The Influence that the Closed Chain Breathing Exercise Affects on Thorax Circumference Increase and ROM of Shoulder Rotation on the Cervical Vertebral Cord Injury Patients (닫힌 사슬 호흡 운동법이 상위 경수 손상 환자의 흉곽둘레길이와 어깨관절의 회전가동범위에 미치는 영향)

  • Purpose : The purpose of this study was to investigate the effects of closed chain breathing exercise on thorax circumference increase and ROM of shoulder rotation for cervical vertebral cord injury patient's. Methods : 12 cervical vertebral cord injury patients volunteer to participate in this study. closed chain breathing exercise group of 4 subjects performed exercise three time a week. The deep breathing exercise group and general exercise group of 4 subjects performed exercise three time a week. We analyaed the descriptive statistics and $3{\times}2$ repeated measures ANOVA by SPSS 12.0 for window. Results : In comparison of rotation of shoulder ROM between pre and post value, the increase of rotation ROM of shoulder was significant in the closed chain breathing exercise group(p<.05). Conclusion : The closed chain breathing exercise helped to increased rotation ROM of shoulder.

The Influence of Treatment in Patients with Shoulder Adhesive Capsulitis for the Proprioceptive Neuromuscular Facilitation and Self-Assistive ROM Exercise (고유수용성 신경근 촉진법과 자가 보조적 관절 가동운동이 견관절 유착성 관절낭염 치료에 미치는 영향)

  • PURPOSE: The purpose of the study was to investigate a influence in Proprioceptive Neuromuscular Facilitation, and self-assisted ROM exercise of shoulder adhesive capsulitis. METHODS: The methods of the study was to investigate a change in range of motion (ROM), an increase in ROM, visual analogue scale (VAS) and disability questionnaire by Proprioceptive Neuromuscular Facilitation, and self-assisted ROM exercise on 32 patients who were diagnosed with shoulder adhesive capsulitis. RESULTS: There was no significant difference between the two groups who were treated in the Proprioceptive Neuromuscular Facilitation and self-assistive ROM exercise group for flexion, extension, internal rotation, external rotation, VAS and disability questionnaire. Both group's flexion, extension, internal ratation, and external rotation levels were significantly different before and after the treatment. And significant statistical decrease in VAS and disability was seen. The extension of the shoulder joint was closely related to external rotation (r=0.84). There was a close relationship between internal rotation and external rotation at the shoulder joint. There was no increased range of extension through the exercise method. However, flexion, extension, internal rotation, external rotation, VAS, and disability questionnaire of Proprioceptive Neuromuscular Facilitation groups were obviously higher than in the self-assisted ROM exercise group. CONCLUSION: Our study suggest that considering Proprioceptive Neuromuscular Facilitation for the patient who has shoulder adhesive capsulitis in clinic.

Effects on Frozen Shoulder as Seen in the Comparison Hold-Relax Technique and ROM Exercise (동결견 환자에 대한 정지-이완기법과 관절가동범위 운동의 비교)

  • The purpose of this study is to evaluate pain and range of motion in the frozen shoulder and to compare Hold-Relax technique with ROM exercise. Twenty-eight frozen shoulder patients between 41 nod 76 years of age were selected and were divided equally by random distribution into a Hold-Relax technique group and a ROM exercise group. To analyze the therapeutic effect between the two groups, two measurements were taken. A measurement of pain was taken by visual analogue scale and a measurement of range of motion was taken by measuring flexion and abduction degrees with a goniometer. The results were as followings : 1. The Hold-Relax group is more effective in decreasing the pain than the ROM exercise group. 2. The Hold-Relax group is more effective in increasing the range of flexion at the shoulder than the ROM exercise group. 3. The Hold-Relax group is more effective in increasing the range of abduction at the shoulder than the ROM exercise group.

Effects of Mobilization with Movement Combined with Exercise(EMWM) on ADH, ROM and Functional Performance in Patients with Impingement Syndrome of the Shoulder (운동과 MWM을 혼합한 중재(EMWM)가 어깨관절 충돌증후군 환자의 AHD, ROM, 기능수행정도에 미치는 효과)

  • Purpose : This study was to identify the effects of Mobilization with Movement combined with exercise (EMWM) on acromio-humeral distance (AHD), range of motion (ROM), pain intensity, and functional performance in patients with impingement syndrome of the shoulder. Methods : The subjects were 40 patients diagnosed with impingement syndrome of the shoulder. Twenty subjects are randomly assigned to each 2 different groups; Group 1. (exercise group), Group 2. (EMWM group). Three weekly interventions were given for 4 weeks. The main outcome measures were ultrasound, goniometer, visual analogue scale (VAS), and Korean Constant shoulder score (K-CSS). The ultrasound (AHD), ROM (flexion, abduction), pain intensity (for shoulder flexion) and functional performance (K-CSS) were compared between the groups. Results : The AHD was significantly increased in Group 2 compared to Groups 1. No significant difference was observed between the groups in the range of abduction of the shoulder, but the range of flexion was significantly increased in Group 2 compared to Groups 1. Pain intensity was significantly lower in Group 2 than in Group 1, and functional performance was significantly increased in Group 2 compared to Group 1. Conclusion : An intervention that combined mobilization with movement with exercise was more effective than exercise alone for rapid recovery from shoulder injury and improvement in functional performance.

Effect of Self Resistance Exercise in Women with Frozen Shoulder (동결견을 가진 여성에 대한 탄력밴드 저항운동의 영향)

  • Background : This study was conducted to know the effect of resistance exercise using thera-band in parallel with conservative therapy in patients with frozen shoulder. Methods : We analyze subjective pain scale, ROM and shoulder functional scale 24 patients diagnosed with adhesive capsulitis were included for study. Among them, 12 patients(experiment group) were managed by resistance exercise as well as conservative physical therapy, another 12 patients(control group) were only treated by conservative therapy alone. All the patients were treated three times a week for 4 weeks, and after, and after each treatment subjective pain scale, ROM, shoulder functional score(SFI) were measured. Results : The VAS were significant reduced between experimental group and control group according to pre-treatment and post-treatment. In experimental group, the ROM and SFI has increased than control group according to pre-treatment and post-treatment. Conclusion : According to the results, we concluded that both conservative therapy and self resistance exercise using thera-band technique are more effective for increasing ROM especially flexion, abduction, internal rotation and SFI than conservative therapy alone.

  • Objectives : The purpose of this report is to prove the effect of The Daoyin exercise program combined with Korean medicine treatment on supraspinatus tendon tear. Methods : Two patients, diagnosed with Supraspinatus tendon tear by shoulder MRI, were treated with The Daoyin exercise program and Korean medicine treatment. VAS(Visual analog scale), ROM(Range of motion) and Physical examination were used to evaluate patients. Results of application : The Daoyin exercise program combined with Korean medicine treatment reduced shoulder pain and improved shoulder ROM of the two patients significantly. Conclusions : The Daoyin exercise program combined with Korean medicine treatment is proved to be effective treatment for reducing pain and improving shoulder ROM of supraspinatus tendon tear patients.

  • Purpose: The study aims to determine the effect of a breathing exercise on shoulder pain, range of motion (ROM), and forward head posture in patients with rotator cuff repair. Methods: In total, 25 patients with rotator cuff repair were included in this study. The experimental group (n = 13) underwent a breathing exercise, while the control group (n = 12) received traditional physical therapy. The visual analogue scales (VASs) for pain, flexion and abduction ROM, and the craniovertebral angle (CVA) of both groups were recorded at both pre- and post-intervention. Paired t-tests were used to determine significant changes in the post-intervention compared with the pre-intervention period, and independent t-tests were used to analyze differences in dependent variables between the two groups. Results: After the two-week intervention, the experimental group experienced a significantly decreased VAS (p < 0.05) and significantly increased ROM and CVA (p < 0.05), while the control group experienced a significantly decreased VAS (p < 0.05). Further, the experimental group that underwent the breathing exercise showed greater improvements in flexion and abduction ROM and in the CVA than the control group (p < 0.05). Conclusion: The results suggested that a breathing exercise can reduce shoulder pain and enhance ROM and posture in patients with rotator cuff repair.

Development of Shoulder Wheel with Adjustable Rotating Speed and Rotating Angle and Application Contents

  • The purpose of this study to improve the shoulder wheel that is used frequently during shoulder circumduction exercise in a park, to link the shoulder wheel to a game application comprising various shoulder circumduction exercise and shoulder ROM exercise, and to enjoy the game during the exercises. A step motor was added to the shoulder wheel, and the rotating speed and rotating angle, and rotating direction can be controlled. The shoulder height, different for different people, can be controlled as well, so that any user can use this device. The developed application content includes the game elements, to enjoy the repetitive and boring exercise with fun and interest, and the control to adjust the degree of difficulty. This was to help users heighten their desire to rehabilitate by inducing active participation during repetitive shoulder rehabilitation exercise because the game vests users with intrinsic motivation and immersion. If the shoulder wheel and application contents proposed in this study are used, shoulder diseases can be prevented in advance by performing muscle strengthening exercises required in an aging society. This study is expected to contribute to the field of research related to serious games and rehabilitation.

Effect of the Proprioceptive Neuromuscular Facilitation Pattern Exercise and Scrambler Treatment on Pain, Range of Motion, and Shoulder Dysfunction in Breast Cancer Patients (고유수용성신경근촉진법 패턴 운동과 스크램블러 치료가 유방암 환자의 통증과 가동범위 및 어깨기능장애에 미치는 영향)

  • Purpose: The study aims to examine the effects of the proprioceptive neuromuscular facilitation pattern exercise and scrambler therapy on pain, range of motion (ROM), and shoulder dysfunction in breast cancer patients. Methods: In total, 30 breast cancer patients were recruited and randomized to group I (n = 10), group II (n = 10), and group III (n = 10). Pain was measured using a visual analogue scale, ROM was measured using a goniometer, and shoulder dysfunction was measured using a shoulder pain and disability index. Group I practiced the proprioceptive neuromuscular facilitation pattern exercise and underwent scrambler therapy, group II underwent scrambler therapy only, and group III practiced the proprioceptive neuromuscular facilitation pattern exercise only. Results: A within-groups comparison showed that all groups demonstrated significant differences in pain, ROM, and shoulder dysfunction after the experiment (p<0.05). Further, according to a comparison of the three groups, group I showed a more significant difference in effectiveness than groups II and III in terms of pain, ROM, and shoulder dysfunction before and after the experiment (p<0.05). Conclusion: This study showed that the proprioceptive neuromuscular facilitation pattern exercise and scrambler therapy are effective in treating pain, ROM issues, and shoulder dysfunction in breast cancer patients.

The Effects of Spencer Technique on the ROM, Pain, Function in Patients with Shoulder Adhesive Capsulitis (스펜서 테크닉이 유착성 관절낭염 환자의 어깨관절 가동범위와 통증, 기능에 미치는 영향)

  • Background: The Purpose of this study was to evaluate the value of Spencer technique on the range of motion (ROM), Pain, function in patients with shoulder adhesive capsulitis. Methods: subjects consisted of 30patients who were diagnosed shoulder adhesive capsulitis. All subjects are randomly assigned to 2groups: Spencer technique (ST) group (n=15), self assistive ROM exercise(S-A ROM E) group (n=15). The subjects performed an intervention program 30 minuets per day and was repeated 3 times a week for 4 weeks a total of 12 times. ROM of flexion, abduction, external rotation, internal rotation were measured using a goniometer. The visual analog scale (VAS), Shoulder pain and disability index (SPADI) were used to measure pain, functional ability. Results: In the intergroup comparisons after the intervention, ROM of flexion, abduction, internal rotation, VAS, SPADI were significantly different(p<.05). Spencer technique was more effective for improving ROM, pain, functional ability than self assistive ROM exercise. Conclusions: Our study suggest that considering Spencer technique for the patient with shoulder adhesive capsulitis. Further studies on Spencer technique are needed in the future.
