심박변이도에 기공(氣功) 자세(姿勢)가 미치는 영향 (The Effects of Qigong Position on Heart Rate Variability)

  • Objectives : This study aimed to investigate the effect of four common types of Qigong position (standing, sitting, supine, and horse-riding position) on the autonomic nervous system. Method : Thirty healthy subjects participated in this study once a week for four weeks. Heart Rate Variability (HRV) was measured three times (before, during, and after the position) while the subject maintained one of four positions for ten minutes. Result : There were significant changes HRV in the standing position. In the sitting position, both the sympathetic and parasympathetic nerves were activated. On the other hand, in the spine position, heart rate (HR) was decreased. Activation of sympathetic and parasympathetic nerves was also observed in this position. Significant increases of indices related to awakening and concentration were observed accompanied by increase of HR and a sympathetic nerve was activated in the riding-horse position. Conclusions : In the present study, it was shown that each Qigong position caused various and significant changes in autonomic nervous system. It would be expected that these results can be applied to choose appropriate Qigong position according to objective of Qigong therapy although it is remained to further evaluate the effects of long-term maintenance of Qigong positions and repeated Qigong training.

측각기를 사용한 견관절 운동범위 측정의 신뢰도에 관한 연구 (Reliaility of Goniometric Measurements of Shoulder Joint Range of Motion)

  • The purpose of this study was to examine the difference in measurements of passive range of motion(PROM) of shoulder when motions are assessed in sitting, as compared with supine and the intratester and intertester reliabilities for goniometric measurements of shoulder PROM using two different sizes of goniometers. * A thesis submitted to the committee of Graduate School of Public Health, Chungnam National University in partial fulfillment of the requirements for the degree of Master of Public Health conferred in February 2001. Fifty one adult male subjects were measured three times, in random order, for the six shoulder motions(flexion, extension, abduction, adduction, external rotation, and internal rotation) while sitting and supine by three different therapists. Data were analyzed to determine intraclass correlation coefficients (ICCs) and paired "t" values between trials for measurements with different goniometer and betweensitting and supine trials. There was statistical difference in measurement of shoulder motion between supine and sitting position and no statistical difference in goniometric type. The intratester intraclass correlation coefficients (ICCs) for all motions ranged from .87 to .98. The ICCs for the intertester reliability of PROM measurements of all shoulder motion ranged from .26 to .89. These findings suggest that reliable measures of PROM of the shoulder can be obtained using standard goniometry in a proper position.

기공(氣功) 자세(姿勢)가 뇌파에 미치는 영향 (The Effects of Qigong Position on Electroencephalogram)

  • This study aimed to investigate the effect of four common types of Qigong position (standing, sitting, supine, and horse-riding position) on the autonomic nervous system. Thirty healthy subjects participated in this study once a week for four weeks. Electroencephalogram (EEG) was measured three times (before, during, and after the position) while the subject maintained one of four positions for ten minutes. There were significant changes in HRV components compared with EEG power spectra in the standing position. Especially, the ratio of low-to-high frequency (LF/HF) which represents a state of balance of autonomic nervous system was increased. In the sitting position, $\beta$ wave which reflects a state of alert consciousness was increased and both the sympathetic and parasympathetic nerves were activated. On the other hand, in the spine position, $\theta$ wave which signifies a state of relaxation was increased and heart rate (HR) was decreased. Activation of sympathetic and parasympathetic nerves was also observed in this position. Significant increases of indices related to awakening and concentration were observed accompanied by increase of HR and a sympathetic nerve was activated in the riding-horse position. In the present study, it was shown that each Qigong position caused various and significant changes in autonomic nervous system. It would be expected that these results can be applied in the choice of appropriate Qigong position according to objective of Qigong therapy although it is remained to further evaluate the effects of long-term maintenance of Qigong positions and repeated Qigong training.

자세에 따른 골반경사운동이 편마비 환자의 발 접촉양상에 미치는 효과 (The Effect of Pelvic Tilt Exercise with Changing the Body Position on Foot Contact Pattern in Hemiplegic Patients)

  • Purpose : The purpose of this study was to investigate the effect of pelvic tilt exercise with changing the body position on foot contact pattern in the hemiplegic patients. Methods : Thirty seven hemiplegic patients were randomly divided 3 groups; control group (CG), sitting exercise group (SIEG) and standing exercise group (STEG). F-mat system and F-scan system were used for the measurement of foot contact pattern of hemiplegic side in walking. Data were analyzed statistically using paired t-test and one-way ANOVA. Results : The results were as follows : 1) Contact area of CG and SIEG were not significant difference in walking. Contact area of STEG was significant increased in walking. 2) Anteroposterior distance of COP of SIEG and STEG were significant increased in walking. Conclusion : These results suggest that pelvic tilt exercise in sitting and standing position are effective in the improvement of Anteroposterior distance of COP and gait stability are increased in only standing position.

A Comparison of Shoulder Muscle Activities on Sitting Posture and Shoulder Angle

  • Background: Sitting posture influences movements of scapulothoracic and glenohumeral joints and changes the shoulder muscle activities. The development and maintenance of correct sitting posture is important for the fundamental treatment of shoulder pain during rehabilitation. Objects: The purpose of this study was to investigate the effects of the sitting postures and the shoulder movements on shoulder muscle activities for both male and female. Methods: Twenty-eight subjects without shoulder-related diseases participated in this experiment. The subjects had randomly adopted three different sitting postures (upright posture, preferred posture, maximum slouched posture) and shoulder flexion angles in scapular plane ($30^{\circ}$, $90^{\circ}$, $120^{\circ}$). Surface electrodes were collected from upper trapezius (UT), anterior deltoid (AD), and posterior deltoid (PD) and the active shoulder range of motion was measured in each sitting posture and shoulder flexion angle. Results: The active range of motions of the shoulder external rotation and the flexion in the scapular plane decreased from the upright posture to the maximum slouched posture (p<.05, mixed-effect linear regression with random intercept, Tukey post-hoc analysis). All muscles showed the highest EMG activities at $120^{\circ}$ shoulder flexion with the maximum slouched posture and did not show the gender differences. Conclusion: Increased shoulder muscle activities may become the potential risk factor for the shoulder impairment and pain if people continuously maintain the maximum slouched posture. Therefore, an upright position is necessary during shoulder exercises, as well as in activities of daily living, including motions involving lifting the arms.

Effect of Visual Biofeedback Training in Real Time on Buttock Pressure and Pelvic Tilting Angles of Hemiplegic Patients During Sitting

  • Background: After a stroke, the control of the trunk muscle may be severely impaired. Due to the importance of trunk control in complex daily postures, the ability to adopt a correct sitting posture is considered a determinant of the recovery of independent function after a stroke. Objects: The purposes of this study were to compare differences in buttock pressure between the left and right sides of hemiplegic patients and differences in their pelvic tilting angles (sagittal and coronal planes) after sitting training with visual biofeedback (VBF) in real time. Methods: Twenty-two individuals with unilateral strokes (11 left-side and 11 right-side hemiplegic stroke patients) participated in this study. Buttock pressure was measured using a pressure mat, and pelvic angles were measured using a palpation meter. Results: The asymmetry of pressure between the right and left (first and third chamber) sides was significantly decreased after the VBF training. The measurements obtained using the palpation meter revealed a significant decrease in the pelvic angles pre- versus post-intervention. Conclusion: VBF training may be distribute a patient's buttock pressure equally while in a sitting posture and increase the length of time a stroke patient can maintain a symmetrical sitting posture. It can also improve pelvic control while sitting in a neutral position.

  • 일반적으로 잘못된 앉은 자세를 장시간 유지하는 것은 척추에 나쁜 영향을 미치는 것으로 널리 알려져 있다. 최근 여러 연구자들이 잘못된 앉은 자세와 척추질환의 인과관계에 관심을 두고 있으며, 척추질환 예방을 위해 앉거나 서는 자세의 변화를 정밀하게 측정할 수 있는 방법을 연구해왔다. 우리는 과거 연구에서 실시간 체위 변환 측정이 가능한 센서 디바이스를 개발하고 운동량 계산 알고리즘을 적용하여 실시간 자세 전환 측정 정확도를 향상시켰으며, 체위 변환 측정 센서의 정확도를 검증하였다. 본 연구에서는 좌압 측정을 통해 체중심의 변화를 고려한 자세 측정 및 분석 장치를 개발하였으며, 개발된 센서는 사용자의 시각적 피드백을 개선하여 자세 교정 훈련 정확도를 높이는 보조 도구로의 활용 가능성을 확인하였다.

치과치료시에 있어서 적성체위에 관한 실험적 연구 (AN EXPERIMENTAL STUDY ON THE EFFECT OF POSTURAL CHANGES UPON THE CLINICAL DENTAL TREATMENT)

  • Healthy adults in the ranges of from 22 to 25 years old, 208 males and 140 females respectively, who have no specific organic diseases in the heart and lung, were experimentally studied on the effect of blood pressure, pulse and breath rates, and blood circulation by the postural changes of the supine, semi-supine, upright, sitting and trendelenburg position. The results obtained were as follows : 1) The blood pressure showed makred differences in the postural changes of the upright>sitting>semisupine>trendelenburg>supine position, orderly. 2) The breath rate did not show specific findings in the postural changes. 3) If the dental chair were suddenly tilted forwardly and backwardly, patients would be subjected to fall in the dangerous state, fainting.

Correlation among triceps surae muscle structure, balance, and gait in persons with stroke

  • Objective: This study aimed to compare muscle structure, balance, and gait parameters between healthy adults and persons with stroke and to analyze the correlation among these variables. Design: Cross-sectional study. Methods: Twenty persons with stroke (11 male, 9 female) and twenty healthy participants (9 male, 11 female) were included. Ultrasound images of the triceps surae and the tibialis anterior were acquired in sitting resting, sitting co-contraction, and standing resting positions and also during the functional reach test (FRT) and single leg anterior reaching test (SLART). Muscle thickness and fascicle length were measured. Spatiotemporal parameters of gait were measured using a pressure walkway. Gait speed, cadence, step length, stride length, stance time, and swing time were measured. Results: Changes in percent fascicle length were significantly greater in the gastrocnemius and soleus (SOL) muscles of healthy adults in the sitting co-contraction position (p<0.05). The percent fascicle length of the SOL in FRT and SLART were significantly greater in healthy adults (p<0.05). The mid-stance phase of stroke patients was shorter than healthy adults (p<0.05). A negative correlation was observed between percent fascicle length of the SOL in the sitting co-contraction position and the proportion of the mid-stance phase (p<0.05). Conclusions: The function of the triceps surae is affected in persons with stroke when compared with healthy adults. This can lead to difficulty in performing tasks that involve forward transfer of weight. If the triceps surae is not sufficiently secured, the possibility of compensation in the stance phase increases during gait.

The Effect of a Task-oriented Upper Arm Exercise on Stable and Unstable Surfaces on Dynamic Balance and Hand Function in Patient with Cerebral Palsy

  • Purpose: The purpose of this study was to investigate the effects of a task-oriented upper arm exercise performed in a sitting position on either an unstable support surface or a stable support surface for children with cerebral palsy. Methods: We prospectively evaluated 18 children with cerebral palsy. Eight subjects were randomly assigned to each of the stable and unstable support surface groups. We performed the upper arm exercise three times a week for 6 weeks. To confirm the effects of the intervention, the berg balance scale test, modified functional reaching test (MFRT), timed up and go test (TUG), and Jebsen-Taylor hand function test were conducted before and after the study. Results: Significant differences were observed in MFRT and TUG between the experimental and control groups (p<0.05). In the Jebsen-Taylor hand function test, there were significant differences between the groups for the items picking up small objects, stacking checkers, lifting large light objects, and lifting large heavy objects (p<0.05), but not for writing and stimulation of feeding. Significant differences were observed between the groups in items of card turning, lifting large light objects, and lifting heavy objects. Conclusion: The purpose of this study was to evaluate the effectiveness of a task-oriented upper extremity exercise program for dynamic balance and hand function performed in a sitting position with either stable or unstable support by cerebral palsy patients. There were improvements in the two groups, but performing the exercise while sitting on an unstable support surface had a greater effect on dynamic balance and hand function than exercise while sitting on a stable supporting surface. The results of this study can be used to improve the daily lives of cerebral palsy patients.
