대한민국 성인에서 비만과 Uric acid의 관련성 (The Association of Obesity and Serum Uric acid in Korean adults)

  • 본 연구는 일부 종합검진 수검자들을 대상으로 비만수준이 혈 중 uric acid에 미치는 영향을 검토하고자 2011년 1월부터 12월까지 광주광역시의 일개 종합병원 건강검진센터에서 종합건강검진을 받았던 20세 이상의 지역주민 1,118명(남자 636, 여자 482)을 분석대상으로 하였다. 우리는 혈청 uric acid, 요소 질소, creatinine 뿐만 아니라 신체계측 (SBP, DBP 및 BMI)에 대해 평가하고, 분석에 포함 하였다. Model I에서, 연령, 성별, SBP, DBP, TC, TG, HDL-C, FBG를 보정하였을 때, 남성(p<0.001)과 여성(p=0.036)에서 uric acid의 평균값은 비만수준이 증가할수록 증가하였다. Model II에서, BUN과 creatinine을 추가로 보정하였을 때, 남성에서는 uric acid의 평균값이 정상 체중군은 $4.89{\pm}0.07mg/dl$, 과체중군은 $5.01{\pm}0.09mg/dl$, 비만군은 $5.35{\pm}0.08mg/dl$로 비만수준이 증가할수록 증가하였다(p<0.001). 그러나 여성에서는 uric acid의 평균값이 정상체중군은 $5.03{\pm}0.08mg/dl$, 과체중군은 $5.19{\pm}0.11mg/dl$, 비만군은 $5.27{\pm}0.09mg/dl$로 유의한 차이가 없었다(p=0.191). 결론적으로, 남성에서 비만수준의 증가는 혈 중 uric acid의 증가와 관련이 있지만, 여성에서는 관련이 없었다.

Direct Determination of Uric Acid in Human Serum Samples Using Polypyrrole Nanoelectrode Ensembles

  • Polypyrrole (PPy) nanotubes have been synthesized by chemical oxidative polymerization of pyrrole within the pores of polycarbonate membrane using the technology of diffusion of solutes. The nanotubes array prepared by the proposed method can be considered as nanoelectrode ensembles (NEEs). An amperometric uric acid sensor based on PPy NEEs has been developed and used for determination of uric acid in human serum samples. The electrode can direct response to uric acid at potential of 0.60V vs. SCE with wide linear range of $1.52{\times}10^{-6}\;to\;1.54{\times}10^{-3}\;M.\;The\;detection\;limit\;is \;3.02{\times}10^{-7}$ M. This sensor has been used to determine uric acid in real serum samples. PPy NEEs is thought of as a good application in the foreground.

Saengmaeksan, a traditional herbal formulation consisting of Panax ginseng, ameliorates hyperuricemia by inhibiting xanthine oxidase activity and enhancing urate excretion in rats

  • Background: Saengmaeksan (SMS) is a traditional Korean medicine composed of three herbs, Panax ginseng, Schisandra chinensis, and Liriope platyphylla. SMS is used to treat respiratory and cardiovascular disorders. However, whether SMS exerts antihyperuricemic effects is unknown. Methods: Effects of the SMS extract in water (SMS-W) and 30% ethanol (SMS-E) were studied in a rat model of potassium oxonate-induced hyperuricemia. Uric acid concentrations and xanthine oxidase (XO) activities were evaluated in the serum, urine, and hepatic tissue. Using renal histopathology to assess kidney function and uric acid excretion, we investigated serum creatinine and blood urea nitrogen concentrations, as well as protein levels of renal urate transporter 1 (URAT1), glucose transporter 9 (GLUT9), and organic anion transporter 1 (OAT1). The effects of SMS on in vitro XO activity and uric acid uptake were also evaluated. The components of SMS were identified using Ultra Performance Liquid Chromatography (UPLC). Results: SMS-E reduced serum uric acid and creatinine concentrations, and elevated urine uric acid excretion. SMS-E lowered XO activities in both the serum and liver, and downregulated the expression of renal URAT1 and GLUT9 proteins. SMS-E reduced renal inflammation and IL-1b levels in both the serum and kidneys. SMS-E inhibited both in vitro XO activity and urate uptake in URAT1-expressing oocytes. Using UPLC, 25 ginsenosides were identified, all of which were present in higher levels in SMS-E than in SMS-W. Conclusion: SMS-E exhibited antihyperuricemic effects by regulating XO activity and renal urate transporters, providing the first evidence of its applicability in the treatment of hyperuricemia and gout.

성인 남성에서 체질량지수로 분류한 비만 유형에 따른 혈청 감마 글루타밀 전이효소와 혈청 요산농도의 차이 비교 (Comparison of the Serum Gamma-Glutamyl Transferase and Serum Uric Acid Levels according to the Obesity Types Based upon on the Body Mass Index in Korean Men)

  • 본 연구는 성인 남성을 대상으로 체질량지수로 분류한 비만 유형에 따른 혈청 GGT와 요산농도의 차이와 관련성을 확인하고자 하였다. 2017년 1월부터 2018년 8월까지 경기지역 종합병원의 건강검진센터를 내원하여 건강검진을 실시한 20세 이상 성인 남성 14,283명을 대상으로 하였다. 비만 기준은 아시아-태평양지역 기준을 적용하였으며, 복부비만은 남성의 허리둘레 90 cm이상을 기준치로 정의하였다. 남성 대상자의 혈청 요산농도 상승은 7.0 mg/dL 이상, 혈청 GGT 상승은 56 IU/L 이상으로 정의하였다. 본 연구결과 혈청 GGT와 요산농도는 정상체중군보다 과체중과 비만군에서 높았으며, 비만군에서는 복부비만을 동반한 경우 혈청 GGT ($53.26{\pm}49.67IU/L$ vs $67.66{\pm}60.16IU/L$) 및 요산농도($6.22{\pm}1.26mg/dL$ vs $6.38{\pm}1.40mg/dL$)가 유의하게 높았다. 그러나 저체중군과는 차이가 없었다. 또한 과체중과 비만군은 정상체중군보다 혈청 GGT 상승과 고요산혈증의 위험이 높게 나타났으나, 저체중은 혈청 GGT 상승과 고요산혈증에 영향을 미치지 않는 것으로 나타났다. 따라서 성인 남성의 과체중 및 비만을 평가하는데 혈청 GGT와 요산이 유용하다고 판단된다.

Chicken serum uric acid level is regulated by glucose transporter 9

  • Objective: Glucose transporter 9 (GLUT9) is a uric acid transporter that is associated with uric absorption in mice and humans; but it is unknown whether GLUT9 involves in chicken uric acid regulation. This experiment aimed to investigate the chicken GLUT9 expression and serum uric acid (SUA) level. Methods: Sixty chickens were divided into 4 groups (n = 15): a control group (NC); a sulfonamide-treated group (SD) supplemented with sulfamonomethoxine sodium via drinking water (8 mg/L); a fishmeal group (FM) supplemented with 16% fishmeal in diet; and a uric acid-injection group (IU), where uric acid (250 mg/kg) was intraperitoneally injected once a day. The serum was collected weekly to detect the SUA level. Liver, kidney, jejunum, and ileum tissues were collected to detect the GLUT9 mRNA and protein expression. Results: The results showed in the SD and IU groups, the SUA level increased and GLUT9 expression increased in the liver, but decreased in the kidney, jejunum, and ileum. In the FM group, the SUA level decreased slightly and GLUT9 expression increased in the kidney, but decreased in the liver, jejunum, and ileum. Correlation analysis revealed that liver GLUT9 expression correlated positively, and renal GLUT9 expression correlated negatively with the SUA level. Conclusion: These results demonstrate that there may be a feedback regulation of GLUT9 in the chicken liver and kidney to maintain the SUA balance; however, the underlying mechanism needs to be investigated in future studies.

Anti-Hyperuricemic Effects of Oenanthe javanica Extracts in Hyperuricemia-Induced Rats

  • Background: This study aimed to investigate the effects of Oenanthe javanica (OJ) extracts on rats with potassium oxonate (PO)-induced hyperuricemia. Methods: The effects of OJ extract on rats with PO-induced hyperuricemia-induced were monitored. Changes in the body weight and organ indices of hyperuricemic rats were calculated to detect anti-hyperuricemic effects. Blood samples were collected to observe the effect of reducing serum uric acid concentration. Kidney tissues were stained to observe histopathological changes under a microscope. The activity of xanthine oxidase (XO), which catalyzes xanthine to uric acid in the liver, was assessed to observe the inhibitory effect of XO. Results: 1. The body weight of hyperuricemic rats showed no considerable differences between the control group and the treatment group. The OJ group had significantly improved liver index, whereas the allopurinol group had improved liver and kidney indices. 2. Serum uric acid levels increased significantly after PO injection, and the OJ and allopurinol groups showed a significant reduction effect. 3. PO injection led to the inflammation of kidney tissues, and OJ improved it significantly. 4. The activity of XO after PO injection was significantly increased, and allopurinol significantly inhibited XO activity in the liver. Conclusion: In the hyperuricemia rat model, OJ extract reduced uric acid concentration and demonstrated its anti-inflammatory effects. Thus, OJ extracts can be used to lower uric acid levels.

Relationship between Serum Uric Acid, Abdominal Obesity, and Waist Circumference in Korean

  • The purpose of this study was to determine the association between serum uric acid, abdominal obesity, and waist circumference (WC) in Korean. A total of 208 healthy examinees were examined at Gimcheon Medical Center from July 2011 to December 2011. WC and body mass index (BMI) of subjects were measured as well as blood pressure, fasting blood sugar level, High Density Lipoprotein (HDL) and Low Density Lipoprotein (LDL) cholesterol, C-reactive protein (CRP), triglycerides, and serum uric acid (SUA). In both men (p<0.05) and women (p<0.01), BMI and SUA were significantly correlated with WC. In men, triglycerides (r=0.501, p<0.01) had a positive correlation with WC, and CRP (r=0.442, p<0.01) had a positive correlation with BMI. In women, triglycerides (r=0.470, p<0.01) and CRP (r=0.366, p<0.01) had a positive correlation with WC. Therefore, WC and SUA may be the most effective anthropometric indices of obesity for screening Korean population.

Utility of Serum Albumin for Predicting Incident Metabolic Syndrome According to Hyperuricemia

  • Background: Serum albumin and uric acid have been positively linked to metabolic syndrome (MetS). However, the association of MetS incidence with the combination of uric acid and albumin levels has not been investigated. We explored the association of albumin and uric acid with the risk of incident MetS in populations divided according to the levels of these two parameters. Methods: In this retrospective longitudinal study, 11,613 non-MetS participants were enrolled among 24,185 individuals who had undergone at least four annual check-ups between 2006 and 2012. The risk of incident MetS was analyzed according to four groups categorized by the sex-specific medians of serum albumin and uric acid. Results: During 55,407 person-years of follow-up, 2,439 cases of MetS developed. The risk of incident MetS increased as the uric acid category advanced in individuals with lower or higher serum albumin categories with hazard ratios (HRs) of 1.386 (95% confidence interval [CI], 1.236 to 1.554) or 1.314 (95% CI, 1.167 to 1.480). However, the incidence of MetS increased with higher albumin levels only in participants in the lower uric acid category with a HR of 1.143 (95% CI, 1.010 to 1.294). Conclusion: Higher levels of albumin were associated with an increased risk of incident MetS only in individuals with lower uric acid whereas higher levels of uric acid were positively linked to risk of incident MetS regardless of albumin level.

신장이식 환자에서 발생되는 Cyclosporin에 의한 고요산혈증과 요산 배설 촉진제인 Benzbromarone의 효과 (Cyclosporin-induced Hypeyuricemia and the Uricosuric Efficacy of Benzbromarone in Kidney Tyansplant Patients)

  • After the introduction of cyclosporin, the graft survival rate of renal transplant and patients' life expectancy have been greatly improved. However, cyclosporin is known to cause several undesirable side effects, one of which is hyperuricemia, which may subsequently cause gouty nephropathy and graft dysfunction. The purpose of this study was to evaluate the frequency and predisposing factors of hyperuricemia in cyclosporin-treated patients within one year of kidney transplantation and uricosuric efficacy of benzbromarone. The patients who were treated with cyclosporin after kidney transplantation in 1998 and the patients who were treated with benzbromarone for the control of cyclosporin-induced hyperuricemia in 1999 were investigated retrospectively. Among the 76 patients in cyclosporin-treated patients in 1998, hyperuricemia occurred in 55 patients $(72.4\%)$ and the mean time from kidney transplantation to occurrence of hyperuicemia was $5.0\pm8.0$ months. In 1999, 22 patients were treated with benzbromarone for hyperuricemia and their mean time from kidney transplantation to occurrence of hyperuricemia was $4.5\pm10.4$ months. Acute rejection developed in one patient $(4.8\%)$ out of 21 normo-uricemic patients and 11 patients $(20.0\%)$ out of 55 hyperuricemic patients in 1998. The difference of rejection rate in these two groups was significant (p<0.001). There was no difference of rejection rate between before and after treatment of benzbromarone. Cyclosporin trough levels did not show a significant correlation with the serum uric acid levels among the three groups. However, hyperuricemic patients showed significantly higher serum creatinine levels than patients with normal uric acid levels (p<0.001). Benzbromarone decreased serum uric acid levels from $8.3\pm2.3\;mg/dl\;to\;5.1\pm2.0\;mg/dl$ (p<0.0001) and normalizing serum uric acid in all of 22 patients. Except for one patient $(4.5\%)$ who experienced diarrhea, no significant side effect was noted.
