Primary Splenic Paraganglioma in a Dog

  • A 6-year-old intact female Shih-tzu dog was referred due to anorexia. Physical examination, complete blood count, serum chemical analysis, radiography, and ultrasonography were evaluated. Physical examination and hematological analysis showed normal findings. Abdominal radiographs and ultrasound revealed well-defined masses in the spleen. Other abdominal organs showed no significant abnormalities. Tissue samples taken via sono-guided fine needle aspiration of the splenic mass showed many bare nuclei, which were variable in size. Results of histopathological and immunohistochemical (IHC) analyses performed after splenectomy were consistent with paraganglioma. Based on these findings, we diagnosed this patient with a paraganglioma of splenic origin. Two months after splenectomy, abdominal ultrasonography revealed a new neoplastic lesion in the liver. The clients refused further management and the patient expired three months after initial diagnosis. Necropsy as well as histopathological and IHC examinations of other systemic organs including the liver, adrenal gland, kidney, brain, urinary bladder, lung, aortic body, carotid body, and pancreas were performed. The neoplastic tissue in the liver also demonstrated features of a paraganglioma, and there were no remarkable findings in all other organs.

Surgical Correction of Single Extrahepatic Portosystemic Shunt Using Ameroid Constrictor in a Dog (Ameroid Constrictor를 이용한 개에서의 단순 간외성 간문맥전신단락증의 치료)

  • A 2-year-old 4.0-kg female Shih Tzu with history of hematemesis and melena was referred to Veterinary Medical Teaching Hospital, Seoul national University for further evaluation and treatment. During physical examination, the dog revealed mild depression, dry mucous membrane and abdominal pain. Hematologic values were normal and serum chemical values showed increased serum bile acid (53.47 umol/l, preprandial), fasting serum ammonia concentration (184 g/dl), alanine transferase (98 U/L), alkaline phosphatase (871 U/L) and gamma glutamyl transpeptidase (21 U/L), and decreased blood urea nitrogen (4 mg/dl), total protein (4.1 g/dl) and albumin (1.2 g/dl). Microhepatica was shown in abdominal radiography. During the ultrasound examination, dilated tortuous vein communicating with caudal vena cava ws observed near the stomach. Intraoperative jejunal vein portography was performed during laparotomy to confirm the location and size of shunt vessel. According to history taking, physical examination, hematologic and serum chemical examination and radiographic study, it was diagnosed as single extrahepatic portosystemic shunt. The anomalous vessel (7 mm, o.d.) that enter the caudal vena cava from the left gastric vein, near the level of the diaphragm, was identified. A Ameroid constrictor (5 mm, i.d.) was applied to the shunting vessel near the caudal vena cava. Hematologic and serum chemical values recovered gradually and were revealed normal values 4 months after surgery. Four month after surgery serum bile acids concentrations were 0.56 $\mu$mol/l (preprandial) and 18.45 umol/l (postprandial). Abdominal radiograph showed normal gastric axis and it revealed normal size of the liver. Fine texture and increased echogenecity of liver and enlargement of portal vein were shown in ultrasonography. Single extrahepatic portosystemic shunt might be treated surgically using Ameroid constrictor.

Imaging Diagnosis: Biphasic Synovial Sarcoma in a Dog (개에서 발생한 이상성 활막육종 증례)

  • An 11-year-old castrated male, Shih-tzu dog was admitted for progressive weight-bearing lameness and mass on the right hindlimb. Radiography and ultrasonography revealed a large well-marginated multinodular mass with mixed echogenicity. On T1 and T2-weighted magnetic resonance imaging (MRI), the mass has a heterogenous signal intensity similar to or higher than muscle. The masses were extended to the pelvic cavi쇼 through obturator foramen and displaced the rectum dorsally. It was diagnosed as synovial sarcoma which was composed with mesenchymal and epithelial elements on histopathological findings of the multifocal biopsied specimen. MRI was helpful to determine the definitive margin for surgical resection of the mass. The mass was recurred at the 6th month after surgery. On the 10th month, the patient was euthanasiuzed by owner's request.

Oral Fluralaner Treatment in a Dog with Desperate Demodicosis: A Case Report

  • A 10-year-old castrated male Shih-tzu dog presented with a history of generalized demodicosis, refractory to conventional therapy with ivermectin and amitraz for a year. The patient was also diagnosed with concurrent deep pyoderma, Malassezia dermatitis, and otitis externa. Treatment with amoxicillin-clavulanate, antifungal drugs (itraconazole, miconazole), and milbemycin oxime resulted in a good response for 90 days. Approximately 4 months later, the first relapse of demodicosis occurred and the miticidal therapy was changed to ivermectin. Additional diagnostic tests were performed to investigate an underlying cause for the recurrence of demodicosis, and endocrinopathies and allergic dermatitis were excluded based on the results. Although ivermectin therapy was sustained for 440 days, a second relapse occurred and amitraz baths were added to the therapy. Despite this therapy, the demodicosis persisted, and the miticidal therapy was changed to oral fluralaner, which led to rapid resolution. Demodicosis did not recur again before death approximately 920 days after administration of oral fluralaner. This case report describes the complete resolution of refractory demodicosis using oral fluralaner in a dog.

Retrospective Study of 143 Dogs with Alimentary Foreign Bodies (개에서 발생한 소화기계 이물에 대한 회고분석 - 143 증례)

  • This study was retrospectively investigated the clinical features, diagnostic imaging, treatment, and prognosis of foreign bodies according to alimentary site and type of foreign body, in 143 dogs. Among 28 breeds, Maltese, Shih Tzu, and terriers including the Yorkshire terrier were presented with high prevalence. Bone was the most commonly identified (33%) foreign body regardless of alimentary site. Approximately 76% of foreign bodies were in the stomach and small intestine. All esophageal foreign bodies were diagnosed on survey radiography, with the most common location being the caudal thoracic esophageal segment. Further diagnostic examinations including ultrasonography or contrast studies were additionally performed to detect radiolucent foreign bodies and perforation or leakage in the stomach and intestine. However, most seeds could be identified based on the characteristic features such as hyperdense thin double lines and inner gas on survey radiography. In conclusion, complications such as peritonitis and intestinal perforation were mainly observed in cases with seeds and linear foreign bodies. Especially, fabric foreign bodies could be induce peritonitis and re-perforation with high prevalence after surgical correction.

Trimethoprim-Sulfamethoxazole for the Treatment of Tear Staining Syndrome in Dogs (개의 눈물착색증후군(Tear Staining Syndrome) 치료를 위한 Trimethoprim-Sulfamethoxazole)

  • 22 dogs (31 eyes) that had treated with trimethoprim-sulfamethoxazole for tear staining syndrome at Snoopy Pet Clinic from October 2000 to September 2002 were reviewed. Of the 22 dogs, 12 were female and 10 male. Their mean (${\pm}$ SD) age was 3.5 (${\pm}\;1.3$) years. The breeds of the dogs consisted of Maltese (8 dogs), Shih tzu (6 dogs), Poodle (5 dogs), Yorkshire terrier (2 dogs), and Mixed (1 dog). The dogs received 30 mg/kg trimethoprim-sulfamethoxazole perorally twice daily for two to six weeks. 26 (19 dogs) of the 31 eyes (22 dogs) recovered completely and did not show relapse at $26{\sim}30$ weeks after treatment. Any complications did not observed. Five eyes of three dogs were not cured. Two eyes (one dogs) of them had not response to medicament and three eyes (two dogs) recurrence but the clinical signs decreased. It was considered that the trimethoprim-sulfamethoxazole was effective for the treatment in dogs with tear staining syndrome.

Magnetic Resonance Imaging in 24 Dogs with Chiari type 1-like Malformations (자기공명영상을 이용한 24마리의 개에서의 Chiari 1형 유사 기형 진단)

  • Chiari type 1 malformation is a developmental condition characterized by cerebellar herniation and syringohydromyelia in human beings. It has been reported as a common condition in the cavalier King Charles spaniel that is similar to human Chiari type 1 malformations. However, there are few documentations of diagnosed Chiari type 1 like malformation in other breed dogs. Abnormalities compatible with such a malformation were identified by magnetic resonance imaging in 24 dogs with neurologic signs in this study. The dogs were 15 females and 9 males. Their breed were variable, and 6 of the dogs were maltese, 5 were shih-tzu. The dogs had a variety of neurological signs and the severity of cerebellar herniation, syringohydromyelia, intracranial intra-arachnoid cyst or hydrocephalus.

Protein-losing Enteropathy Induced by 4 Different Etiologies (4가지 다른 병인들에 의해 유발된 단백소실성 장병증 증례)

  • Four dogs (case 1; 2-year-old spayed female Schnauzer, case 2; 3-year-old spayed female Cocker Spaniel, case 3; 9-year-old castrated male Yorkshire Terrier, and case 4; 9-year-old intact female Shih-tzu) were referred to us with gastrointestinal signs such as diarrhea, vomiting, and anorexia. Results of blood analysis revealed hypoproteinemia and hypoalbuminemia in all dogs. Case 4 showed large circular mass which is connected with small intestine on abdominal ultrasonography and other 3 cases showed no remarkable findings on abdominal radiography and ultrasonography. We performed enterectomy in case 4 and gastrointestinal endoscopic examination with biopsy in other 3 patients. Finally, 4 patients diagnosed to protein losing enteropathy with 4 different etiologies.

Mammary gland tumors in three male dogs

  • Mammary gland tumors are very rare in male dogs. In this study, four mammary gland tumors from 3 male dogs (2 intact, 1 neutered) were collected from local animal hospitals. The dogs included two purebred Shih Tzu (1 intact, 1 neutered) and one intact purebred Cocker Spaniel. The mean age of dogs with mammary gland tumors was 9 years (5-12 years). Two dogs had a solitary mass, whereas one dog had two mammary masses. Of the four tumor masses, three were observed in the fourth or fifth mammary glands, and one was observed in the third mammary gland. According to histopathologic examinations, all four mammary masses from three dogs were benign tumors including two benign mixed tumors in one case and two complex adenomas. There were no history of obesity, testicular tumors, diabetes, and sex hormonal therapy in any male dogs with mammary tumors. Surgical excision was the only reported treatment for these tumors. No recurrence or metastasis was recorded up to 25 months after surgery.

Pericardial mesothelioma in a dog with lymph node metastasis and chylothorax (개에서 림프절 전이와 유미흉을 동반한 심낭막 중피종 증례 보고)

  • Pericardial mesothelioma is a rare neoplasm in dogs. This report describes a case of pericardial mesothelioma in a 13-year-old Shih Tzu that presented with a clinical history of dyspnea. Hemorrhagic pericardial effusion and chylous pleural effusion with reactive mesothelial cells were identified by radiograph and cytology. Necropsy revealed multiple round nodules throughout the pericardium and regional lymph nodes in addition to chylothorax. Histopathology revealed invasive neoplasm on the pericardial surface with metastasis to the lymph nodes. The neoplastic cells were immunopositive to both cytokeratin and vimentin. Diagnosis of pericardial mesothelioma with regional lymph node metastasis was made.
