Nonpharmacological management and psychosocial support for children and adolescents with type 1 diabetes

  • Compared to that in the Caucasian population, type 1 diabetes mellitus (T1DM) incidence rates are very low in Koreans. Therefore, compared to the recent development of pharmacological therapy applicable to Korean children with T1DM, interest in nonpharmacological therapy and psychosocial support systems remains low, as is the development of Korean-style T1DM education programs for therapeutic application. Children who have been newly diagnosed with diabetes are placed in completely new environments for treatment. For appropriate control of diabetes, patients have to self-monitor blood glucose levels and inject insulin several times a day and must use extreme self-control when they eat foods to avoid increases in blood glucose levels. Blood glucose excursions resulting from impaired pancreatic ${\beta}$ cell functions cause mental stress due to vague fears of chronic complications of diabetes. In addition, children with diabetes cannot be excluded from the substantial amount of studies required of Korean adolescents, and the absolute shortage of time for ideal control of diabetes adds to their mental stress. Many of these patients are psychologically isolated in school where they spend most of their time, and they are not appropriately considered or supported with respect to blood glucose control in many cases. In this respect, this author will introduce some of the newest views on nonpharmacological therapy and psychosocial support systems that account for important parts of T1DM management and seek measures to apply them in conformity with the social characteristics of Korea.

간호대학생의 지각된 스트레스, 자아존중감, 전공만족도가 시험불안에 미치는 영향 (Effect of perceived stress, self esteem, major satisfaction on the test anxiety of nursing students)

  • 본 연구는 간호대학생을 대상으로 지각된 스트레스, 자아존중감, 전공만족도가 시험불안에 미치는 영향을 확인하여 시험불안을 조절하기 위한 기초자료를 제공하고자 시도되었다. 연구참여자는 B시의 간호대학생 251명을 대상으로 하였다. 자료분석은 SPSS 22.0 프로그램을 이용하여 실수, 백분율, 평균, 표준편차, t-test, ANOVA, 피어슨 상관계수, 다중회귀분석으로 분석하였다. 대상자의 시험불안은 2.97±.54점이었고, 일반적 특성에 따른 시험불안의 차이를 분석한 결과 성별(t=-3.241, p=.001), 대학생활만족도(F=5.682, p=.004)에서 유의한 차이를 나타냈다. 연구대상자의 시험불안은 지각된 스트레스(r=.361, p<.001)와는 정적상관관계, 자아존중감(r=-.186, p=.003)과는 부적상관관계를 나타냈다. 대상자의 시험불안에 영향을 미치는 요인은 지각된 스트레스, 성별로 나타났고, 설명력은 14%로 확인되었다. 본 연구결과를 통하여 간호대학생을 대상으로 시험불안에 영향을 미치는 요인을 확인함으로써 시험불안을 중재할 수 있는 기초자료로 활용할 수 있을 것이다.

주장훈련 프로그램을 적용한 국내학위논문 분석 (An analysis of studies on Assertiveness Training reported in Korea from 1982 to 1997.)

  • The purpose of this study is analysis of 52 dissertations on assertiveness training which presented from 1982 to 1997 in Korea. It was diversified the research purpose to by 1) time of publication or presentation 2) research design, characteristics of subjects used in each study 3) related concepts, the effects of nursing interventions according to related concepts 4) the effects of nursing interventions according to the subjects 5) the effects of nursing interventions according to the frequency and the period of assertiveness training 6) the effects of nursing interventions according to the group size. 7) the effects of nursing interventions according to the necessary time per round. Findings obtained in the study were as follows : 1. At the point of the numbers of studies presented by year, the number of the studies tend to increase, especially the most plentiful papers were presented during five years from 1991 to 1995. 2. Research design of the studies belong to experimental research. The subjects of research consists of 5 Nursing related subjects and 47 Non-nursing related subjects so that Non-nursing related subjects contain much large proportion. 3. When studies were classified by the related concepts, they were classified that 17 studies were by assertiveness & assertive behaviour, 15 ones anxiety, 9 ones self & ego, 7 ones depression. It can be inferred that studies related to assertive behaviour, self & ego, sociality and power of learning were increasing. Otherwise, studies related to anxiety, depression, aggression, shyness and anguish were decreasing. 4. At the point of the nursing intervention effect by the subjects, the concepts of assertive behavior, feeling of self-respect and sociality were increasing, but the concepts of conflict and interpersonal stress were decreasing and the concepts of anxiety and sociality were ineffective in the subjects related Nursing. On the contrary, the concepts of assertive behavior, sociality and personalty were increasing, but the concepts of anxiety, depression, aggression, shyness, self-estranged feeling were decreasing and the concepts of assertive behavior, aggression, personalty, power of learning, anxiety, self-concepts and self-expression were ineffective in the subjects related Non-Nursing. 5. 18 studies can be found, which investigated 10 times for 5 weeks from the point of the nursing intervention effect by the times and the period of assertiveness training. In case of over 5 times of assertiveness training, assertiveness, assertive behavior and personality(tranquility, depression) increased. In 6 times, feeling of self-respect & egorespect increased but depression, interpersonal stress and anxiety decreased. In over 10 times, shyness, shame and feeling of self-estrangement decreased. In over 12 times, interpersonal anxiety decreased. 6. In the event of nursing intervention effect by the size of the group, it was greatest within 10 persons in 21 studies. In case of within 10 to 20, only feeling of self-expression were ineffective. Particularly, in case of over 31 persons, there were no ineffective concepts. 7. At the viewpoint of nursing intervention effect by the necessary time of one time, it was greatest within 1 to 2 hours in 32 studies. In case of within 1hour, aggression, power of learning and assertiveness were ineffective. In case of within 1 to 2 hours, assertive behavior, self-expression, personality and anxiety were ineffective. Particularly, in case of within 2 to 3 hours, there were no ineffective concepts.

간호여대생의 자아존중감, 식이 자기효능감, 생활스트레스가 식행동에 미치는 영향 (Effects of self esteem, dietary self-efficacy and life stress on dietary behavior of female nursing students)

  • 본 연구는 간호여대생의 식행동의 정도를 알아보고, 자아존중감, 식이 자기효능감, 생활스트레스가 식행동에 미치는 영향을 알아보기 위하여 시도되었다. 연구대상자는 B, C시의 간호여대생 442명을 대상으로 하였다. 자료수집은 2016년 5월 15일부터 5월 30일까지 였다. 자료분석은 SPSS WIN/21.0을 이용하여 평균과 표준편차, t-검정, 일원분산분석, 피어슨 상관계수, 다중회귀분석을 사용하였다. 연구결과 간호여대생의 식행동은 $3.01{\pm}0.49$점으로 나타났다. 간호여대생의 일반적 특성에 따른 식행동의 차이에서 식행동은 학년(F=4.823, p=.008), 주거형태(F=6.067, p<.001), 성적(F=3.907, p=.021), 건강상태(F=7.975, p<.001), 건강에 대한 관심도(F=4.189, p=.016)에서 유의한 차이를 나타냈다. 간호여대생의 자아존중감, 식이 자기효능감, 생활스트레스, 식행동과의 관계를 분석한 결과 식행동과 자아존중감(r=-.238, p<.001), 식이 자기효능감(r=-.438, p<.001)과는 유의한 부적 상관관계가 나타났고, 생활스트레스(r=.194, p<.001)와는 정적 상관관계가 나타났다. 간호여대생의 식행동에 영향을 미치는 요인은 식이 자기효능감, 생활스트레스, 주거형태, 건강에 대한 관심도로 나타났고, 설명력은 26.7%로 나타났다. 본 연구의 결과를 통하여 간호여대생을 대상으로 식행동에 영향을 미치는 다양한 요인을 확인하기 위한 반복 연구가 필요하고, 식행동을 향상시킬 수 있는 교육 및 중재프로그램 개발이 필요하다.

여드름 피부가 청소년의 스트레스, 우울증, 자아존중감에 미치는 영향 - 중학생 중심으로 - (Influence of Acne Skin on Teenager's Stress, Depression, and Self-esteem - Focusing on middle school students -)

  • 현재 청소년들은 대중매체의 근접성에 의한 외모적 아름다움을 중요시 하고 있다. 따라서 피부에 발생하는 여드름은 중증의 질병은 아니지만 잘못 관리하면 안면부에 흉터와 자국을 남기게 되고 이는 학생들의 자신감을 상실시키고 우울증과 스트레스를 동반한다. 이에 본 연구에서는 여드름 피부가 청소년의 스트레스, 우울증, 자아존중감에 어떠한 영향을 미치는 가를 분석하여 여드름을 단순한 피부질환의 하나로 생각할 것이 아니라 정신건강에도 밀접한 관계가 있음을 인지하고 여드름 피부를 관리하는데 좀 더 효과적이며 피부를 관리하는 기초자료로 활용하고자 3가지 가설을 수립하였다. 그 결과 중학생의 여드름 발생이 스트레스와 우울증에 정(+)의 영향을 미치는 것으로 나타나 여드름 피부관리는 표면적 관리가 아닌 정신건강에 필요한 관리로 사료된다.

대학생들의 스트레스와 인터넷 중독의 관계에 관한 연구 (A Study on the Relationship between Internet Addiction and Stress of College Students)

  • 본 연구의 주요 목적은 대학생들을 대상으로 스트레스와 인터넷 중독의 관계를 규명하는 것이다. 인터넷 중독 정도를 측정하기 위해 미국 온라인 중독센터에서 사용하고 있는 인터넷 중독검사를 이용했으며, 20-49점을 정상이용집단, 50-79점을 중독위험집단, 80점 이상을 인터넷 중독집단으로 분류하였다. 연구결과 스트레스 요인 중 대인관계에서 느끼는 스트레스, 가정생활에서 느끼는 스트레스, 학교생활에서 느끼는 스트레스가 인터넷 중독에 유의한 영향을 미치는 것으로 분석되었다.

유아의 일상적 스트레스 및 부적응 행동 : 유아의 개인적 변인과 어머니의 심리사회적 변인을 중심으로 (The Effects of Personal and Parental Variables on Young Children's Daily Stress Levels and Maladjustment Behaviors)

  • The purpose of this study was to establish the relative influences of individual variables (sex, age, temperament) and mother's psychosocial variables (maternal attitude, parenting stress, marriage satisfaction) on daily stress levels and maladjustment of young children attending at daycare centers. Subjects were 224 pairs of children and their mothers, and 18 teachers from 5 child-care centers in Gwangju City. Children were tested individually using 22-item daily stress inventory with illustrations which described certain stressful situations. Questionnaires were also administered to mothers and teachers. Collected data were subjected to Cronbach's $\alpha$, correlation and multiple regression analysis using SPSS (WIN 14.0) program. The major findings were as follows; Overall average score of children's daily stress was equivalent with the median. 'Blame and attack situation' and 'anxiety and frustrated situation' were perceived as more stressful for children than 'lose self-respect situation'. Overall average score of children's maladjustment was lower than the median. 'Overactivity' was highest among subscores of maladjustment. Age was the only significant influential factor on children's daily stress level. Sex was the most influential factor on 'overactivity', 'aggression', and 'regression' of young children. Activity and maternal stress were the second most influential factor on 'overactivity' and 'aggression', respectively.

응급실 간호사의 직무 스트레스, 스트레스 대처방식, 강인성이 소진에 미치는 영향 (Impact of Job Stress, Coping Behavior, Hardiness on Burnout in Nurses in the Emergency Department)

  • Purpose: This study aimed to identify the effect of job stress, coping behavior, and hardiness on burnout in nurses in the emergency department (ED). Methods: Data were acquired via self-reporting questionnaires from 148 nurses in EDs at one general hospital, four university hospitals, and six hospitals with >200 beds. Data were analyzed using the SPSS 23.0 program through descriptive statistics, t-test, analysis of variance, Pearson's correlation coefficient, and stepwise multiple regression. Results: A statistically significant difference in burnout was observed with respect to the age of the nurses. Burnout was positively correlated with job stress and wishful thinking and detachment, among coping behaviors, and was negatively correlated with hardiness. Job stress and wishful thinking, among coping behaviors, were significant predictors of burnout and were observed in 14.9% of the nurses who experienced burnout. Conclusion: According to our results, job stress was identified as the most significant factor which influenced burnout in ED nurse's. Therefore, it is necessary to develop and implement educational programs for reducing job stress and minimizing the use of wishful thinking as a coping behavior in order to alleviate the burnout in ED nurses.

북한이탈청소년의 학교중도탈락 의도에 영향을 미치는 요인 (Factors Affecting School Drop-out Intention of North Korean Refugee Youth)

  • 본 연구의 목적은 북한이탈 청소년의 학교중도탈락의 영향요인과 그 영향의 정도를 밝히고, 결과를 근거로 사회복지실천 개입전략과 정책적 제안을 마련하는 것이다. 성별, 학업성취역량 인식, 자아존중감, 학교 문화적응스트레스와 같은 개인내적 특성이 학교중도탈락의 주요 영향요인이며, 부모 양육의 질, 또래애착, 가족의 경제수준, 정착기간 등의 환경적 요인들은 유의한 영향을 미치지 않는 것으로 나타났다. 결론에서 본 연구가 갖는 이론적 기여, 실천 정책적 함의에 대하여 논의하였다.

Effect of PCE superplasticizers on rheological and strength properties of high strength self-consolidating concrete

  • A variety of polycarboxylate ether (PCE)-based superplasticizers are commercially available. Their influence on the rheological retention and slump loss in respect of concrete differ considerably. Fluidity and slump loss are the cardinal features responsible for the quality of concrete. These are related to the dispersion of cement particles and the hydration process which are greatly influenced by type of polycarboxylate ether (PCE)-based superplasticizers. On the backdrop of relatively less studies in the context of rheological retention of high strength self-consolidating concrete (HS-SCC), the experimental investigations were carried out aiming at quantifying the effect of the six different PCE polymers (PCE 1-6) on the rheological retention of HS-SCC mixes containing two types of Ordinary Portland Cements (OPC) and unwashed crushed sand as the fine aggregate. The tests that were carried out included $T_{500}$, V-Funnel, yield stress and viscosity retention tests. The supplementary cementitious materials such as fly ash (FA) and micro-silica (MS) were also used in ternary blend keeping the mix paste volume and flow of concrete constant. Low water to binder ratio was used. The results reveal that not only the PCEs of different polymer groups behave differently, but even the PCEs of same polymer groups also behave differently. The study also indicates that the HS-SCC mixes containing PCE 6 and PCE 5 performed better as compared to the mixes containing PCE 1, PCE 2, PCE 3 and PCE 4 in respect of all the rheological tests. The PCE 6 is a new class of chemical admixtures known as Polyaryl Ether (PAE) developed by BASF to provide better rheological properties in even in HS-SCC mixes at low water to binder mix. In the present study, the PCE 6, is found to help not only in reduction in the plastic viscosity and yield stress, but also provide good rheological retention over the period of 180 minutes. Further, the early compressive strength properties (one day compressive strength) highly depend on the type of PCE polymer. The side chain length of PCE polymer and the fineness of the cement considerably affect the early strength gain.
