Comparison of elective single cleavage-embryo transfer to elective single blastocyst-embryo transfer in human IVF-ET

  • Objective: This study was carried out to compare the clinical outcome of elective single cleavage-embryo transfer (eSCET) to that of elective single blastocyst-embryo transfer (eSBET) in human IVF-ET. Methods: This study was a retrospective study which analyzed for 614 women who visited the Daegu Maria Clinic from August 2008 to December 2009. All were under 37 years old and had more than 8 mm of endometrial thickness on the day of hCG administration and at least one good quality embryo on day 3. The eSCETs were performed on day 3 (n=450) and the eSBETs were conducted on day 5 (n=164). Results: The numbers of retrieved oocytes, fertilized oocytes, and day 3 good quality embryos were significantly lower in the eSCET group (12.1${\pm}$6.0, 8.2${\pm}$4.6, and 4.2${\pm}$3.1, respectively) compared to the eSBET group (16.7${\pm}$7.2, 12.1${\pm}$5.0, and 8.5${\pm}$4.5, respectively; p<0.001). However, the clinical pregnancy, implantation, on-going pregnancy, and live birth rates of the eSCET group (46.7, 46.9, 40.0, and 36.7%, respectively) were not statistically different from those of the eSBET group (51.2, 51.8, 45.1, and 43.9%, respectively; p=0.318, 0.278, 0.254, and 0.103, respectively). Conclusion: These results suggested that elective single embryo transfer should be performed regardless of the developmental stage to women less than 37 years old who had more than 8 mm of endometrial thickness on the hCG administration day and at least one good quality embryo on day 3 in order to reduce the twin pregnancy rate without reducing the whole pregnancy rate.

The pregnancy outcomes of day-5 poor-quality and day-6 high-quality blastocysts in single blastocyst transfer cycles

  • Objective: This study compared the outcomes of single blastocyst transfer cycles, using day- 5 poor-quality blastocysts and day-6 high-quality blastocysts. Methods: We analyzed 462 frozen-thawed embryo transfer (FET) cycles performed at our center from January 2014 to December 2019. The cycles were divided into two groups: a day-5 poor-quality blastocyst transfer group (group A) and a day-6 high-quality blastocyst transfer group (group B). The clinical outcomes were tested. Results: In groups A and B, respectively, the clinical pregnancy rate (CPR; 61.65% vs. 67.17%, p=0.258), implantation rate (IR; 61.65% vs. 67.17%, p=0.258), and live birth rate (LBR; 69.51% vs. 77.83%, p=0.134) showed no significant differences. Moreover, when day-3 embryo quality was considered, the CPR, IR, and LBR were also similar in group A and group B (p>0.05). Conclusion: The clinical outcomes of day-5 poor-quality blastocysts and day-6 high-quality blastocysts were similar, suggesting that the developmental speed of the embryo might be more important than embryo quality for the clinical outcomes of single blastocyst transfer in FET cycles.

Associations of post-warming embryo or blastocyst development with clinical pregnancy in vitrified embryo or blastocyst transfer cycles

  • Objective: To investigate whether the degree of post-warming embryo or blastocyst development is associated with clinical pregnancy in vitrified embryo or blastocyst transfer cycles. Methods: Ninety-six vitrified cleavage-stage embryos and 58 vitrified blastocyst transfer cycles were selected. All transfer cycles were performed from February 2011 to March 2019, and all vitrified embryos or blastocysts were warmed from 4 PM to 6 PM and then transferred the next morning from 9 AM to 10 AM. The scores of the cleavage-stage embryos and blastocysts were assessed at warming and at transfer using the modified Steer method and the Gardner method, respectively. The mean embryo or blastocyst score, score of the single top-quality embryo or blastocyst, and the difference in the score between warming and transfer were compared between nonpregnant and pregnant women. Results: In the cleavage-stage embryo transfer cycles, both the top-quality embryo score at transfer and the difference in the score between warming and transfer were significantly associated with clinical pregnancy. A top-quality embryo score at transfer of ≥ 60.0 (area under the curve [AUC], 0.673; 95% confidence interval [CI], 0.531-0.815) and a difference in the score between warming and transfer of ≥ 23.0 (AUC, 0.675; 95% CI, 0.514-0.835) were significant predictors of clinical pregnancy. In blastocyst transfer cycles, the top-quality blastocyst score at transfer was the only significant factor associated with clinical pregnancy. A top-quality blastocyst score at transfer of ≥ 38.3 was a significant predictor of clinical pregnancy (AUC, 0.666; 95% CI, 0.525-0.807). Conclusion: The top-quality embryo score at transfer and the degree of post-warming embryo development were associated with clinical pregnancy in vitrified cleavage-stage embryo transfer cycles. In vitrified blastocyst transfer cycles, the top-quality blastocyst score at transfer was the only significant factor affecting clinical pregnancy.

  • This study was carried out to establish an effective system for embryo transfer of techniques by analyzing several factors affecting the gestation length and the weight changes of calves produced from embryo transfer in Korean cattle. The results obtained in study on factors affecting the gestation length and the weight changes of calves produced from embryo transfer were as fallow; 1) The gestation length and the birth weight did not differ between male and female, but the weight changes after birth were remarkablely different between sex(P<0.05). 2) The gestation length between heifers and cows was not different, and body weights at birth and 6 months were remarkabley heavy in cows(P<0.05). Weight changes after 6 months were not different. 3) The gestation length and the birth weight were significantly different between the single and twin calving (P<0.05). Weight of twin at 6 and 12 months were remarkabely light. 4) Calving seasons did not affect the gestation length the and the birth weight. Weaning weight was significantly heavy(P<0.05), but weight changes after weaning were no different among the calving seasons. Conclusivley, this results suggest that cows will be better when considering growth of calves and twin produced from embryo trnsfer in Korean cattle.

Impact of co-transfer of embryos produced by somatic cell nuclear transfer using two types of donor cells on pregnancy outcomes in dogs

  • Objective: The present study analyzed the influence of co-transferring embryos with high and low cloning efficiencies produced via somatic cell nuclear transfer (SCNT) on pregnancy outcomes in dogs. Methods: Cloned dogs were produced by SCNT using donor cells derived from a Tibetan Mastiff (TM) and Toy Poodle (TP). The in vivo developmental capacity of cloned embryos was evaluated. The pregnancy and parturition rates were determined following single transfer of 284 fused oocytes into 21 surrogates and co-transfer of 47 fused oocytes into four surrogates. Results: When cloned embryos produced using a single type of donor cell were transferred into surrogates, the pregnancy and live birth rates were significantly higher following transfer of embryos produced using TP donor cells than following transfer of embryos produced using TM donor cells. Next, pregnancy and live birth rates were compared following single and co-transfer of these cloned embryos. The pregnancy and live birth rates were similar upon co-transfer of embryos and single transfer of embryos produced using TP donor cells but were significantly lower upon single transfer of embryos produced using TM donor cells. Furthermore, the parturition rate for TM dogs and the percentage of these dogs that remained alive until weaning was significantly higher upon co-transfer than upon single transfer of embryos. However, there was no difference between the two embryo transfer methods for TP dogs. The mean birth weight of cloned TM dogs was significantly higher upon single transfer than upon co-transfer of embryos. However, the body weight of TM dogs did not significantly differ between the two embryo transfer methods after day 5. Conclusion: For cloned embryos with a lower developmental competence, the parturition rate and percentage of dogs that remain alive until weaning are increased when they are co-transferred with cloned embryos with a greater developmental competence.

  • The sheep can be reproduced by natural mating as well as applied reproductive biotechnology, embryo transfer (ET). However, this method in sheep is influenced by several factors such as season, photoperiod, latitude, temperature, nutrition, and breed. In addition, there is still less research on assisted reproductive technologies in small ruminants, compared to other livestock species such as cattle and pigs. Because there has been a need for an optimization and a continuous improvement of ET techniques in small ruminants. the main objective of this study was to evaluate the conception rate obtained after ET in Mongolian sheep (Dorper breed). After embryo recover, code 1 and 2 embryos (morula or blastocyst stage) for ET in the present study were 63% (63/100) and 24% (24/100), respectively. Then Each single embryo was transferred to a synchronized recipient who prepared by estrous synchronization protocol with fluorogestone acetate-cloprostenol sodium. The results demonstrated that an average conception rate and lambing rate was 35.6% (31/87) and 33.3% (29/87), respectively. Further study is still necessary, but these results indicated that single embryo of Mongolian sheep with the present protocol was enough to conducting ET when the genetically superior sheep were necessary to be expanded.

  • This experiment was carried out to investigate the factors affecting superovulation in rabbits and to determine the effect of pFSH and PMSG on ovarian superovulatory responses and embryo production, and the effect of superovulation treatment with a single injection of pFSH dissolved in polyvinylpyrrolidone on the ovarian responses and the embryo quality. The results obtained were suonmerized as follows: Superovulatory response resulted in significantly (P<0.05) higher ovulation rates and more embryos in spring or autumn, compared with summer or winter. Repeated superovulatory treatments with PMSG leaded to a significantly(P<0.05) decreased number of total follicles and recovered ova. Superovulation with pFSH resulted in the higher number of ovulated follicles and recovered ova than with PMSG. A single subcutaneous injection of pFSH dissolved in 25% PVP resulted in the more ovulation points(33.2) and recovered embryos(30.2), which were comparable to the multiple injections of pFSH(44.8 vs 37.7).These results indicated that the treatment with a single injection of FSH dissolved in PVP was an efficient and simple alternative method to the conventional multiple FSH injections for superovulation in rabbits.

  • This study was conducted to determined if single subcutaneous (s.c.) injection of Folltropin-V dissolved in polyethyleneglycol (PEG) can replace as the standard multiple intramuscular (i.m.) injection. The results suggest that the s.c. treatment produced more corpora lutea, embryos recovered and transferable embryos as compared to the i.m. treatment (p<0.05). This study indicates that a single s.c. injection of Folltropin-V dissolved in PEG was effective for superovulatory response and embryo yield in Korean cattle.

Efficacy of dual progesterone administration (intramuscular and vaginal) for luteal support in fresh day 3 or day 4 embryo transfer cycles

  • Objective: The aim of this study was to compare in vitro fertilization outcomes between fresh day 3 or day 4 embryo transfer cycles with dual progesterone (P) administration (intramuscular and vaginal) and cycles with single intramuscular P administration for luteal support. Methods: We selected 124 cycles from 100 women (under age 40 years) who underwent oocyte pick-up (number of trials ≤ 3, 4-14 oocytes obtained) and transfer of two or three day 3 or day 4 embryos at two infertility centers from January 2014 to June 2019. Dual P (intramuscular P [50 mg] daily+vaginal P) was used in 52 cycles and a single intramuscular administration of P (50 mg daily) was used in 72 cycles. Results: Women's age, infertility factors, number of oocytes retrieved, number of transferred embryos, and mean embryo score were similar between the dual P group and the single P group. Although the number of trial cycles was significantly higher (1.9 vs. 1.5), and the mean endometrial thickness on the trigger day (10.0 mm vs. 11.0 mm) was significantly lower in the dual P group, the implantation rate, clinical pregnancy rate, ongoing pregnancy rate, and miscarriage rate for both day 3 and day 4 transfers were similar between the two groups. Conclusion: In fresh day 3 or day 4 embryo transfer cycles, dual P administration did not demonstrate any clinical advantages. Intramuscular P alone appears to be sufficient for luteal support.
