Hemodynamic Study after Left Lung Reimplantation and Right Pulmonary Artery Ligation (황견에서 좌측폐재식수술 및 우측 폐동맥결찰수술후 혈역학적 연구)

  • We have performed four left lung reimplantation and right pulmonary artery ligation in dogs for six months from March 1989 to September 1989 at the Thoracic & Cardiovascular Surgery department, Yonsei University, College of Medicine, Seoul, Korea. Excised left lungs were perfused with 200cc of 4oC cold saline at a pressure of 60cmH2O through left pulmonary artery & preserved in 4 oC cold saline for about 20 mins. Left lung reimplantation were proceeded with inferior pulmonary vein, superior pulmonary vein, left pulmonary artery and left main bronchus in order. The main pulmonary artery pr. were 39/21[31], 22/12[15], 25/9[15] and 54/17[37] mmHg each after right pulmonary artery ligation on left reimplanted dogs, Right pul a. ligations were performed 9 days, 12 days, 16 days and 19 days after left lung reimplantation. Two dogs died at 10 days 21 days, after right pul. a. ligations in left reimplanted dogs. The remaining two dogs were sacrificed at 18 days, 21 days after right pul. a. ligation in left reimplanted dogs. Autopsy findings showed narrowing of left superior pul. vein anastomotic site in two narrowing of left pulmonary artery anastomotic site in one narrowing of left sup. pul vein & pneumonia of left lung in one. In the lung transplantation, it was thought to be important that the anastomosis of pulmonary artery and especially pulmonary vein be done with particular precaution for early and late stenosis.

Safe arm posture when using vertical rescue sack (수직 강하식 구조대 사용 시 안전한 팔 자세)

  • This study is about the safe arm posture in case of emergency escape using the vertical dive rescue sack at the fire site. The experimental results are as follows. First, the arms extended upward contact with the endothelium and narrowing part of the rescue sack minimized the scratches did not occur. Second, the bent position with both arms open was subject to light abrasions of on the elbows due to friction between the elbows and the scapula and the endothelium. Third, in the posture where both arms were gathered in the chest, the body passed through the narrowing part and friction between the bag's narrowing part, All subjects had light abrasions on their elbows. Fourth, because the arms are lowered, the legs are extended to the width of the shoulders when descending, so that the back of the hand has friction with the narrowing part of the bag and the endothelial skin. Finally, posture with both arms below the front increased the volume of the front of the body, resulting in a slight back injury. As a future research task, it is necessary to study the proper posture of legs and the posture of landing on the ground.

Esophageal Foreign Bodies : Experiences of 235 cases for 12 years (식도이물;12년간 235례의 경험)

  • We have clinico-statically analysed the 235 cases of esophageal foreign body during the period the past 12 years from March InS to December 1996 at the Department of Otolaryngology-Head and Neck Surgery, Korea University College of Medicine, Guro Hospital. The results were as follow : 1) Coin was the most frequent foreign body in the esophagus(172 cases : 73.2%). Bone was the next frequent foreign body (21 cases 8.8%). 2) In sex distribution, 124 cases(52.8%) of patients were the female and 111 cases(47.2%) were the male, so the sex ratio was 1:1.12. 3) In the age distribution, 134 cases(56.9%) were under 5 years of age, coin was the most frequent materials in this age group(111 cases 47.2%). 4) The most common symptom was vomiting(136 cases : 57.9%). The next common symptom was dysphagia and odynophagia was 41 cases(17.4%) 5) The most prevalent site of lodgement was the first narrowing(181 cases : 77.0%), the third narrowing(31 cases : 13.2%) and the second narrowing of esophagus(23 cases 9.8%) in order. 6) In duration of lodgement, 207 cases(88.1%) were lodged for one day. 7) The foreign bodies of esophagus were removed successfully by esophagoscopic procedure under local or general anesthesia. There were 3 cases of complication, one case of esophageal bleeding and two cases of esophageal rupture.

Effects of Solvent Viscosity on Conformational Dynamics of Heme-pocket in Myoglobin and Hemoglobin

  • The influence of solvent viscosity on conformational dynamics of the heme-pocket, a small vacant site near the binding site of myoglobin (Mb) and hemoglobin (Hb), and playing a functionally important role by serving as a station in ligand binding and escape, was studied by probing time-resolved vibrational spectra of CO photodissociated from MbCO and HbCO in $D_2O$, 75 wt% glycerol/$D_2O$, and trehalose at 283 K. Two absorption bands ($B_1$ and $B_2$) of the sample in viscous solvents, arising from CO in the heme pocket, are very similar to those in $D_2O$. Two bands in Mb and Hb under all three solvents exhibit very similar nonexponential spectral evolution ($B_1$ band; blue shifting and broadening, $B_2$ band; narrowing with a negligible shifting), suggesting that in the present experimental time window of 100 ps, the extents of the spectral shift and narrowing is much influenced neither by the viscosity of solvent nor by the quaternary contact of Hb. Spectral evolution can be described by a biexponential function with a fast universal time constant of 0.52 ps and a slow time constant ranging from 13 to 32 ps. For both proteins in all three solvents majority of spectral evolution occurs with the fast universal time constant. The magnitude of the slow rate in the spectral shift of B1 band decreases with increasing solvent viscosity, indicating that it is influenced by global conformational change which is retarded in viscous solvent, thereby serve as a reporter of global conformational change of heme proteins after deligation.

Takayasu`s Arteritis; A Case Report (Takayasu 동맥염;치험 1례)

  • Recently we experienced a case of Takayasu`s arteritis involving the major aortic branches. A 30 year-old female patient admitted with the complaints of dizziness, visual disturbance, headache and tingling sensation of upper extremities. Aortogram revealed nearly complete obstruction of the origin site of both common carotid arteries and right vertebral artery, and irregular luminal narrowing of the origin site of innominate artery and left subclavian artery, but opacification of right subclavian artery and left vertebral artery. Successful surgical treatment was accomplished with a bypass from the ascending aorta to the left common carotid artery using a tube graft. The left subclavian artery and right axillary artery were revascularized distal to the stenosis with tube grafts that extended from the aortic graft. Postoperative complications were atelectasis, lymph leakage and left phrenic nerve palsy. She discharged uneventually at postoperative 22 days and most of symptoms were relieved.

Surgical Treatment of Intrahepatic Inferior Vena Cava Obstruction: Three Cases Report (간내 하공정맥 폐쇄증: 수술 3례 보고)

  • We operated three cases of intrahepatic inferior vena caval obstruction from January, 1969 to February, 1978 at the department of Thoracic and Cardiovascular Surgery in Seoul National University Hospital. They were all female. And their ages were 33, 38 and 32 respectively. The lesions were the same site; just above the opening of the hepatic vein of inferior vena cava. In first case, segmental narrowing of the inferior vena cava without thrombosis was noted. In second case; thrombus occluded the inferior vena cava and left hepatic vein. In third case, we observed membrane of 4mm thickness which occluded the lumen completely. They were totally operated by using cardiopulmonary bypass. Their post-operative courses were good in the last two cases and re-obstruction was suspected in the first case.

  • Coarctation of the aorta is classically a congenital narrowing of the upper descending thorac aorta adjacent to the site of attachment of the ductus arteriosus which is sufficiently severe that there is a pressure gradient across the area. Recently we have experienced two cases of coarctation of the aorta and successfully performed resection of the sites of coarctation and end to end anastomosis of the aorta. The first case was a juxtaductal type of coarctation of the aorta with PDA and the pathology of the lesion was a diaphragm with central narrow opening. And the resection length was about 0.5cm and aortic clamping time was 20 minutes. The second case was also juxtaductal type coarctation of the aorta with mild tubular hypoplasia of aortic isthmus, left SVC and the pathology was also a diaphragm with central narrow opening. And the resection length was about 0.5cm and aortic clamping time was 29 minutes. Both postoperative course was uneventful and the patients were discharged two weeks after operation.

Interrupted Aortic Arch(Type A) associated with PDA, VSD, Mitral Regurgitation and Single Coronary Artery (대동맥궁 결손증의 완전교정 치험 1예)

  • We experienced a case of interrupted aortic arch[Type A] associated with PDA, VSD, mitral regurgitation and single coronary artery. The patient was 7 years old boy, who showed congestive heart failure[NYHA functional class III]. One stage total correction was performed under profound hypothermia with total circulatory arrest. Aortic continuity was established using PDA with anterior wall of main pulmonary artery flap. VSD was closed with Dacron patch and mitral regurgitation repaired by Reed`s annuloplasty method. The postoperative cardiac catheterization revealed no pressure gradient between ascending aorta and descending aorta, decreased pulmonary artery pressure and trivial residual shunt[Qp/Qs: 1.28]. The aortogram showed good continuity of the aorta without narrowing of the anastomotic site. During the period of 1 year follow up, heart failure symptoms were nearly subsided.

A Severe Retropharyngeal Hematoma after Stellate Ganglion Block -A case report- (성상신경절차단 후 생긴 심한 후인두 혈종 -증례보고-)

  • A 73-year-old man with sudden sensory neural hearing loss received a stellate ganglion block. Two hours after the block, the patient complained of newly developed neck discomfort. After an additional two hours, the neck swelled up gradually and neck pain and dyspnea developed. A plain radiograph of neck revealed narrowing of the upper airway; a tracheostomy was performed and the dyspnea was improved. On the next day, the pain site extended to the right scapula and a CT image revealed a huge retropharyngeal hematoma. Hematoma evacuation and bleeder ligation were then performed and the patient was discharged on the fourth day after admission without any complications. A practitioner should always remember to educate the patients about possible complications and undertake intensive observation when performing procedures, even in patients who do not initially present with a compromised airway.

Esophageal Leiomyomatosis in a patient with Alport Syndrome (Alport syndrome과 동반된 식도 평활근종증)

  • A-13-urar-old with a history of Alport syndrome had been suffering from progressive dysphagia and postprandial vomiting for over 7 years. Exophagogram and manoemtric studies were consistent with achalasia. Barum study demonstrated marked esophageal dilatation and smooth tapered narrowing of the distal esophagus. However in spite of the medical treatment including the injection of the distal esophagus. However in spite of the medical treatment including the injection of botulinum toxin at the lesion site using an endoscope symptom did not improved and he suffered growth failure and malnutrition. Esophagectomy and esophagogastrostomy were performed to relieve the dysphagia. A firm circumferential intramural mass about 7$\times$5$\times$5 cm was found in the distal esophagus. The lumen of the esophagus was markedly dilated and esophageal wall was hypertrophied. Histologic examination of the neoplasm revealed a rather ill defined tumor tissue consisting of interlacing or whirling spindle cells without significant mitosis and esophageal leiomyo-matosis was confirmed. The patient was discharged uneventfully.
