A Case of Clonorchis Sinensis with Multiple Low Echogenic Nodules in a Child (소아에서 진단된 다발성 저음영 결절 형태를 보인 간흡충증 1례)

  • Clonorchiasis, known as the parasitic fluke in the intrahepatic bile ducts, occurs throughout Korea, Japan, Taiwan, and Vietnam with high frequency. Most of the infections follow the consumptions of raw or undercooked freshwater fish, which explains its rare occurrence to childhood even in those areas mentioned earlier above. The authors have experienced it with an 8 year old boy. Abdominal ultrasonogram revealed multiple low echogenic nodules in the right lobe of liver and lymph nodes proliferation around the porta hepatis area. And abdominal C.T. scan also found multiple low attenuated nodules with delayed enhancement in liver parenchyma with hepatomegaly and multiple conglomerated lymph nodes in porta hepatis. Both abdominal ultrasonogram and C.T. scan didn't show any dilatation of peripheral intrahepatic bile ducts. In repeated stool examination, Clonorchis sinensis eggs were found in feces and the result of the skin test for Clonorchis sinensis was positive. The patient had a history of having eaten raw pond smelt back in Cheongju a month before the symptom onset. On the basis of the patient's history and the fact that Clonorchis eggs were found in feces 3 weeks after eosinophilia and symptoms were noted, the authors estimated that the findings of the patient's abdominal sonogram and C.T. scan were the radiologic findings of the acute phase of clonorchiasis, of which no report had ever been made before. So the authors report a case of clonorchiasis with atypical radiologic findings in an 8 year old boy.

Seven-Year Follow Up of Microscopic Polyangiitis Presenting with Rapidly Progressive Glomerulonephritis (급속 진행성 사구체신염으로 시작된 현미경적 다발성 동맥염(Microscopic Polyangiitis) 환자의 7년간의 장기 추적관찰 1례)

  • Microscopic polyangiitis(MPA) is a systemic necrotizing vasculitis that involves many organ systems including the skin, joint, kidneys, and lungs. In spite of early diagnosis and intensive care, the five-year actuarial patient and kidney survival rates are 65% and 55%. We experienced a case in 7-year-old girl of microscopic polyangiitis presenting with rapidly progressive glomerulonephritis which was confirmed by renal biopsy and positive serum perinuclear antineutrophil cytoplasmic autoantibodies(p-ANCA). The diagnosis of patients first renal biopsy was MPA, p-ANCA-associated crescentic glomerulonephritis. The patients second renal biopsy was done 5 years 6 months later since first renal biopsy, and pathologic diagnosis was chronic sclerosing glomerulonephritis, advanced, due to MPA. We began methylprednisolone pulse therapy, combined with a low dose of cyclophosphamide and plasmapheresis therapy. ACE inhibitor, angiotensin II receptor blocker, and cyclophosphamide were used until now and the patients current age is 14 years old. On admission, the patients laboratory findings showed BUN 117 mg/dL and Cr 2.3 mg/dL, while on the hospital day BUN and Cr values fell to 20.8 mg/dL and 1.6 mg/dL. But renal function was progressed to chronic failure with latest laboratory data BUN 51.7 mg/dL and Cr 3.2 mg/dL. ACE inhibitor, angiotensin II receptor blocker and small dose of immunosuppressant with close observation is the key to maintain the patient survival.

Antioxidant, anti-inflammatory, and anti-pruritic effects of grape branch extract (포도가지 추출물의 산화방지, 항염증 및 항가려움 효과)

  • This study was designed to investigate the antioxidant activities of the ethanol extracts (GBE) of grape branches (Campbell Early). The total polyphenol and flavonoid content of GBE was $201.42{\pm}4.16$ and $11.85{\pm}0.44mg\;GAE/g$, respectively. The antioxidant activity of GBE was measured using the ABTS and DPPH assays, and the $IC_{50}$ values were $45.60{\pm}0.09$ and $299.13{\pm}0.22$, respectively. GBE inhibited the production of pro-inflammatory mediators (NO, iNOS, $PGE_2$, COX-2, $IL-1{\beta}$, and IL-6) in lipopolysaccharide-stimulated RAW 264.7 macrophages in a dose-dependent manner. Moreover, GBE treatment significantly suppressed the production of $TNF-{\alpha}$ and IL-6 cytokines in phorbol 12-myristate 13-acetate plus calcium ionophore A23187-stimulated HMC-1 human mast cells. Furthermore, the administration of GBE markedly inhibited the scratching behavior induced by the compound 48/80 in ICR mice. These results suggested that GBE has potential as a therapeutic agent against inflammation and itch-related skin diseases.

Analysis of Phytochemicals in Popular Medicinal Herbs by HPLC and GC-MS (HPLC와 GC/MS를 활용한 약용식물 유래 phytochemicals 분석)

  • Oriental herbs are reported as having potent functions for preventing many types of diseases. They also appear to have positive effects and potential capabilities for skin care. Among the many oriental herbs that are available, we chose to analyze four medicinal herbs, Korean red ginseng, Artemisia capillaries Thunb, Schizonepeta tenuifolia Briq, and Foeniculum vulgare Mill, because all are popular and considered as favorite medicinal plants in Korea. Extracts of the herbs were obtained by various methods such as using distilled water, ethyl ether, methanol, ethanol, benzene, 1-butanol, and chloroform. Nine phytochemicals were detected in the extracts: maltol, adenosine, b-pinene, menthone, pulegone, limonene, anethole, estragole, and fenchone, which reportedly have multi-functionalities. All phytochemicals were analyzed quantitatively by various chromatographic techniques such as HPLC and gas chromatography-mass (GC-MS) spectrometry. This article also presents the optimum conditions for extracting these 9 targeted phytochemical compounds that were derived from 4 popular oriental herbs, which could be useful for the efficient preparation of each phytochemical.

Three Cases of Pulmonary Paragonimiasis in a Family after Ingestion of Raw Fresh-water Crayfishes caught in a Stream of Wolchulmountain (월출산 민물가재 생식에 의한 한 가족내 폐흡충증 3례)

  • Human infection with the lung fluke Paragonimus westermani has become rare in Korea. Human paragonimiasis is caused by eating raw fresh-water crayfishes or crabs infected with larval metacercariae. Recently, we experienced three cases of pulmonary paragonimiasis in a family. They ate raw fresh-water crayfishes that lived in a stream in Wolchulmountain. All the parients had hypereosinophilia and pulmonary infiltrates with pleural effusion or hydropneumothorax, which did not improve on antibiotics. Ingestion of raw crayfishes was a clue for paragonimiasis. Positive results were shown both on intradermal skin test and ELISA for Paragonimus westermani specific IgG. After treatment with praziquantel, the patients showed an improvement. This is the first familial human paragonimiasis, reported from Wolchulmountain in Chonnam Province where there had been no previous cases of paragonimiasis.

  • The cloning of canids was succeeded in 2005, several years after the birth of Dolly the sheep and also after the cloning of numerous other laboratory and farm animal species. The delay of successful somatic cell nuclear transfer (SCNT)was due to the unique reproductive characteristics of the female dogin comparison to other domestic mammals, such as ovulation of immature canine oocyte and a requirement of 25 days for the completion of meiosis within the oviduct (Holst & Phemister, 1971). When the technology for the recovery of in vivo matured oocyte was established, the application of cloning also became possible and cloned dog offspring were obtained. This report summarizes the progress of technical procedures that are required for cloning canids and the application of this technique. The first cloned dog, Snuppy, was achieved using an in vivo-matured oocyte which was enucleated and transferred with an adult skin cell of male Afghan hound. After establishment of a criterion of well-matured oocyte for the improvement of SCNT efficiency, we obtained three cloned female Afghan hound and a toy poodle cloned from 14 year-old aged Poodle using SCNT through this factor. To date, cloned dogs appeared to be normal and those that have reached puberty have been confirmed to be fertile. Through application of canine SCNT technique, first, we demonstrated that SNCT is useful for conserving the breed of endangered animal from extinction through cloning of endangered gray wolves using inter-species SCNT and keeping the pure pedigree through the cloning of Sapsaree, a Korean natural monument. Secondly, we showed possibility of human disease model cloned dog and transgenic cloned dog production through cloning of red fluorescent protein expressing dog. Finally, SCNT can be used for the propagation of valuable genotypes for making elite seed stock and pet dog. In summary, dog cloning is a reproducible technique that offers the opportunity to preserve valuable genetics and a potential step towards the production of gene targeted transgenic cloned dogs for the study of human diseases.

Antioxidative Activities and Whitening Effects of Ethyl Acetate Fractions from The Immature Seeds of Abeliophyllum distichum (미선나무 미성숙 종자의 항산화 및 미백 활성)

  • Abeliophyllum distichum Nakai is deciduous shrubs of flowering plant in Oleaceae. It is important plant resources and consists of one species in the world. Also the endemic plant of A. distichum has been protected and designed endangered plant in Korea. For this reason, study on the immature seeds of A. distichum (ADS) hasn't progressed. In the present study, we evaluated the antioxidant activity and inhibitory effects on proteins and mRNA levels were related in the whitening effect in B16F10 cells. ADS was effective for reaction oxygen species (ROS). ROS causes various diseases such as aging, inflammation, cancer, and etc. Antioxidant properties were evaluated DPPH, ABTS radical scavenging activity and Reducing power. Plants were known that contained phenolic compounds were related in antioxidant activity. Phenolic compounds were phytochemicals commonly named natural polyphenols. These are secondary metabolites of plants involved in the defense against different types of stresses. In results, ADS suppressed the expression and transcription of Tyrosinase, TRP-1, TRP-2, and Microphthalmia-associated transcription factor (MITF). Tyrosinase, tyrosinase-related protein 1 (TRP-1), tyrosinase-related protein 1 (TRP-2) are known to play an important role in melanin biosynthesis. MITF regulated the expression and transcription of Tyrosinase, TRP-1, and TRP-2. In conclusion, ADS was effective in both antioxidant activities and whitening effects. Also, they were associated with the content of phenolic compounds. We suggested that ADS can be use antioxidants and skin-whitening functional cosmetics material derived from natural plant resources.

Effect of Lime Sulfur on Changes of Fungal Diversity in Pear Fallen Leaves (석회유황합제가 배나무 낙엽의 진균 다양성 변화에 미치는 영향)

  • This study was conducted to examine changes in the fungal community on fallen leaves of pear by treatment with lime sulfur. Although the lime sulfur could reduce the primary inoculum of several pathogens on spring season, the effect of lime sulfur has not been well determined scientifically. Fallen leaves infected by pear diseases in pear orchards in Naju were collected and treated with lime sulfur or water as a control. To determine the fungal diversity from each treatment, rDNA internal transcribed spacer (ITS) regions were analyzed after extraction of fungal genomic DNA from lime sulfur-treated or water-treated fallen leaves, respectively. The most common fungal species were Ascomycota and Basidiomycota in both treated leaves. However, the population dynamics of several fungal species including Alternari sp., Cladosporium sp., and Phomopsis sp., which are known as pear pathogens for skin sooty dapple disease, were quite different from each treated leaves. These results indicated that lime sulfur treatment led to changes of fungal communities on pear fallen leaves and could be applicable as a dormant spray.

[ $5{\alpha}-Reductase$ ] Inhibitors from Native Plants and their Sebosuppressive Effects in Cultured Human Sebaceous Gland Cells (천연물 유래의 $5{\alpha}-Reductase$ 저해제의 개발과 인체 유래 피지선 세포의 배양을 이용한 피지분비 억제 효과 측정)

  • This study was performed to develop new cosmeceutical agents with sebosuppressive activity from native plant extracts in Korea. Inhibitory efforts of the extracts on $5{\alpha}-reductase$ (5-AR) were evaluated by enzyme kinetics analysis using UV-spectrophotometric method. Two kinds of enzyme suspensions as 5-AR sources were prepared from rat liver tissue and cultured hSG cells. The sebosuppressive effects were determined by measuring the total lipid quantity produced in cultured hSG cells after incubation with the extracts. As a result, Pinus thunbergii extracts showed the most potent 5-AR inhibitory effects. Its $K_i$ values were 0.0002% and 0.0014% for rat liver 5-AR and human sebaceous gland 5-AR, respectively. Addition of Pinus thunberii extract to hSG cells showed 48% reduction in total lipid production at 0.005% concentration. In conclusion, Pinus thunbergii extracts can be used as a cosmeceutical agent to regulate sebum production and to alleviate the sebum-involved skin diseases, such as acne and seborrheic dermatitis.

Evaluation of Using Pattern of the Topical Agents in Korean Community (한국 외래형 국소용 피부 외용제의 사용 현황에 대한 기초 평가)

  • Topical agents including of steroids are easily exposed to Korean patients in community setting. Those topical agents must be use cautiously because can cause adverse drug reaction unlike other oral medication. We evaluate the using pattern topical agents of Korean in community setting. We performed a questionnaire survey in three different pharmacies (general community pharmacy, dermatology clinic oriented pharmacy, general hospital oriented pharmacy) and in three university town. The survey asked necessity of topical agents, the number of applications per day, application period, site, and exist of steroids ingredient, etc. Total 330 people was participated in the survey from May 1st to July 31st, 2014. We analysed the survey by the frequency of each question and cross-analysis of the compliance between general public and pharmacy groups. In results, Korean people used topical agents for sudden skin diseases (66.9%), was gotten by dermatologist's prescription (89.1%), and did not know their diagnosis (39.2%). The people applied topical agents on face (22.3%), less one half FTU (finger tip unit) per application (61.9%), less than one week (62.9%). Though they was received drug education from pharmacist (52.2%), did not heard about adverse effects (78.4%). And compliance of applied times in pharmacy group was higher than in-non-pharmacy group (56% vs 38%, p=0.002). The highest compliance between 3 pharmacy subgroup was general community pharmacy (76.3% vs 61.6%, 61.2%, p=0.039). Compliance of applied duration was no difference between groups. Majority of respondents did not know diversity of ingredients, potency, application area of topical steroids, and replied to feel some vague reluctance and to think so dangerous about topical steroids. In conclusion, Korean people in community use topical agents well but still some exposed to risk from topical agents.
