Discharge Assessment Tool in Stroke and Elderly Patients: A Systematic Review (뇌졸중 및 노인 환자의 퇴원 평가도구에 대한 체계적 고찰)

  • Objective : The aim of this systematic review is to analyze the assessment items and tools used for the discharge of stroke and elderly patients and to help select appropriate assessment tools for discharge planning. Methods : The studies included in this review were collected from the PubMed, Medline Complete (EBSCOhost), and Scopus databases from January 2009 to December 2018. A total 22 studies were selected. The research types, the areas and sub-areas of assessment, as well as the assessment tools according to the areas were analyzed. Results : Descriptive research (59.2%) was the most frequent type of research. The 12 main areas of assessment were medical condition, activities of daily living, swallowing function, sensation, mobility, cognition and perception, communication, emotion, home environment, patient knowledge and readiness for discharge, social support, and well-being. Among the assessment tools, the most frequently used tools were Bathel Index; Functional Independence Measure, to assess activities of daily living; Short-Form Health Survey, to assess well-being, and Timed Up and Go test, to assess mobility. Conclusion : This study will help select the assessment areas and tools to be considered at discharge of stroke patients and serve a basis for the development of comprehensive assessment tools for discharge planning.

Effects of Computerized Cognitive Training Program Using Artificial Intelligence Motion Capture on Cognitive Function, Depression, and Quality of Life in Older Adults With Mild Cognitive Impairment During COVID-19: Pilot Study (인공지능 동작 인식을 활용한 전산화인지훈련이 코로나-19 기간 동안 경도 인지장애 고령자의 인지 기능, 우울, 삶의 질에 미치는 영향: 예비 연구)

  • Objective : We investigated the efficacy of an artificial intelligence computerized cognitive training program using motion capture to identify changes in cognition, depression, and quality of life in older adults with mild cognitive impairment. Methods : A total of seven older adults (experimental group = 4, control group = 3) participated in this study. During the COVID-19 period from October to December 2021, we used a program, "MOOVE Brain", that we had developed. The experimental group performed the program 30 minutes 3×/week for 1 month. We analyzed patients scores from the Korean version of the Mini-Mental State Examination-2, the Consortium to Establish a Registry for Alzheimer's Disease Assessment Packet for Daily Life Evaluation, the short form Geriatric Depression Scale, and Geriatric Quality of Life Scale. Results : We observed positive changes in the mean scores of the Stroop Color Test (attention), Stroop Color/Word Test (executive function), SGDS-K (depression), and GQOL (QoL). However, these changes did not reach statistical significance for each variable. Conclusion : The study results from "MOOVE Brain" can help address cognitive and psychosocial issues in isolated patients with MCI during the COVID-19 pandemic or those unable to access in-person medical services.

Case Series of 5 Failed Back Surgery Syndrome Patients Who were Treated by Direct Moxibustion Therapy (직접구 치료에 의해 호전된 척추수술후통증증후군 환자 5례)

  • Objectives : To evaluate the effectiveness of direct moxibustion in patients with failed back surgery syndrome (FBSS). Methods : 5 patients (3 females and 2 males) with FBSS underwent direct moxibustion on ouch points (阿是穴) for 10 to 15 weeks (once a week). The effectiveness of the treatment was evaluated with NRS (numerical rating scale), ODI (Oswestry disability index), BDI (Beck depression inventory) score, SF-36 (short form 36 health survey) score and PSQI (Pittsburgh sleep quality index) before and after the treatment. Results : Within observation period, average value changed from 10.0 to 4.2 in NRS of pain, from 37.0 to 20.6 in ODI, from 20.6 to 12.0 in BDI score, 42.2 to 62.6 in SF-36 score, and from 8.6 to 5.2 in PSQI. Conclusions : Direct moxibustion on FBSS patients showed more or equivalent effectiveness when compared to conventional FBSS treatment modalities.

  • The production of an extracellular polysaccharide, EPS-R, from the marine bacterium Hahella chejuensis was investigated at various aeration rates in a batch culture. Higher aeration rate resulted in enhanced EPS production and increased the viscosity of the culture broth. At an aeration rate of 1.5 vvm, EPS-R (12.2 g/L) was obtained with a yield (Y$\_$p/s) of 0.6 from the STN medium after 72 h of cultivation. The H. chejuensis cells changed rod morphology to a short-rod form in the stationary growth phase.

  • Traditional syntax-oriented serializability notions are considered to be not enough to handle in particular various types of transaction in terms of duration of execution. To deal with this situation, altruistic locking has attempted to reduce delay effect associated with lock release moment by use of the idea of donation. An improved form of altruism has also been deployed in extended altruistic locking in a way that scope of data to be early released is enlarged to include even data initially not intended to be donated. In this paper, we first of all investigated limitations inherent in both altruistic schemes from the perspective of alleviating starvation occasions for transactions in particular of short-lived nature. The idea of two-way donation locking(2DL) has then been experimented to see the effect of more than single donation in distributed database systems. Simulation experiments shows that 2DL outperforms the conventional two-phase locking in terms of the degree of oncurrency and average transaction waiting time.

  • This paper presents a modified quasi-static approximate solution derived from the Maxwell's equations of integral form for the calculation of magnetic shielding effectiveness in a non-uniform enclosure such as metal-clad high-voltage test laboratory. It also describes the simplified relationship between the electrical parameters applicable to the engineering calculations of electromagnetic absorption loss which comprise the resultant effects due to the welding seams and short-circuited slots as well as the shielding material properties. A numerical example shows the fairly good agreements with experimental results measured on the absorption loss vs. incident wave frequency without any unreasonable rapid increase.

Application of Plate and Circumferential Wiring with Resin to Bilateral Mandibular Fractures in a Calf: Case Report

  • A 2-month-old, 50 kg male Korean native calf was referred with bilateral mandibular fractures. The bilateral mandible was hanging loose, with inability to use the jaw, indicated by excessive salivation. Radiography revealed a long oblique fracture in the right mandible, and a short oblique fracture in the left mandible. Computerized Tomography, a different form of radiography, revealed the fracture in the left mandible to be a comminuted fracture. In order to stabilize the bilateral mandible fractures, a combination of bone plate and wiring was applied; in addition, resin was applied to enhance stabilization of the mandible. Six weeks after surgery, the calf was able to masticate and ruminate well. In this case, application of the plate and wiring with resin to bilateral mandibular fractures presented a successful functional recovery. Accordingly, the combination of bone plate and wiring with resin can be an effective technique for treating mandibular fractures.

Computational Investigation of Isomeric and Conformeric Structures of Methyl Fluoroperoxide and Fluoromethyl Fluoroperoxides (Methyl fluoroperoxide와 fuoromethyl fluoroperoxides의 conformers와 isomers 구조에 대한 이론연구)

  • The ab initio calculations for fluoromethyl fluoroperoxides have been carried out using MP2/6-311G(d,p) and B3LYP/6-311++G(d,p) method. The structural optimizations were performed for several isomers and conformers of methyl fluoroperoxide, $CH_3OOF$ and the vibrational frequencies were calculated. The most stable conformer of $CH_3OOF$ is skew form and has fairly short O-O bond distance. The trans and cis conformers have 8-12 kcal/mol higher energies than skew form and the other isomers are very unstable. The structures of $CH_2FOOF$, $CHF_2OOF$ and $CF_3OOF$ are also optimized and vibrational frequencies were calculated. These molecules also have skew forms as the lowest energy conformers. The O-O bond distances are longer and C-O bond distances are shorter than $CH_3OOF$, but the structural parameters are almost independent of the number of fluorine atoms in methyl group.

  • Crystals of salicylaldehyde-4-morpholinothiosemicarbazone, $C_{12}H_{15}O_2N_3S$, are orthorhombic with space group Pna21. Unit-cell dimensions are a = 11.85(5), b = 15.45(5) c = 7.18(3)${\AA}$ with z = 4. Three-dimensional intensity data were collected from the multiple-film equi-inclination Weissenberg photographs taken with $CuK{\alpha}$ radiation. The intensities were estimated visually. The structure was solved by Patterson and Fourier methods and refined by the block-diagonal least-squares methods until the final R value becomes 0.11 for the 1064 observed independent reflections. The morpholine ring has a chair form. The rest atoms of salicylaldehyde-4-morpholinothiosemicarbazone molecule excluding morpholine ring and sulfur atom approximately lie on a plane. The hydroxyl group of the salicylaldehyde and the nitrogen atom of the thiosemicarbazone form an intramolecular hydrogen bond, $O-H{\cdot}{\cdot}{\cdot}N$, of 2.67${\AA}$. The short intermolecular distances all appear to be normal van der Waals contacts.

Effects of Exercise Intervention on Physical Fitness and Health-relalted Quality of Life in Hemodialysis Patients (운동요법이 혈액투석 환자의 체력과 건강관련 삶의 질에 미치는 효과)

  • Purpose: The purpose of this study was to investigate the effect of stretching, muscle strengthening, and walking exercise on the cardiopulmonary function and health-related quality of life in hemodialysis patients. Methods: Twenty-one patients in the intervention and the control group participated in the exercise respectively on maintenance hemodialysis at four university hospitals. The exercise was composed of 20 to 60 min per session, 3 sessions a week for 12 weeks. The effect of exercise was assessed by cardiopulmonary function (peak oxygen uptake, peak ventilation, peak respiration rate, maximal heart rate, and exercise duration) using a cycle ergometer. Grip strength was measured by dynamometer, and flexibility was measured by sit and reach measuring instrument. Health-related quality of life was measured using Medical Outcomes Study Short Form-36. Results: Peak oxygen uptake, peak ventilation, peak respiration rate, exercise duration, grip strength, flexibility, and physical component scale were significantly improved in the intervention group after 12 week's exercise compared to the control group. Conclusion: These findings indicate the exercise can improve cardiopulmonary function, grip strength, flexibility, and physical component scale of health-related quality of life in hemodialysis patients.
