Effects of a Trunk Stabilization Exercise with Gym Ball on Scapular Position for Patients with Arthroscopic Rotator Cuff Repair

  • Purpose: The present study was to investigate effects of scapular position and pain on a trunk stabilization exercise with gym ball for patients with arthroscopic rotator cuff repair. Methods: Subjects were patients after 2 weeks of rotator cuff repairs. Subjects were randomly assigned to conservative treatment group (CTG, n=10) or trunk stabilization with gym ball group (SBG, n=10), and participated one of those groups for 2 weeks. Measurements about pain and scapular position were assessed in before and after treatment programs. Results: Pain reduced in both rest and night in SBG compared to CTG. In the case of scapular position, decreased scapular retraction (increased protraction) was found in CTG and vice versa in SBG. Reduced scapular anterior tilting was also found on SBG. Conclusion: The present study showed that early performed trunk stabilization with gym ball could bring a scapular alignment that increases subacromial space and reduces pain. This suggests early trunk stabilization with gym ball programs to restore shoulder functions for patients with rotator cuff repair.

파워 숄더 재킷의 디자인 및 패턴전개 방법 (Pattern Making Method and Design of Power Shoulder Jackets)

  • 여성복에 있어서 09/10년 시즌에 가장 주목할 만한 특징은 파워 숄더 룩이다. 파워 숄더 룩은 단순히 패션니스타들에서만이 아니라 당당히 스트리트 패션의 하나로 자리 잡았으며 파워 숄더 룩의 50%이상을 차지하는 재킷이 여성복의 핵심 아이템의 하나로 부활하면서 재킷의 어깨 각도는 0도에서 30도 이상까지 다양하게 나타난다. 다양한 어깨 각도 중 10도, 15도, 20도의 형태가 70%이상을 차지하고 있으며 패턴의 형태는 몸판의 어깨선을 자연스럽게 올리고 확장한 어깨변형 패턴, 어깨선과 함께 소매산 부분을 절개한 소매산 변형 패턴, 몸판과 소매가 연결된 패턴이 주를 이루고 있다. 파워 숄더 패턴 전개는 어깨 각도가 높아질수록 각이 시작되는 점이 목옆점에 가깝게 하고 어깨끝점이 확장되어야 소매의 외관형태가 편안하고 피트성이 좋아진다. 어깨 변형의 경우 앞뒤 진동선의 시작점인 어깨점과 소매산의 정점 부분의 선이 완만하게 이어지도록 S라인형태를 유지하는 것이 가장 중요하며 소매산의 ease분량이 일반 재킷 소매보다 많아야 파워 숄더 실루엣이 자연스럽게 연출된다. 소매변형 패턴은 중심쪽 소매의 소매산 높이는 기본소매 소매산높이의 1.5cm를 외곽 소매분량으로 처리한 것을 고려하여 소매산 높이를 설정하고 어깨 각이 커질수록 외곽소매의 소매산 폭을 크게 함으로서 소매의 어깨점이 목옆점 방향으로 휘지 않고 각이 선 파워 숄더가 완성된다. 소매변형과 소매와 몸판연결 패턴의 경우 어깨점과 상완이 연결되는 부분에 충분한 여유가 필요하며 어깨각도도 다른 형태에 비해 상대적으로 낮아 보인다. 이와 같이 파워 숄더 재킷 패턴은 패턴 제작시 디자인적인 요소를 충분히 감안한 패턴 설계와 함께 소매의 달림선 위치, 어깨너비, 어개 높이 확장량, 소매통의 여유량 등을 고려하여 제작하는 것이 바람직하다.

자가 등 가동운동이 어깨 통증 및 기능에 미치는 효과 (The Effect of Self Thoracic Mobilization Exercise on Shoulder Pain and Function)

  • 본 연구는 자가등 가동운동(STME, Self Thoracic Mobilization Exercise)이 어깨 기능장애 환자에게 어깨통증과 장애정도, 어깨의 기능에 미치는 영향을 알아보고자 하였다. 어깨 기능장애 환자 32명을 대상으로 무작위로 STME와 대조군으로 할당하였으며, 시각 상사척도와 어깨기능장애, 어깨의 가동범위 평가를 중재 전-후 실시하였다. 16명의 STME 중재 그룹은 폼롤러를 활용하는 방법과 네발자세에서 이용하여 굽힘, 폄을 자가로 실시하였고, 16명의 대조군은 전기치료 패드를 이용한 플라세보 효과를 활용하였다. 두 집단은 20분 동안 주당 3회를 4주간 실시하였다. STME군이 대조군 보다 통증과 기능장애에서 모두 유의한 향상을 보였으며(p<0.05), 어깨의 가동범위는 STME을 이용한 그룹이 플라세보 그룹 보다 굽힘, 벌림, 가쪽 돌림에서 유의한 효과를 보였다(p<0.05), 하지만 폄과 안쪽돌림에는 유의한 효과를 보이지 못했다(p>0.05). 따라서 STME은 어깨통증과 기능장애 해결에 효과적이며, 향후 어깨 통증 환자에게 보다 쉬운 접근방법의 중재방법을 교육하고 효과적인 치료를 할 수 있을 것이다. 나아가 사회적 치료비용 또한 경감시킬 수 있을 것이다.

어깨뼈 후전사방향촬영에서 Tube 각도와 환자의 회전각도 관한 연구 (A Study of Tube Angle and Patient's Rotation Angle in Scapular Y View)

  • 어깨뼈 검사는 일상생활, 스포츠 활동 시에 어깨관절의 후측부나, 후외측부에 강한 외력이 가해서 발생하는 직접외상에 의한 경우와 상완부에 주로 외전, 신전, 외회전력이 가해져 어깨관절 탈구 등 병변의 유무를 관찰하는데 유용하게 사용되고 있다. 본 연구의 목적은 어깨관절 후전사방향촬영(shoulder PA oblique projection, scapula Y-view)의 촬영각도에 따른 영상의 왜곡을 방지하고 방사선학적 진단에 가장 유용한 촬영 각도를 알아보고자 하였다. 어깨뼈체부와 봉우리(acromion) 및 부리돌기(coracoid process)는 Y자를 이루게 되는데 $10^{\circ}$에서는 어깨뼈와 봉우리사이의 각도가 협착되어 나타나고, $20^{\circ}{\sim}25^{\circ}$에서는 Y자에 위팔뼈머리가 겹쳐 보이며, 어깨관절과의 위치도 적절해 보였다. $35^{\circ}$에서는 위팔뼈머리가 Y자에 겹쳐 보이지만, 위팔뼈머리와 어깨관절이 왜곡되어 보였다. 환자체위는 $45^{\circ}$ 거상시킬 때 가장 적합한 측면상을 관찰되며, 봉우리(acromion) 및 부리돌기(coracoid process)도 Y 자를 이루었다.

푸쉬업 플러스 운동과 키네시오 테이핑 적용이 견관절 근막동통증후군의 통증과 기능에 미치는 효과 (The Effect of Push-up Plus Excercise with Kinesio-Taping on the Pain and Function in Myofascial Pain Syndrome of Shoulder)

  • Background: The purpose of this study was conducted to investigate the effects of Push-up plus exercise with kinesio taping on the pain and shoulder motor function in patient with shoulder myofacial pain syndrome. Methods: The patients with myofacial pain syndrome were randomly divided 2 groups. kinesio taping group (n=20) was taken physical therapy program and kinesio taping. Push-up plus exercise with taping group (n=22) was taken physical therapy and kinesio taping with Push-up plus exercise. The kinesio taping applied on upper trapezius and levator scapulae. The Push-up plus exercise performed in standing position and qudripad position during 10 seconds on 15 time, 3 set per each positions. We mearsured the pain degree using PPT, VAS, MPSSI and shoulder motor function using CSA before and after experiment. The significant test of PPT, VAS, MPSSI, CSA according to applying the kinesio taping and Push-up plus exercise between groups used ANCOVA. Results: In the result following analysis, there was significance on PPT (F=7.378, p=.016), VAS (F=13.071, p=.031), CSA (F=5.302, p=.026) between kinesio taping group and Push-up plus exercise with tapping group. Then, Push-up plus exercise with kinesio taping group has significance on the PPT, VAS, CSA in patients with myofacial pain syndrome. Conclusions: It may suggest that kinesio taping combined with Push-up plus exercise will be helpful of the pain and shoulder function improvement in the patients with myofacial pain syndrome.

관절과 상완 관절의 회전운동이 견갑흉곽 운동에 미치는 영향 (The Influence of the Glenohumeral Rotation on the Scapulothoracic Motion)

  • This study was performed to evaluate the influences of the passive glenohumeral rotation on the scapulothoracic motion. We took anteroposterior radiograms of the right shoulders including the thoracic vertebrae with supine position in 10 normal male adults, at 0 degree abduction, 45 degrees abduction and 90 degrees abduction in scapular plane and in neutral rotation, maximal internal rotation and maximal external rotation in each abduction view. The scapulothoracic motion was measured as the distances between the vertical line drawn from the spinous process of the 7th cervical vertebra and the inferior and superior angles of the right scapula respectively. At 0 degree abduction, the distances were not changed in internal rotation relative to neutral rotation, but decreased significantly in external rotation, that is, the scapula shifts medially on external rotation. At 45 degrees abduction, the distances were increased significantly only in internal rotation, that is, the scapula shifts laterally on internal rotation. At 90 degrees abduction, the scapula rotated laterally on internal rotation and medially on external rotation. In conclusion, when a physician examines the rotation of the shoulder joint, he cannot exclude the scapulothoracic motion just by examining the patient with supine position. And we concluded that the rotatory movement of the shoulder is not solely contributed to the glenohumeral motion.

키네시오 테이핑 요법과 상지 협응 운동이 견관절 근막동통증후군의 통증과 기능에 미치는 효과 (Effects of Kinesio Taping and Upper Coordination Exercises on Pain and Function of Shoulder Myofascial Pain Syndrome)

  • Purpose: The purpose of this study was conducted to investigate the effects of kinesio taping and coordination exercise on the myofascial pain syndrome and shoulder function. Methods: The patients with myofascial pain syndrome were participated in this study and divided randomly 2 groups. Control group (n=22) was taken only physical therapy program. Experimental group (n=22) was taken physical therapy with Kinesio taping and coordination exercise. The Kinesio taping in experimental group applied on levator scapulae and supraspinatus. The coordination exercise performed in supine position and sitting position on 15 times during 10 seconds each positions. We measured the pain degree using visual analog scale (VAS), pain rating score (PRS), pressure pain threshold (PPT), myofascial pain subjects symptoms index and shoulder motor function using constant shoulder assessment scale; CSA before and after experiment. Results: The significant test of CSA, myofascial pain subjects symptoms index, VAS, PRS according to applying the Kinesio taping and coordination exercise between groups used ANCOVA. In the result following analysis, there was significance on VAS (F=13.071, p=.031), PRS (F=12.130, p=.014), PPT (F=7.378, p=.016), CSA (F=5.302, p=.026) between control group and experimental group. Conclusion: Then, Kinesio taping and coordination exercise has benefit on the VAS, PRS, PPT, CSA in patients with myofascial pain syndrome. So, it may suggest that Kinesio taping combined with coordination exercise will be helpful of the pain and shoulder function improvement the patients with myofascial pain syndrome.

아래등세모근 강화운동 시 어깨관절의 벌림 각도에 따라 둥근 어깨에 미치는 영향 (Effects of Shoulder Abduction Angles on Rounded Shoulders During Lower Trapezius Strengthening Exercise)

  • Purpose : The purpose of this study is to find out which shoulder abduction angle among the three representative angles of lower trapezius strengthening exercises most effectively improves rounded shoulder posture. Methods : Thirty-one people with rounded shoulders, the subjects of this study, were selected. With the use of a random number table, group A was randomly assigned to a Y-type exercise, group B to a T-type exercise, and group C to an MPC exercise. In the prone position, both arms are abducted at the angles suggested for each group (145 °, 90 °, and 45 °, respectively), and the hands are holding dumbbells to assume a functional posture. With the start signal, the thumb raised the arm to the sky and the arm is raised to the level of the ear for 10 seconds and then returned to the original position. Each week, the strength required was increased by changing the resistance weight, number of times, and set, and the exercise was performed three times a week for a total of four weeks. Results : There was a significant difference in the change in shoulder height of each group after four weeks of exercise in comparison to the amount of change in height before and after exercise in all groups (p<.05). However, no significant difference was observed between all groups in the change in muscle activity of the lower trapezius muscle in each group following four weeks of exercise (p>.05). Conclusion : The results of this study showed that the three lower trapezius strengthening exercises have a positive effect on the improvement of rounded shoulders. In consideration of these results, it is therefore proposed that the various lower trapezius strengthening exercises be applied in parallel during physical therapy for patients with rounded shoulders in clinical practice.

Measurement of Uncertainty Using Standardized Protocol of Hand Grip Strength Measurement in Patients with Sarcopenia

  • Background: The aim of this study was to determine the accuracy and error range of hand grip strength measurement using various methods. Methods: Methods used for measurement of hand grip strength in 34 epidemiologic studies on sarcopenia were analyzed. Maximum grip strength was measured in a sitting position with the elbow flexed at 90 degrees, the shoulder in 0 degrees flexion, and the wrist in neutral position (0 degrees). Maximum grip strength in standing position was measured with the shoulder in 180 degrees flexion, the elbow fully extended, and the wrist in neutral position (0 degrees). Three measurements were taken on each side at 30 sec intervals. The uncertainty of measurement was calculated. Results: The combined uncertainty in sitting position on the right and left sides was 1.14% and 0.38%, respectively, and the combined uncertainty in standing position on the right and left sides was 0.35 and 1.20, respectively. The expanded uncertainty in sitting position on the right and left sides was 2.28 and 0.79, respectively, and the expanded uncertainty in standing position on the right and left sides was 0.71 and 2.41, respectively (k=2). Conclusions: Uncertainty of hand grip strength measurement was identified in this study, and a significant difference was observed between measurement. For more precise diagnosis of sarcopenia, dynamometers need to be corrected to overcome uncertainty.

Comparison of the Muscle Activities of Upper Trapezius and Middle Deltoid between Subjects with and without Elevation of Shoulder Girdle during Arm Elevation

  • Purpose: Muscle imbalance between upper trapezius (UT) and serratus anterior (SA) during arm elevation is a factor causing shoulder dysfunction. However, there is no study to compare the muscle activities of the UT and middle deltoid (MD). The purpose of this study was to compare the muscle activities of the UT and MD between with and without elevation of shoulder girdle (ESG) during shoulder abduction. Methods: The subjects without (control group=9) or with (ESG group=8) participated in this study. The muscle activities of the UT and MD were measured using a electromyography during $90^{\circ}$ shoulder abducted position in both group. The data in middle of 3-second of the 5-second periods were used. The mean value of three trials was used in the data analysis. For each muscle, independent t-tests were performed to compare for group differences. Results: The muscle activity of UT was significantly greater in ESG group, compared to that of the control group (p<0.05). The muscle activity of MD was significantly smaller in ESG group, compared to that of the control group (p<0.05). Conclusion: These findings showed that low muscle activation of MD as well as SA may contribute to hyperactivity of UT during arm elevation.
