Purpose: This study was conducted to develop and test an explanatory model on functional capacity in patients with chronic obstructive pulmonary disease using path analysis. Methods: Data were collected from 149 chronic obstructive pulmonary disease patients using 6-minute walk test, measurement of oxygen saturation, pulmonary function test, and self-reported questionnaires from June to October, 2005. The collected data were analyzed using SPSS/WIN 12.0 program and AMOS/WIN 4.0 program. Results: The overall fitness indices of modified model were good($x^2$ = 14.324, p = .281 GFI = .981, RMSEA = .006, AGFI = .944, NFI = .927, NNFI = .999, CFI = .999, PNFI = .613, $x^2$/df = 1.194). Functional capacity was influenced directly by age(${\beta}$ = -.304, p = .000), dyspnea(${\beta}$ = -.278, p = .000), self-efficacy(${\beta}$ = .240, p = .000), social support(${\beta}$ = .175, p = .004), pulmonary function(${\beta}$ = .169, p = .008), and oxygen saturation(${\beta}$ = .099, p = .048). These variables explained 39.3% in functional capacity. Conclusion: The findings of this study suggest that comprehensive nursing interventions should focus on decreasing dyspnea and increasing self-efficacy, social support, and oxygen saturation. In this perspective, pulmonary rehabilitation would be an effective strategy for improving functional capacity in patients with chronic obstructive pulmonary disease.
Background: Truck driving is a highly sedentary occupation that places workers at risk for chronic health conditions, such as obesity and high blood pressure. The primary purpose of this study was to objectively describe truck drivers' typical physical activity (PA) patterns. Methods: We used ~7-10-day baseline PA actigraphy data samples from drivers in the Safety & Health Involvement For Truckers (SHIFT) study (n = 394). Driver PA patterns (e.g., average number of ≥10 minute Freedson bouts per week, time in bouts, and common days/times for PA) were summarized with descriptive analyses. We also compared objective accelerometer data to self-reports. Results: Drivers' weekly PA averaged 14.4 minutes (SD = 37.0), and most PA occurred between 5-6 pm on Tuesdays and Wednesdays. Drivers overestimated self-reported weekly exercise by over 60 min/week compared to accelerometer data. Conclusion: Our results suggest that objective PA assessment may be warranted over self-report when possible, and timing may be key in future PA intervention work with truck drivers.
Purpose: Reported insomnia and decreased sleep quality are common among older adults. The purpose of this study was to identify factors contributing to insomnia and decreased sleep quality among elderly persons living within a community. The data will be verified with Actigraph which is a non-invasive measurement of rest/activity cycles. Methods: 137 participants over 65 years old were recruited from a senior welfare center. Their mean age was $74.07{\pm}5.18years$ old and majority was women (69.3%). The quality and quantity of nighttime sleep were assessed using Varren-synder-Halper Sleep Scale and a sleep log. Depression and fatigue were assessed using Geriatric Depression Scale Short Form and Fatigue Severity Scale, respectively. Pain was assessed using 11-point Numeric rating scale. An actigraph was used to verify the self-reported sleep. Results: Ninety-two participants (67.2%) reported insomnia. Sixty-eight subjects (49.6%) reported good sleep. Of the participants, 56.2% reported depression and 33.6% reported fatigue. In logistic regression, depression was related to insomnia (OR=3.09, p=.003). Sleep quality was related to depression (OR=2.13, p=.045) and fatigue (OR=2.24, p=.044). The data from the Sleep logs correlated with data obtained from the actigraph. Conclusion: Depression significantly influenced insomnia and sleep quality whereas fatigue only influenced sleep quality. Thus, depression and fatigue should be closely monitored among elderly persons with sleep disturbances.
Purpose: We gathered information for the development of a blood pressure measurement education program by analyzing the accuracy of reading taken using a blood pressure simulator by Korean paramedic students. Methods: Data from 131 students were collected in November 12-20, 2013, and April 2-4, 2014. A 27-item questionnaire was administered, the accuracy of measurements confirmed using a blood pressure simulator (BT-CEAB), and the data analyzed (SPSS v 21.0). Results: The accuracy of systolic and diastolic blood pressure readings (${\leq}2mmHg$) was relatively low (27.5%). The mean blood pressure knowledge score was 67.61 points; significant differences were noted considering the sex (p = .001), hours of practice (p =.007), numbers of practice (p = .001), and reported self-confidence (p = .026). The blood pressure measurement accuracy group did not show a significant difference in its knowledge of blood pressure (p = .198). Conclusion: Most subjects needed several practice sessions to master the skill of measuring blood pressure. The feedback provided by individual assessment and the practice education program will serve as the basis for clinical and prehospital practice.
Purpose: The purpose of this study was to determine if osteoarthritis patients would benefit in terms of pain, fatigue, difficulty with physical activity, joint stiffness, and flexibility of the joints from a structured self-help program. Method: This self-help program was carried out 2-3 hours once a week for 6 weeks in 2005-2006, and evaluated in one group pretest-posttest pre-experimental design. The subjects of this study who were diagnosed osteoarthritis were recruited at two different Community Health Centers in Kangwon. The subjects who agreed with the purpose of this study and participated both pretest and post-test were 55 patients. Mean age is 63.48 (9.48) years, mean duration of disease is 7.95 (7.66) years. The self-help program was consisted of weekly health contract, exercise, health education, group discussion, group counseling, and recreation. At every meeting, researcher and trained public health nurse evaluated the program, and prepared the next program. The measurement tools were pain rating scale (0-10), fatigue rating scale (0-10), Korean WOMAC (Western Ontario and McMaster University Osteoarthritis) Index, ruler, and goniometer. Results: At the completion of 6 weeks of self-help program, the subjects reported significantly less pain and difficulty with physical activity and more flexibility in both shoulder and knee joints compared to pretest. Conclusion: The self-help program would be helpful on pain, physical activity, and joint flexibility for arthritis patients.
To identify osteoporosis and examine the relationship between health behavior and the self-efficacy of middle-aged women, a descriptive survey was conducted. The subjects were 465 healthy female residents of Kyunggido, Korea, who underwent ultrasound measurement and a health examination at the Kangmeung-Ci Health Center between July and August 2000. They were 40 to 60 years old, with a mean age of 46.7 years. The subjects completed a self-reported questionnaire that consisted of seven items concerning health behavior and twelve items concerning self-efficacy related to osteoporosis. Bone density was evaluated from ultrasound measurements of the right heel. All the data were analyzed using the program SAS-PC. The study found that the mean T score related to bone density was -1.30, and ranged from -3.52 to 3.06. Based on the T score, 74.8% of the subjects were normal, 12.9% had osteopenia, and 12.3% had osteoporosis. The mean osteoporosis self-efficacy score was 41.17 and ranged from 12 to 60. Osteoporosis self-efficacy differed significantly with health behavior related to health supplementary food (t=5.63, p=.018), exercise (t=6.65, p=.010), alcohol drinking(t=10.80, p=.001), and smoking (t=10.23, p=.001). A community-based health promotion program should be developed to prevent osteoporosis in middle-aged women.
This study performed to identify the relationship between job stress and self-efficacy based on the related research review and text network analysis. For the literature review, we performed the search process at three domestic and one foreign database using key words, 'nurse', 'stress', 'self-efficacy'. A total of 18 papers were selected as the target literature. Nine of these studies reported a statistically significant negative correlation between nurses' job stress and self-efficacy. It was difficult to compare between studies' results because of the optional usage of the questionnaires. In addition, a text network analysis was conducted by extracting keywords from the 18 papers. The keyword with the highest frequency of appearance was job stress, and the main words with high frequency of emergence were self-efficacy, hospital, and correlation. To clarify the relationship between the keywords, it is proposed to perform a survey on the influence factors through the development of Korean version measurement.
This study was done to analyze the trends of research on coping in Korea, to suggest future direction, for research on coping, and ultimately to contribute to an increase in explanation of adaptation. This article reviewed 79 nursing research papers on coping done since 1978 by examining them according to the period of publication or presentation, research design, type of subjects, measurement instruments, research for a degree or not, range of reliability, and association of coping and related variables. The results are as follows : The number of studies on coping increased rapidly from the mid-1980's and decreased slowly from the mid-1990's. The maority of the studies were surveys, comparative studies, or correlational studies. The subects of the 46 studies were healthy people, while those in the remaining studies were patients with a variety of illnesses. Thirth-eight studies on coping were done for master's thesis, three for dissertion, and 38 were not degrees. The Bell and Jalowiec coping scales have not been used since the early 1990's. In contrast, Lazarus and Folkman's W.C.C.L. has been used increasingly since that time. The reliabilities of the coping scale were reported in 37 cases and the Cronbach's alpha coefficients were .71 to .86. All subjects reported using more problem-oriented coping than emotion-oriented coping in short-term or emotion-oriented coping and healthy groups did more long-term coping. It was difficult to describe consistently the relationship between stress and coping according to the type of coping scale or research subjects, but generally moderate relationships were found. This was due to instrumental problems and no consideration of situational context. The subject group who used more short-term coping and less long-term coping reported poorer mental status, and higher scores in burnout and state anxiey than others. That is, the relationship between stress and adaptation increased the power of explanation with intervening the mediating effect of coping. The association of locus of control, mastery, social support, and self-concept with coping showed positive relationships : those of uncertainty and severity in illness with coping showed negativerelationships ; those of state anxiety and depression with short-term coping were positive, and those of self-esteem with long-term coping or problem-oriented coping were negative. There were significant differences in the scores of types of coping according to religion, level of education, and socio-economic status. That is, Presbyterians and Catholics, those with higher education levels and higher socio-economic status used more long-term or problem-oriented coping. On the basis of the above findings the following recommendations are made : 1. There is a need to test the mediating effect of coping variable in order to clarify the concept. 2. Longitudinal studies are needed to determine the patterns of change in coping strategies when stressful events are encountered. 3. It's necessary to develop a reliable and variable measurement tool for coping. 4. There is a need to identify subscales of coping to increase explanation of variance 5. It's necessary to consider personal, situational, and antecedent variables : the characteristics of subject populations, the natures of illness and treatment situations. 6. The power of explanation of studies designed to identify the stress-adaptation process should be increased using the combination model of process-oriented coping and cognitive-structural model.
Objective: The purpose of this study is to verify the structural relationships among work-family balance, resilience, job satisfaction, parenting self-efficacy and happiness of double income couples with 3~5 years child(ren). Method: This study distributed structured self-reported questionnaires and used the data of 229 couples with 3~5 years child(ren) for an analysis. A confirmatory factor analysis was conducted for checking the measurement unity and a structural equation model was performed to reveal the structural relationship using AMOS software. Results: In a structural relationships among work-family balance, resilience, job satisfaction, parenting self-efficacy and happiness, a total of 13 paths appeared significant. The influence of the husband on the wife was more significant than that of the wife on the husband, and resilience was found to have a strong influence on the happiness of wife and husband. The work-family balance was different for wife and husband. Conclusion: These results suggest that it is necessary to develop various programs and approaches for the development of resilience to promote their happiness. As a policy direction in the future, with a goal of a shift to the paradigm, 'raising children together,' an institutional support should be prepared for the concretization of the formation and practice of the value of co-parenting children.
The purpose of this study is to compar self-reported fatigue and fatigue-regulating behaviors of rheumatoid arthritic patients and normal persons. This study collected the data from 75 rheumatoid arthritic patents visited the departments of internal medicine or orthopedics of four general hospitals T-city and K-city by means of direct interview and questionnaires. in this study also collected data from 75 normal persons who had not been exposed to any other disease in T-city and K-city by means of direct interviews anti questionnaires which were conducted by two trained nurses. This experiment was conducted from August 1, 1998 to October 15, 1998. This study used both MAP(Multi-Dimentional Assessment of Fatigue) developed by Belza(1995) to measure fatigue and the measurement developed by Kwon, Young-Eun to investigate fatigue regulating behaviors. The collected materials were analyzed by means of descriptive statistics, t-test, and the ANCOVA according to the SPSS PC+ program. The findings are as follows : 1. There was the statistically significant difference(t =5.07, p=.000), between rheumatoid arthritic patients(32.76 points) and normal persons(25.81 points) in t-test comparison by group about fatigue. A fatigue degree of rheumatoid arthritic patient group was high in five kinds of lower realms such as common fatigue degree, fatigue severity to be experienced, distress due to fatigue, daily fatigue degree, and fatigue timing at the last week by dimension. 2. There was the significant difference in the number of fatigue-regulating behaviors between rheumatoid of fatigue arthritic patients(9.37 times) and normal persons (8.15 times), but there wasn't any significant difference in the efficiency between rheumatoid arthritic patients(2.85 points) and normal persons (2.78 points) This research suggests two kinds of things as follows : 1. It is necessary to develop an educational program for improving efficiency of fatigue-regulating behaviors as well as some nursing arbitration measures for reducing fatigue of rheumatoid arthritic patients. 2. It is necessary for the future studies to continuously grasp characteristics of fatigue by gender variable by selecting more rheumatoid arthritic male patients.
