Proliposomal Clenbuterol Patch for Transdermal Delivery (프로리포솜을 이용한 클렌부테롤의 경피흡수 제제화)

  • Proliposomal patch of clenbuterol, ${\beta}_2-agonist$ bronchodilator, was prepared and its feasibility as a novel transdermal drug delivery system was examined. Proliposomal granules containing clenbuterol was prepared by a standard method using sorbitol and lecithin with (Rx 2) or without cholesterol (Rx 1). The porous structure of sorbitol in the proliposomes was maintained allowing tree flowability of the granules. Following contact with water, the granules were converted probably to liposomes almost completely within several minutes. It indicates that proliposomes may be hydrated, when they are applied on the skin under occlusive condition in vivo, by the sweat to form liposomes. Clenbuterol release from Rx 1 and Rx 2 proliposomes to pH 7.4 isotonic phospate buffer (PBS) across cellulose membrane (mol. wt. cut-off of 12000-14000) was retarded significantly compared with that from the mixture of clenbuterol powder and blank proliposomes. Interestingly, proliposomes prepared with lecithin and cholesterol (i.e., Rx 2 proliposomes) showed much more retarded release of clenbuterol than proliposomes prepared only with lecithin (i.e.. Rx 1 proliposomes), indicating that clenbuterol release from proliposomes can be controlled by the addition of cholesterol to the proliposomes. Proliposomal patches were prepared using PVC film as an occlusive backing sheet, two sides adhesive tape (urethane, 1.45 mm thickness) as a reservoir for proliposome granules and Millipore MF-membrane (0.45 mm pore size) as a drug release-controlling membrane. Rx 1 or Rx 2 proliposomes containing 4.6 mg of clenbuterol were loaded into the reservoir of the patch. Clenbuterol release from the patches to pH 7.4 PBS was determined using USP paddle (50 rpm)-over-disc release method. Clenbuterol release from the proliposomal patches was much more retarded even than from a matrix type clenbuterol patch (Boehringer Ingelheim ltd). Being consistent with clenbuterol release from the proliposomal granules, the release from the patches was highly dependent on the presence of cholesterol in the proliposomes : Patches containing Rx 2 proliposomes showed several fold slower drug release than patches containing Rx 1 proliposomes. When the patch containing Rx 1 proliposomes was applied on to the back of a hair-removed rat, clenbuterol concentration in the rat blood was maintained during 6-72 hrs. Transdermal absorption of clenbuterol from the patch was accelerated when the patch was prehydrated with 50 ml of pH 7.4 PBS before topical application. Above results indicate that sustained transdermal delivery of clenbuterol is feasible using proliposomal patches if the cholesterol content and pore size of the release rate-controlling membrane of patches, for example, are appropriately controlled.

Arthroscopic Cannulated Screw Fixation Technique for Avulsion Fracture of the Intercondylar Eminence of the Tibia (삽관 나사못(Cannulated screw)을 사용한 경골 과간 융기부 견열 골절의 관절경적 치료 기법)

  • Avulsion fractures of the intercondylar eminence of the tibia are not uncommon. In the displaced avulsion fracture, anatomical reduction and firm fixation of fracture fragments are needed but the most of the conventional operative techniques including arthroscopic technique are relatively complex and need. The results were not always satisfactory due to the risk of postoperative complications such as wound infection, premature epiphyseal closure and loss of fixation after early motion etc. So we describe a simple and safe modified method of arthroscopic reduction and fixation for avulsion fractures of the intercondylar eminence of the tibia. In our thirteen cases, we achieved anatomical reduction and secure fixation using cannulated screw through the three arthroscopic portals (anterolateral, medial mid-patellar and central). Postoperatively, immediate limited range of motion of the knee and partial weight bearing were possible. Additional use of the washer afforded safe fixation of comminuted avulsion fracture. The advantage of this technique includes its technical simplicity, easy removal of hardware, ability to treat comminuted type IV fracture with washer, no additional skin incision, no damage to growing plate in growth children and less morbidity.

Isolation and Characterization of Bacterial Cellulose-Producing Bacteria for Silver Nanoparticle Synthesis (은 나노입자 합성을 위한 Bacterial Cellulose 생산 세균의 분리 및 특성)

  • As a basic study for environment-friendly production of bacterial cellulose (BC) dressing with antimicrobial activity, we isolated and identified acetic acid bacteria which are resistant to silver ions and can biosynthesize silver nanoparticles. Furthermore, conditions of BC production by selected strain were also investigated. Strain G7 isolated from decayed grape skin was able to grow in the presence of 0.1 mM $AgNO_3$ which was identified as Acetobacter intermedius based on 16S rRNA gene analysis. BC production was the highest in a medium containing 2% glucose as a carbon source, 2% yeast extract as a nitrogen source, and 0.115% acetic acid as a cosubstrate. Structural properties of BC produced in optimal medium were studied using Fourier-transform infrared spectroscopy and X-ray diffractometer, and it was found that BC produced was cellulose type I that was the same as a typical native cellulose. When strain G7 was cultured in an optimal medium containing 0.1 mM $AgNO_3$, the color of the culture broth turned into reddish brown, indicating that silver nanoparticles were formed. As a result of UV-Vis spectral analysis of the culture, it was found that a unique absorption spectrum of silver nanoparticles at 425 nm was also observed. Scanning electron microscopic observations showed that silver nanoparticles were formed on the surface and pores of BC membrane.

Breeding of New Cultivar 'Hongsil' in Forcing Culture of Strawberry for Export (수출용 촉성재배 딸기 신품종 '홍실' 육성)

  • The 'Hongsil' strawberry ($Fragaria{\times}ananassa$ Duch.) is a new cultivar released by Gyeongsangnam-do Agricultural Research & Extension Services in 2015. This cultivar originated from a 2011 cross between '802a312' and 'Okmae' and exhibited suitable fruit characteristics for export due to the high soluble solids content and firmness of the fruit. The breeding line '1116-74-15' was selected as an elite line after examining its characteristics and productivity from 2013 to 2014. A field trial of this line was conducted on a farm in 2015, and it was registered as 'Hongsil' thereafter. The 'Hongsil' strawberry demonstrates intermediate plant type and moderate vigor in growth. It is suitable for forcing culture due to its early flower bud differentiation and low chilling requirement. The soluble solids content of 'Hongsil' is $10.0^{\circ}Brix$, which is approximately $0.4^{\circ}Brix$ higher than that of 'Seolhyang' The Fruit firmness of 'Hongsil' is $13.1g{\cdot}mm^{-2}$, similar to that of 'Maehyang'. Fruits of 'Hongsil' are long conical having a red skin color and marketable yield is $4,269kg{\cdot}10a^{-1}$ which is 16% higher than that of 'Maehyang'. The 'Hongsil' cultivar is sensitive to powdery mildew, and it therefore warrants further investigation regarding disease control.

Clinical Cases Analysis of Forearm Free Fasciocutaneous Flaps on Oral Cavity Defect Area (구강 결손부에 적용된 요골 유리전완 피부피판 적용례 분석)

  • The radial forearm free flap (RFFF) has become a workhorse flap as a means of reconstructing surgical defects in the head and neck region. We have transferred 12 RFFFs with fasciocutaneous type on oral cavity defects in 12 patients after cancer resection and submucous fibrotic lesion ablation from 2005 to 2007 at Department of oral and maxillofacial surgery, Pusan National University Hospital. We reviewed retrospectively patients' charts and followed up the patients. Clinical analysis on the cases with RFFFs focusing on flap morbidity, indications and available vessels was done. The results of study are follows: 1. RFFF could be applied for all kind of defects after resection of tongue, floor of mouth, buccal mucosa, denuded bone of palate, maxilla, and mandible. 2. All free flaps could be used for primary reconstruction. The survival rate of 12 RFFFs was 92%. Partial marginal loss of the flaps was shown as 3 cases among 12 cases. Large size-vessels like superior thyroid artery, facial artery, internal jugular vein were favorable for microvascular anastomosis. 3. Parenteral nutrition instead of nasal L-tube also can be favorable for postoperative a week for better healing of the flap if the patients couldn't be tolerable with nasal tubing. 4. Donor sites with thigh skin graft were repaired with wrist band for 2 weeks. The complications included scarring, abnormal sensation on hand, and reduced grip strength in few patients, but those didn't induce major side effects. 5. Most RFFFs were well healed even if mortality rate of cancer patients was shown as 50% (5/10 persons). The mortality of patients was not correlated with morbidity of the flaps. We could identify the usefulness of RFFF for restoration of oral function, esthetics if the flap design, tissue transfer indications, and well controlled operation are proceeded.

The study on the causal agent of Streptococcicosis (Lactococcus garvieae), isolated from cultured marine fishes (해산 양식어류로부터 분리된 연쇄구균증의 원인균, Lactococcus garvieae에 대한 연구)

  • The purpose of this study was to investigate the microbiological characteristics and the distributions of the bacteria causing streptococcicosis occurred in marine fish farm, Korea. Many kinds of cultures fishes suffered from the disease accompanied with typical symptoms, including darkening of the skin, exophthalmia, petechiae inside of the opercula and distended abdomen. The isolates from the diseased fishes were compared with Lactococcus garvieae by biochmical, biophysical and serological methods and polymerase chain reaction(PCR) assay. We isolated 35 strains of the geuns Streptococcus from the diseased olive flounder, Paralichthys olivaceus, yellow tail, Seriola quinqueradiata and Korean rockfish, Sebastes schlegeli. 15 strains out of the isolates were identified to L. garvieae and the others were not because of their different biochemical and biophysical charateristics. Seven strains of the isolates were agglutinated by rabbit serum raised against L. garvieae $KG^+$ phenotypic cells(ATCC49156)as a reference strain. Twenty-one strains of the isolates identified to L. garvieae since they were formed the expected band through performing PCR assay using specific primers, pLG-1(5'-CATAACAATGAGATCGC-3') and pLG-2(5'-GCACCCCGCGGTTG-3'). In the present study, it showed that L. garvieae was a dominant strain causing streptococcicosis in the tested area due to occurrence of 21 strains as L. garvieae out of all the isolates, 9 atrains as Streptococcus sp. and 5 strains as Enterococcus sp.

A Study on Efficiency of Essential Oil for Dandruff Symptoms Relief - Focused on Tea tree oil - (비듬 증상 완화를 위한 에션셜 오일의 유효성 연구 - 티트리 오일을 중심으로 -)

  • The reason that aromatherapy began to draw attention again as the alternative therapy is because the essential oils that are used for aromatherapy not only allow the human body to maintain homeostasis but also have an effect on curing a mental feeling of fatigue and because these efficacies and effects are proceeding with being proved scientifically. Also, with coming to be high in a voice of concern about severally side effects that the modern medicine and the synthetic chemistry have, the aromatherapy came to be paid attention as the replacement therapy that can substitute for and supplement this. In particular, at the time of requiring a study on a substance to substitute for this after having been announced in Japan for 1998 a risk of Zincpyrithione that is being used as a component of dandruff treatment, it is thought that the aroma oil that was known for having an anti-dandruff effect has a merit of being easy in a user's access, thus it will be effective in curing the symptom of dandruff. Accordingly, this study conducted the following experiment in a bid to examine whether the aroma oil has the anti-microbial effect on the actual malassezia. First of all, it measured the minimum concentration of aroma oil in which the growth of malassezia is inhibited, by carrying out MIC(Minimum Inhibition Concentration) test in terms of anti-microbial activity against Malassezia furfur KCTC 7744 while using oils of Patchouli, Rosemary, Lavender, and Tea tree among aroma oils that are being used for dandruff treatment. As a result of that, Tea tree oil has the most excellent anti-microbial activity, and the measurement result of anti-microbial activity was shown in order of Lavender oil>Rosemary oil>Patchouli oil. Next, it carried out the is actually eased by applying Tea tree oil directly to the scalp of dandruff. In consequence of the experiment, the massage of Tea tree oil indicated the result of relieving the symptoms of dandruff such as scale, itch, pain, inflammation, and sebum, and among these symptoms of dandruff, it was shown to be most effective in easing itch. Especially, this clinical experiment was indicated that the massage of Tea tree oil has the most effect on relieving itch of targets with a dry-skin type who had no experience of dandruff treatment.

Gracilis Muscle Transplantation in Neglected Brachial Plexus Palsy (진구성 상완 신경총 마비에 대한 유리박근이식술)

  • Complete denervation after severe brachial plexus injury make significant muscle atrophy with loss of proper function. It is much helpful to reconstruct the essential function of the elbow flexion movement in patient with total loss of elbow flexion motion after brachial plexus lesion which was not recovered with nerve surgery or long term conservative treatment from onset. In whole arm type brachial plexus injury, if there were no response to neurotization or neglected from injury, the volume of the denervated muscle is significantely reduced month by month. About 18 months most of the muscle fibers change to fibrous tissues and markedly atrophied irreversibly, further waiting is no more meaningful from that period. Authors performed 14 cases of functioning gracilis muscle transfer from 1981 to 1995 with microneurovascular technique, neuromusculocutaneous free flaps were performed for reconstruction of lost elbow flexion function. Average follow-up period was 5 years and 6 months. We used couple of intercostal nerves as a recipient nerve which were anastomosed to muscular nerve from obturator nerve in all cases. Recipient vessels were three deep brachial artery and eleven brachial artery which were anastomosed to medial femoral circumflex artery with end to end or end to side fashion. Average resting length of the transplanted gracilis were 24 cm. We can get average 54 degree flexion range of elbow with fair muscle power from flail elbow. There were one case of muscle necrosis with lately developed thrombosis of microvascular anastomosed site which comes from insufficient recipient arterial condition, 3 cases of partial marginal necrosis of distal skin of the transplanted part which were not significant problem with spontaneously solved with time goes by gracilis muscle has constant neurovascular pattern with relatively easy harvesting donor with minimal donor morbidity. Especially it has similar length and shape with biceps brachii muscle of upper arm and longer nerve pedicle which can neurorrhaphy with intercostal nerve without nerve graft if sufficient mobilization of the nerves from both sides of gracilis and intercostal region. Authors can propose gracilis muscle transplantation with intercostal nerves neurotization is helpful method with minimal donor morbidity for neglected brachial plexus palsy patients.

Musculocutaneus Island Flap Based on the Distal Vascular Pedicle of Gracilis Muscle (박근의 원위혈관경을 이용한 도서형 근피판술)

  • The gracilis that is frequently used as a donor of free muscle trasfer is appropriate in the muscular shape and vascular position. This muscle is belonged to the second type of muscle group by the classification of the pattern of muscular nutrient vessel. The adductor branch or first perforating branch of deep femoral artery which supplies the proximal 1/3 of this muscle is a dominant one and this is used for the microscopic anastomosis of muscle or musculocutaneous flap. The minor vascular pedicles which enter the distal 1/3 of this of this muscle are branches of the superficial femoral artery and it is 0.5mm in diameter, 2cm in length with two venae comitantes. These minor pedicles supplies distal half of the gracilis muscle. This island musculocutaneous flap using distal vascular pedicle can be used to cover the defect of soft tissue around the distal femoral supra-condylar area, knee joint and proximal tibial condyle area which cause limitation of motion of knee joint, or in the cases that usual skin graft is impossible. The important operative procedure is as follows; The dissection is carried proximally and distally and the entire gracilis muscle including proximal and distal pedicle is completely dissected. After temporary blocking of the proximal vascular pedicle, the adequate muscle perfusion by the distal pedicle is identified and it is rotated to the recipient site around knee joint. The advantages of this procedure are simple, no need of microscopic vascular anastomoses and no significant functional loss of donor site. Especially in the cases of poor condition of the recipient vessel, this procedure can be used effectively. From 1991 to 1996, we performed 4 cases; complete survival of flap in 3 cases and partial survival of flap with partial necrosis in 1 case. This procedure is though to be useful in the small sized soft tissue defect of distal femoral supra-condylar area, knee joint and proximal tibial condylar area, especially in the defect of anterior aspect which expected to cause limitation of motion of knee joint due to scar contracture. But the problems of this procedure are the diameter of distal vascular pedicle is small and the location of distal vascular pedicle is not constant. To reduce the failure rate, identify the muscular perfusion of distal vascular pedicle after blocking the proximal pedicle, or strategic delay will be helpful.

External Fixation of Retaining Ligament in Correction of Facial Disfigurement in Type-1 Neurofibromatosis Patients (유지 인대의 외부 고정을 통한 제1형 신경섬유종증 환자의 안면부 변형 교정)

  • Purpose: In neurofibromatosis patients, complete surgical excision of the mass is almost impossible and surgical treatment usually consists of multiple serial excisions that only result in a debulking effect. Remnant tumor mass has a gravitational effect on facial soft tissues that leads to sagging of skin and soft tissue, and eventually, facial disfigurement and asymmetry. The purpose of our surgical method is to perform soft tissue lifting with longer lasting effect with less surgical risk of damaging facial nerve and vessels. With external fixation using K-wire or surgical screw, the procedure only called for a short incision length and had additional adhesive properties that enabled anchoring of soft tissue in a lifted position for a longer postoperative period. Methods: A total of 5 neurofibromatosis patients (NF-1) visited our clinic for mass reduction and face lifting. The age of patients ranged from 13 to 42 (mean 28.8 years), and most patients had a long history of multiple excisions in the past. Face lifting was performed in 2 different areas, the periorbital area in 3 patients, and the midface in 2 patients. The materials used in fixation of retaining ligament were K-wire (n=3) and titanium screw (n=2). Results: Follow up period was from 5 month to 3 years and 1 month (mean=2 years and 1 month). All patients conveyed satisfaction with the results and no major complications were reported. The lifting effect lasted for as long as 3 years, and there were no complaints of relapse of soft tissue depression or sagging within the operated area. 1 patient (M/13) needed secondary k-wire insertion and additional mass excision in 1 year and 10 months postoperatively due to tumor growth. In two patients with K-wire fixation, mild dimpling and tenderness were observed in the follow up period, but in about 2 months postoperatively, dimpling was relieved and there was no need for removal of fixating material. Conclusion: Surgical lifting in neurofibromatosis patients can be challenging, for mass excision cannot be done completely and gravitational effect by residual mass can be persistent. External fixation of the retaining ligament in patients with neurofibromatosis can give satisfactory results-for incision length is relatively shorter, and the lifting effect can last longer compared to other various face lifting techniques.
