  • Breast augmentation is becoming more common, be it for cosmetic reasons or post-mastectomy. Multiple articles in the literature describe the imaging findings of various types of cosmetic breast augmentation. Some articles describe imaging findings for different types of post-mastectomy reconstructions. This essay aims to serve as a comprehensive reference for the multimodality imaging of various types of breast augmentations in native breast and post-mastectomy reconstructions. Familiarity with these findings will facilitate the detection of complications and new or recurrent breast malignancies in patients. With the extensive illustrations provided in this essay on normal and abnormal imaging findings of augmented breasts, readers will receive exposure that will facilitate effective practice.

Usefulness of High-Resolution Ultrasonography after Foreign Body Injection on Aesthetic Plastic Surgery (성형외과 영역에서 이물질 주사에 대한 고해상초음파 검사의 유용성)

  • Purpose: The purpose of this study is to demonstrate the usefulness of the high resolutional ultrasonographic features in patients with foreign body. Methods: From September 2007 to August 2009, we retrospectively reviewed high resolutional ultrasonogram using 5~12 MHz linear transducer of 13 patients presenting with inflammation after foreign body injection. They were referred for complications after foreign body injection. Injected foreign bodies were 4 silicone, 4 paraffin, 2 artecoll, and 3 unknown. We treated them with foreign body removal (7), foreign body removal and corrective plastic surgery (4), and conservative treatment with antibiotics and steroid injection (2). Results: High resolutional ultrasonography well demonstrated the existence of foreign body and it's overall size, location within the tissue layer, and vascularity. Comparison between preoperative and postoperative ultrasonographic findings was useful not only to evaluate the prognosis but also to plan the treatment. These ultrasonographic findings aided in precise assessment of the contour and location of the foreign body and led to an accurate surgery. We were able to acquire various information in order to set a detailed plan for the operation which in turn, led to a precise, successful surgery. After the treatment, complication did not occur in 12 patients, except 1 patient. But this patient was also treated after reoperation. Postoperative high resolutional ultrasonography shows almost foreign body removed and inflammation disappeared. Conclusion: Considering the usefulness of highresolution ultrasonography in foreign body injection, highresolution ultrasonography would be necessary for both the patient and the doctor. Preoperative and postoperative high resolutional ultrasonography is highly accurate, safe, inexpensive and easy. It can be a useful modality in foreign body after plastic surgery.

Self Induced Foreign Body Injection Patient: A Case Report (안면부 및 경부에 이물질을 자가 주입한 환자 1례: 증례보고)

  • Purpose: In the past, laymen or beauty parlor employees commonly injected people with unproven foreign material such as paraffin, silicone, and cooking oil. We have treated a patient who injected foreign material into her face and neck. Methods: For the last 20 years, a 43-year-old woman has been injecting herself in the face and neck with foreign material. There was no tenderness, erythema, pain, ulceration, or necrosis. However many visible, touchable subcutaneous masses were found. Her face was extremely ugly and disfigured by the foreign material. We could not recognize the boundary between her neck and mandible. The occipital scalp drooped extremely. Over the past 3 years, from February 2005 to October 2007, we performed 15 operations. Results: We conducted a cephalometric facial analysis to compare preoperative and postoperative facial diameter. The patient's upper face diameter decreased from 67 cm to 60 cm, the mid face diameter from 82 cm to 59 cm, the lower face diameter from 63 cm to 50 cm, and the neck circumference diameter from 53 cm to 44 cm, respectively. The mid sagittal diameter decreased from 26 cm to 23 cm. The total excised tissue weight was 4023.7 gram after 15 operations. Conclusion: Serial excision of face and neck masses in a patient who injected herself with foreign material resulted in a satisfactory outcome.

The Etiology and Treatment of the Softened Phallus after the Radial Forearm Osteocutaneous Free Flap Phalloplasty

  • Background The radial forearm osteocutaneous free flap is considered to be the standard technique for penile construction. One year after their operation, most patients experience a softened phallus, so that they suffer from difficulties in sexual intercourse. In this report, we present our experience with phalloplasty by radial forearm osteocutaneous free flap, as well as an evaluation of the etiology and treatment of the softened phallus. Methods Between March 2005 and February 2010, 58 patients underwent phalloplasty by radial forearm osteocutaneous free flap. Most of their neophallus had been softened subjectively and among them, 12 patients who wanted correction were investigated. We performed repetitive fat injection, artificial dermis grafting, silicone rod insertion, and rib bone with cartilaginous tip graft. Physical examination, plain radiograph, computed tomography, bone scintigraphy, and satisfaction scores were investigated. Results Most of the participants' penises have been softened after phalloplasty, and the skin elasticity had been also decreased. On plain radiograph, the distal end of the bone was self-rounded; however, the bone shape of the neophallus had no significant interval changes or resorption. Computed tomography showed equivocal density of cortical bone. On bone scintigraphy, the bone metabolism was active at 3 months postoperatively, and remained active 9 years postoperatively. Conclusions The use of a rib bone with cartilaginous tip graft could be an option for improvement of the softened phallus. Silicon rod insertion is also worth considering for rigidity of the softened phallus. Decreased rigidity due to soft tissue atrophy could be alleviated with repeated fat injection and artificial dermis grafting.

Optimum design of injection mold heater for uniform curing of LSR seal for waterproof connector (방수 커넥터용 LSR Seal의 균일 경화를 위한 사출 금형 히터의 최적 설계)

  • Automotive waterproof connectors are highly functional parts that must be air-tight in a complex environment. In the LSR multi-cavity injection molding process for manufacturing waterproof connectors, it is important to maintain a uniform curing temperature between the cavities in order to obtain a quality product. For this purpose, we designed the capacity of the cartridge heater differently for each position, and then linked the heat transfer analysis and optimization module to obtain the optimal cartridge heater capacity. As a result of the optimization analysis, the temperature deviation between cavities was decreased from $13.1^{\circ}C$ to $8.1^{\circ}C$ compared with the case in which constant heater capacity was applied, so that the design criterion could be satisfied within a temperature deviation of $10^{\circ}C$ for uniform curing. This study suggests that this method can be applied efficiently to the design of a large area multi-cavity LSR mold heater.

An experimental study on the characteristics of transverse jet into a supersonic flow field (초음속 유동장에서의 충돌제트 특성에 대한 실험적 연구)

  • When a secondary gaseous flow is injected vertically into a supersonic flow through circular nozzle, a complicated structure of flow field is produced around the injection area. The interaction between the two streams produces a strong bow shock wane on the upstream side of the side-jet. The results show that bow shock wave and turbulent boundary layer interaction induces the boundary layer separation in front of the side-jet. This study is to analyze the structure of flow fields and distribution of surface pressure on the flat plate according to total pressure ratio using a supersonic cold-flow system and also to study the control force of affected side-jet. The nozzle of main flow was designed to have Mach 2.88 at the exit. The injector has a sonic nozzle with 4mm diameter at the exit of the side-jet. In experiments, The oil flow visualization using a silicone oil and ink was conducted in order to analyze the structure of flow fields around the side-jet. The flow fields are visualized using the schlieren method. In this study, a computational fluid dynamic solution is also compared with experimental results.

  • Unilateral vocal cord paralysis is induced by various causes and its effective treatment has been diversely searched out until now. Currently used treatment modalities are intracordal injection of exogenous materials such as Teflon or Silicone, and thyroplasty and so forth. But, with the above mentioned modalities, it has been not satisfactory to obtain a good postoperative results especially in cases when the glottal incompetence is very severe or the level difference between the vocal cords is large. In such cases, vocal cord adduction can be accomplished by anteromedial traction of the muscular process of paralyzed vocal cord via surgical exposure resulting improvement of voice quality. Recently, authors performed arytenoid adduction in 3 cases of unilateral vocal cord paralysis to obtain a better improvement of voice quality, and experienced satisfiable postoperative results.

  • In this study, the yield of the modules in LCD production lines, improving current TFT-LCD production process is essential for high-pressure silicone injection equipment, and precision control system was developed. This full-scale production of the future through the development of next-generation display production line is being prepared, being transferred to China in LCD production facilities can make the most of efficient equipment. Therefore, minimize the cost of new investment and help create the maximum effect to control the detailed behavior of the sequence H/W and S/W system was installed on the production line. In addition, Fast-evacuating the structure proposed for the Vacuum pump, Pump control circuit design and experimental results has been completed.

Fabrication and Processing Method of Ophthalmic Hydrogel Tinted Lens Containing Indium Tin Oxide-Composited Materials

  • In this study, a multifunctional ophthalmic lens material with an electromagnetic shielding effect, high oxygen permeability, and high water content is tested, and its applicability is evaluated. Metal oxide nanoparticles are applied to the ophthalmic lens material for vision correction to shield harmful electromagnetic waves; the pyridine group is used to improve the antibacterial effect; and silicone substituted with urethane and acrylate is employed to increase the oxygen permeability and water content. In addition, multifunctional tinted ophthalmic lens materials are studied using lens materials with an excellent antibacterial effect (2,6-difluoropyridine, 2-fluoro-4-pyridinecarboxylic acid) and functional (UV protection, high wettability) lens materials (2,4-dihydroxy benzophenone, 2-hydroxy-4-(methacryloyloxy)benzophenone). To solve problems such as air bubbles generated during the polymerization process for the manufacturing and turbidity of the lens surface, polymerization conditions in which the defect rate is minimized are determined. The results show that the polymerization temperature and time are most appropriate when they are $110^{\circ}C$ and 40 minutes, respectively. The optimum injection amount of the polymerization solution is 350 ms. The turbid phenomenon that appears in lens processing is improved by 10 to 95 % according to the test time and conditions.

Etiology of Delayed Inflammatory Reaction Induced by Hyaluronic Acid Filler

  • The etiology and pathophysiology of delayed inflammatory reactions caused by hyaluronic acid fillers have not yet been elucidated. Previous studies have suggested that the etiology can be attributed to the hyaluronic acid filler itself, patient's immunological status, infection, and injection technique. Hyaluronic acid fillers are composed of high-molecular weight hyaluronic acids that are chemically cross-linked using substances such as 1,4-butanediol diglycidyl ether (BDDE). The mechanism by which BDDE cross-links the two hyaluronic acid disaccharides is still unclear and it may exist as a fully reacted cross-linker, pendant cross-linker, deactivated cross-linker, and residual cross-linker. The hyaluronic acid filler also contains impurities such as silicone oil and aluminum during the manufacturing process. Impurities can induce a foreign body reaction when the hyaluronic acid filler is injected into the body. Aseptic hyaluronic acid filler injections should be performed while considering the possibility of biofilm formation or delayed inflammatory reaction. Delayed inflammatory reactions tend to occur when patients experience flu-like illnesses; thus, the patient's immunological status plays an important role in delayed inflammatory reactions. Large-bolus hyaluronic acid filler injections can induce foreign body reactions and carry a relatively high risk of granuloma formation.
