여대생의 지각된 건강상태와 건강증진행위에 대한 연구 (The Relation between Perceived Health Status and Health-Promoting Behaviors in Female College Students)

  • Purpose: The purpose of this study was to investigate the relation between perceived health status and health-promoting behaviors in female college students. Method: Data were collected from December 10 to 31. 2007. In 160 female college students, perceived health status was measured using the Medical Outcomes Short-Form Health Survey (SF-36), and health-promoting behaviors using the Health Promoting Lifestyle Profile (HPLP). Results: The major findings of this study were as follows: 1) The mean scores of the level of health promoting behaviors, physical health status, and mental health status were 2.44 (SD=0.39), 51.9 (SD=8.03), and 42.56 (SD=10.77), respectively. 2) Health-promoting behaviors were correlate with physical health status (r=-.361) and mental health status (r=.498). 3) Health-promoting behaviors were significantly associated with allowance and mental health in capability, which explained 28.9% of variance in health-promoting behaviors. Conclusion: The results suggest that it is necessary to strengthen female college students' practice of exercise and responsibility for health in order to improve their health promotion behaviors. Also, the results of this study give useful information for designing interventions and program development for female college students' appropriate health promoting life.

Is Caffeine Intake Associated With Urinary Incontinence in Japanese Adults?

  • Objectives: To investigate whether caffeine intake is associated with urinary incontinence (UI) among Japanese adults. Methods: A total of 683 men and 298 women aged 40 to 75 years were recruited from the community in middle and southern Japan. A validated food frequency questionnaire was administered face-to-face to obtain information on dietary intake and habitual beverage consumption. Urinary incontinence status was ascertained using the International Consultation on Incontinence Questionnaire-Short Form. Results: Mean daily caffeine intake was found to be similar between incontinent subjects (men 120 mg, women 94 mg) and others without the condition (men 106 mg, women 103 mg), $p$=0.33 for men and $p$=0.44 for women. The slight increases in risk of UI at the highest level of caffeine intake were not significant after adjusting for confounding factors. The adjusted odds ratios (95% confidence interval) were 1.36 (0.65 to 2.88) and 1.12 (0.57 to 2.22) for men and women, respectively. Conclusions: No association was evident between caffeine intake and UI in middle-aged and older Japanese adults. Further studies are required to confirm the effect of caffeine in the prevention of UI.

  • Purpose: The purpose of this study was to investigate whether subjective health and social support influence depression directly or indirectly through self-control of the physically disabled. Method: A five-item General Health Short Form (SF-36) Health Survey Questionnaire was used to measure subjective health. Social support and self-control were measured by Social Support Questionnaire 6 and Mastery Scale. The level of depression was measured by CES-D (Center for Epidemiologic Studies Depression). Result: The level of depression was high (M=22.23). Seventy percent of the subjects were depressed. Subjective health and satisfaction with social support influenced self-control. Subjective health influenced the level of depression directly and indirectly, but self-control influenced the level of depression only directly. Conclusion: Subjective health and satisfaction with social support influenced depression through self-control in physically disabled persons. Therefore, when we manage these subjects' depression, it is seemed that program which lowers the level of depression by assessing and correcting these factors should be prepared.

장기이식 대기자와 수혜자의 불안, 우울 및 삶의 질 비교 (Comparison of Anxiety, Depression, and Quality of Life between Organ Transplant Candidates and Recipients)

  • Purpose: The aim of this study was to compare the levels of anxiety, depression, and quality of life (QOL) among kidney or liver transplant candidates and recipients. Methods: A cross-sectional descriptive design was utilized. The 160 subjects were recruited and assigned to three different conditions: awaiting transplant group; post-transplantation (TPL) group within less than one year of surgery; and post-TPL group with one year and no more than two years post surgery. The levels of anxiety and depression were measured by the Hospital Anxiety and Depression Scale. The level of quality of life was measured by the Medical Outcomes Study Short Form-36 version 2. Results: The anxiety score was significantly higher in the waiting group compared with the 1~2 years post TPL group. The depression score was significantly higher in the waiting group compared with the post TPL groups. The QOL was significantly lower in the waiting group compared with the post TPL groups. Conclusion: Our results show a high prevalence of anxiety and depression and a marked reduction in the QOL in transplant candidates compared to the recipients. This study indicates that the group waiting for transplants may have unmet needs during that time period. Health professionals should be encouraged to attend to the psychological distress of the transplant candidates in order to improve their quality of life.

Functional Reconstruction of a Combined Tendocutaneous Defect of the Achilles Using a Segmental Rectus Femoris Myofascial Construct: A Viable Alternative

  • The composite anterolateral thigh flap with vascularized fascia lata has emerged as a workhorse at our institution for complex Achilles defects requiring both tendon and soft tissue reconstruction. Safe elevation of this flap, however, is occasionally challenged by absent or inadequate perforators supplying the anterolateral thigh. When discovered intraoperatively, alternative options derived from the same vascular network can be pursued. We present the case of a 74-year-old male who underwent composite Achilles defect reconstruction using a segmental rectus femoris myofascial free flap. Following graduated rehabilitation, postoperatively, the patient resumed full activity and was able to ambulate on his tip-toes. At 1-year follow-up, active total range of motion of the reconstructed ankle exceeded 85% of the unaffected side, and donor site morbidity was negligible. American Orthopaedic Foot and Ankle Society and Short Form-36 scores improved by 78.8% and 28.8%, respectively, compared to preoperative baseline assessments. Based on our findings, we advocate for use of the combined rectus femoris myofascial free flap as a rescue option for reconstructing composite Achilles tendon/posterior leg defects in the setting of inadequate anterolateral thigh perforators. To our knowledge, this is the first report to describe use of this flap for such an indication.

폐암 환자의 기능상태에 관한 연구 (Functional Status in Lung Cancer Patients)

  • Purpose: To exam how functional status varied according to age, gender, stage, treatment protocols, and pathologic types of cancer in lung cancer patients. Method: A Cross-sectional and descriptive study was used. Functional status was measured with the Medical Outcome Study Short Form-36. A total of 106 lung cancer patients participated. Their mean age was 61 years. Majorities were male, in advanced stages, and receiving chemotherapy. Result: Functional status of the subjects were relatively low compared to the results of previous studies. Women had more severe functional limitations in Role-Emotion(t=2.17, p <.05). Generally, older patients(> 60 yrs.) had relatively more severe limitations in all subcategories, but the difference was not statistically significant. Subjects in late lung cancer stage(stage III & IV) had more severe functional limitation in all subcategories. But the statistical difference was found only in General Health(t=2.10, p<.05). In terms of treatment protocol, no-current treatment group had lower General Health than those of the chemotherapy group(F=3.42, p<.01). There were no statistical differences in functional status among pathological cancer cell types. Conclusion: The results of this study suggest that effective management may be achieved when these factors are considered on individual basis in the clinical management of lung cancer patients.

Factors Associated with Health-Related Quality of Life in Tuberculosis Patients Referred to the National Research Institute of Tuberculosis and Lung Disease in Tehran

  • Background: In tuberculosis (TB) patients, health-related quality of life (HRQoL) is significant in self-management, which in turn can be effective in therapeutic acceptance and prevention of treatment failure due to multi-drug resistant TB. This study was conducted to evaluate HRQoL and associated factors in TB patients referred to the National Research Institute of Tuberculosis and Lung Disease (NRITLD). Methods: In this study, patients were selected from TB clinics of the NRITLD in Tehran. In addition to an Iranian version of the Short-Form Health Survey (SF-36), demographic and disease characteristic questionnaires were used for data collection. The data were then analyzed using SPSS software. Results: Two hundred five TB patients, with the average age of $42.33{\pm}17.64$ years, participated in this study. The HRQoL scores in different domains ranged from $14.68{\pm}11.60$ for role limitations due to emotional problems to $46.99{\pm}13.25$ for general health perceptions. The variables of sex, marital status, education, job status, place of residence, and cigarette smoking, influenced the HRQoL scores in different dimensions. Conclusion: According to the study findings are the important variables that influenced the HRQoL of TB patients. The consideration of its can improve the HRQoL of TB patients.

Effects of Virtual Reality Exercise Program on Balance, Emotion and Quality of Life in Patients with Cognitive Decline

  • Purpose: In this study, we investigated the effectiveness of a 12-week virtual reality exercise program using the Nintendo Wii console (Wii) in improving balance, emotion, and quality of life among patients with cognitive decline. Methods: The study included 30 patients with cognitive decline (12 female, 18 male) who were randomly assigned to an experimental (n=15) and control groups (n=15). All subjects performed a traditional cognitive rehabilitation program and the experimental group performed additional three 40-minute virtual reality based video game (Wii) sessions per week for 12 weeks. The berg balance scale (BBS) was used to assess balance abilities. The short form geriatric depression scale-Korean (GDS-K) and the Korean version of quality of life-Alzheimer's disease (KQOL-AD) scale were both used to assess life quality in patients. Statistical significance was tested within and between groups before and after treatment, using Wilcoxon signed rank and Mann-Whitney u-tests. Results: After 36 training sessions, there were significant beneficial effects of the virtual reality game exercise on balance (BBS), GDS-K, and KQOL-AD in the experimental group when compared to the control group. No significant difference was observed within the control group. Conclusion: These findings demonstrate that a virtual reality-training program could improve the outcomes in terms of balance, depression, and quality of life in patients with cognitive decline. Long-term follow-ups and further studies of more efficient virtual reality training programs are needed.

Mediating Effect of Depression in the Relationship Between Health-Related Quality of Life and Pain of the Elderly with Physical Dysfunction

  • This study aimed to determine the effect of depression between the health-related quality of life and pain in elderly persons with physical disabilities. A total of 111 patients who were treated at hospital B (Seoul, Daejeon, Daegu, Gwangju, and Busan) participated in the Survey. The SF-36 Health Assessment was used to determine the quality of life of subjects, the Korean version of the Geriatric Depression Scale-Short form to assess the level of depression, and Numeric Rating Scale to determine the level of pain. To determine the moderating effect of depression on the relationship between quality of life and pain, simple regression analysis, and the Sobel test were performed. There was a significant negative correlation between health-related quality of life and pain (r<-.3, p<.05), and a significant positive correlation between depression and pain (r=.251, p<.05). Thus, health-related quality of life had a simple regression relationship with depression and pain. Depression also showed a mediating effect between health-related quality of life and pain. The results of this study suggest that depression mediates between pain and quality of life.

Effect of Lower Trapezius Strengthening Exercise Program on Unstable Support Surface for Neck Pain Patients with Forward Head Posture: A Randomized Controlled Trial

  • Objective: The purpose of this study was to investigate the effect of a lower trapezius strengthening exercise program on an unstable support surface on pain, neck dysfunction, psychosocial factors, and postural alignment in neck pain patients with forward head posture. Design: A randomized controlled trial Methods: A total of 36 neck pain patients participated in this study. Screening tests were performed and assigned to experimental group (n=18) and control group (n=18) using randomization program. Both groups performed the lower trapezius strengthening exercise program. In addition, in the experimental group, an exercise program for lower trapezius muscle strengthening was performed on an unstable support surface. All interventions were performed 3 times a week, for a total of 5 weeks. quadruple visual analogue scale (QVAS), neck disability index (NDI), short form (SF)-12, and postural alignment were measured before and after the intervention to compare their effectiveness. Results: Both groups showed significant differences in QVAS, NDI, SF-12, and postural alignment before and after intervention (p<0.05). In addition, the experimental group showed significant differences in NDI and postural alignment compared to the control group (p<0.05). Conclusions: The lower trapezius strengthening exercise program on unstable support surfaces is an effective intervention method with clinical significance in improving neck disability and postural alignment in patients with neck pain with forward head posture.
