Purpose: The purpose of this study was to identify the effects of cancer-overcome BeHaS exercise program on shoulder joint function, stress, body image and self-esteem in women who have had surgery for breast cancer. Method: A non-equivalent control group pre-post test design with an experimental group (n=25) and a control group (n=25) was used. The experimental group participated in the program once a week for eight weeks. Data were analyzed using descriptive statistics and Chi square and t-test with the SPSS Win 17.0. Results: There were significantly increased in shoulder joint function (p=.012), body image (p=.001), and self-esteem (p=.013), and significantly decreased in stress (p=.003). Conclusion: The results suggest that breast cancer-overcome BeHaS exercise program had beneficial effects on shoulder joint function, body image, self-esteem and stress in patients who have had surgery for breast cancer.
Purpose: The cane is one of the most popular assistive devices for stroke patients. Clinical complaints of sensorimotor functions on the ipsilateral upper limb were appealed in stroke patients who had used a cane for a long period. Therefore, we investigated whether cane usage for a long-term period affected sensoriomotor dysfunctions on the non-affected upper limb, in terms of pain presence, shoulder joint sense, a nine-hole pegboard test, and a tracking task. Methods: We recruited 12 stroke patients, who were divided into the cane-using (CU) group or the non-cane using (NCU) group, according to cane usage experience. We evaluated joint position sense for the integrity of proprioceptive reposition sense in the shoulder joint, used a nine-hole pegboard test for upper limb dexterity evaluation, and a tracking task for visuomotor coordination. Results: Four patients in the CU group had complained of shoulder pain none did in the NCU group. In addition, the CU group showed more reposition errors on the shoulder joint than the NCU group did. In addition, the CU group had more difficulty in proprioceptive sense perception and in performance of the nine-hole pegboard teat and tracking task, compared with the NCU group. Conclusion: Our findings suggest that cane usage for a long period in stroke patients could give rise to trigger joint pain and decrease proprioceptive sense. In addition, complex motor performance in the ipsilateral upper limb could deteriorate. In stroke patients who had used acane for long period, careful observation and proper intervention will be necessary.

Purpose: In the traumatic anterior shoulder instability, the laxity of joint capsule and ligament is frequently demonstrated. Although a arthroscopic procedure to address anterior instability with joint capsular redundancy have generally provided good results, its recurrence rate is higher than open procedure. By reducing the capsular redundancy, thermal shrinkage is likely to improve the outcome of arthroscopic anterior stabilization. The objective of this study was to evaluate additional thermal capsular shrinkage as a treatment of joint capsular redundancy in anterior shoulder instability. Materials and Methods: From March 1999 to June 2000, 25 shoulders of 23 patients of recurrent anterior shoulder dislocation underwent arthroscopic Bankart repair with shrinkage procedure. The mean follow up was 29 months and average age at the time of operation was 26 years. Of these patients, 20 were male and 3 were female who had been experienced the average 8 times of dislocation before operation. Thermal shrinkage alone without Bankart repair was performed in two cases who did not have Bankart lesion. The clinical result was evaluated in according to Modified Rowe Score. Results: The Modified Rowe Score was improved from preoperative 35 points to postoperative 88 points. None of cases showed recurrence of dislocation. But, in two cases, temporary sensory hypesthesia of the axillary nerve was developed and in two cases of postoperative stiffness, arthroscopic capsular release and brisement were performed. Conclusion: Additional capsular shrinkage in arthroscopic technique to address recurrent anterior shoulder instability could treat effectively the capsular redundancy.
Objectives : The purpose of this study was 1) to investigate the possibility of taking blood stasis pattern as a clinical parameter of frozen shoulder, using blood stasis pattern questionnaire (BSPQ) to frozen shoulder group comparing with normal group, and 2) to find out the relationship of the severity of main frozen shoulder symptoms (pain and limited range of motion of shoulder) and the level of blood stasis pattern through BSPQ analysis. Methods : During the period of January 2010 to July 2010, fifty five frozen shoulder patients who visited outpatient clinic of department of acupuncture and moxibustion in East-West Neo Medical Center, Kyung Hee University and fifty five normal people without shoulder pain nor limited ROM in shoulder joint were evaluated through BSPQ, and to the frozen shoulder group, also evaluated three different visual analogue scales (VAS) of pain (pain on average, pain at night, and pain on motion) and active / passive range of motion (ROM) of shoulder joint. Results : Mean blood stasis pattern score of frozen shoulder patients group was significantly higher than the score of normal group (patients vs normal group : $4.85{\pm}1.68$ vs $3.49{\pm}1.54$). Three different types of pain VAS (on average, at night and on motion) showed low to very low positive correlation with BSPQ scores when analyzed with Pearson's correlation coefficient. ROM levels in shoulder joint were not significantly related to BSPQ scores, though active external rotation range showed low positive correlation with BSPQ scores. Conclusions : Patients with frozen shoulder showed higher blood stasis pattern score in BSPQ but the level of blood stasis pattern is not significantly related to the severity of pain or limitation of ROM in shoulder joint.
Background: The treatment for acromioclavicular joint injuries (ACJI) ranges from a conservative approach to extensive surgical reconstruction, and the decision on how to manage these injuries depends on the grade of acromioclavicular (AC) joint separation, resources, and skill availability. After a thorough review of the literature, the researchers adopted a simple cost-effective technique of AC joint reconstruction for acute ACJI requiring surgery. Methods: This was a prospective single-center study conducted between April 2017 and April 2018. For patients with acute ACJI more than Rockwood grade 3, the researchers performed open coracoclavicular ligament reconstruction using synthetic sutures along with an Endobutton and a figure of 8 button plate. This was followed by AC ligament repair augmenting it with temporary percutaneous AC K-wires. Clinical outcomes were evaluated using the Constant Murley shoulder score. Results: Seventeen patients underwent surgery. The immediate postoperative radiograph showed an anatomical reduction of the AC joint dislocation in all patients. During follow-up, one patient developed subluxation but was asymptomatic. The mean follow-up period was 30 months (range, 24-35 months). The mean Constant score at 24 months was 95. No AC joint degeneration was noted in follow-up X-rays. The follow-up X-rays showed significant infra-clavicular calcification in 11 of the 17 patients, which was an evidence of a healed coracoclavicular ligament post-surgery. Conclusions: This study presents a simple cost-effective technique with a short learning curve for anatomic reconstruction of acute ACJI. The preliminary results have been very encouraging.

While shoulder hemiarthroplasty is still used to treat young patients with shoulder pathology, the use of this procedure has substantially declined in recent years due to its significant complication profile. Glenoid wear with arthrosis is one of the major postoperative complications following shoulder hemiarthroplasty, and efforts to prevent this complication led many scientists to explore alternative weight-bearing surfaces on arthroplasty implants to decrease joint wear and improve patient outcomes. Pyrolytic carbon, or pyrocarbon, is a material that has better biocompatibility, survivorship, strength, and wear resistance compared to the materials used in traditional shoulder hemiarthroplasty. Pyrocarbon implants have been used in orthopedics for over 50 years; recently, their utility in shoulder hemiarthroplasty has garnered much interest. The purpose behind the use of pyrocarbon in shoulder hemiarthroplasty is to decrease the risk of progressive glenoid wear, especially in young active patients in whom joint preservation is important. Promising survivorship and outcomes have been demonstrated by recent studies, including limited glenoid wear following pyrocarbon hemiarthroplasty. Nevertheless, these clinical studies have been limited to relatively small case series with limited long-term follow-up. Accordingly, additional research and comparative studies need to be conducted in order to properly assess the therapeutic efficacy and value of pyrocarbon hemiarthroplasty.
This study was to investigate the effects of joint mobilization with two different electrotherapy methods on shoulder external rotation range of motion and pain in mastectomy patients. Thirty mastectomy patients were divided into STMG (joint mobilization+pain scrambler therapy, n=15) and TENMG (joint mobilization+transcutaneous electrical nerve stimulation, n=15). The measurements were performed shoulder external rotation range of motion (ROM), pain (VAS). In both groups, there was a significant difference in the shoulder external rotation ROM, pain after intervention (p <.05). STMG was more decrease in VAS score than TENMG (p <.05). There was no significant difference in shoulder external rotation ROM between the two groups. In order to decrease the pain of mastectomy patients, it was confirmed that STMG was more effective than TENMG.. It is thought that a control group is added to confirm various differences, and more subjects are needed.
A single subject experimental design (alternating treatment design) was used to compare the effects of only ultrasound and ultrasound combined with stretching of the joint capsule on the ROM increase and pain reduction in patients with frozen shoulder. Two subjects were included in each group. In the only ultrasound treatment sessions, ultrasound was applied at the pain point of the shoulder joint in supine position. In the ultrasound combined with stretching treatment sessions, ultrasound was applied at the pain point of the shoulder joint positioned in external rotation and abduction in sitting position. Only ultrasound treatment and ultrasound combined with stretching treatment were alternately performed on each patient. Pain and disability was measured by shoulder pain and disability index (SPADI), and range of motion (ROM) was measured by scratch test. The results of this study showed that ultrasound combined with stretching treatment were more effective than only ultrasound treatment in ROM increase and pain reduction. However, disability score was not significantly different.
Background: To evaluate clinical and radiological outcome using AO hook locking plate in acute acromioclavicular joint injuries. Methods: This study was based on patients with Rockwood type 3 or 5 acromioclavicular joint injuries who received surgery with AO hook locking plate from June 2008 until June 2009. Among the 22 patients, 19 of them were male and 3 were female, the mean age was $44.4{\pm}15.57$ years (20-72 years) and follow-up period was $15.5{\pm}3.90$ months (12-23 months). Preoperatively, postoperatively, and at the final follow-up after the plate removal, both coracoclavicular distances were measured from the anteroposterior radiograph. Also, the Shoulder Rating Scale of the University of California at Los Angeles scores (UCLA scores), the American Shoulder and Elbow Surgeons scores (ASES scores), Constant scores, and the Korean Shoulder Society scores (KSS scores) were measured at the final followup to evaluate the function of the shoulder joint. Results: At the time of injury, the mean coracoclavicular distance of the injured side was $17.69{\pm}4.23mm$ (9.57-27.82 mm) and the unaffected side was $7.55{\pm}2.20mm$ (3.24-13.05 mm). The mean coracoclavicular distance measured postoperatively and at the final follow-up was $6.87{\pm}2.34mm$ (4.07-14.13 mm) and $8.47{\pm}2.96mm$ (4.37-17.48 mm), respectively. The mean UCLA, ASES, Constant, and KSS scores measured in the final follow-up were $33.5{\pm}1.30$ (31-35), $90.8{\pm}8.36$ (72-100), $78.6{\pm}8.80$ (62-100), and $94.4{\pm}5.08$ (84-100) each. Conclusions: From this short-term research, the surgical treatment using AO hook locking plates in acute acromioclavicular joint injuries is clinically and radiographically satisfying and considered as a useful treatment method.

Objectives The purpose of this study was to observe effectiveness of ultrasound-guided Soyeom pharmacopuncture therapy at acromioclavicular joint of shoulder in patients with anterior shoulder pain. Methods We analyzed medical records of 9 patients hospitalized with anterior shoulder pain who had admitted to Department of Korean Rehabilitation Medicine, College of Korean Medicine, Daejeon University from March 1, 2021 to June 13, 2021. 9 patients were treated the treatment of Soyeom pharmacopuncture therapy on acromioclavicular joint under ultrasonic guidance with Korean medicine treatment (such as herbal medicine, acupuncture, moxabustion, etc). The study was conducted as a retrospective observation study which analyze the patient's medical records. We used numeric rating scale (NRS) to evaluate pain reduction and the shoulder range of motion (ROM) to evaluate function improvement twice (before pharmacopuncture treatment and 5 days later treatment). Statistical analysis was performed using the IBM SPSS Statistics version 25 program (IBM Co., Armonk, NY, USA). Results The average of numeric rating scale reduced statistically significantly from 5.56±1.13 to 3.11±1.36 (p=0.004). The average of shoulder flexion ROM increased statistically significantly from 166.67±8.17° to 175.00±8.37° (p<0.05). And the average of shoulder abduction ROM increased statistically significantly from 158.57±20.35° to 172.86±12.53° (p<0.05). Both NRS and shouler ROM showed statistically significantly improve after treatment. Conclusions This study shows ultrasound-guided Soyeom pharmacopuncture therapy at acromioclavicular joint of shoulder has a meaningful clinical effect on the improving shoulder pain and ROM especially on flexion and abduction.
