A Study on the Relationship between Hope and Self-Care Agency in Hemodialysis Patients (혈액투석환자의 희망과 자가간호역량과의 관계)

  • This study examined and identified the relationship between the level of hope and self-care agency in hemodialysis patients. The purpose of the study was to reveal that hope is an essential factor to enhance self-care agency and to prepare the basic materials to help nurse the patients. The subjects for this study consisted of 108 persons who undergoing hemodialysis from 2 hospital, 2 hemodialysis center in Pusan, Korea. The degree of hope was measured by the questionnaire that has been developed by Miller. The self-care agency was measured by the questionnaire that has been developed by Kearney B.Y & Fleisher B.J. The collected data were analyzed by descriptive statistics and t-test. one-way ANOVA and Pearson's Correlation Coefficient with SPSS WIN program. Results were obtained as follows: 1) The mean scores of hope were $131.77{\pm}25.77$(range 67-195). The rate of hope classified said that the lowest hope was 2.8% and low hope was 27.8%. 2) The mean scores of self-care agency were $143.78{\pm}23.63$(range 74-187). The degree of self-care agency classified said that the rate of self-care agency deficiency was 0.9% and low self-care agency was 12.0%. 3) The hope was significantly related to self-care agency. That is, the higher the hope score was, the higher the level of self-care agency score(r=0.668,p=0.001) was. In this result, there were hemodialysis patients who have very low hope score and self-care agency score. In this cases self-care agency can be increased by encouraging the patients to be more hopeful. Conclusively nurses should access the degree of hope and self-care agency of hemodialysis patients individually and apply the hope therapy in order that the patient have higher hope than before.

  • The purpose of this study was to identity degree on self-care agency & self efficacy of hemodialisis patients. The subjects for the study consisted of 260 hemodialisis patients and the data were collected from April 1st. 2002 to April 30. 2002. The instruments used for this study were the Self-carer As Inventory Scale(Genden & Taylor. 1998) and Concrete Self Efficacy Scale(Joo-Hyen Kim. 1995). The Self-carer As Inventory translated by So Hyang-Sook and was modified by auther of this study. The data were analysed by using SPSS/WIN program and included number. percentange. t-test, ANOV A and Pearson Correlation Coefficient. The results of the study are summarized as follows. 1. As the average score Self-care agency indicated $132.21\pm19.11$ points and Self efficacy showed $597.33\pm130.95$points. it can be said that Self-care agency and Self efficacy were high. 2. In the relationship between general and hemodialisis cure characteristics and Self care agency were significant difference in age(F=3.065. p=0.018). married(F=3.160. p= 0.029). religion(F=4.128. p=0.003). hemodialisis cure duration(F=2.615. p=0.049). 3. In the relationship between general and hemodialisis cure characteristics and Self efficacy were significant difference in age (F=2.992. p=0.019). married(F=4.427. p=0.013), job(before attack-F=1.926. p=0.044. after attack-F=1.784. p=0.048). incomes (F=2.235. p=0.041), hemodialisis cure frequency (F=1.718. p=0.042). diet practice (F=2.248. p=0.025). 4. Self efficacy was significantly related to Self-care agency(r=0.474. p<(0.001). That is the higher the self efficacy. the higher the level of Self-care agency. The result in the above showed that more concern and endeavors need to improve hope. medical support. self efficacy, self-care agency of hemodialisis patients.

Factors Affecting the Self Care Behavior of Korean High School Students (고등학생의 자가간호행위 영향요인)

  • Purpose: The purpose of this study was to analyze the factors affecting Self Care Behavior in Korean high school students. Method: Data were collected from November 9 to December 28, 2003. The participants in this study were 293 students(134 boys, 159 girls), recruited from two High School located in Seoul. Data collection was conducted through the use of Questionnaire that was modified by the investigator. The data were analyzed with descriptive statistics, Pearson Correlation coefficients and stepwise multiple regression. The SPSS win 10.0 program was used. Results: The mean of total item score the Self Care Behaviors scales was 3.37, which was slightly low. There was a significant correlation between Self Care Behavior, Hope, Social Support, and Self Care Agency(γ= .30 ~ .65, p<.001). Stepwise multiple regression analysis showed that Self Care Agency was a predictor of Self Care Behavior and accounted for 28% of the variance. Self Care Agency account for 31% of the variance in Self Care Behavior in the high school girls. Self Care Agency and Hope accounted for 27% of the variance in Self Care Behavior in High school boys. Conclusion: Self Care Agency accounted for variance in Self Care Behavior in these high school students. Therefore it is necessary to develop nursing interventions to increase Self Care Agency in high school students in order to increase the Self Care Behavior.

Factors Influencing Quality of Life in Type II Diabetes Mellitus Patients Registered at Public Health Center (일개 시 보건소에 등록한 제2형 당뇨병 대상자의 삶의 질 영향요인)

  • Purpose: The purpose of this study was to examine the relationships among self-care, self-care agency, self-efficacy, and quality of life in type II diabetic patients registered at a public health center. Method: The study subjects were 128 type II diabetic patients who were living in G city. The data were collected from March 2001 to February 2002. The instruments used for this study were the self-care scale developed by Jeung(1997) and designed by Park (1984) based on the original scale, the self-care agency scale developed by So (1992), the self-efficacy scale developed by a Jeung (1997) and designed by Paek (1996) based on the original scale, and the quality of life scale developed by Ro (1988). The data were analyzed using descriptive statistics, pearson correlation coefficient, and stepwise multiple regression. Results: 1. The relationships among self-care, self-care agency, self-efficacy, and quality of life were significant. Self-care was significantly related to self-care agency (r=.609. p<.01), self-efficacy (r=.763. p<.01), and quality of life (r=.493. p<.01). 2. The stepwise multiple regression analysis was performed to identify factors influencing quality of life of the subjects. The most powerful predictor was self-care agency (48.4%). The combination of self-care, complication status, age, education level, and self-efficacy accounted for 88.7% of the variance of quality of life in type II diabetic patients. Conclusion: The results suggest that self-care, self-care agency, self-efficacy, and quality of life are important variables for development of nursing intervention programs for patients with diabetes.

Effects of Empowerment Education Program for Hemodialysis Patients on Self-efficacy, Self-care Agency, Self-care Activities and Physiologic Parameters (능력증강 교육프로그램이 혈액투석환자의 자기효능감, 자가간호역량, 자가간호행위 및 생리적 지표에 미치는 효과)

  • Purpose: The purpose of this study was to determine the effects of empowerment education program on self-efficacy, self-care agency, self-care activities and physiologic parameters among hemodialysis patients. Method: Nonequivalent control group pretest-posttest design was used. There were 36 subjects in the experimental group and 35 subjects in the control group. The empowerment education program was administered for 4 weeks (1 time per week). Data were collected pre-treatment and post treatment. The $X^2$-test or Fisher's exact test, ANCOVA, t-test and paired t-test were performed to determine the group differences, using SPSS 14.0 version. Results: Self-efficacy, self-care agency, self-care activities and weight variation significantly improved after the empowerment education program in hemodialysis patients. Conclusion: The empowerment education program for hemodialysis patients was effective to improve the degree of self-efficacy, self-care activities and self-care agency and to decrease interdialytic weight variation.

A Study on the Relationship Between Learned Helplessness and Self-care Agency in Dialysis Patients (투석환자의 학습된 무기력과 자가간호역량에 관한 관계 연구)

  • The purpose of this study was to identify the degree of learned helplessness and self-care agency in dialysis patients and the relationship between learned helplessness and self-care agency. The subjects were 168 dialysis patients who were undergoing hemodialysis and peritoneal dialysis from 1 university hospital, 1 hospital, 1 hemodialysis center in Seoul, 1 hospital in Incheon, Korea. The data were collected with two interview questionnaires which were Learned Helplessness Scale (LHS) and Self-as-Carer Inventory (SCI). The collected data were analyzed by descriptive statistics and t-test, One-way ANOVA and Pearson Correlation with SPSSWIN program. Results were obtained as follows : 1) The mean scores of learned were 45.93(range 20-80). The mean learned helplessness scores of peritoneal dialysis patient and hemodialysis (HD) patient were not different significantly. However by the aging process, the mean scores of learned helplessness has a tendency to get higher and who had a job were likely to low score of learned helplessness. 2) The mean scores of self-care agency were 142.48 (range 33-198). The mean self-care agency scores of peritoneal dialysis (PD) patient much higher than the score of it with hemodialysis. The score of self-care agency were significantly different between peritoneal dialysis patient and hemodialysis patient. The patients who have job were likely to be higher score of self-care agency than other groups. 3) There was significant relationship between the score of learned helplessness and self-care agency. In conclusion, there was an inverse relationship between learned helplessness and self-care agency in dialysis patients. Considering this, the high level of learned helplessness of dialysis patients should be intervened by nurse with a well developed edicational program or cognitive behavioral therapy.

  • This study examined the relationship between the level of hardiness and self-care agency of persons with a colostomy. The purpose of the study was to explore the effect of hardiness on self-care agency of persons with a colostomy and to identity factors influencing on. The ultimate goal was to help nurses improve the person's self-care agency and to contribute theoretical base for the planning of supportive nursing interventions to increase the level of self care. The subjects for the study consisted of 43 persons aged above eighteen below seventy had had Miles operation and permanent colostomy. The data were collected from July, 1998 to September 15, 1998. The instruments used for this study were the Health-Related Hardiness (Pollock, 1984) and the Self-carer As Inventory Scale(Genden & Taylor, 1988). The Self-Carer As Inventory translated by So Hyang-sook and was modified by author of this study. The data were analysed by using SAS computerlized program and included number, percentage, t-test, ANOVA and Pearson Correlation Coefficient. The results of the study are summarized as follows. 1. Hardiness was significantly related to Self-care agency(r=-0.531. p=0.0002). That is, the higher the hardiness, the higher the level of Self-care agency. 2. In the relationship between general characteristics and traits of persons with a colostomy and hardiness were significant difference in monthly income (F=2.79, p=0.03), problem in stoma management (F=3.29, p=0.01). 3. In the relationship between general characteristics and traits of persons with a colostomy and Self-care agency were significant difference in monthly income (F=2.93, p=0.03), problem in stoma management(F=2.93, p=0.01). Therefore, in order to increase the level of Self-care agency of ostomate Nurses should be helpful to provide them with hardiness training to strengthen their control, commitment and challenge(subconcepts of hardiness).

A Study on the Relation Between the Perceived Social Support and Self-Care Agency of High School Students (일 고등학교 학생이 지각한 사회적 지지와 자가간호역량과의 관계 연구)

  • This study was designed to identify the relation between the Perceived Social Support and the Self-care Agency of high school students in a rural area. The subjects for this study were 250 students living in Chonnam province: among first grade, second grade and third grade students were 98, 89 and 63 respectively. The data were collected during the period from April 2 to 4, 2001. The instruments used in this study were the Generally Perceived Social Support Scale developed by Park, J. W.(1985) and Self-Care Agency Questionnarie developed by Deneys(1981). The data were analyzed by descriptive statistic, t-test, ANOVA, and Pearson's correlation using the SAS PC+ Program. The results were as follows: 1. The mean score of the Perceived Social Support was 3.19. 2. The mean score of the Self-care Agency was 2.65. 3. There was a significant differences in school year(F=3.11, p=.046), educational level of the father(F=3.41, p=.035) that of the mother(F=4.07, p=.019), and economic status(F=8.99, p=.000), school performance(F=16.37, p=.000) from Perceived Social Support between general characteristics. 4. There was a significant differences in economic status(F=4.55, p=.004), school performance(F=6.72, p=.002) from self care agency between general characteristics. 5. The relation between the score of the Perceived Social Support and Self-care Agency was significant(r=.49, p=.0001). The relation between the score of the direct Perceived Social Support and Self-care Agency was significant(r=.50, p=.0001) and the relation between the score of the indirect Perceived Social Support and Self-care Agency was also significant(r=.40, p=.0001). In conclusion, it was found that higher score of the Social Support was a higher level of the Self-care Agency, especially the direct Perceived Social Support. The score of the Social Support and Self-care Agency was significantly differentiated according to economic status and school performance.

  • Purpose: This study examined the relationship between level of activity of daily living (ADL) and Self-Care Agency in patients who have had a spinal cord injury. Method: The participants in the study were 78 persons who had a spinal cord injury and were between 20 and 70 years of age. The data were collected from November 1, 2003 to January 30, 2004. The instruments used in this study were the ADL Check List and Self-Care Agency Scale. The data were analyzed using the SPSS program and included numbers, percentages, t-test and ANOVA, and Pearson correlation coefficients. Results: The total mean score for activity of daily living was $44.21{\pm}16.63$. The total mean score fur Self-Care Agency was $107.45{\pm}15.71$, There was a significant difference between scores for ADL and Self-Care Agency. Conclusion: In order to increase the level of ADL, rehabilitation nurses should ensure that these patients receive training in ADL. Also, it was identified that informational support is important, that is, nurses should help to provide these patients with Self-Care Agency training for ADL.

The Relationships among Postpartum Fatigue, Depressive Mood, Self-care Agency, and Self-care Action of First-time Mothers in Bangladesh

  • Purpose: Postpartum fatigue can impact maternal well-being and has been associated with levels of perceived self-care. This study aimed to examine the relationship among fatigue, depressive mood, self-care agency, and self-care action among postpartum women in Bangladesh. Methods: A descriptive cross sectional survey was done with 124 first-time mothers from two tertiary hospitals in Dhaka, Bangladesh. The Modified Fatigue Symptoms checklist, Denyes' Self Care Instrument, the Edinburgh Postnatal Depression Scale, and items on sociodemographic and delivery-related characteristics, were used in Bengali via translation and back-translation process. Results: High fatigue levels were found in 18.5% (n=23) and 73.4% had possible depression (n=91). There was a significant negative relationship between fatigue and self-care agency (r=-.31, p<.001), and self-care action (r=-.21, p<.05). Fatigue differed by level of self-care agency (t=4.06, p<.001), self-care action (t=2.36, p=.023), newborn's APGAR score (t=-2.93, p=.004), parental preparation class participation (F=15.53, p<.001), and postpartum depressive mood (t=-4.64, p<.001). Conclusion: Findings suggest that high level of self-care efficacy and behaviors can contribute to fatigue management, and highlight the need for practical interventions to better prepare mothers for postpartum self-care, which may, in turn, alleviate postpartum fatigue.
