Thailand Consensus on Helicobacter pylori Treatment 2015

  • Management of Helicobacter pylori infection is an important aspect of many upper gastrointestinal tract diseases, such as chronic gastritis, peptic ulcer disease, gastric cancer and mucosa-associated lymphoid tissue (MALT) lymphoma. The Thailand Consensus on H. pylori treatment 2015 consisted of 22 national experts who took active roles, discussed all important clinical information and investigated clinical aspects in four workshops, focuising on: (1) Diagnosis (2) Treatment (3) Follow-up after eradication and (4) H. pylori infection and special conditions. Experts were invited to participate on the basis of their expertise and contribution to H. pylori works and/or consensus methodology. The results of each workshop were taken to a final consensus vote by all experts. Recommendations were developed from the best evidence and availability to guide clinicians in management of this specific infection associated with variety of clinical outcomes.

잘게 저민 생선의 드럼건조기에 의한 건조특성 (Drying Characteristics of Minced Fish on Drum Dryers)

  • 단(單)원통형 및 원통형 드럼건조기를 이용하여 잘게 저민 생선으로부터 플레이크를 성공적으로 만들 수 있었다. 생선의 높은 지방함량으로 인해 저민 생선이 드럼에 잘 들러붙지 않는 문제점은 타피오카가루의 형태로 2.5%의 전분을 저민 생선에 첨가함으로서 건조양상에는 영향을 미치지 않으면서 해결될 수 있었다. 제품의 수분함량을 일정한 수준으로 유지할 경우 건고형분(乾固形分)의 생산속도는 드럼간의 간격을 최소로 줄였을때 최고에 도달하였다. 건고형분(乾固形分)의 생산속도는 드럼의 회전속도가 증가할수록 선형적으로 증가하였고 제품의 품질이 손상되지 않는 범위에서 스팀의 압력이 높을수록 증가하였다. 총괄 열전달계수는 드럼의 회전수가 증가함에 따라 증가한 반면에 드럼간의 간격이 커짐에 따라 증가하지는 않았다. $MR\;=\;A\;\exp(\;-k{\theta})$로 표시되는 가장 보편적인 건조 모델식이 드럼건조에도 적용될 수 있었다.

Comparison of Gut Microbiota between Lean and Obese Adult Thai Individuals

  • Current reports suggest that obesity is a serious global health issue. Emerging evidence has predicted strong links between obesity and the human gut microbiota. However, only a few such studies have been conducted in Asia, and the gut microbiota of lean and obese adult Asians remains largely unexplored. Here, we investigated the potential relationship between gut microbiota, body massindex (BMI), and metabolic parameters in adults from Thailand, where obesity is increasing rapidly. Fecal and blood samples were collected from 42 volunteers who were allocated into lean, overweight, and obese groups. The fecal microbiota was examined by quantitative PCR analysis. Bacteroidetes, Firmicutes, and Staphylococcus spp. and methanogens were most abundant in lean volunteers. Overweight volunteers majorly harbored Christensenella minuta and Akkermansia muciniphila, ${\gamma}-Proteobacteria$, and bacteria belonging to the genus Ruminococcus. Methanogens and bacteria belonging to the phylum Bacteroidetes were negatively correlated with adiposity markers (BMI and waist circumference), but positive correlated with high-density lipoprotein, suggesting that they can be used as leanness markers. While some of our results agree with those of previous reports, results regarding the contributions of specific taxa to obesity were inconsistent. This is the first study to report the adult gut microbiota in Southeast Asian populations using molecular techniques and biochemical markers and provides a foundation for future studies in this field.

Accuracy of Self-Checked Fecal Occult Blood Testing for Colorectal Cancer in Thai Patients

  • Purpose: Colorectal cancer (CRC) screening with fecal occult blood testing (FOBT) has been associated with a reduction in CRC incidence and CRC-related mortality. However, a conventional FOBT requires stool collection and handling, which may be inconvenient for participants. The EZ-Detect$^{TM}$ (Siam Pharmaceutical Thailand) is a FDA-approved chromogen-substrate based FOBT which is basically a self-checked FOBT (no stool handling required). This study aimed to evaluate the accuracy of EZ-Detect for CRC detection. Methods: This prospective study was conducted in the Faculty of Medicine, Siriraj Hospital, Bangkok, Thailand between November 2013 and May 2014. Some 96 patients with histologically-proven CRC and 101 patients with normal colonoscopic findings were invited to perform self-checked FOBT according to the manufacturer's instructions. Results were compared with endoscopic and pathologic findings. Sensitivity, specificity, positive predictive value (PPV), and negative predictive value (NPV) for CRC detection were calculated. Results: The present study revealed the sensitivity, specificity, PPV and NPV of this self-checked FOBT for CRC detection to be 41% (95% CI: 31-51), 97% (95% CI: 92-99), 93% (95% CI: 81-98) and 63% (95% CI: 55-70), respectively. The overall accuracy of the self-checked FOBT for identifying CRC was 70%. The sensitivity for CRC detection based on 7th AJCC staging was 29% for stage I, 32% for stage II and 50% for stage III/IV (P=0.19). The sensitivity was 33% for proximal colon and 42% for distal colon and rectal cancer (P=0.76). Notably, none of nine infiltrative lesions gave a positive FOBT. Conclusions: The self-checked FOBT had an acceptable accuracy of CRC detection except for infiltrative tumors. This home-administrated or 'DIY' do-it-yourself FOBT could be considered as one non-invasive and convenient tool for CRC screening.

아시아 일부지역의 광천수 수질특성에 관한 연구 (A Study on the Mineral Water Quality in Asia Partial Area)

  • From March 1, 2005 to August 31, 2006 mineral water wells were selected for 13 areas in Southeast Asia (Indonesia, Philippines, Thailand, Singapore) and 20 areas in Northeast Asia (Korea, North Korea, China, Japan) and the study on their mineral water quality was analyzed. - Mineral water quality was the best in Korea. Mineral spring waters in some area of China and North Korea and in the whole area of the Southeast Asia were of poor quality. - The hardness of mineral water was the low in Korea $(10{\sim}47mg/L\;as\;CaCO_3)$, Japan $(4{\sim}66mg/L\;as\; CaCO_3)$, Geumgang-san North Korea Area $(4mg/L\;as\;CaCO_3)$. Mineral spring water in Thailand, Indonesia $(1{\sim}97.5mg/L\;as\;CaCO_3)$ and in the other area $(120{\sim}1205mg/L\;as\;CaCO_3)$ were high degree of hardness. - pH value in the mineral water of Southeast Asia $(pH\;6.7{\sim}8.2)$ and Northeast Asia $(pH\;5.9{\sim}7.9)$ was up to WHO standard $(pH\;6.5{\sim}8.5)$. Fluorine of negative ion was found in 10 mineral waters: Indonesian mineral water 'ATARTN'(0.02mg/L), Thailand mineral water 'SIAM' (0.6mg/L), 'MASAFI' (0.02mg/L). Korean mineral water 'SAEMMULNARA' (1.1mg/L), 'SANSU'(0.6mg/L), 'ICIS'(0.3mg/L), 'DONGWON SAEM-MUL'(0.03mg/L), 'PYEONGCHANG' (0.6mg/L), North Korean mineral water 'KUMGANGSAN'(0.1mg/L), Japanese mineral water 'CRYSTAL GEYSER'(0.55mg/L). However Fluorine in the other 23 mineral waters were not detectable.

Changes in Growth Characteristics of Seven Foliage Plants Grown in an Indoor Bio-Wall System Depending on Irrigation Cycle

  • In order to increase the indoor air purification effect of plants, plants need to be placed on 5-10% of indoor spaces. To increase the density and utilization of plants in indoor spaces, studies on bio-wall, a vertical green wall system, have been recently conducted. The purpose of this study was to investigate the growth characteristics of 7 indoor plants introduced to the system and their rooting zones at different irrigation cycles. This study was conducted to investigate a proper irrigation cycle for the continuous maintenance of bio-wall systems. The conditions of their growth environment were maintained as follows: light intensity, 20-50 μmol·m-2·s-1 PPFD; and temperature, 20 - 25℃. For fertilization, Hyponex diluted with water at the ratio of 1:1,000 was supplied to plants. Irrigation was treated at intervals of 1, 3, 5, and 7 days for 1 hour at a time. As a result, there was no significant difference in the growth of plants between different irrigation cycles. Dieffenbachia 'Marianne' showed a significant decrease in the number of leaves at the irrigation cycle of 7 days. In addition, the chlorophyll content was relatively low at the irrigation cycle of 7 days. In terms of the color of leaves, a decrease in L value and b value and an increase in a value were observed, resulting in changes in brightness and color. Ardisia pusilla 'Variegata' showed a slightly higher photosynthetic activity and stomatal conductance when it was watered every day and once per 5 days, while Epipremnum aureum showed a relatively higher photosynthetic activity and stomatal conductance at the irrigation cycle of 3 days. In the case of root activity, it was found that the longer irrigation cycle, the higher root activity compared to daily irrigation. The development of roots of Peperomia clusiifolia was promoted by watering at long intervals. However, in the case of Aglaonema 'Siam-Aurora', the total number of roots decreased at the interval of 7 days. In conclusion, a proper irrigation cycle for the sustainable maintenance of vertical bio-wall systems seems to be 3 days.

Anatomical knowledge of zygomatico-buccal plexus in a cadaveric study

  • The details of the facial nerve pattern were clearly explained in the parotid gland (PG), lateral area of the face, and periorbital areas to prevent the unexpected outcome of medical intervention. However, it remains unclear whether information about the zygomatico-buccal plexus (ZBP) in the masseteric and buccal regions. Therefore, this study aimed to help clinicians avoid this ZBP injury by predicting their common location. This study was conducted in forty-two hemifaces of twenty-nine embalmed cadavers by conventional dissection. The characteristics of the buccal branch (BB) and the ZBP were investigated in the mid-face region. The results presented that the BB gave 2-5 branches to emerge from the PG. According to the masseteric and buccal regions, the BB were arranged into ZBP in three patterns including an incomplete loop (11.9%), a single-loop (31.0%), and a multi-loop (57.1%). The mean distance and diameter of the medial line of the ZBP at the corner of the mouth level were 31.6 (6.7) and 1.5 (0.6) mm respectively, while at the alar base level were 22.5 (4.3) and 1.1 (0.6) mm respectively. Moreover, the angular nerve arose from the superior portion of the ZBP at the alar base level. The BB formed a multiloop mostly and showed a constant medial line of ZBP in an area approximately 30 mm lateral to the corner of the mouth, and 20 mm lateral to the alar base. Therefore, it is recommended that physicians should be very careful when performing facial rejuvenation in the mid-face region.

Initial Risk Assessment of Disodium Disulphite in OECD High Production Volume Chemical Program

  • Disodium disulphite, the HPV chemical, was assigned to Korea in order to implement OECD SIDS program in 1999. It was produced about 3,200 ton/year in 1998. This report evaluates the toxic potency of disodium disulphite based on the environmental and mammalian effects as well as human exposure. Oral $LD_{50}$ in rats is 1,540 mg/kg b.w. and effects was observed to the stomach, liver and the GI track that was filled with blood. For repeated dose toxicity, the predominant effect was the induction of stomach lesion due to local irritation. The no observed adverse effect lever for local (stomach irritation) was about 217 mg/kg bw/day. There is no evidence that disodium disulphite is genotoxic in vivo. No reproductive or developmental toxicty of disodium disulphite was observed for the period up to 2 yr and over three generation. In humans, urticaria and asthma with itching, edema, rhinitis, and nasal congestion were reported. Disodium disulphite is unlikely to induce respiratory sensitization but may enhance symptom of asthma in sensitive individuals. This chemical would be mainly transported to water compartment when released to environmental compartments since it is highly water soluble (470 g/l at 20). Low K oc (2.447) indicates disodium disulphite is so mobile in soil that it may not stay in the terrestrial compartment. The chemical has been tested in a limited number of aquatic species. hem acute toxicity test to fish, 96 hr-$LC_{50}$ was > 100 mg/1. For algae, 72 hr-$XC_{50}$ was 48.1 mg/1. For daphnid, the acute toxicity value of 48 hr-$EC_{50}$ was 88.76 mg/1, and chronic value of 21day-NOEC was > 10 mg/1. Therefore, PNEC of 0.1 mg/l for the aquatic organism was obtained from the chronic value of daphnid using the assessment factor of 100. Based on these data the disodium disulphite was recommended as low priority for further post-SIDS work in OECD.

  • In Korea, 2,320 tonnes of acetanilide were mostly wed as intermediates for synthesis in phar-maceuticals or additives in synthesizing hydrogen peroxide, varnishes, polymers and rubber. Only small amount of 120 kg were wed as a stabilizer for hydrogen peroxide solution for hair colouring agents in 1998. Readily available environmental or human exposure data do not exist in Korea at the present time. However, potential human exposures from drinking water, food, ambient water and in work places are expected to be negligible because this chemical is produced in the closed system in only one company in Korea and the processing factory is equipped with local ventilation and air filtering system. Acetanilide could be distributed mainly to water based on EQC model. This substance is readily biodegradable and its bioaccumulation is low. Acute toxicity of acetanilide is low since the L $D_{50}$ of oral exposure in rats is 1,959 mg/kg bw. The chemical is not irritating to skin, but slightly irritating to the eyes of rabbits. horn repeated dose toxicity, the adverse effects in rats were red pulp hyperplasia of spleen, bone marrow hyperplasia of femur and decreased hemoglobin, hematocrit and mean corpuscular hemoglobin concentration. The LOAEL for repeated dose toxicity in rats was 22 mg/kg/day for both sexes. Acetanilide is not considered to be genotoxic. In a reproductive/developmental toxicity study, no treatment-related changes in precoital time and rate of copulation, impregnation, pregnancy were shown in all treated groups. The NOAELs for reproduction and developmental toxicity (off-spring toxicity) are considered to be 200 mg/kg bw/day and 67 mg/kg bw/day, respectively. Ecotoxicity data has been generated in a limited number of aquatic species of algae (72 hr- $E_{b}$ $C_{50}$; 13.5 mg/l), daphnid (48hr-E $C_{50}$ > 100 mg/l) and fish (Oryzias latipes, 96hr-L $C_{50}$; 100 mg/l). Form the acute toxicity values, the predicted no effect concentration (PNEC) of 0.135 mg/1 was derived win an assessment factor of 100. On the basis of these data, acetanilide was suggested as currently of low priority for further post-SIDS work in OECD.in OECD.D.

Spatiotemporal Trends of Malaria in Relation to Economic Development and Cross-Border Movement along the China-Myanmar Border in Yunnan Province

  • The heterogeneity and complexity of malaria involves political and natural environments, socioeconomic development, cross-border movement, and vector biology; factors that cannot be changed in a short time. This study aimed to assess the impact of economic growth and cross-border movement, toward elimination of malaria in Yunnan Province during its pre-elimination phase. Malaria data during 2011-2016 were extracted from 18 counties of Yunnan and from 7 villages, 11 displaced person camps of the Kachin Special Region II of Myanmar. Data of per-capita gross domestic product (GDP) were obtained from Yunnan Bureau of Statistics. Data were analyzed and mapped to determine spatiotemporal heterogeneity at county and village levels. There were a total 2,117 malaria cases with 85.2% imported cases; most imported cases came from Myanmar (78.5%). Along the demarcation line, malaria incidence rates in villages/camps in Myanmar were significantly higher than those of the neighboring villages in China. The spatial and temporal trends suggested that increasing per-capita GDP may have an indirect effect on the reduction of malaria cases when observed at macro level; however, malaria persists owing to complex, multi-faceted factors including poverty at individual level and cross-border movement of the workforce. In moving toward malaria elimination, despite economic growth, cooperative efforts with neighboring countries are critical to interrupt local transmission and prevent reintroduction of malaria via imported cases. Cross-border workers should be educated in preventive measures through effective behavior change communication, and investment is needed in active surveillance systems and novel diagnostic and treatment services during the elimination phase.
