A 51 year old man was admitted to the Thoracic and Cardiovascular Department of Kyungpook University Hospital on April 7, 1976, with chief complaints of orthopnea and the chest pain for about 3 months. Physical examination showed narrow pulse pressure, puffy face, engorged neck veins at sitting position, distant heart sound, enlarged liver and edematous upper extremities. The chest roentgenogram demonstrated markedly enlarged cardiac silhouette. Low voltage and the low to diphagic T`s were noted on the electrocardiogram. Paroxysmal ventricular tachycardia was developed intermittently and was subsided spontaneously. Repeated pericardiocentesis were performed each of which yielded from 100 to 300ml. but intractable cardiac failure was progressed. The bacteriology and cytology of the pericardial fluid were not revealed any specific findings. The pericardiectomy was performed to release the intractable cardiac tamponade. Pericardium was found to be thickened and cardiac constriction was noted. The thickened pericardium was easily removed. A large hen`s egg sized dark blue tumor mass occupied the anterior wall of the right atrium and two thumb tip sized pearl gray tumors were placed at the just below portion of the main pulmonary artery. The biopsy report revealed primary fibrosarcoma of the heart. The patient was improved from the symptoms of the cardiac failure during the postoperative course.
The purpose of this study was to analyze the environmental privacy on the perspective of integration and segregation of spatial areas in facilities for the elderly, through investigating required environmental privacy at each hierarchical space, the contextual situation and counterplan for the privacy. For the study, a content analysis for 35 foreign academic journal articles published from 1990 to August of 2013 was conducted. The results of the study were as follows: First, four hierarchical spaces had somewhat integrated characteristics as being required various types of privacy beyond the original features of each space. Especially semi-public and public spaces were needed the privacy similar to in private or semi-private areas. Second, the contextual situation for the privacy was mainly from undesirable access and psychological pressure of other residents, staff and etc. to the residents' personal space, stuff, and behaviors. The other was from no space (no sitting) or no equipments for privacy. Third, the counterplan for privacy was categorized as improvements of physical environments and making defensible space or keeping physical distance of the elderly. It is suggested that welfare of the aged act in Korea about the room capacity for the elderly facilities needs to be reconsidered for the privacy in their individual rooms and the privacy in semi-public and public area must be considered to make the behavior scope of the elderly extended.
The purpose of this study was to collect data of skin temperature and physiological responses which is useful for standardization of insulation measurement in various garments. And we investigated sex and season difference of physiological responses of 60's males and 60's females in hot and cold environment. Healthy ten 60's males and ten 60's females volunteered as subjects. The experiment was conducted three times; One for winter condition($5^{\circ}C$, 45%), another for summer condition($30^{\circ}C$, 65%) and the other for nude condition as control($30^{\circ}C$, 65%). The subjects were sitting for 1 hour with suitable ensemble on each experimental condition in climate chamber. We measured skin temperature, rectal temperature, heart rate, oxygen uptake, sweat rate, blood flow, blood pressure and subjective sensations. We found that skin temperature and most of physiological responses were higher in male subject, summer condition than in female subject, winter condition.
Reduction cranioplasty is one of the treatment modality among many treatment options for macrocephaly with hydrocephalus. The most previous techniques of reduction cranioplasty have some disadvantages such as difficult airway maintenance, pressure sore due to modified prone position, severe venous infarct due to obstruction of venous blood flow and large amount of bleeding from the dissection of superior sagittal sinus to obtain bone fragment needed. A 28-month-old girl had extreme macrocephaly. She couldn't control head rotation and keep sitting position. The operation was performed at supine position with adequate exposure of entire calvarium and the hinge was made in occipital bone fragment that covered posterior part of superior sagittal sinus. Bleeding volume and the other complications were decreased and acceptable reduction was achieved with this method.
Background: A number of researchers have attempted to improve the balance of stroke patients, however there is still a question as to whether taping is effective in increasing balance. Objective: To determine the effect of paretic and non-paretic side taping on the balance ability in patients with stroke. Design: A single-blind randomized controlled trial Methods: This randomized single-blind controlled clinical trial with a repeated measures study included 45 subjects who were randomly assigned to paretic side taping groups (n=15), non-paretic side taping groups (n=15), and trunk exercise groups (n=15). Trunk exercise and paretic side taping groups had taping on the paralyzed erector spinae, while the non-paretic side taping group had taping on the non-paralyzed erector spinae. Trunk exercises were performed for 30 minutes to promote core muscles. The balance ability measured the center of pressure movement (paretic side, non-paretic side,forward, backward, limit of stability) in the sitting position. All measurements were evaluated using BioRescue. Results: All three groups showed significant increase in all variables after 4 weeks. The paretic and non-paretic side taping groups had a significant increase in all variables after 30 min of attachment. However, there was no significant difference among the three groups. Conclusions: Paralysis and non-paralysis taping improved the balance ability of patients with stroke in an immediate effect of 30 min. However, after 4 weeks of intervention, taping with trunk exercise did not differ from single trunk exercise. In future studies, various analyses need to be conducted through more diverse evaluations.
Objective: Adolescent idiopathic scoliosis patients make up 40% of all scoliosis patients, and it is likely to increase even more because of the increase in sitting times due to the pandemic. Method: The subject of this study was a 16-year-old female student. The Cobb's Angle at initial value was 42° at the thoracic and 33° at the lumbar. The subject's height was 161.6 cm, and the type of scoliosis was 3CL. The brace was built with fabric materials with the size information from the X-ray information and actual measurements. The brace was made for the adolescents to wear for a longer time by making them put pressure on the same pressure points of the existing braces. The subjects were required to wear the device for 16 hours every day for three months. Additional features to check the pressure and time were synchronized through an app for easier communication and management with the responsible investigator. Results: After wearing the 3D Fabric brace, Cobb's angle changed from 42° to 33° at the thoracic and 33° to 23° at the lumbar. The ATR changed from 9° to 8° at the thoracic and 11° to 6° at the lumbar. As a result, the changes in the ATR angle do relate to the decrease of Cobb's angle, which made the angle of scoliosis that is bent in a three-dimensional way improve, making the height of the subject increase from 161.6 cm to 163.5 cm. Conclusion: Through this study, developing a brace that is made in the form of the 3CL to align the strap direction and putting pressure on the proper pressure points makes Cobb's angle and the ATR smaller. This means that there is a positive effect on the changes in height. A brace made of light fabric material is a good brace to help treat adolescent idiopathic scoliosis. There was an opinion that it is more comfortable to wear than existing braces, but it seems necessary to conduct a quantitative study about the before and after of wearing the brace and a survey for Korean specific cases.
This study finds out the aged consumers' needs on the developing elderly friendly and sensitive bathtub. This bathtub focuses on enhancement of multi-senses and enhancement of health, safety & convenience during bath. The enhancing multi-senses would have four functions; therapy with color and aroma, the massage with micro water bubbles, the visual and auditory with TV & cinema, and the auditory with music and radio. Also, enhancing others of bathtub would have seven functions; emergency bell, blood-pressure check system for health condition, water temperature indicator, keeping water warm, reading a book in that, and controlling of house entrance door and a getting the phone during bath. The aged consumers' needs find throughout a questionnaire survey, and 497 data was analyzed. The main results are as follow. They consider a full bath behavior as a healthcare. They have an intention to purchase or use the elderly and friendly bathtub, and they prefer using it in common facilities to using it at home. They highly need health related function of bathtub, which are massage (64.4%), blood-pressure check system (55.1%) and aroma therapy (45.7%). The emergency bell (67.0%), keeping water warm (62.2%), and water temperature indicator (49.7%), related to safety & information function of bathtub, are needed. Also, they think tolerance for error and low physical effort in bathtub design alternative, opening door and inside sitting support, to be important.
The purpose of this study was to investigate the change in blood velocity(mm/sec) when compression and/or heat were applied to the knee joint for the elderly who has both normal and painful legs with osteoarthritis(OA). Experimental compression knee band was prepared from the 3D knee data of the average women in 60's. 3D replica of knee was reduced by 7, 10, and 13% from the nude pattern in course direction. Clothing pressure was measured at the front and back of each healthy and painful knee of elderly women for one minute while standing and sitting on the chair. Blood velocity was measured at 13 cm upper from the mid-patella for 15 minutes. Results are as follows: first, compression or heating treatment itself did not change blood velocity of both legs; second, combination treatment with heating and compression was effective to increase blood velocity. In details, for healthy legs, combination treatments with compression by 10% reduced pattern(about 1.3kPa) and heating($43^{\circ}C$) induced the maximal blood velocity, however, for knees with OA, 7% reduced pattern(about 1.0kPa) with simultaneous heating($43^{\circ}C$) was more effective than other cases. These results indicated that pain and spasticity of knee joint with OA could be reduced by applying heat and compression therapy, where the compression level of painful knee should be slightly lower than of healthy leg.
This paper presents the development of a robotic system for rehabilitation of the trunk's ability to maintain postural control under different balance conditions. The system, developed with extensive input from rehabilitation and biomedical engineering experts, consists of a seat mounted on a robotic mechanism capable of moving it with four degrees of freedom (3 rotational and 1 translational). The seat surface has built in instrumentation to gauge the movements of the user's center of pressure (COP) and it can be moved either to track the movements of the COP or according to operator given commands. The system allows two types of leg support. A ground mounted footrest allows participation of legs in postural control while a seat connected footrest constrains the leg movement and limits their involvement in postural control. The design evolution over several prototypes is presented and computer aided structural analysis is used to determine the feasibility of the designed components. The system is pilot tested by a stroke patient and is determined to have potential for use as a trunk rehabilitation tool. Future works involve more detailed studies to evaluate the effects of using this system and to determine its efficacy as a rehabilitation tool.
Objective: The study analyzed degrees of participation in physical activities, demographic and sociological characteristics of middle and high school students, and the relations to mental health related factors, based on "Korea youth risk behavior web-based survey." Therefore, the study can be used as preliminary data for establishing mental health promotion plans for youth and setting contents and directions of health education. Methods: The study analyzed the data, collecting from 75,066 middle and high school students, regarding their demographic and sociological characteristics, mental health, and relations to physical activities by using SPSS Win 15.0. Results: 19.1% of the students answered they had seriously contemplated suicide during the past 12 months, and 43.2% said they are usually under enormous pressure. 55.3% answered that academic achievements and the pressure from future paths are major factors of their stress. The students showed noteworthy differences in whether they think about suicide depending on whether the students live with their father or mother. The study found that the students whose parents' levels of education are low, or their academic records are low, or economic conditions are poor are more likely to think about suicide. The students tend to think more about killing themselves if they spend more time sitting to study than being engaged in physical activities, or spend little time walking. And they are more likely to think about committing suicide if they spend little time exercising, or are not engaged in physical activities on the playground or a gym in PE classes.
