The Effect of γ-Aminobutyric Acid Intake on UVB- Induced Skin Damage in Hairless Mice

  • The skin, the largest organ in the body, undergoes age-related changes influenced by both intrinsic and extrinsic factors. The primary external factor is photoaging which causes hyperpigmentation, uneven skin surface, deep wrinkles, and markedly enlarged capillaries. In the human dermis, it decreases fibroblast function, resulting in a lack of collagen structure and also decreases keratinocyte function, which compromises the strength of the protective barrier. In this study, we found that treatment with γ-aminobutyric acid (GABA) had no toxicity to skin fibroblasts and GABA enhanced their migration ability, which can accelerate skin wound healing. UVB radiation was found to significantly induce the production of matrix metalloproteinase 1 (MMP-1), but treatment with GABA resulted in the inhibition of MMP-1 production. We also investigated the enhancement of filaggrin and aquaporin 3 in keratinocytes after treatment with GABA, showing that GABA can effectively improve skin moisturization. In vivo experiments showed that oral administration of GABA significantly improved skin wrinkles and epidermal thickness. After the intake of GABA, there was a significant decrease observed in the increase of skin thickness measured by calipers and erythema. Additionally, the decrease in skin moisture and elasticity in hairless mice exposed to UVB radiation was also significantly restored. Overall, this study demonstrates the potential of GABA as functional food material for improving skin aging and moisturizing.

IMRT(Intensity Modulated Radiation Therapy)전달 기법과 FFF(Flattening Filter Free) 빔을 이용한 요철 부위 피부 선량 증가 방법에 대한 고찰 (A Study on IMRT (Intensity Modulated Radiation Therapy) Delivery Technique and FFF (Flattening Filter Free) Beam to Increase Skin Dose to Irregularly Shaped Skin Surface.)

  • 목 적: 볼루스 적용이 어려운 코와 같은 요철 부위 치료 시 피부 선량 확보에 어려움이 있는 경우, IMRT(Intensity Modulated Radiation Therapy)전달 기법 과 평균 에너지가 낮은 FFF(Flattening Filter Free)를 조합한 치료 계획으로 피부 선량 증가 여부를 팬텀 실험을 통해 알아보고자 하였다. 대상 및 방법: 두경부암 치료시 가장 흔하게 사용하는 6MV-FF(Flattening Filter) VMAT(Volumetric-Modulated Arc Therapy) 치료계획을 기준으로 정하고, VMAT와 IMRT, FF와 FFF, 5 mm 볼루스 적용 유무를 조합하여 비교군을 생성하였다. 란도 팬텀 콧등에 가상 타겟을 생성하였고, 5 mm 의 가상의 볼루스를 란도 팬텀 콧등 위에 완전 밀착을 가정하여 적용하였다. 팬텀의 코를 기준으로 5개 측정지점을 정하고 각각의 치료계획 을 치료테크닉과 bolus 적용유무에 맞게 치료계획 당 3회를 조사하여 흡수선량을 측정하였다. 결 과: FF vs FFF 에서의 피부선량 차이는 VMAT bolus off에서 FFF인 경우 증가하였고, IMRT bolus off의 경우 차이를 나타내지 않았다. VMAT bolus 5 mm, IMRT bolus 5 mm에서는 FFF에서 오히려 피부선량이 감소되는 것을 확인 할 수 있었다. VMAT vs IMRT 피부선량 차이는 FFF bolus off 의 경우에만 피부선량이 증가했고, 나머지는 통계적 차이가 없었다. bolus off vs bolus 5 mm 에서의 피부선량 차이는 IMRT FFF 을 사용했을 경우만 제외하고는 모두 bolus 5 mm 에서 피부선량이 증가하는 것을 확인하였다. IMRT와 FFF를 조합한 치료 계획은 통상적으로 사용되는 VMAT와 FF를 사용하여 5 mm 볼루스를 적용한 치료 계획 피부선량의 측정값을 분석한 결과 통계적으로 유의미한 차이를 발견하지 못했다. 따라서 IMRT_FFF 를 사용함으로써 VMAT_FF 에 5 mm 볼루스 적용한 것과 비슷한 수준의 피부선량 전달이 가능할 것이라 사료되며, 이는 높은 피부선량이 필요하지만, 볼루스 적용이 어려운 환자와 같은 경우에 유용할 수 있다. 결 론: 볼루스 적용이 어려운 환자의 경우, VMAT와 FF에 비교하여 IMRT와 FFF를 적절히 조합한 치료 계획으로 피부 선량 증가를 기대할 수 있다.

토모테라피 시 종양의 위치에 따른 피부선량 검증 (Verification of skin dose according to the location of tumor in Tomotherapy)

  • 목 적 : 토모테라피를 이용한 방사선치료 시 종양의 위치 변화에 따른 피부선량을 알아보기 위해 Gafchromic EBT3 film을 이용하여 피부선량을 측정하고, 치료계획 선량과 비교하여 그 차이를 알아보고자 한다. 대상 및 방법 : 피부선량 측정을 위한 팬톰은 I'm RT Phantom(IBA Dosimetry, Germany)을 사용하였으며, 2.5 mm CT영상을 획득한 후 Pinnacle(ver9.2, Philips Medical System, USA)을 이용하여 종양의 위치와 피부 선량 측정 점을 설정하였다. 종양(40.75 cm3)의 위치는 팬톰의 중앙과 표면으로부터 5 mm, 10 mm 거리로, 토모테이블 감약을 고려하여 팬톰의 천장과 바닥방향으로 대칭하게 각각 설정하였으며, 피부 선량의 측정점은 표면으로부터 3 mm, 5 mm 두께로 3회 반복 측정하였다. TomoHD(TomoHD treatment system, Tomotherapy Inc., Madison, Wisconsin, USA)를 이용하여 토모테라피 치료 계획을 수립하고, Gafchromic EBT3 필름을 팬톰 내 삽입하여 3회 반복 측정한 뒤 측정된 피부 선량과 치료계획 선량을 비교하였다. 결 과 : 팬톰의 상부 측 피부선량은 종양이 중앙에 위치한 경우 5 mm, 3 mm에서 7.53 cGy, 7.25 cGy였으며, 천장 측 피부로부터 5 mm 떨어져 위치한 경우 각각 18.06 cGy, 16.89 cGy, 10mm 떨어진 경우 각각 20.37 cGy, 18.27 cGy의 값을 나타내었다. 팸톰 하부 측 피부선량은 종양이 중앙에 위치한 경우 5 mm, 3 mm에서 각각 8.82 cGy, 8.29 cGy였으며, 하부 측 피부로부터 5mm 떨어져 위치한 경우 각각 21.69 cGy, 19.78 cGy, 10mm 떨어진 경우 각각 20.48 cGy, 19.57 cGy로 나타났다. 종양이 중앙에 위치한 경우의 피부 선량은 치료계획 시보다 3.2~17.1%이상 증가한 반면, 종양이 천장 측 5 mm에 위치한 경우 2.8~9.0%로 감소하였고, 토모테이블 방향으로 피부선량은 치료계획 선량에 비해 평균 11%이상 증가함을 보였다. 결 론 : 이번 연구 결과 토모테라피를 위한 치료 계획 시 피부선량은 실제 피부선량과 오차가 발생하였으며, 특히 토모 테이블 방향으로의 피부 선량은 치료계획상의 피부선량보다 증가하는 것을 알 수 있었다. 따라서 토모 테이블과 근접하여 위치한 종양의 치료 시 정확한 선량 계산 시스템과 피부선량의 검증 노력이 필요할 것으로 사료된다.

Anti-obesity effect of resveratrol-amplified grape skin extracts on 3T3-L1 adipocytes differentiation

  • Resveratrol (3,4,5-trihydroxy-trans-stilbene), a phytoalexin found in grape skin, grape products, and peanuts as well as red wine, has been reported to have various biological and pharmacological properties. The purpose of this study was to investigate the anti-obesity effect of resveratrol-amplified grape skin extracts on adipocytes. The anti-obesity effects of grape skin extracts were investigated by measuring proliferation and differentiation in 3T3-L1 cells. The effect of grape skin ethanol extracts on cell proliferation was detected by the MTS assay. The morphological changes and degree of adipogenesis of preadipocyte 3T3-L1 cells were measured by Oil Red-O staining assay. Treatment with extracts of resveratrol-amplified grape skin decreased lipid accumulation and glycerol-3-phosphate dehydrogenase activity without affecting 3T3-L1 cell viability. Grape skin extract treatment resulted in significantly attenuated expression of key adipogenic transcription factors, including peroxisome proliferator-activated receptor, CCAAT/enhancer-binding proteins, and their target genes (FAS, aP2, SCD-1, and LPL). These results indicate that resveratrol-amplified grape skin extracts may be useful for preventing obesity by regulating lipid metabolism.

New possibility of chlorogenic acid treatment on skin aging

  • It is well-documented that decreased antioxidant defense system by ultraviolet(UV) irradiation is the most important reason to induce the skin aging, especially photoaging. Chlorogenic acid(CA), a nonflavonoid catecholic compound, is present in the diet as part of fruits, tea, coffee and wine and has been reported to have anti-inflammatory, antimutagenic and anticarcinogenic activities. In this study, we examined the effects of CA on the UV -induced photoaging. Firstly, we investigated the protective effect of CA on antioxidant defense system in HaCaT human keratinocytes after UV irradiation treatment. UV irradiation decreased antioxidant defence enzyme activities of superoxide dismutase, catalase and GSH contents, which were restored by CA. To elucidate the effect of CA, 1% of CA and vehicle were applied to human buttock skin before and after UV irradiation (2MED). CA prevented UV -induced matrix metalloproteinase-1 mRNA expression and procollagen mRNA depression. And CA also increased CD1a(Langerhans cell) expression significantly. Our results suggest that CA has protective effects on UV -induced photoaging by increasing cellular antioxidant defense system. Therefore, CA may be a useful anti-aging agent for cosmetic purpose.

The Use of Matriderm and Autologous Skin Graft in the Treatment of Full Thickness Skin Defects

  • Background For patients with full thickness skin defects, autologous Split-thickness skin grafts (STSG) are generally regarded as the mainstay of treatment. However, skin grafts have some limitations, including undesirable outcomes resulting from scars, poor elasticity, and limitations in joint movement due to contractures. In this study, we present outcomes of Matriderm grafts used for various skin tissue defects whether it improves on these drawbacks. Methods From January 2010 to March 2012, a retrospective review of patients who had undergone autologous STSG with Matriderm was performed. We assessed graft survival to evaluate the effectiveness of Matriderm. We also evaluated skin quality using a Cutometer, Corneometer, Tewameter, or Mexameter, approximately 12 months after surgery. Results A total of 31 patients underwent STSG with Matriderm during the study period. The success rate of skin grafting was 96.7%. The elasticity value of the portion on which Matriderm was applied was 0.765 (range, 0.635-0.800), the value of the trans-epidermal water loss (TEWL) was 10.0 (range, 8.15-11.00)$g/hr/m^2$, and the humidification value was 24.0 (range, 15.5-30.0). The levels of erythema and melanin were 352.0 arbitrary unit (AU) (range, 299.25-402.75 AU) and 211.0 AU (range, 158.25-297.00 AU), respectively. When comparing the values of elasticity and TEWL of the skin treated with Matriderm to the values of the surrounding skin, there was no statistically significant difference between the groups. Conclusions The results of this study demonstrate that a dermal substitute (Matriderm) with STSG was adopted stably and with minimal complications. Furthermore, comparing Matriderm grafted skin to normal skin using Cutometer, Matriderm proved valuable in restoring skin elasticity and the skin barrier.

두시(豆豉) 추출물을 이용한 하태독법(下胎毒法)이 NC/Nga 생쥐에서 유발된 아토피 유도 피부염에 미치는 항염증 효과 (The Anti-inflammatory Effects of Hataedock Taken Douchi Extracts on Atopic Dermatitis-like Skin Lesion of NC/Nga Mouse)

  • Objectives Hataedock is a Korean herbal medical oral treatment that removes fetal toxic heat and meconium from new born babies. The purpose of this study is to evaluate whether Hataedock treatment of Duchi extracts has anti-inflammation effects on AD (Atopic Dermatitis)-induced NC/Nga mice. Methods After Hataedock treatment of Duchi extracts on days 0, 3-week-old NC/Nga mice were sensitized on days 28, 35, 42 by exposure of DNFB (dinitrochlorobenzene) and were induced to have AD. Immunohistochemistry of NF-${\kappa}B$ p65, iNOS, COX-2 and TUNEL assay of apoptotic body was used to identify changes of skin damages and anti-inflammation effects. Results The alleviate effect of the skin damage and angiogenesis was observed in DT group. The damage of stratum corneum, hyperplasia, edema, infiltration of lymphocytes and distribution of capillary were decreased in DT group. Also, the study results suggested that Hataedock treatment of Duchi extracts in DT group remarkably downregulated levels of NF-${\kappa}B$ p65 by 70% (p < 0.001), as well as COX-2 by 51%, iNOS by 62% (p < 0.001). Additionally, Hataedock treatment of Duchi extracts in DT group up-regulated apoptosis of inflammatory cells by 68% in atopic dermatitis-like skin lesion. Conclusions From the study results, we observed that Hataedock treatment of Duchi extracts alleviates AD through diminishing various inflammatory cytokines in the skin lesions, which are involved in the initial steps of AD development. It is anticipated to have potential applications for prevention and treatment of atopic dermatitis.

Concordant Surgical Treatment: Non-melanocytic Skin Cancer of the Head and Neck

  • Background: Skin cancer is the most common type of cancer. Of the 4 million skin lesions excised annually worldwide, approximately 2 million are considered cancerous. In this study, we aimed to describe a regional experience with skin cancers treated by a single senior surgeon and to provide a treatment algorithm. Methods: The medical records of 176 patients with head and neck non-melanocytic skin cancer (NMSC) who were treated by a single surgeon at our institution between January 2010 and May 2016 were retrospectively reviewed, and their data (age, sex, pathological type, tumor location/size, treatment modality) were analyzed. Patients with cutaneous squamous cell carcinoma (cSCC) who were classified as a high-risk group for nodal metastasis underwent sentinel node mapping according to the National Comprehensive Cancer Network guidelines. Results: Among the patients with NMSC who were treated during this period, basal cell carcinoma (BCC; n=102, 57.9%) was the most common pathological type, followed by cSCC (n=66, 37.5%). Most lesions were treated by complete excision, with tumor-free surgical margins determined via frozen section pathology. Thirty-one patients with high-metastasis-risk cSCC underwent sentinel node mapping, and 17 (54.8%) exhibited radiologically positive sentinel nodes. Although these nodes were pathologically negative for metastasis, 2 patients (6.5%) later developed lymph node metastases. Conclusion: In our experience, BCC treatment should comprise wide excision with tumor-free surgical margins and proper reconstruction. In contrast, patients with cSCC should undergo lymphoscintigraphy, as nodal metastases are a possibility. Proper diagnosis and treatment could reduce the undesirably high morbidity and mortality rates.

Effects of red ginseng on the elastic properties of human skin

  • Background: Red ginseng contains components, including microelements, vitamins, essential oils, and fatty acids, that can be used in skincare to delay the aging process. We investigated the effects of red ginseng treatment on skin elasticity by assessing cellular stiffness and measuring collagen protein synthesis. Methods: Human dermal fibroblasts were treated with red ginseng, and the resulting changes in stiffness were investigated using atomic force microscopy. Cytoskeletal changes and mRNA expression of biomarkers of aging, including that of procollagens I and VII, elastin, and fibrillin-1, were investigated. Collagen in a human skin equivalent treated with red ginseng was visualized via hematoxylin and eosin staining, scanning electron microscopy, and atomic force microscopy. Results and conclusion: The stiffness of fibroblasts was significantly reduced by treatment with red ginseng concentrations of ≥ 0.8 mg/mL. The ratio of F-actin to G-actin decreased after treatment, which corresponded to a change in fibroblast stiffness. The storage modulus (G') and loss modulus (G'') of the skin equivalent were both lowered by red ginseng treatment. This result indicates that the viscoelasticity of the skin equivalent can be restored by red ginseng treatment.

폴리프로필렌 복합소재의 아르곤 플라즈마 처리로 표면층 제거와 젖음성 향상 (Improvement of Wettability and Removal of Skin Layer on Ar-Plasma-Treated Polypropylene Blend Surface)

  • 아르곤(Ar) 플라즈마 처리된 폴리프로필렌 복합소재의 표면 개질 및 특성 변화를 X-선 광전자 분광 분석(XPS), 적외선 분광 분석(FTIR), 주사 전자 현미경 분석(SEM) 및 접촉각 측정 등을 이용하여 조사하였다. Ar 플라즈마 처리 시간의 증가는 폴리프로필렌 복합소재 표면의 젖음성, 극성 관능기를 갖는 산소 성분, 탈크 함량 및 표면조도의 증가를 초래하였다. 주사 전자 현미경 분석을 통한 자세한 관찰은 폴리프로필렌 성분으로 구성된 표면층(skin layer)이 존재함을 확인하였다. 폴리프로필렌과 고무 입자간의 점도차는 표면층의 생성을 촉진시켰다. 하지만 Ar 플라즈마 처리시간의 증가는 표면층의 두께를 감소시키는 것을 확인하였다. 사출성형 공정 동안, 표면층을 제거할 수 있는 추가적인 방법론에 대해서도 토의하였다. Ar 플라즈마 처리에 의한 표면 개질 및 모폴로지의 변화는 폴리프로필렌 복합소재 표면 상에 친수화 상태를 부여하고, 이에 따라 젖음성 향상을 유도하였다.
