환자 프라이버시 보호행동 실천에 관한 연구 - 물리치료사와 작업치료사를 중심으로 - (A Study on the Therapists' Protective Actions of Medical Information Privacy - With a focus on physical and occupational therapists -)

  • 본 연구는 물리치료사와 작업치료사의 환자 프라이버시 보호행동 실천도를 알아보기 위해 시도되었다. 전국에 소재한 종합병원 및 재활치료 전문병원의 물리치료사와 작업치료사 297명을 대상으로 2012년 1월 2일부터 1월 13일까지 설문조사를 실시하였다. 연구결과 총 실천도 평균은 $3.54{\pm}.733$으로 나타났으며, 각 영역별 평균은 의사소통 영역 $3.62{\pm}.766$ 직접접촉 영역 $3.57{\pm}1.013$, 유지 관리 영역 $3.53{\pm}.780$ 순으로 나타났다. 일반적 특성 중 각 영역별로 분석한 결과 연령이 증가 할수록, 근무경력이 많을수록 실천도가 높았다. 환자 의료정보의 프라이버시는 100% 보호되어져야 하는 것이며, 실천도는 매우 잘 하고 있음의 수준(5)에 근접해야 할 것이나 환자의 기능 개선의 중추적 전문가인 치료사로서 의료정보 보호에 관한 관심이 적고 그 내용에 대해 거의 인지하지 못하는 것으로 나타났다. 이에 지속적인 홍보와 교육을 통하여 환자 프라이버시 보호에 대한 인식을 고취시고, 윤리의식을 확고하게 심어주는 마인드에 대한 제고가 필요하다.

선택 실험법을 이용한 친환경 보일러의 시장 점유율 예측

  • 최근 온실가스 배출 규제와 에너지 안보에 대한 관심이 고조되면서 친환경연료인 목재 펠릿을 이용한 펠릿 보일러가 주목받고 있다. 그러나 친환경 기술인 펠릿 보일러는 아직 시장 초기단계로 정부에서 펠릿 보일러에 대한 지원정책을 통해 펠릿 보일러 보급사업을 실시하고 있다. 따라서 펠릿 보일러에 대한 미래시장 점유율을 예측함으로써 정부와 기업이 보다 효과적인 보급정책과 시장전략을 추진할 수 있을 것이다. 이를 위해서는 펠릿 보일러에 대한 소비자 선호를 분석해야 하며 본 연구는 신기술이 갖는 무형의 비용을 반영한 선택실험법을 적용하여 펠릿 보일러에 대한 시장 점유율을 추정하였다. 신기술의 시장 점유율이 확산되고, 기술 신뢰도가 향상됨에 따라 무형의 비용이 감소하게 된다. 상이한 무형의 비용을 가정한 시나리오별로 전기보일러, 가스보일러, 기름보일러, 펠릿 보일러의 선호를 선택 실험법에 의해 조사하였다. 다항로짓모형을 이용하여 속성별 파라메터를 추정하고, 시나리오별로 보일러별 시장 점유율을 예측하였다. 추정 결과 무형의 비용이 감소함에 따라 펠릿 보일러의 시장점유율이 점차 증가함을 알 수 있었다. 또한 가스보일러와 전기보일러에 대한 대체효과가 크게 나타났다. 한편 목재 펠릿 전용 사용자 집단과 일반 보일러 사용자 집단간의 속성별 지불용의액과 시장 점유율에 차이가 발생하고, 소득, 교육, 가족수, 신기술 인식 수준과 같은 개인별 속성에 대해 선호의 이질성이 부분적으로 존재함을 확인하였다.

국내 중고령층의 민간의료보험 가입에 대한 영향 요인 (Factors Associated with the Middle-aged or the Old-aged Koreans' Enrollment in Private Health Insurance)

  • 이 연구는 중 고령자의 민간의료보험 가입에 대한 영향 요인과 민간의료보험 가입자를 대상으로 민간의료보험 가입 개수 및 월 평균 납부액에 영향을 주는 요인이 어떤 것인지를 분석한 연구이다. 분석을 위해 2008년 제2차 고령화연구패널조사(KLoSA) 표본 중 의료급여 대상자를 제외한 8,167명을 대상으로 로지스틱 회귀분석과 다중선형회귀분석을 실시하였다. 분석결과 민간의료보험 가입 여부는 인구 사회 경제학적 요인의 영향을 많이 받았고, 규칙적 운동, 흡연, 인지기능, 주관적 건강상태, 입원, 외래진료횟수, 무료 1차 건강검진 수혜에 대한 영향을 받았다. 민간의료보험 가입자를 대상으로 한 민간의료보험 가입 개수는 연령, 가구총소득, 주관적 건강상태, 음주, 무료 1차 건강검진 수혜에 영향을 받았고, 민간의료보험 월 평균 납부액은 연령, 결혼생활상태, 경제활동상태, 주관적 계층의식, 가구총소득, 음주, 입원에 대한 영향을 받는 것으로 나타났다. 추후 본 연구가 민간의료보험의 역할을 제시하고, 지속적으로 추진되고 있는 건강보험 보장성 강화 정책에 방향성을 제시하는 데 도움이 될 것으로 기대된다.

자살 충동 예방을 위한 음악치료적 접근 (Music Therapy Interventions for Prevention of Suicidal Ideation)

  • 근래 자살예방에 대한 관심이 증가되면서 이에 대한 실질적인 전략들이 많이 개발되었다. 특히 서구에서는 지역사회 정신건강 기관에서 자살예방을 위한 치료 프로그램이 특성화되어 운영되는 것은 물론, 대학 및 병원과 연계하여 자살 충동의 원인이 되는 다양한 정신 병리적 혹은 사회 심리적인 원인들을 규명하고 이에 대한 계속적인 연구가 시행되고 있다. 본 연구에서는 음악치료가 자살의 중요한 요인인 우울에 미치는 영향을 중심으로 하여 자살예방을 위한 음악치료적인 접근을 제시하고자 하였다. 접근방법으로는 노래심리치료(Song Psychotherapy), MI(Music Imagery), 역할악기즉흥연주(Role Play Instrumental Improvisation)심리치료를 주요방법으로 구성한 음악심리치료 프로그램이 제공되었다. 연구대상은 우울을 동반한 정신적 외상상태로 자살을 시도했던 30대 여성이었으며 치료기간은 주 1회 90~120분의 7회의 세션이 실시되었다. 음악치료 전개에 따라 우울척도와 자살생각척도를 통해 변화를 양적 질적으로 연구 방법들을 사용하였다. 연구 결과 내담자는 음악심리치료를 통해 안정감과 편안함을 느끼고 방어나 저항 없이 자신의 감정의 생각, 그리고 내면의 세계를 탐구할 수 있었다. 이 경험들을 통하여 내담자는 자아개념과 새로운 가치를 발견하게 되고 부정적인 정서가 긍정적인 정서로 바뀌게 됨에 따라 우울감이 감소되었다. 또한 자살에 대한 생각보다는 현재 삶에 대한 구체적인 계획을 행동으로 옮기게 되었다. 특히 역할악기 즉흥연주 과정의 음악은 기분의 변화, 타인과의 관계변화, 태도의 변화 등 다양한 영역변화를 가져오는 주체로서 '치료' 그 자체로 사용되어졌다. 그리고 음악은 해결되지 않은 정신적 외상상태에 있었던 내담자의 심리적 위축과 긴장감, 무기력한 에너지 등을 표출시키고 본능과 감정을 그대로 경험할 수 있도록 하였다. 더 나아가 '거짓자아'와 분리되어 '참 자아'와의 통합을 이루어가도록 도움을 주었다. 결과적으로 음악심리치료는 우울을 감소시키고 자살충동 예방에 효과적으로 적용될 수 있음을 시사하고 있다.

전자상거래에서의 소비자보호에 관한 법.제도적 연구 (A Legal and Systematic Study On Consumer Protection In Electronic Commerce)

  • 전자상거래는 1990년대 후반 이후부터 폭발적인 성장을 거듭해 가고 있다. 그러나 전자상거래의 비대면성과 익명성, 국제성, 일방성, 현혹가능성 등과 같은 특성으로 인하여 소비자의 피해가 증가하고 있다. 따라서 본 논문에서는 소비자보호를 위한 법 제도적 개선점을 다음과 같이 종합적으로 도출해 보았다. 첫째, 전자상거래소비자보호법상 청약철회권 행사기간을 연장할 필요가 있다. 둘째, 전자상거래는 비대면적으로 이루어지기 때문에 소비자가 착오를 일으킬 가능성이 높으며, 소비자가 자신의 착오를 입증하기가 용이하지 않으므로 사전에 착오가 발생하지 않도록 정확한 정보가 제공되도록 해야 한다. 셋째, 전자약관에 대한 인증마크 제도를 도입하여야 한다. 넷째, 소비자의 안전한 개인정보관리를 통한 개인정보유출과 이용에 대한 불안감이 없도록 법적, 제도적 그리고 기술적 대책이 강구되어야 한다. 다섯째, 인터넷 신뢰마크에 대한 엄격한 감독이 요구된다. 마지막으로 온라인 조정에 있어서 절차상 전송된 메시지나 문서에 대한 해킹 등을 통하여 제3자의 개입과 간섭이 있을 수 있으므로 당사자간의 명확한 인증을 통하여 시스템상에서의 보안성을 높일 필요가 있다.

고등학생의 학교폭력에 영향을 미치는 요인에 관한 연구 : 인성요인을 중심으로 (A study on factors affecting high school students of school violence - Focusing on personality factors -)

  • 학교는 학생의 효과적인 학업성취와 지적능력 개발을 위해 공교육을 실현하는 거점임과 동시에 최전선의 위치를 담당하고 있다. 비단 학업뿐만 아니라 사회의 한 구성원으로써 타인과 함께 하기위한 배려와 이해의 폭을 넓혀주는 공간이기도 하다. 그러나 우리나라 학교교육의 현실은 타인을 배려하고 책임감을 느끼는 삶의 태도와 관련된 인성교육에 대해서는 부족한 면이 많다는 지적을 받고 있다. 한 개인이 성인으로 성장해 나가는 데에는 지적능력뿐만 아니라 정서적인 측면이 반드시 필요하고, 학교에서는 그와 관련된 다양한 방법을 가르치는 것이 의무화되어 있다. 그럼에도 불구하고 우리나라 중등교육은 교사들의 과도한 행정업무를 비롯한 기타 업무로 인해 인성교육의 과정이 소홀해 지거나 형식적인 활동을 벗어나지 못하고 있다. 이러한 영향으로 학생들의 문제적 행동을 개선하지 못하는 결과를 가져왔으며, 결국 학교폭력과 같이 타인에게 직접적인 피해를 주는 심각한 사회문제를 야기한 측면이 있다. 따라서 이 연구는 교육학과 범죄학의 청소년비행 행위에 기초하여 인성의 개념을 구체화하고 학교폭력과의 관계를 경험적 연구를 통해 검증하고자 하였다. 이에 따라 2013년 7월 1일부터 2013년 9월 31일 까지 전국 277개교에 재학 중인 고등학교 3학년 학생들 중 경기도에 소재한 K대학의 입학설명회 및 모의전형에 참여한 학생들 중 무응답과 분석에 사용할 수 없다고 판단된 자료를 제외하고 최종분석에서 총 1045명의 자료가 활용되었다. 그 결과 학교폭력 피해경험이 많고, 남자학생일 수 록 학교폭력의 가해 경험이 많을 것으로 나타났다. 또한 건강한 학생일수 록 학교폭력의 가해경험이 많고, 분노조절과 공감, 배려의식이 낮은 학생이 학교폭력의 가해경험이 많은 것으로 나타났다.

임상실습 교육개선을 위한 일 실습지도자 활용모델 (preceptorship model)의 적용 및 효과에 관한 연구 -암센타, 재활센타, 중환자실 실습을 중심으로- (Application and Effectiveness of a Preceptorship for the Improvement of Clinical Education)

  • Clinical practice in nursing education provides an opportunity for students, through the process of ap-plying theoretical knowledge to practice, and to learn nursing skills as well as being socialized into nursing and as such decrease the reality shock of actual nursing practice. Because of a shortage of nursing faculty, the job of achieving the objectives of the clinical practice had been turned over to the head nurses. This resulted in many problems, such as, unclear location of responsibilities and inadequate feedback from head nurses. Therefore this study was done to introduce and evaluate the use of preceptors as a way to minimize the above problems, and to maximize the achievement of the clinical practice objectives. Using an adaptation of Zerbe's (1991) three-tiered team model, clinical practice was done using a preceptor, a head nurse and a clinical instructor, each with different and well defined roles. The subjects of this study were 67 senior students of the College of Nursing of Y University in Seoul whose clinical practice in adult nursing was carried out between May 1, 1994 and December 8, 1994. There were 22 preceptors who had at least two years of clinical experience and who were recommended by their head nurses. They were given additional education on the philosophy and objectives of the College of Nursing, on communication skills, on the theory and practice of education, and on nursing diagnosis and education evaluation. The role of the preceptor was to work one-to-one with students in their practice. The role of the head nurse was to supervise and evaluate the preceptors. The role of the clinical instructor was to provide the education program for the preceptors, to provide ad-vice and suggestions to the preceptors and to maintain lines of communication with the college. With each of these roles in place, it was thought that the effectiveness and efficiency of the clinical practice could be increased significantly. To evaluate the effectiveness of the preceptorship, the three - tiered model, Lowery's Teacher Evaluation Opinion Form translated and adapted to Korea was used to measure student statisfaction. The Clinical Practice Compentency Evaluation Tool developed by Lee et ai was also used to measure student competencies. The results of this study are as follows 1. The satisfaction with clinical practice was higher with the introduction of the perceptors than it was before they were used. (t=-5.96, p=<.005) 2. The clinical practice competencies were higher with the introduction of the preceptors than it was before they were used(t=-5.l3, p<.005) 3. In order to analyze areas not measured by the quantitative tools additional analysis of the open questions was done. The results of this analysis showed that : 1) The students felt positive about their sense of security, confidence, handling of responsbility, and being systematic. They also felt positive about improvements in knowledge, opportunities for direct care, and socialization. 2) The students felt negative about the technical part of their role, lack of knowledge by the preceptor, unprofessional attitudes on the part of the preceptor, difficulty in the role of the professional nurse(student). 3) The preceptors felt positive about their responsibility, motivation, and relationship with the college. 4) The preceptors felt negative about their bur-den. Introduction of the preceptorship model will lead to change and improvement in the negative factors discussed above, solve problems in the present clinical education system, increase continuity in the education of the students, help with socialization of the students and motivation of the preceptors to up-grade their education and increase their confidence. These objectives must be obtained to further the development of professional nursing, and thus, making the preceptorship a reality is our job for the future.

임대주택단지의 생활안전 위해요인 해소방안 (Resolution Method of Hazard Factor for Life Safety in Rental Housing Complex)

  • The government has been constructing and supplying public rental housing to ordinary people in order to stabilize housing since 1989. However, the public rental houses initially supplied to ordinary people are at high risk for safety accidents due to the deterioration of the facilities. Therefore, this study is aimed to propose a solution to solve the life safety hazards of the old rental housing complex as a follow-up study of Analysis of Accident Patterns and Hazard Factor for Life Safety in Rental Housing Complex. Types of life safety accidents that occur in public rental housing complexes are sliding, falling, crash, falling objects, breakage, fire accidents, traffic accidents and criminal accidents. The types of safety accidents that occur in rental housing complexes analyzed in this study are sliding, crashes, falling objects, and fire accidents. Although the incidence of safety accidents such as falling, breakage, traffic accidents and crime accidents in public rental housing complexes is low, these types are likely to cause safety accidents. The method of this study utilized interviews and seminar results, and it suggested ways to solve the life safety hazards in rental housing complexes. Interviews were conducted with residents and managers of rental housing complexes. Seminars were conducted twice with experts in construction, maintenance, asset management, housing welfare and safety. Through interviews and seminars, this study categorizes the life safety hazards that occur in rental housing complexes by types of accidents and suggests ways to resolve them as follows. (1) sliding ; use of flooring materials with high friction coefficient, installation of safety devices such as safety handles, implementation of maintenance, safety inspections and safety education, etc. (2) falling ; supplementation of safety facilities, Improvement of the design method of the falling parts, Safety education, etc. (3) crash ; increase the effective width of the elevator door, increase the effective width of the lamp, improve the lamp type (U type ${\rightarrow}$ I type), etc. (4) falling objects and breakage ; design of furniture considering the usability of residents, replacement of old facilities, enhancement of safety consciousness of residents, safety education, etc. (5) fire accidents ; installation of fire safety equipment, improvement by emergency evacuation, safety inspection and safety education, etc. (6) traffic accidents ; securing parking spaces, installing safety facilities, conducting safety education, etc. (7) criminal accidents; improvement of CCTV pixels, installation of street lights, removal of blind spots in the complex, securing of security, etc. The roles of suppliers, administrators and users of public rental housing proposed in this study are summarized as follows. Suppliers of rental housing should take into consideration the risk factors that may arise not only in the design and construction but also in the maintenance phase and should consider the possibility of easily repairing old facilities considering the life cycle of rental housing. Next, Administrators of rental housing should consider the safety of the users of the rental housing, conduct safety checks from time to time, and immediately remove any hazardous elements within the apartment complex. Finally, the users of the rental housing needs to form a sense of ownership of all the facilities in the rental housing complex, and efforts should be made not to cause safety accidents caused by the user's carelessness. The results of this study can provide the necessary information to enable residents of rental housing complexes to live a safe and comfortable residential life. It is also expected that this information will be used to reduce the incidence of safety accidents in rental housing complexes.

  • One of the latest fields of research in the area of output devices is tactual display devices [13,31]. These tactual or haptic devices allow the user to receive haptic feedback output from a variety of sources. This allows the user to actually feel virtual objects and manipulate them by touch. This is an emerging technology and will be instrumental in enhancing the realism of wearable augmented environments for certain applications. Tactual displays have previously been used for scientific visualization in virtual environments by chemists and engineers to improve perception and understanding of force fields and of world models populated with the impenetrable. In addition to tactual displays, the use of wearable audio displays that allow sound to be spatialized are being developed. With wearable computers, designers will soon be able to pair spatialized sound to virtual representations of objects when appropriate to make the wearable computer experience even more realistic to the user. Furthermore, as the number and complexity of wearable computing applications continues to grow, there will be increasing needs for systems that are faster, lighter, and have higher resolution displays. Better networking technology will also need to be developed to allow all users of wearable computers to have high bandwidth connections for real time information gathering and collaboration. In addition to the technology advances that make users need to wear computers in everyday life, there is also the desire to have users want to wear their computers. In order to do this, wearable computing needs to be unobtrusive and socially acceptable. By making wearables smaller and lighter, or actually embedding them in clothing, users can conceal them easily and wear them comfortably. The military is currently working on the development of the Personal Information Carrier (PIC) or digital dog tag. The PIC is a small electronic storage device containing medical information about the wearer. While old military dog tags contained only 5 lines of information, the digital tags may contain volumes of multi-media information including medical history, X-rays, and cardiograms. Using hand held devices in the field, medics would be able to call this information up in real time for better treatment. A fully functional transmittable device is still years off, but this technology once developed in the military, could be adapted tp civilian users and provide ant information, medical or otherwise, in a portable, not obstructive, and fashionable way. Another future device that could increase safety and well being of its users is the nose on-a-chip developed by the Oak Ridge National Lab in Tennessee. This tiny digital silicon chip about the size of a dime, is capable of 'smelling' natural gas leaks in stoves, heaters, and other appliances. It can also detect dangerous levels of carbon monoxide. This device can also be configured to notify the fire department when a leak is detected. This nose chip should be commercially available within 2 years, and is inexpensive, requires low power, and is very sensitive. Along with gas detection capabilities, this device may someday also be configured to detect smoke and other harmful gases. By embedding this chip into workers uniforms, name tags, etc., this could be a lifesaving computational accessory. In addition to the future safety technology soon to be available as accessories are devices that are for entertainment and security. The LCI computer group is developing a Smartpen, that electronically verifies a user's signature. With the increase in credit card use and the rise in forgeries, is the need for commercial industries to constantly verify signatures. This Smartpen writes like a normal pen but uses sensors to detect the motion of the pen as the user signs their name to authenticate the signature. This computational accessory should be available in 1999, and would bring increased peace of mind to consumers and vendors alike. In the entertainment domain, Panasonic is creating the first portable hand-held DVD player. This device weight less than 3 pounds and has a screen about 6' across. The color LCD has the same 16:9 aspect ratio of a cinema screen and supports a high resolution of 280,000 pixels and stereo sound. The player can play standard DVD movies and has a hour battery life for mobile use. To summarize, in this paper we presented concepts related to the design and use of wearable computers with extensions to smart spaces. For some time, researchers in telerobotics have used computer graphics to enhance remote scenes. Recent advances in augmented reality displays make it possible to enhance the user's local environment with 'information'. As shown in this paper, there are many application areas for this technology such as medicine, manufacturing, training, and recreation. Wearable computers allow a much closer association of information with the user. By embedding sensors in the wearable to allow it to see what the user sees, hear what the user hears, sense the user's physical state, and analyze what the user is typing, an intelligent agent may be able to analyze what the user is doing and try to predict the resources he will need next or in the near future. Using this information, the agent may download files, reserve communications bandwidth, post reminders, or automatically send updates to colleagues to help facilitate the user's daily interactions. This intelligent wearable computer would be able to act as a personal assistant, who is always around, knows the user's personal preferences and tastes, and tries to streamline interactions with the rest of the world.

암환자의 희망 예측요인 (Predictive Factors of Hope in Patients with Cancer)

  • It has been believed that cancer is an omnious factor threatening the future and life itself. Patients having the disease experience anxiety, fear, feeling of weakness, depression and feelings of uncertainty and hopelessness. Most cancer patients, however, have expectations of possible recovery and a better future, very different from the patients who feel hopeless. Therefore. hope allows people to respond effectively to the fatal disease they have and prevents them from detoriorating physically and spiritually, positively influencing their survival, response to treatment and sense of security. Studies previously performed showed that hope is positively correlated with social and family supports, self-esteem, spiritual well-being, responsive action, health promotion behavior and quality of life. Thus, the study attempted to provide basic information on nursing cancer patients by investigating their levels of hope and determining predictive factors which influence hope. For the study 200 cancer patients in two university hospitals located in Pusan were sampled as subjects. Data were collected for twenty nine days from Feburary 1, 1999 to March 1. Instrumets for the study included 10 items from the self-esteem scale by Rosenberg (1965), 39 hope measurements by Kim and Lee(1965), 16 of the social support scale by Tae(1986) and 16 of the general characteristics scale, all of which totaled 81 items. The data were analyzed using the SPSS program. General characteristics of the investigated based on numbers and percentage. Hope, self-esteem and social support were analyzed using means, minimum, maximum and standard deviation. Relations among the foregoing three factors were analyzed using Pearson' correlation coefficient. Levels of hope in cancer patients were determined using t-test, ANOVA and Scheffe test. Predictive factors influencing hope were investigated using multiple stepwise regression analysis. Results of the study are summarized as follows: 1. An average level of hope was $185.55{\pm}23.39$ points(96 min. and 234 max.) 2. Levels of hope showed a significant difference among them according to sex (t=-3.69, P=.000), age(F=4.714, P=.000), job(F=3.247, P=.008), monthly income (F=6.113, P=.003), treatment charge (F=3.796, P=.011), supportive resources (F=10.554, P=.000), diagnosis(F=2.287, P=.029), perceived health status(F=22.184, P=.000), level of pain(F=3.334, P=.021), religion (F=4.911, P=.001) and religion's effect in life (F=11.706, P=.000), 3. For the subjects, self-esteem and social support were $38.32{\pm}7.21$(13 min, and 50 max.) and $52.97{\pm}8.49$points(28 min, 80 max.). Concerning social support, average levels of family support and medical support were found $35.95{\pm}6.05$(18 min, and 40 max) and $27.02{\pm}4.99$ points(20 min and 40 max). The hope the cancer patients showed significant correlations with self-esteem (r=.588, P=.000), family support(r=.224, p=.001) and medical support(r=.221, P=.002). 4. The five variables related to hope (self-esteem, religion's effect in life, perceived health status, social support and age) accounted for 54.2 percent of the hope level; especially, self-esteem was the highest at 34.6%. As shown in the above results, predictive factors which most influence hope in cancer patients were self-esteem and religion's effect of life. Therefore, nursing interventions to increase self-esteem should be developed. Regarding religion's effects, studies on spiritual aspects should be carried out in a way that contributes to promotion of hope.
