질벽의 실리콘 액 주사에 의한 폐색전종 및 급성 호흡곤란 증후군 1예 (A Case of Acute Respiratory Distress Syndrome with Pulmonary Embolism Induced by Injection of Silicone at Vaginal Wall)

  • 실리콘 액은 미용 및 성형수술에 널리 사용되어지고 실리콘에 의한 불법적인 의료행위가 증가함에 따라 국소적 부작용 뿐만 아니라 폐 색전증에 의한 급성 호흡곤란 증후군등의 치명적인 폐침범의 부작용을 일으킨다는 보고가 있으나 국내보고는 아직 없었다. 본 증례에서는 건강한 39세 젊은 여자 환자가 질벽에 실리콘액을 주사한 후에 발생한 폐 색전증을 동반한 급성 호흡곤란 증후군 1예을 경험하였기에 문헌고찰과 함께 보고하는 바이다.

Silastic을 이용한 내전 갑상성형술-적용 및 술기 (Medialization Thyroplasty with Silastic- Decision Making & Practical Points)

  • Unilateral vocal fold paralysis resulting in glottal incompetence can cause significant morbidity attributable to impaired speech, swallowing, and ability to protect the airway. The treatment of unilateral vocal cord paralysis has a long history, marked by technical innovations and improvements. These methods typically use endoscopic injection or implants to augment the volume of the affected vocal fold. The first known treatment, reported by Brunnings in 1911, was paraffin injection. The first thyroplasty medializing the paralysed vocal cord was performed by Payr in 1915 ; here, a cartilage door-flap was created from the thyroid ala to obtain better voice quality. In the 1970s, Isshiki systematized and developed the use of the external medialization by Payr. Later he modified his original technique, and achieved safer and better results. Many other methods were introduced for external medialization during the 1980s and 1990s. There has been couple of materials using for medialization laryngoplasty: silicone bloc, cartilage, goretex (polytetrafluoroethylene), titanium, etc. Among them, silicone bloc is the most popularly used material. Type I thyroplasty in combination with arytenoid adduction is a proven technique for medialization of the paralysed vocal fold. In this paper, personal experience for using silicone bloc type I thyroplasty : decision making and practical points, long-term results and complication of the procedure will be discussed.

  • Recently, Silicone that one of the thermo-sets is used to making optical products such as LED lenses because of excellent thermal properties. LED lenses are required to keep the precise dimensions, so they must be molded to have the minimum deformation. Thermo-sets have the expansion characteristic on the part of thermal property, it is important to optimize the cure condition so that the deformation of the part become minimum. In this study, to investigate the relationship between the shrinkage by the curing and expansion by the thermal properties of the resin, reactive injection experiment was performed by setting the variables such as mold setting temperature, cure time. As a result, it was confirmed that there was a interval while the thermal properties were transferred to more active during the cure process. It is expected to help in determining the reactive injection molding conditions of the thermo-set parts as well as LED lens in order to reduce the amount of deformation.

  • A micro syringe which can be attached to the end of a micro intravascular endoscope for drug injection is fabricated and its characteristic is tested. The syringe consists of a drug chamber and an actuator chamber which are separated by a silicone rubber membrane. The drug chamber is filled with liquid drug by the membrance actuation caused by the vaporization and condensation of the working liquid in the actuator chamber. The liquid drug is ejected by the electrolysis of the working liquid. The membrane deflection by each actuation method has been measured. The liquid ejection image has been captured during the electrolysis of the electrolyte. Also, the successful operation of the micro syringe under the normal blood pressure was verified.

Silicone Rubber-Filled 허니콤 샌드위치 복합재료의 댐핑 특성 (Damping Characteristic of Silicone Rubber-Filled Honeycomb Sandwich Composite)

  • In this paper a new sandwich composite is developed by injection silicone rubber into the honeycomb core. This composite material is designed to have a improved damping performance. For verification damping tests were conducted to the specimens with different stacked USNl25 carbon/epoxy prepreg laminate facesheets, $[0/90]_{4s},\;[0/45-45/90]_{2s},\;[45/-45]_{4s}$. Frequency response, displacement response and damping ratio were checked and compared for the both groups of specimens, with and without rubber fillings. The experimental results provided a good agreement with our original material design concept.

커플링제로 처리된 질화규소 분말의 사출성형 (Injection Molding of Silicon Nitride Powders Treated with Coupling Agents)

  • The effects of silane coupling agents on the injection molding process were investigated using silicone nitride mixtrues with a binder system containing polypropylene as a major binder (55vol% solid loading). The formation of bonding between silicon nitride powder and coupling agents was confirmed through the analyses of powder surface. The use of coupling agents improved mixing characteristics judged by the torque change during mixing process. the coupling agents also reduced molten viscosity of the mixture considerably, which is a main factor to determine the flow of the mixture. However, the bonding between coupling agents and polymers had a negative effect on the debinding process by retarding the thermal decomposition.

  • Based on eco-friendly advantages and the enhanced development in the chemical and physical characteristics, liquid silicone rubber (LSR) is widely used in producing precision parts in the automotive, medical, electronics, aeronautical and many other industries. In the current work, a thermoplastic-thermoset multi component injection molding (MCM) was developed for a waterproof automotive connector housing using PBT and LSR resins. Measurements of the rheological characteristics of PBT and LSR were made to improve the reliability of the numerical analysis for the multi component injection process. With the measured viscosity, pvT and curing data, numerical analysis of the multi cycle injection molding was conducted using simulation software (Sigma V5.0).

  • Direct injection of foreign material, such as liquid paraffin and silicone, into the breast can induce a foreign body granulomatous reaction and fibrosis, resulting in hard, nodular breast masses and architectural distortion that can mimic neoplasm. Conventional methods, including physical examination, mammography, and ultrasonography are of little use to differentiate between foreign body-induced mastopathy and breast cancer. In patients with foreign body injection such as breast augmentation, dynamic contrast enhanced MR imaging is an excellent imaging modality. Here, the authors report the MR imaging and pathological findings of ductal carcinoma in situ (DCIS) with multicystic changes in a 41-year-old woman with a previous history of interstitial mammoplasty by paraffin injection.

Novel Resectable Myocardial Model Using Hybrid Three-Dimensional Printing and Silicone Molding for Mock Myectomy for Apical Hypertrophic Cardiomyopathy

  • Objective: We implemented a novel resectable myocardial model for mock myectomy using a hybrid method of three-dimensional (3D) printing and silicone molding for patients with apical hypertrophic cardiomyopathy (ApHCM). Materials and Methods: From January 2019 through May 2020, 3D models from three patients with ApHCM were generated using the end-diastolic cardiac CT phase image. After computer-aided designing of measures to prevent structural deformation during silicone injection into molding, 3D printing was performed to reproduce anatomic details and molds for the left ventricular (LV) myocardial mass. We compared the myocardial thickness of each cardiac segment and the LV myocardial mass and cavity volumes between the myocardial model images and cardiac CT images. The surgeon performed mock surgery, and we compared the volume and weight of the resected silicone and myocardium. Results: During the mock surgery, the surgeon could determine an ideal site for the incision and the optimal extent of myocardial resection. The mean differences in the measured myocardial thickness of the model (0.3, 1.0, 6.9, and 7.3 mm in the basal, midventricular, apical segments, and apex, respectively) and volume of the LV myocardial mass and chamber (36.9 mL and 14.8 mL, 2.9 mL and -9.4 mL, and 6.0 mL and -3.0 mL in basal, mid-ventricular and apical segments, respectively) were consistent with cardiac CT. The volume and weight of the resected silicone were similar to those of the resected myocardium (6 mL [6.2 g] of silicone and 5 mL [5.3 g] of the myocardium in patient 2; 12 mL [12.5 g] of silicone and 11.2 mL [11.8 g] of the myocardium in patient 3). Conclusion: Our 3D model created using hybrid 3D printing and silicone molding may be useful for determining the extent of surgery and planning surgery guided by a rehearsal platform for ApHCM.

  • The purpose of this experiment is to modify diagnosis criterion of isothermal relaxation current(IRC) measurement equipment which is using for distribution cable diagnosis. We're using this system for several years in the field instead of DC leakage current measurement and lots of cables were replaced. But we have to investigate on the reliability of this equipment for our cables because we didn't carried out condition assessment of extracted cables after field diagnosis by this equipment. It is important thing for cable maintenance. If the replacement criterion is improper we can not prevent failures or will waste budget on account of replacement of the sound cables. In this paper we selected field installed cables and injected silicone fluid to the cables for insulation rehabilitation. In order to prove reliability of the diagnosis equipment we compared diagnosis results and AC breakdown strength according to operating time after silicone treatment. This is the results of the field test for 1 year.
