본 연구는 성장기 II급 부정교합자를 골격 형태에 따라 분류하여 액티베이터 사용 효과를 비교, 분석하고 그 결과를 진단, 치료 계획 수립 등 임상 과정에 연계시키기 위해 시행되었다. 실험은 수완부 방사선 사진상 Fishman 분류의 2, 3, 4 단계로 최대성장기 이전의 II급 부정교합자를 대상으로 하였다. 대조군은 ANB 3도 이상이며 악정형 장치 치료를 받지 않고 치열교정 치료만 받은 환자로 총 25명(남자 15명, 여자 10명)이고 실험군은 액티베이터 치료를 받은 환자로 총 116명(남자 53명, 여자 63명)이었다. Articular angle과 gonial angle을 이용하여 골격 형태를 hyperdivergent type과 hypodivergent type으로 분류하여 액티베이터 치료효과를 비교한 결과 hypodivergent한 골격 형태를 가진 환자에서 더 큰 효과를 보였다. 따라서 성장기 II급 부정교합자의 진단과 치료계획 수립 시 안모의 골격 형태 분류를 통해 액티베이터의 효과를 예측할 수 있다고 생각한다.
This study was designed to investigate the difference between craniofacial characteristics of the normal occlusion and those of Class II Div. 1 malocclusion. The sample was divided into 2 groups, the 50 subjects of Normal occlusion, the 50 subjects of Class II Div. 1 malocclusion in both sexes. Both groups aged from 11 to 14 years. The results of this study were as follows; 1. No significant difference was observed in cranial base shape between both groups, but anterior cranial base size of Class II Div. 1 malocclusion group was larger than that of normal group. 2. No significant difference in antero-posterior position of Maxilla to cranial base was founded between both groups. 3. No difference in Mandibular shapes and Mandibular plane angles to the cranial base was observed between Class II Div. 1 malocclusion and normal occlusion, but Mandibular position in Class II Div. 1 malocclusion was posterior to that of normal group. 4. Antero-posterior relationship of Maxilla and Mandible was significant difference between both groups, but vertical relationship of those was no difference. 5. Maxillary incisor position to cranial base of Class II Div. 1 malocclusion was anteior to normal occlusion, and Maxillary posterior teeth was posterior. Mandibular incisor and mandibular posterior teeth position was no difference. 6. Upper and lower lip position to esthetic line of Class II Div. 1 malocclusion was anterior to normal occlusion.
Objective: To compare crown-root angulations of the permanent maxillary anterior teeth in skeletal Class I, Class II, and Class III Korean malocclusion patients using cone-bean computed tomography (CBCT) images. Methods: Sixty CBCT images were collected from orthodontic patients archive based on skeletal Class I (0˚< A point-nasion-B point angle [ANB] < 4˚), Class II (ANB ≥ 4˚), and Class III (ANB ≤ 0˚) to have 20 samples in each group. Mesiodistal crown-root angulation (MDCRA) and labiolingual crown-root angulation (LLCRA) were evaluated after orientation of images. Crown-root angulations were compared among Class I, Class II, and Class III groups and among the maxillary anterior teeth in each group. Results: LLCRAs of the maxillary central incisor and the lateral incisor were significantly lower in Class III group than those in Class I group. However, those of the canine showed no significant differences among groups. MDCRAs of the maxillary anterior teeth did not significantly differ among groups either. Conclusions: Our results suggest that skeletal Class III malocclusion might affect LLCRA of the maxillary incisors, especially the central incisor.
This investigation was designed to categorize Angle's class II malocclusion groups through analyzing horizontal and vertical components of craniofacial skeleton in Angle's class II malocclusion. The material selected for this study consisted in standard lateral cephalogram of two hundred and twenteen children, eighty eight boys and one hundred twenty four girls, aged 6 through 18 years, having Angle's class II malocclusion. On the basis of findings of this study, the following results were obtained. 1. In horizontal skeletal classifications, 16 groups were classified according to FMN-A-B, SE-FMN-A, Ba-SE-Me, Ba-Se/Ra P. The sequences that have relatively high frequency are as follows: a) Horizontal Group 16 b) Horizontal Group 12 c) Horizontal Group 13 d) Horizontal Group 9 & 15 2. In vertical skeletal classification, 8 groups were classified according to the PMV/PP, PMV/Occ. P. PMV/Mn. P. To study the calcification of second molars in skeletal class II malocclusion, the author examined cephalograms, orthopantomograms, periapical films of 391 children from 7y 0m to 15y 11 m years old who had skeletal class II malocclusion, and observed the difference in the calcification stage between upper and lower second molars. The result s are as follows. 1. The mean ages of crown completion of upper and lower second molars are $8.7{\pm}1.75$, $8.8{\pm}1.13$ in boys, and $8.4{\pm}0.81$, $8.5{\pm}0.91$ in girls. 2. The mean ages of root completion of upper and lower second molars are $14.0{\pm}1.09,\;14.5{\pm}0.52$ in boys, and $13.7{\pm}1.15,\;13.8{\pm}1.18$ in girls. 3. The calcification stages of upper second molars are more advanced than those of lower second molars at $5\%$ level in both sexes.
정상교합자와 골격성 III급 부정교합자의 저작운동형태를 비교연구하기위해서 30명의 정상교합자와 20명의 골격성 III급 부정교합자를 대상으로 츄잉검을 사용하고 BioPAK system을 이용하여 전두면상에서 저작운동을 기록, 분석하였다. 각 개체마다 대표하는 저작경로를 정하여 저작폭경, 개구거리, 개구각, 페구각, 최대개구속도, 최대폐구속도를 알아보았다. 또 저작경로의 형태에 따라 특징적인 7가지 패턴으로 분류하여 다음의 결과를 얻었다. 1. 정상교합자군에 비해 골격성 III급 부정교합자군에서는 좀 더 다양하고 수직적인 저작양상을 나타내었다. 2. 저작폭경의 비교에서는 골격성 III급 부정교합자군이 정상교합자군에 비해 더 좁게 나타났다(p<0.01) 3. 개구거리에서는 골격성 III급 부정교합자군이 정상교합자군에 비해 더 작게 나타났으나 통계학적으로 유의성은 없었다(p>0.05). 4. 개구각과 폐구각에서는 골격성 III급 부정교합자군에서 정상교합자군보다 더욱 예각으로 나타났다(p<0.01). 5. 최대개구속도, 최대폐구속도에서는 정상교합자군에 비해 골격성 III급 부정교합자군에서 느리게 나타났으나 통계학적으로 유의성은 없었다(p>0.05). 6. 저작운동형태에 따른 분류에서 정상교합자군에서는 Type II가 73.4%로 가장 많은 비율을 차지하였으나, 골격성 III 급 부정교합자군에서는 Type III가 35.0%, Type II가 30.0%로 많은 비율을 차지하였다. 7. 저작운동형 태에 따른 분류에서 골격성 III급 부정교합자군에서는 Type IV(chopping type)가 25.0%로 정상교합자군 3.3%에 비해 많은 비율을 차지하였다.
The study was designed to examinate the discrimination rate of 4 antero-posterior cephalometric measurements from Angle's malocclusion groups. The material was 246 cephalometric radiographs taken from 42 Normal occlusions, 83 Class I malocclusions, 64Class II Division I malocclusions, 57Class III malocclusions. ANB angle, APDI, AB/OP angle, and Wits appraisal as measurements of antero-posterior skeletal relationship were measured on the cephalometric radiographs and statistically analyzed by the Canonical Discriminant Function. The results of this study were as follows: 1. ANB angle, APDI, AB/OP angle, and Wits appraisal were clinically useful measurements for the evaluation of the antero-posterior skeletal relationship. 2. The rates that discriminate actual malocclusion groups were $77.45\%$ in Wits appraisal, $74.02\%$ in AB/OP angle, $71.08\%$ in ANB angle, and $70.59\%$ in APDI. 3. The discrimination rate of actual Class III malocclusion show above $93\%$, but actual Class I and Class II division I malocclusions were relatively low.
Objective: Treating Class II subdivision malocclusion with asymmetry has been a challenge for orthodontists because of the complicated characteristics of asymmetry. This study aimed to explore the characteristics of dental and skeletal asymmetry in Class II subdivision malocclusion, and to assess the relationship between the condyle-glenoid fossa and first molar. Methods: Cone-beam computed tomographic images of 32 patients with Class II subdivision malocclusion were three-dimensionally reconstructed using the Mimics software. Forty-five anatomic landmarks on the reconstructed structures were selected and 27 linear and angular measurements were performed. Paired-samples t-tests were used to compare the average differences between the Class I and Class II sides; Pearson correlation coefficient (r) was used for analyzing the linear association. Results: The faciolingual crown angulation of the mandibular first molar (p < 0.05), sagittal position of the maxillary and mandibular first molars (p < 0.01), condylar head height (p < 0.01), condylar process height (p < 0.05), and angle of the posterior wall of the articular tubercle and coronal position of the glenoid fossa (p < 0.01) were significantly different between the two sides. The morphology and position of the condyle-glenoid fossa significantly correlated with the three-dimensional changes in the first molar. Conclusions: Asymmetry in the sagittal position of the maxillary and mandibular first molars between the two sides and significant lingual inclination of the mandibular first molar on the Class II side were the dental characteristics of Class II subdivision malocclusion. Condylar morphology and glenoid fossa position asymmetries were the major components of skeletal asymmetry and were well correlated with the three-dimensional position of the first molar.
Purpose: This retrospective study aimed to evaluate the nasopharyngeal and oropharyngeal dimensions of individuals with skeletal class II, division 1 and division 2 patterns during the pre-peak, peak, and post-peak growth periods for comparison with a skeletal class I control group. Materials and Methods: Totally 124 lateral cephalograms (47 for skeletal class I; 45 for skeletal class II, division 1; and 32 for skeletal class II, division 2) in pre-peak, peak, and post-peak growth periods were selected from the department archives. Thirteen landmarks, 4 angular and 4 linear measurements, and 4 proportional calculations were obtained. The ANOVA and Duncan test were applied to compare the differences among the study groups during the growth periods. Results: Statistically significant differences were found between the skeletal class II, division 2 group and other groups for the gonion-gnathion/sella-nasion angle. The sella-nasion-B-point angle was different among the groups, while the A-point-nasion-B-point angle was significantly different for all 3 groups. The nasopharyngeal airway space showed a statistically significant difference among the groups throughout the growth periods. The interaction among the growth periods and study groups was statistically significant regarding the upper oropharyngeal airway space measurement. The lower oropharyngeal airway space measurement showed a statistically significant difference among the groups, with the smallest dimension observed in the skeletal class II, division 2 group. Conclusion: The naso-oropharyngeal airway dimensions showed a statistically significant difference among the class II, division 1; class II, division 2; and class I groups during different growth periods.
Background: This study aimed to determine the skeletal and dental effects in pediatric and adolescent Korean patients with Class II Division 1 malocclusion treated using the Invisalign Mandibular Advancement (MA®) appliance. Methods: The study included patients aged 6 to 18 years who received orthodontic treatment with the MA® appliance for Class II Division 1 malocclusion at the Department of Pediatric Dentistry, Wonkwnag University Daejeon Dental Hospital, between July 1, 2018, and December 31, 2021. The treatment group consisted of 20 patients, 10 boys and 10 girls. The control participants were also 10 boys and 10 girls. Lateral cephalometric radiographs were taken before and after treatment, and 41 measurements of skeletal and dental changes were measured and analyzed using the V-CephTM 8.0 (Osstem Implant). All analyses were performed using SPSS software (IBM SPSS for Windows, ver 26.0; IBM Corp.), and statistical significance was tested using paired and independent samples t-tests for within-group and between-group comparisons, respectively. Results: The patients in the treatment group showed significant decreases in ANB (A point, Nasion, B point), maxillary protrusion, maxillary anterior incisor labial inclination, and maxillary protrusion after treatment. However, when compared with the growth changes observed in the control group, only ANB and maxillary protrusion decreased, with no significant differences in SNA, SNB, and mandibular length. Conclusion: Collectively, the results of this study confirm that the use of MA® appliance in pediatric and adolescent Korean patients with Class II Division 1 malocclusion results in a reduction of anteroposterior skeletal and dental disharmony.
