  • Background: In this study, we analyze the performance of the Acute Physiology and Chronic Health Evaluation (APACHE) II, APACHE IV, Simplified Acute Physiology Score (SAPS) 3, and Mortality Probability Model $(MPM)_0$ III in order to determine which system best implements data related to the severity of medical intensive care unit (ICU) patients. Methods: The present study was a retrospective investigation analyzing the discrimination and calibration of APACHE II, APACHE IV, SAPS 3, and $MPM_0$ III when used to evaluate medical ICU patients. Data were collected for 788 patients admitted to the ICU from January 1, 2015 to December 31, 2015. All patients were aged 18 years or older with ICU stays of at least 24 hours. The discrimination abilities of the three systems were evaluated using c-statistics, while calibration was evaluated by the Hosmer-Lemeshow test. A severity correction model was created using logistics regression analysis. Results: For the APACHE IV, SAPS 3, $MPM_0$ III, and APACHE II systems, the area under the receiver operating characteristic curves was 0.745 for APACHE IV, resulting in the highest discrimination among all four scoring systems. The value was 0.729 for APACHE II, 0.700 for SAP 3, and 0.670 for $MPM_0$ III. All severity scoring systems showed good calibrations: APACHE II (chi-square, 12.540; P=0.129), APACHE IV (chi-square, 6.959; P=0.541), SAPS 3 (chi-square, 9.290; P=0.318), and $MPM_0$ III (chi-square, 11.128; P=0.133). Conclusions: APACHE IV provided the best discrimination and calibration abilities and was useful for quality assessment and predicting mortality in medical ICU patients.

  • Two major processes that affect the vertical distribution of apparent pH in the Subarctic region of the Northeastern Pacific Ocean are (1) apparent oxygen utilization by marine organisms and (2) carbonate dissolution. Simplified mathematical approximations show that, in the pH ravge of 7.2 to 8.3, the utilization of 0.1 mM of oxygen (2.24 ml/liter) lowers the pH by 0.20 units and 0.1 mM of carbonate dissolution increases the pH by 0.25 units. Since Oxygen utilization exceeds 0.3 mM while the carbonate dissolution, with respect to the surface, is about 0.05mM, the effect of oxygen utilization is much greater than the dissolution effect of carbonate on the pH of seawater.

  • The aim of this study is to evaluate clinical usefulness of Korean red ginseng (RG) on various postmenopausal syndromes. Total plasminogen inhibitor-l (tPAI-l) in peripheral blood from 9 postmenopausal women with climacteric syndromes (CS) was measured before and 3 months after treatment with daily oral administration of 6 g RG and that from 8 postmenopausal women without any CS was also measured as healthy controls. Blood samples were collected in the early morning on the bed-rest. Psychological conditions of postmenopausal women with CS were measured before and 3 months after treatment with RG using simplified menopausal index (SMI). In addition, OKETSU (blood stagnation) syndrome scores and KI deficiency (generalized energy stagnation) scores proposed by Terasawa et al., were recorded before and 3 months after treatment with RG in postmenopausal women with CS and in healthy postmenopausal women. OKETSU syndrome scores and tPAI-l levels in postmenopausal patients with CS were significantly (P<0.001 and P<0.01) higher than those in healthy postmenopausal women without CS. Similarly, SMI scores and KI deficiency scores in postmenopausal patients with CS were about three-fold higher than those without any CS. When RG was administered for 3 months, KI deficiency scores and OKETSU scores as well as SMI scores declined around the levels of healthy postmenopausal women. Although tPAI-1 levels significantly (P<0.05) decreased after treatment with RG, those did not reach the levels of healthy postmenopausal women. Clinical usefulness of administration of RG to postmenopausal women with CS was confirmed from evaluation not only by modem medicine but also by traditional KAMPO medicine.

  • This paper presents a method to estimate subpixel accuracy motion vectors using a mathermatical model withoug interpolation. the proposed method decides the coefficients of mathematical model, which represents the motion vector which is achieved by full search. And then the proposed method estimates subpixel accuracy motion vector from achieved mathematical model. Step by step mathematical models such as type 1, type 2, type 3, modified bype 2, modified type 3, and Partial Interpolation type 3 are presented. In type 1, quadratic polynomial, which has 9 unknown coefficients and models the 3by 3 pixel plane, is used to get the subpixel accuracy motion vectors by inverse matrix solution. In type 2 and 3, each quadratic polynomial which is simplified from type 1 has 5 and 6 unknown coefficients and is used by least square solution. Modified type 2 and modified type 3 are enhanced models by weighting only 5 pixels out of 9. P.I. type 3 is more accurate method by partial interpolation around subpixel which isachieved by type 3. LThese simulation results show that the more delicate model has the better performance and modified models which are simplified have excellent performance with reduced computational complexity.

  • The purpose of this study is to increase efficiency of CPR through comparing the chest-compression quality between rescuer's simplified verbal order method and the continued verbal order method by utilizing voice meter during CPR. Subjects were 89 people(45 people for the experimental group, 44 people for the control group) who completed the 15-week CPR curriculum as undergraduates for the department of Emergency Medical Technology in C Province and were carried out by being randomly extracted. The group division was set for the experimental group as the group with the simplified verbal order and for the control group as the group with the continued verbal order. The period of measurement was progressed primarily(November 10, November 28, 2011) and secondarily(September 3-September 4, 2012). An analysis was used SPSS WIN 12.0 program. As a result of research, as for the implementation of appropriate chest compression(time, %), the quality was higher(p<.05) in the experimental group(102.86 times, 67.79%) than the control group(85.31 times, 55.84%). As a result of research, the chest compression(time, %) in the experimental group(102.86 times, 67.79%) had the higher effect of chest compression quality(p<.05) than the control group(85.31 times, 55.84%). On the other hand, the operation of weak chest compression(time) was higher in control group(61.13 times) than experimental group(35.54 times). The proper chest compression was shown(p<.05) in men of the experimental group as for gender and in over 60kg of the experimental group as for weight.

  • For structures with underground basement walls, the soil-structure-interaction between the side soil and the walls affects the response of the system. There is interest in quantifying the relationship between the lateral earth pressure and the wall displacement using p-y curves. To date, passive p-y curves in available limited studies were assumed elastic-perfectly plastic. In reality, the relationship between earth pressure and wall displacement is complex. This paper focuses on studying the development of passive p-y curves behind rigid walls supporting granular soils. The study aims at identifying the different components of the passive p-y relationship and proposing a rigorous non-linear p-y model in place of simplified elastic-plastic models. The results of the study show that (1) the p-y relationship that models the stress-displacement response behind a rigid basement wall is highly non-linear, (2) passive p-y curves are affected by the height of the wall, relative density, and depth below the ground surface, and (3) passive p-y curves can be expressed using a truncated hyperbolic model that is defined by a limit state passive pressure that is determined using available logarithmic spiral methods and an initial slope that is expressed using a depth-dependent soil stiffness model.

  • In this part, an implicit time dependent solution is presented for the Boltzmann transport equation discretized by the analytic coarse mesh finite difference method (ACMFD) over the spatial domain as well as the simplified P3 (SP3) for the angular variable. In the first part of this work we proposed a SP3-ACMFD approach to solve the static eigenvalue equations which provide the initial conditions for temp dependent equations. Having solved the 3D multi-group SP3-ACMFD static equations, an implicit approach is resorted to ensure stability of time steps. An exponential behavior is assumed in transverse integrated equations to establish a relationship between flux moments and currents. Also, analytic integration is benefited for the time-dependent solution of precursor concentration equations. Finally, a multi-channel one-phase thermal hydraulic model is coupled to the proposed methodology. Transient equations are then solved at each step using the GMRES technique. To show the sufficiency of proposed transient SP3-ACMFD approximation for a full core analysis, a comparison is made using transport peers as the reference. To further demonstrate superiority, results are compared with a 3D multi-group transient diffusion solver developed as a byproduct of this work. Outcomes confirm that the idea can be considered as an economic interim approach which is superior to the diffusion approximation, and comparable with transport in results.
