Purpose: Although the sural nerve is the most commonly used donor for autologous nerve graft, its morbidity after harvesting is sparsely investigated. The sural nerve being a sensory nerve, complications such as sensory changes in its area and neuroma can be expected. This study was designed to evaluate the donor site morbidity after sural nerve harvesting. Methods: Among the 13 cases, who underwent sural nerve harvesting between January 2004 and August 2009, 11 patients with proper follow up were included in the study. The collected data included harvested graft length, actual length of the grafted nerve, anesthetic and paresthetic area, presence of Tinel sign and symptomatic neuroma, and scar quality. Results: In 7 patients, no anesthetic area could be detected. Of the patients with a follow up period of more than 2 years, all the patients showed no anesthetic area except two cases who had a very small area of sensory deficit ($225mm^2$) on the lateral heel area, and large deficit ($4,500mm^2$) on the lateral foot aspect. The patients with a short follow up period (1~2 m) demonstrated a large anesthetic skin area ($6.760mm^2$, $12,500mm^2$). Only one patient had a Tinel sign. This patient also showed a subcutaneous neuroma, which was visible, but did not complain of discomfort during daily activities. One patient had a hypertrophic scar in the retromalleolar area, whereas the two other scars on the calf were invisible. Conclusion: After a period of 2 years the size of anesthetic skin in the lateral retromalleolar area is nearly zero. It is hypothesized that the size of sensory skin deficit may be large immediately after the operation. This area decreases over time so that after 2 years the patient does not feel any discomfort from nerve harvesting.

Objective : The purpose is to analyze the effects of parent training interventions in reducing the symptoms of Attention Deficit Hyperactivity Disorder (ADHD) and to provide a basis for the effects. Methods : We collected literature published in international academic journals from 2009 to 2019 through ProQuest and Scopus databases. The search key terms were (ADHD OR Attention Deficit Hyperactivity Disorder) AND (Parent OR Mother OR Father) AND (Training OR Program OR Therapy OR Intervention) AND (Randomized OR Randomised OR Randomly). The PEDro scale was used to evaluate the quality of the studies, and a meta-analysis was conducted through Comprehensive Meta-Analysis 3.0. Results : Of 628 studies that we reviewed, 20 were eligible for inclusion. of parental training interventions for children with ADHD appeared close to a medium effect size while parent-child interventions (0.639) appeared to have a medium to large effect size. Oppositional defiant behavior and conduct disorder (0.737) in children and children's emotions (0.679) appeared close to a large effect size. ADHD symptoms hyperactivity and impulsivity (0.590) and child behavior (0.521) appeared to be over a medium effect size. We chose a random-effects model since heterogeneity tests showed significant results. Conclusion : We were able to confirm the effects of parent training interventions on the symptoms of children with ADHD. This will be used as evidence to provide a clinical basis for occupational therapists. More research related to parental training interventions should be carried out to ensure better reporting in the future.

Objective : This study was designed to find the latest intervention trends in treatment of children with attention deficit hyperactivity disorder (ADHD) in South Korea. Methods : Studies on ADHD published in Korean journals from January 2004 to December 2018 were reviewed. The databases used were DBpia, Google Scholar, RISS, and KISS. The search terms were "attention deficit hyperactivity," "child," and "intervention." A total of 14 studies were selected. The results of this study were as follows: Patient, Intervention, Comparison, Outcome. Results : The level of evidence was the highest in the single subject study of level IV, and the methodological quality was the most common study. The interventional metronome (IM) intervention was the most frequently used, and the evaluation tools used in the measurement were the Long Form Assessment of the Interactive Metronome and the Korean Behavior Child Checklist. The Korean ADHD interventions showed statistically significant improvement in concentration, hyperactivity, task performance, excessive language use, and timing. Conclusions : Various types of interventions were provided for children with ADHD in South Korea. The interventions showed positive effects, and provided basic data for evidence-based implementation in clinical practice. In the future, a model study should be conducted, to establish an effective service delivery system, as well as ongoing research on children with ADHD.
Purpose: This study examined the improved sensory, hand function, postural balance and activities of daily living (ADL) through somatosensory stimulation, such as the facilitation of functional reaching and tactile, proprioceptive stimulus of the upper limb (UL) and hand. Methods: Seventeen stroke patients having problems with motor and somatosensory deficits were selected in Bobath Memorial Hospital adult rehabilitation center. The patients were divided into two groups; the sensorimotor deficit group (SMDG) and motor deficit group (MDG). Somatosensory stimulation on the UL, physical therapy and occupational therapy were carried out three times a week over a six week these treatments were performed in both group period. To compare each group, the following assessment tools were used: such as tactile detection thresholds (TDT), two point discrimination on the affected side (TPDas), unaffected side (TPDus) stereognosis (ST) manual function test, hand function on the affected side (HFas) and unaffected side (HFus), Postural Assessment Scale for Stroke (PASS) and Korean version Modified Barthel Index (K-MBI). Results: In the SMDG, somatosensory stimulation on the UL was statistically important for TDT, TPDas, TPDus (except for the thener), ST, hand function on HFas, on HFus, PASS length of displacement with foam (LDFSEO), and K-MBI. In the MDG, somatosensory stimulation on the UL was important for TDT, TPDas, TPDus (except index finger) length of displacement with the eyes open, LDFSEO, HFas, HFus, PASS and K-MBI. In addition, there was a significant difference in the PASS between SMDG and MDG. Conclusion: Somatosensory stimulation on the UL affects the sensory, hand function, postural control and ADLs performance.

Objective : The purpose of this study was to identify the effect of Interactive Metronome (IM) training on postural control and hand writing performance of children with Attention Deficit Hyperactivity Disorder (ADHD). Methods : Participant was a third grade elementary school student diagnosed with ADHD. ABA design was used and a total of 30 sessions were held for 3 sessions every week for a total of 10 weeks. In the intervention period, IM training was conducted for 40~50 minutes before intervention for writing, and the writing task was carried out. We evaluated the handwriting legibility and speed. Before baseline A and within a month after A' phase, Clinical Observation of Motor and Postural Skills (COMPS) was evaluated to examine the changes in postural control of the student. Results : After the IM intervention, the postural control of the student improved in the score of slow movement, finger-nose touching, and asymmetrical tonic neck reflex. The handwriting legibility and speed has also tended to increase during the intervention period, but it has not significantly changed. Conclusion : This study could be used as an evidence that the IM training aimed at postural control and handwriting ability could enhance the ability to improve postural control and thereby provide fundamental knowledge for future studies.

Objective : The purpose of this study was to provide for the intervention and outcome measurement tools of children with Attention Deficit Hyperactivity Disorder (ADHD) through ICF model. Methods : The systematic review methods were used. Papers published in the journal between January, 2000 and July, 2014 were searched through MEDLINE/PubMed, Sciencedirect, Ovid. The main terms searched were 'ADHD, Children, intervention, outcome measure', and 8 papers were analyzed. Results : 1. The subjects of ADHD were pure ADHD (75.8%), ADHD with dyslexia (9.1%), ADHD with conduct disorder (5.8%), ADHD with tic disorder (3.8%), ADHD with DCD (3.0%), ADHD with emotional disorder (2.5%). 2. The nonpharmacologic intervention of ADHD were functioning and disability (80%) and contextual factors (20%). Most frequently used intervention were body function and structure (60%). 3. The outcome measurement tools of ADHD were functioning and disability (80.5%) and contextual factors (19.5%). Most frequently used outcome measurement tools were body function and structure (70.8%). Conclusion : This study can provide information on the intervention and outcome measurement tools of ADHD.

Objective : The purpose of this study was to examine the differences between motor functions and executive functions according to the timing of children with attention deficit hyperactivity disorder (ADHD). Methods : The subjects were 32 children with ADHD aged between 6 and 12 living Busan. To assess the timing, Long Form Assessment (LFA) of Interactive Metronome (IM) was used. Bruininks-Oseretsky of Motor Proficiency, Second Edition (BOT-2) were also used to assess motor functions. STROOP Color and Word Test and Children's Color Trails Test were used to evaluate executive functions. Mann-Whitney U tests were used to determine the differences between the executive functions and the motor functions according to the timing. Results : Comparing the inter-group motor functions according to the timing, there was a statistically significant difference in the Fine manual control and Fine motor precision in BOT-2 (p<.05). Comparing the inter-group executive functions according to the timing, there was not statistically significant difference (p>.05). Conclusion : Among the deficits in ADHD children, we could see the differences between motor function and executive function according to timing function. This study would be meaningful in that the results could be a basic data for study on the timing of children of ADHD in the future.
Miller Fisher syndrome is characterized by acute external ophthalmoplegia, ataxia and areflexia in the abscence of significant motor or sensory deficit in the limbs and usually results in a complete recovery. Most cases have anteceding events like upper respiratory infection or other viral infections. Its accurate anatomic lesion sites and pathogenesis is still unknown. Recently we experienced a 47 year-old man who had a sudden onset of complete total ophthalmoplegia, ataxia, diplopia and whose condition was improved through Oriental medical treatment.
노인에 있어 낙상 (falls)의 결과는 신체 기능상의 상실을 가져 올 뿐 아니 라 종종 사망에 이르게 하는 원인이 되기도 하여 중대한 건강상의 문제로 다루어지고 있다. 정상적인 노화과정은 감각(sensory)과 운동 (motor)계의 감퇴 그리고 주의력의 쇠퇴와 연관되어 있는데, 노인들은 이러한 감각(sensory)과 운동(motor)계 (system)의 손상으로 여러 가지 자세(posture)와 보행 (walking)등을 수행하는데 어려움을 겪는다. 또한 노화와 관련된 변화들은 자세를 조절 (postural control) 하는데 있어서 주의력 (attention capacity)을 감소시킨다. 이러한 조건 하에서 노인들은 이중과업 (dual task)을 수행하는데 젊은 사랑들보다도 더 많은 어려움을 느끼며 이는 곧바로 낙상 (falls)의 가능성을 증가시키는 중요한 원인이 되고 있다. 이러한 점들을 고려할 때 낙상(falls)을 방지하기 위한 훈련 프로그램 (training program)은 단순한 신체운동(physical exercise) 보다는 자세조절 (postural control)시의 인지시스템 (cognitive system)이 포함된 중추 통합 기전(central integrative mechanisms)을 최적화 (optimize) 시킬 수 있는 방향으로 구성되어야 한다.

In 1983, Sten H$\ddot{a}$kanson first reported the clinical safety and efficacy of retrogasserian glycerol injection as a treatment of typical trigeminal neuralgia in 96 of 100 patients during a follow-up period of 1~6 years. Since September 1987, we have injected sterile pure glycerol into the trigeminal cistern using an anterior percutaneous approach via the foramen ovale (H$\ddot{a}$rtel route) for treatment of tic douloureux in 12 patients who were suffering from attacks pain of following discontinuation of carbamazepine. The results were as follows; 1) Eight patients were completely free from pain attacks with a single dose of glycerol (0.4 ml). The remaining four patients needed a second dose (0.4 ml) several days later following the single dose. The degree of patient's subjective satisfaction by those injections was very good in 11 and fair in one. 2) During the follow-up period (1~13 months), persistent sensory deficit as determined by the pin prick test, appeared to be mild in 10 and moderate in one patient. There was no sensory deficit in one patient. further attacks of pain from those injections were still noted. 3) As a transient complication, there was headache in all patient, facial hematoma in 4, nausea and vomiting in two each, and vertigo and herpes simplex in one each. In conclusion, we confirmed that the above glycerol injections into the trigeminal cistern were clinically very effective as a treatment of tic douloureux even though the follow-up period was short.
