Intra-articular Osteochondroma of the Head of Third Metacarpal Bone -A Case Report- (중수골 두부에 발생한 관절내 골연골종 - 1례 보고-)

  • The osteochondroma is the most common benign bone tumor which arise in the metaphyseal or metaphysiodiaphyseal portion of long bone. Osteochondroma is rare in hand and extremely rare in the intraarticular location of the hand. We report a case of intraarticular osteochondroma which occurred at the third metacarpal head of the hand in a 53 year old female patient. The size of the excised mass was $10{\times}8{\times}5$ mm with definite cartilage cap.

Effects of Tween 80 Pretreatment on Dry Matter Disappearance of Rice Straw and Cellulolytic Bacterial Adhesion

  • An in situ experiment was conducted to find out whether Tween 80 improves rice straw digestion through increased adhesion of major fibrolytic bacteria. Rice straw was sprayed with various levels of Tween 80 non-ionic surfactant or SDS ionic surfactant 24 h before incubation in the rumen of Holstein steers. Dry matter (DM) disappearance and adhesion of F. succinogenes, R. flavefaciens and R. albus on rice straw after in situ incubation were measured by real-time PCR. Application of Tween 80 increased DM disappearance, which was more noticeable at an application level of 1% compared to lower application levels. Application of SDS resulted in an opposite response in DM disappearance with highest reduction in DM disappearance at 1% level. In a subsequent in situ experiment, higher Tween 80 was applied to rice straw in an attempt to find the optimum application level. Tween 80 at 2.5% gave better DM disappearance than 1% with a similar result at 5%. Therefore, an adhesion study was carried out using rice straw treated with 2.5% Tween 80. Our results indicated that Tween 80 reduced adhesion of all three major rumen fibrolytic bacteria to rice straw. Present data clearly show that improved DM disappearance by Tween 80 is not due to increased bacterial adhesion onto substrates.

  • Purpose: This study was to determine the current trend of nursing research as exploring both quantitative and qualitative methodologies, and to provide the explicit direction to improve the quality of published papers. Methods: Total of 366 articles published between 2004 and 2006 was reviewed using the criteria of analysis. Results: There was more number of quantitative studies than qualitative studies. More studies were conducted with subjects who had health problems, and studies that targeted women and elderly population have been significantly increased. In quantitative methodology, utilization of experimental and quasi-experimental designs has been increased, however descriptive study was dominant as yet. In qualitative methodology, studies using grounded theory and phenomenology were frequently published. It was noted that theoretical framework and rational for sample size were rarely presented in quantitative study. Philosophical position and the process of preparation for study, which guided the research, were not clearly described in qualitative study. Conclusion: The findings of this review suggest that published studies have been improved and diversified, however, detailed and clear evaluation tool that assesses study process and method should be developed as a way to further improve the quality of published papers.

Factors Affecting the Profitability of Private Hospitals in Korea (민간병원의 수익성 관련요인)

  • This study was performed to identify factors affecting the profitability of private hospitals in Korea different and to make informations that could be very helpful to hospitals in improving profitability. Data used in this study were collected from 112 hospitals with complete general data of present conditions as well as financial statements(balance sheets, income statements). They were chosen from hospitals that passed the standardization audit undertaken by the Korean Hospital Association from 1998 to 2001 for the purpose of accrediting training hospitals. The dependent variables were used operating margin to total assets and operating margin to gross revenues as proxy indicators for profitability. The independent variables were ownership type, location, bed size, period of establishment, debt to total assets, current ratio, fixed ratio, total asset turnover, average length of stay, bed occupancy rate, admission ratio of outpatients, personnel costs per adjusted inpatient, and fiscal years. The factors had significantly positive effect on operating margin to total assets and operating margin to gross revenues were bed size, total asset turnover. And the factors had significantly negative effect on operating margin to total assets and operating margin to gross revenues were period of establishment, debt to total assets, average length of stay, personnel costs per adjusted inpatient. The adjusted $R^2$ of multiple regression equation was 25.2%, 21.4% respectively. It is very important for private hospitals to improve profitability by raising total asset turnover, and reducing debt to total assets, average length of stay, and personnel costs per adjusted inpatient using the rational and efficient business strategy.

Cross-Sectional Velocity Variability and Tidal Exchanger in a Bay (만구를 통한 해수유출입과 만내수괴의 해수교환성)

  • Chinhae Bay, included small ports, is the region which the red tide phenomenon is occurred frequently in summer season. Field sampling of 4 cross-sections in the bay resulted in detailed informations on cross-sectional velocity distributions, salt concentrations and discharge during one consecutive tidal cycle in summer season, 1983. High velocity cores reoccur two times a semi-diurnal tidal cycle at the same cross-sectional location, lower layer, in Kadok Channel during the spring tide. The tidal exchange ratio was estimated by Eulerian method. The range of exchange ratios in central Kadok Channel are 9.3-17% at the spring tide and 16.9-21.8% at the neap tide. On the other hand, its range in Masan bay-mouth is 8.7% at the spring tide and 2.0% neap tide, respectively.

A Study on Utilization of Processed Foods and Recognition of Food Labels among University Students (대학생들의 가공식품 구매실태와 식품표시 인지 정도)

  • This study examined the utilization of processed food and the recognition of food labels among 257 university students (201 females and 56 males) in Seoul and Kangwon-do. The results of the study showed that the most important considered information on food labels are shelf life, nutrition facts, and price in sequential order. Female students' recognition of nutrition facts was significantly higher than that of male students (p<0.05). Female students more attentively checked the manufacturer, origin of the products, and shelf life information on food labels than did the male students. In checking out the food labels information, the most checked items on food labels were calories, trans fatty acid and cholesterol in that order and the least checked information was the sodium contents. Among processed foods, male students' consumption was dominant in instant noodles, frozen dumplings, and canned goods, while female students had more candies/chocolates. In selecting processed foods, male students showed strong preference for cheaper and quantitative products, while female students chose more tasty, brand new, well-known brand, and products of domestic origin. Frequency of canned and frozen food consumption showed a positive correlation with BMI, while candies/chocolates showed a negative correlations with BMI. Negative correlations were found in the attitude of selecting food with longer shelf life and BMI. The results of this study suggest that university students need to be well informed to make wise food choices that contribute to a healthy diet. Additionally, food manufacturers and government authorities concerned should make certain that consumers know how to use food label information more easily and effectively through proper education.

Fine Needle Aspiration Cytology of Pulmonary Hamartoma - A Report of Two Cases - (폐 과오종의 세침흡인 세포학적 소견 - 2예 보고 -)

  • Pulmonary hamartoma is an uncommon benign tumor consisting of a mixture of loose fibromyxoid tissue, cartilage, fat, and cleft-like spaces lined by cuboidal or ciliated epithelium. Cytologically, the presence of a mesenchymal component is essential for the diagnosis of pulmonary hamartoma. We report the fine needle aspiration cytologic findings of two cases of pulmonary hamartoma. Case 1 was a 71-year-old woman with a mass, measuring $1.8{\times}1.5cm$ in the upper lobe of the right lung. Case 2 was a 51-year-old woman with a mass, measuring $2.3{\times}2.0cm$ in the lower lobe of the right lung. Fine needle aspiration cytology of both pulmonary masses revealed several sheets of loose fibromyxoid tissue fragments with focal cartilaginous differentiation and a few clusters of bland cuboidal epithelial cells on the bloody background. The diagnosis was histologically confirmed by needle biopsy.

Differences in Spatial Variation of Soil Chemistry Between Natural and Anthropogenic Soils

  • The Agricultural Land Remodeling Project was launched for agricultural fields with potential risk of flooding which were placed in low-lying area as a part of agricultural sectors of the National 4 River Project. It induced of the reclaimed agricultural fields on a national scale. The arable lands reestablished by reclamation have caused several big problems such as destruction of soil physicochemical properties, and thus the techniques to solve disadvantages were urgently required. In this study, we collected experimental samples from top soils in three agricultural areas, one from conventional agricultural fields (Hwasun, Jeollanam-do) and the others from reclaimed (remodelled) agricultural fields (Naju, Jeollanam-do and Gumi, Gyeongsangbuk-do), The soil chemistry data were analyzed using statistic tools such as semi-variance and kriging, and differences between natural and reconstructed soils were examined. The score, R (Ao) which indicates a dependence distance between each chemical element, was as follows; 21.8~43.5 (Conventional, Hwasoon), 4.4~70.6 (Remodelled, Naju) and 5.3~43.6 (Remodelled, Gumi). These results suggested that chemical properties of the reclaimed agricultural fields had a huge variation. Moreover, the result of kiriging maps also represented a ununiform pattern in the reclaimed lands. As a result of this study, it is strongly required to build up the soil type-specific management techniques for the reclaimed agricultural lands.

Repair of the Cleft Lip using Triangular Cheiloplasty-A Case Report (삼각피판법을 이용한 편측 불완전 구순열 환자의 구순 교정 수술-증례 보고)

  • After Tennison introduced a triangular flap method which, for the first time, preserved the Cupid's bow, Randall gave this method a sound mathematical basis. This method is also called as an inferior triangular cheiloplasty which is characterized by making a small triangular flap from the lateral border of cleft destined to be fitted into an incision on the medial side of cleft. He postulated that the height obtained was equal to the sum of the median of the two triangles used in the cheiloplasty. Using this technique, a 22 month-old male patient with incomplete unilateral cleft lip was corrected primarily. The deviation of the columella and flattening of the nostril on the cleft side were minimal. The operation was done under general anesthesia and patient was healed uneventfully. We tried to improve the symmetry and esthetic feature of philtrum, nostril sill, alar-facial groove, preventing the notch formation on the nostril floor, and to reconstruct the muscle sling in the upper part of lip. The shape of Cupid's bow was restored, and the symmetry of columella was regained as a result. In summary, the inferior triangular cheiloplasty is effective to correct the primary unilateral cleft lip, results in the restoration of favorable anatomy and function.

What kind of specialized services attract patients? (환자유치를 위한 특화서비스에 대한 연구)

  • Background: Competition among hospitals has intensified, because hospitals and beds are oversupplying and the global medical market has opened in Korea. Method: we developed questionnaires of the patient's satisfaction about 20 items. We surveyed 297 out-patients and 302 in-patients. We analyzed the descriptive statistics and regression by SPSS(Version 17) and Excel. Results: The relations with the patient's loyalty and the specialized hospital service are 1) Recommend to other person: FHR use and the assistant for follow-up treatment were related positively for out-patient and PHR and FHR use were related positively for out-patient. 2) Revisiting: PHR use and the need of an assistant for follow-up patients were related for them and the PayTV were negatively related. Conclusion: FHR, PHR and assistant system for follow-up patient's condition were very useful factors to increase patient's loyalty.
