Size of Prostatitis Symptoms Using Prostatitis Symptom Index(PSI): The Effect of Prostatitis Symptoms on Quality of Life (전립선염 증상지수를 이용한 전립선염 증상의 규모와 삶의 질에 미치는 영향)

  • Objectives : To determine the prevalence of prostatitis symptoms in the general population by questionnaire survey and to measure the effect of prostatitis symptoms on quality of life(QOL). Materials & Methods : A cross sectional community-based epidemiologic study was performed on 2,034 men, living in the Seoul metropolitan area using stepwise random sampling. Out of 2,034 interviewees, 1,356 men who were older than 40 and provided sufficient information were selected for this study. The questionnaires were completed by well trained interviewers. Contents of the questionnaires included demographic data, the Prostatitis Symptom Index(PSI), the International Prostate Symptom Score(IPSS), a general health questionnaire section and a sexual health questionnaire section. The PSI was composed of a sum of the scores from three questions about dysuria, penile pain and perineal pain and it ranged 0 to 12. Incidence of prostatitis symptoms was defined by a score of 4 or more and the reference group was defined as consisting of those with a score of 3 or less. The rate of incidence of prostatitis symptoms was assessed according to age and the difference of QOL between the prostatitis symptoms group and the reference group. Results : The overall positive rate of prostatitis symptoms measured by the PSI, in men older than 40, living in the Seoul metropolitan area, was 4.5%(61/1,356), adjusted to 4.8% by the relative proportion of this age group in the general population of the Seoul metropolitan area as compared to Korea and the World. The proportion of the group with prostatitis symptoms assessed by the PSI did not increase with age although the proportion of participants with moderate to severe lower urinary tract symptoms (LUTS) did increase with age. The group with prostatitis symptoms suffered from a much greater incidence of LUTS compared to the reference group (p<0.05). The QOL scores of the IPSS, and the general health and sexual health status of the group with prostatitis symptoms, were worse than those of the reference group.(p<0.05). Conclusions : The positive rate of prostatitis symptoms in men older than 40, living in the Seoul metropolitan area, was 4.8% and it didn't increase with age. The general QOL of the group with prostatitis symptoms was much worse than that of the reference group.

The Effect of Application of Injury Area to Overcrowding Indices in Local Emergency Department (지역응급의료센터에서 손상구역 운용이 응급실 과밀화 지표에 미치는 영향)

  • Purposes: There have been many efforts to improve the service of emergency centers. In spite of these, no evidence is showing any landmark advancement of emergency services, especially in the hospital stage, exists. We need some efficient standard criteria to evaluate emergency service in the hospital stage, and a useful method might utilize the overcrowding index. We want to know the change in the overcrowding index at a regional emergency center after injury area administration. Injury area means an area in which only an assigned duty physician manages patients with injuries such as those from traffic accidents, falls, assualts, collisions, lacerations, amputations, bums, intoxication, asphyxia, drowning, animal bites, sexual assualts, etc. Methods: We started to operate an injury area in our emergency department from late 2004, and from January to June in 2004 and in 2005, we collected patients' data, age, sex, assigned department, and result from hospital order communication system to figure out overcrowding indices and result indices. We found the daily number of patients, the turnover rate, the admission rate, the ICU admission rate, the emergency operation rate, the ED stay duration, and the ED patient volume to be overcrowding indices. Also we found the withdrawal rate, the transfer rate, and mortality to be result indices. We compared these indices between 2004 to 2005 by using a t-test. Results: There was a significant increase in the daily number of visiting patients in 2005, overcrowding indices, such as the turnover rate, the admission rate, the ICU admission rate, and the emergency operation rate, also showed statistically significant increases in 2005 (P<0.001). As for the result indices, there was a noticeable decrease in the number of withdrawals (11.77/day in 2004 to 4.53/day in 2005). Conclusion: Operating an injury area in a mildly overcrowded local emergency center is beneficial. Evaluating the effect of operating an injury area and it's impact on hospital finances by conducting a similar study analyziing patients for a longer duration would be valuable.

Spatial-Temporal Pattern Analysis of Unplanned Factory Locations in the Seoul Metropolitan Area Using FEMIS Data (수도권 개별입지 공장의 시공간적 입지특성 분석)

  • The purpose of this study is to analyze the spatial-temporal proliferation of individual factories in the Seoul Metropolitan Area and to provide policy implications. The location of the factory is divided into individual locations and planned locations. In the case of individual locations, it takes a long time to establish factories. Also, due to high cost and regulation, the establishment of individual factories is not easy. However, since the establishment process is simple and the price of land is low, the establishment of individual factories has led to an increase in the number of individual factories. The problem of the undeveloped factories is that the lack of infrastructure such as the road and environmental pollution treatment facilities around the factory deteriorates the pleasant environment and the cityscape and deteriorates the health of the residents in the surrounding area. In this study, we analyzed the location of individual factories established in the Seoul metropolitan area from 2001 to 2016 by using ArcGIS Pro. The results of the analysis are as follows. First, individual location factories are formed around existing industrial complexes and industrial sites. The reason for this is considered to be the external effect that can be obtained from the surrounding area. Secondly, since the Seoul city's individual location factories are established in many residential areas, it shows the conflicting result of the mixture of residence and factory. Third, the Gyeonggi province's individual location factories have a high proportion of non-urban areas. This is because the Gyeonggi province's management area occupies a larger proportion than other areas in Gyeonggi province. This study analyzed the spatial - temporal spreading process of individual factories and the unfolding of individual factories in the metropolitan area, and it can provide policy implications to control the over development of individual factories in the future.

  • A 69 year-old woman was admitted with sudden left facial weakness. She had no other neurologic deficit, except for left peripheral type facial palsy. She had a presumptive diagnosis of Bell's palsy. The blink test was indicative of left facial neuropathy due to left medullary lesion. Diffusion weighted (DWI) brain MRI demonstrated high signal signal lesion in left dorsolateral ponto-medullary junction. Apperant diffusion coefficient (ADC) brain MRI showed low signal lesion in the same area. We present an unusual case of ipsilateral peripheral facial palsy in dosolateral ponto-medullary infarction without other neurologic deficits.

Nearly single crystal, few-layered hexagonal boron nitride films with centimeter size using reusable Ni(111)

  • Hexagonal boron nitride (hBN) is a dielectric insulator with a two-dimensional (2D) layered structure. It is an appealing substrate dielectric for many applications due to its favorable properties, such as a wide band gap energy, chemical inertness and high thermal conductivity[1]. Furthermore, its remarkable mechanical strength renders few-layered hBN a flexible and transparent substrate, ideal for next-generation electronics and optoelectronics in applications. However, the difficulty of preparing high quality large-area hBN films has hindered their widespread use. Generally, large-area hBN layers prepared by chemical vapor deposition (CVD) usually exhibit polycrystalline structures with a typical average grain size of several microns. It has been reported that grain boundaries or dislocations in hBN can degrade its electronic or mechanical properties. Accordingly, large-area single crystalline hBN layers are desired to fully realize the potential advantages of hBN in device applications. In this presentation, we report the growth and transfer of centimeter-sized, nearly single crystal hexagonal boron nitride (hBN) few-layer films using Ni(111) single crystal substrates. The hBN films were grown on Ni(111) substrates using atmospheric pressure chemical vapor deposition (APCVD). The grown films were transferred to arbitrary substrates via an electrochemical delamination technique, and remaining Ni(111) substrates were repeatedly re-used. The crystallinity of the grown films from the atomic to centimeter scale was confirmed based on transmission electron microscopy (TEM) and reflection high energy electron diffraction (RHEED). Careful study of the growth parameters was also carried out. Moreover, various characterizations confirmed that the grown films exhibited typical characteristics of hexagonal boron nitride layers over the entire area. Our results suggest that hBN can be widely used in various applications where large-area, high quality, and single crystalline 2D insulating layers are required.

Evaluation of Emission Reduction Program for In-use Diesel Vehicles Using PM10 Concentrations of Road-side and Background Monitoring Network (도로변 및 배경농도 측정망 PM10 자료를 이용한 노후 운행경유차 배출가스 저감사업 효과 해석)

  • Emission reduction program for in-use diesel vehicles (ERPDV) has been enacted since 2004 over the Seoul metropolitan area, and diesel emission reduction is forced to fulfill this regulation. This study was performed to evaluate the ERPDV using $PM_{10}$ concentrations of both road-side monitoring and national background network during the period of 2004-2010. In order to assess the pure road emission, we first eliminated the long range transport effect by deducting the trend of annual national background concentrations from the road-side $PM_{10}$ concentrations, and then analyzed the time series of the resultant $PM_{10}$ concentrations over Seoul metropolitan area. The annual rates of variations of road-side $PM_{10}$ with the deduction of trend of background level show -3.2, +0.4, and -2.4 ${\mu}g/m^3$/year, in Seoul, Incheon, and Gyonggi province, respectively. There are steadily decreasing trend in Seoul with all of statistic parameters such as mean, mediam, 5%ile, 10%ile, 25%ile, 75%ile, 90%ile, and 95%ile concentrations. Incheon shows some fluctuations with positive with no significant trend, and Gyonggi province shows overall decreasing but not consistent. Student-t test shows 95% significant level of ERPDV effect in Seoul, but there exists no significant level greater than 90% in both Incheon, and Kyonggi province. Total annual averaged trend over the whole Seoul metropolitan area is estimated to lie in approximately $-2.9{\mu}g/m^3$/year in this study, implying the intimate involvement of ERPDV to a large extent. This is also suggesting that the further research cost-effectiveness of ERPDV with consideration of the long range transport process would be needed over the Seoul metropolitan area.

ORIGINAL ARTICLE - Analysis of RFA related to stabilities by types and areas of dental implants (ORIGINAL ARTICLE - 임플란트 종류 및 식립부위에 따른 안정성에 대한 RFA 분석)

  • Objective : This research compared stabilities between two types of dental implant ($SLA^{TM}$, Institut Straumann AG, Waldenburg, Switzerland and $SSII^{TM}$, Osstem co, Busan, Korea) using Osstell Mentor (Integration Diagnostics AB, Goteborg, Sweden) considering surgery methods, surgery area, diameter of implant, systemic disease, and smoking for obtaining prognosis information when installing fixture of dental implant. Materials & Methods : 206 implants of 131 patients taken by resonance frequency analysis (RFA) were determined as a final sample. Dental implants were installed as protocol of supplier by a excellent dentist who had 10 years experience about dental implants. Before connecting abutments (3 months after installation of fixture), RFA were measured twice for buccal and lingual direction to obtain average value. Results : Dental implants at mandible showed significantly higher stabilities significantly than at maxilla (p<0.001). Diameter 4.8 implants had also higher stabilities than diameter 4.1 in case of $SLA^{TM}$ implants (p<0.001). $SLA^{TM}$ implants showed more excellent stabilities than $SSII^{TM}$ implants, especially at posterior area of mandible (p=0.045) and premolar area of maxilla (p=0.032). Conclusions : This research revealed higher stabilities of $SLA^{TM}$ implants than $SSII^{TM}$ implant, especially at posterior area of mandible (p=0.045) and premolar area of maxilla (p=0.032).

Study on Trip Generation Characteristics of Single-Person Household in Seoul Metropolitan Area (수도권 1인가구의 통행발생 특성에 관한 연구)

  • Single-person households in the Seoul Metropolitan Area have increased sharply during recent 20-30 years. Despite of decreasing in the total population, the number of single-person household is predicted to increase continuously. However, the effect of single-person household growth on the domestic transport sector has not been studied concretely. In this study, the differences on trip generation characteristics by household size and attributes were figured out by analyzing Seoul Metropolitan Area Household Travel Behavior Survey (SMA-HTBS). Firstly, trip generation rates (trips/day/person) were produced by household attribute, household member attribute and trip attribute based on SMA-HTBS. Secondly, trip generation rate of single-person household and that of multi-person (2 or more) household were compared by significance test. It was found that trips generation characteristics of single-person household is quite different from those of multi-person household by housing type, residential type, living area, and transport mode. The result of this paper is expected to contribute developing more sophisticated trip generation model and transport policy reflecting trip generation characteristics of single-person household.

ANALGESIC EFFICACY OF CAPSAICIN IN PERIODONTAL THERAPY (Capsaicin이 치주 치료시 통증에 미치는 영향)

  • The purpose of present study is to assess the effects of capsaicin topically applicated to the chronic periodontal pain suffering area. In the First study, twenty patients with chronic pain caused by mild periodontal disease were selected, and periodontal pack containing capsaicin(PPC) was attached to these patients gingiva around pain suffering area. Then the presence of discomfort had been recorded every ten minutes for the first 1 hour. After 1 hour again, It had been recorded according to the presence of pack and to the existence of pain. In the second study, twenty moderate periodontitis patients were selected. After subgingival curettage of two quadrant area, non-euginol periodontal pack or PPC were attached to the curetted gingival margins of them (Non-euginol pack bearing area and capsaicin pack bearing area is supposed to control group and test group respectively.), and the degrees of pain with time had been recorded eight times with 1 hour interval (at that day) or recorded once in a day (from the next day to the next appointment day). The results are as follows : 1. PPC has caused discomfort accompanied by burning sensation to the mild periodontitis patients with chronic pain. 2. PPC has given little effects to improve the pain after subgingival curettage of moderate periodontitis patients.
