacement intervals for dressing and care sets are similar to those recommended for adults. Umbilical catheters should not be used longer than 5 days for the umbilical arterial catheter and 14 days for the umbilical venous catheter. It is most important to regularly educate, train and give feedback to the medical staffs about the various preventive measures required at each stage from before insertion to removal of the catheter. Continuous efforts are needed to develop effective and safe infection control strategies for neonates and young infants.

The Effect of Hand Reflexology on Pain, Skin Temperature and Nursing Practice (입원 환자에서 손 반사요법의 통증, 피부체온 및 간호실무 효과)

  • Purpose: This study is aimed to examine the effectiveness on pain, feeling and nursing practice of hand reflexology applied to in-patient in the clinical setting. Method: The pre/post test methods using VAS for pain and feeling, physiologic measuring, and questionnaire of nursing care were conducted. Subjects were selected 45 in-patients from a surgical ward in hospitals during Nov.-Dec. 2005. 5 minute hand reflexology therapy was applied to both hands of participant by two research assistants at the same time. Data were analyzed based on SAS program using frequency, $x^2$-test, paired t-test, ANOVA and Pearson's correlation coefficients. Result: 1. After receiving hand reflexology therapy, the subjects showed significant pain relief(t=-4.94, p=.0001), improvement in feeling(t=19.44, p=0001) and an increase in skin temperature(t=3.54, p=.001). 2. The applied skills that the participants preferred were press-rotate(80.0%), press-walk(35.56%) and press-rolling(31.11%). The effectiveness of nursing practice scored $3.99{\pm}1.97$ out of 5. Nurse-Patient Relationship and effective response of nursing intervention ranked the highest 4.31. Conclusion: Hand reflexology was considered as an effective nursing intervention in the clinical settings, but in order to evaluate re-verifies and reliances for this effects, it is necessary that comparative studies should be conducted.

Analysis of Nursing Care Activities of Nursing Students in Clinical Experience (간호학생의 임상실습 간호활동시간 분석)

  • The competence of newly graduated nurses is based on various clinical expriences gained when they were students. Therefore, instructors of nursing students, professors in nursing schools or directors in hospitals must play a critical role in assisting them to obtain various knowledge and experienced nursing skills. The purpose of this study was to investigate nursing care activities and nursing care hours practiced by nursing students in a general hospital. The subject students were total 214 nursing students, 2nd graders(sophomores) and 3rd graders(juniors) from 5 Junior Nursing Colleges in Seoul and they practiced at S general hospital to gain clinical experience. The data were collected for 4 days. The tools for this study were the direct nursing care activity list consisted of 15 nursing areas and the indirect nursing care activity list consisted of 9 nursing areas. The subject students were supposed to record their own score. The results of this study are ; 1. The nursing care hours per nursing student 1) The average total nursing care hours a day per each nursing student are 362.65 mins(6.04hr), the direct nursing care hours per each nursing stuent are 202.09 mins(direct nursing care rate 56.0%) and it is higher than the indirect nursing care hours, 159.75mins(indirect nursing care rate 44.0%). The direct nursing care rate of each student by a team approach in the evening shift(56.3%) is higher than that in the day shift(55.8%). 2) The hours of checking vital signs are the longest(47.35mins) among the direct nursing care activites and next is in order of counseling 8l emotional support, nurse rounds, and accompaning a patient during examination. The hours of reporting are the longest(32.39mins) among the indirect nursing care activites, and next is the activities related to education such as reviewing chart, looking up references, etc. 3) The freqency of checking vital signs practiced by nursing student is the highest(the average of 55.7 times) among the direct nursing care activities and next is in order of nurse rounds, assistance of feeding, and counseling & emotional support. The required time for nursing students to accompany their patient during examination is the longest(20.7mins) and next are in order of restriction on patient' activity, orientated by a head nurse, skin care, sitz bath, bathing & hair shampoo, and assisting with patients' exercise. 2. The nursing care hours per grader 1) The average hours of total nursing care per a nursing student are 369.2mins(6.2hrs) to 2nd graders, 355.9mins(5.9 hrs) to third graders. The direct nursing care rate per each nursing student to 3rd graders(59.3%) was higher than that to End graders(52.8%). 2) For 2nd graders, the highly marked nursing activities are teaching associated with direct nursing care activities such as drawing up papers, looking up references, reviewing charts, and being orientated by staff nurses. For 3rd graders, measurments, observations, and nurse rounds in indirect nursing care activities are taken highly 3) The most frequent practice of the nursing care activites is checking vital signs : 65 times to 3rd graders and 46.5 times to 2nd graders. Our suggestions based on the results of this study are : 1. It is recommanded to repeat the same designed study in a variety of clinical fields for further study. 2. It is recommanded to collect data not by self-record method but by observated method. 3. It is needed for nursing instructors in Nursing Schools and in hospitals to develop the guidelines and check-list of clinical practice courses.

An Importance Analysis on the NCS-Based Skin Care Qualification L3 Level of Education in Life Care (라이프케어의 피부미용 NCS기반 자격 L3수준의 교육 중요도 연구)

  • The recent phenomenon of job "Miss Match", which is inconsistent with knowledge in the demand of educational training institutes and industries, has spread to an increase in private education costs for reeducation and employment of new hires, resulting in weak individual job competency and poor employment capability, as well as economic and material waste at the national level. To compensate for these problems, the National Competency Standards(NCS), which are available immediately in practice and look for a standard point of national job competency with the aim of fostering human resources sought by industries, were developed, and even the NCS-based qualification system was launched in line with the stream of times. This study is intended to look into the importance and priority of competency units and competency unit elements at the NCS-based qualification L3 level in the skin care field for an overall check of the NCS-based qualification level at a time when educational institutes are organizing and operating the school curriculums according to the NCS and NCS-based qualification level. And it is attempted to provide basic data for the development of curriculum in fostering professional human resources required by industries. To analyze the needs for competency units and competency unit elements at the L3 level, a survey using AHP method was carried out to a group of field experts and a group of education experts. In addition, the SPSS(Statistical Package for Social Science) ver. 21.0 and Expert Choice 2000, an AHP-only solution was used to do statistical processing through the processes of data coding and data cleaning. The findings showed that there was a partial difference of opinion between a group of field experts and a group of education experts. This indicates that the inconsistencies between educational training institutes and industrial sites should be resolved at this time of change with the aim of fostering field customized human resources with professional skills. Consequently, the solution is to combine jobs at industrial sites and standardized educations of educational institutes with human resources required at industrial sites.
